Pro-Con Debate: Universal Versus Selective Continuous Monitoring of Postoperative Patients.

In this Pro-Con commentary article, we discuss use of continuous physiologic monitoring for clinical deterioration, specifically respiratory depression in the postoperative population. The Pro position advocates for 24/7 continuous surveillance monitoring of all patients starting in the postanesthesia care unit until discharge from the hospital. The strongest arguments for universal monitoring relate to inadequate assessment and algorithms for patient risk. We argue that the need for hospitalization in and of itself is a sufficient predictor of an individual's risk for unexpected respiratory d

Interdisciplinary collaboration in critical care alarm research: A bibliometric analysis.

BACKGROUND: Alarm fatigue in nurses is a major patient safety concern in the intensive care unit. This is caused by exposure to high rates of false and non-actionable alarms. Despite decades of research, the problem persists, leading to stress, burnout, and patient harm resulting from true missed events. While engineering approaches to reduce false alarms have spurred hope, they appear to lack collaboration between nurses and engineers to produce real-world solutions. The aim of this bibliometric analysis was to examine the relevant literature to quantify the level of authorial collaboration b

Learning to live on a Mars day: fatigue countermeasures during the Phoenix Mars Lander mission.

STUDY OBJECTIVES: To interact with the robotic Phoenix Mars Lander (PML) spacecraft, mission personnel were required to work on a Mars day (24.65 h) for 78 days. This alien schedule presents a challenge to Earth-bound circadian physiology and a potential risk to workplace performance and safety. We evaluated the acceptability, feasibility, and effectiveness of a fatigue management program to facilitate synchronization with the Mars day and alleviate circadian misalignment, sleep loss, and fatigue. DESIGN: Operational field study. SETTING: PML Science Operations Center. PARTICIPANTS: Scienti

Cross-Cultural Adaptation and Validation of the Japanese Charité Alarm Fatigue Questionnaire (CAFQa) Among ICU Nurses and Physicians: A Multicentre Study.

Alarm fatigue threatens patient safety in intensive care units (ICUs), yet no validated Japanese instrument exists. We cross-culturally adapted the Charité Alarm Fatigue Questionnaire (CAFQa) into Japanese following COSMIN guidelines and evaluated it in 129 clinicians (103 nurses, 26 physicians) at five hospitals, with 102 retested. Confirmatory factor analysis supported the two-factor structure (CFI = 0.922; RMSEA = 0.041; SRMR = 0.076), but the inter-factor correlation was near zero (r = 0.05), suggesting that Alarm Stress and Alarm Coping behave as largely independent dimensions. Actionable Ventricular Tachycardia During In-Hospital ECG Monitoring and Its Impact on Alarm Fatigue.

BACKGROUND: Ventricular tachycardia (V-tach) is the most common lethal arrhythmia, yet 90% of alarms are false and contribute to alarm fatigue. We hypothesize that some true V-tach also causes alarm fatigue because current criteria are too sensitive (i.e., ≥6 beats ≥100 beats/min [bpm]). PURPOSE: This study was designed to determine (1) the proportion of clinically actionable true V-tach events; (2) whether true actionable versus nonactionable V-tach differs in terms of heart rate and/or duration (seconds); and (3) if actionable V-tach is associated with adverse outcomes. METHODS: This was a

Automated stress detection using mobile application and wearable sensors improves symptoms of mental health disorders in military personnel.

Leading causes in global health-related burden include stress, depression, anger, fatigue, insomnia, substance abuse, and increased suicidality. While all individuals are at risk, certain career fields such as military service are at an elevated risk. Cognitive behavioral therapy (CBT) is highly effective at treating mental health disorders but suffers from low compliance and high dropout rates in military environments. The current study conducted a randomized controlled trial with military personnel to assess outcomes for an asymptomatic group (n = 10) not receiving mental health treatment, a

Embedding social determinants in mHealth for pediatric oncology: co-designing a patient-centred tool for febrile neutropenia in resource-limited settings.

BACKGROUND: Febrile neutropenia (FN) is a potentially life-threatening complication of chemotherapy in pediatric oncology. In resource-limited settings, timely care is often hampered by structural and social barriers. Broader social determinants of health (SDoH)—including food insecurity, housing instability, limited transport, digital exclusion, and caregiver mental health conditions—undermine caregivers’ capacity to respond to FN episodes effectively. METHODS: This study aimed to identify and operationalize key SDoH influencing FN management and integrate them into the co-design of the Salud

Reducing inappropriate infectious disease serology tests through the use of decision support in a provincial electronic health system.

Diagnostic stewardship improves patient care and reduces health costs by promoting appropriate test use. Ordering serologic tests for infectious diseases can be challenging, with indications differing based on the clinical picture, previous results, or pathogens. We aimed to use our electronic health system (EHS) to provide real-time, patient-specific education on test appropriateness to reduce inappropriate testing. This observational study took place from October 2023 to 2024. The study included all users of the province-wide, integrated EHS in Alberta, Canada (population 4.9 million), which

Blue-light filtering spectacle lenses for visual performance, sleep, and macular health in adults.

BACKGROUND: 'Blue-light filtering', or 'blue-light blocking', spectacle lenses filter ultraviolet radiation and varying portions of short-wavelength visible light from reaching the eye. Various blue-light filtering lenses are commercially available. Some claims exist that they can improve visual performance with digital device use, provide retinal protection, and promote sleep quality. We investigated clinical trial evidence for these suggested effects, and considered any potential adverse effects. OBJECTIVES: To assess the effects of blue-light filtering lenses compared with non-blue-light f

SPIRIT-PRO Extension explanation and elaboration: guidelines for inclusion of patient-reported outcomes in protocols of clinical trials.

Patient-reported outcomes (PROs) are used in clinical trials to provide valuable evidence on the impact of disease and treatment on patients' symptoms, function and quality of life. High-quality PRO data from trials can inform shared decision-making, regulatory and economic analyses and health policy. Recent evidence suggests the PRO content of past trial protocols was often incomplete or unclear, leading to research waste. To address this issue, international, consensus-based, PRO-specific guidelines were developed: the Standard Protocol Items: Recommendations for Interventional Trials (SPIRI

Carcinomas arising in multilocular thymic cysts of the neck: a clinicopathological study of three cases.

BACKGROUND: The high prevalence rate of malaria due to the proliferation of drug-resistant strains of Plasmodium falciparum could mitigate the realization of sustainable development goals in Nigeria, necessitating research on the discovery of novel chemotherapies against resistant strains of the parasite. However, Solanum anomalum with ethnopharmacological claims could foster this breakthrough, prompting an investigation on the antiplasmodial and hepatoprotective activities of the plant's fruit, semi-purification and characterization of its constituents, as well as in silico studies on its com
