Alkylating agents for Waldenstrom's macroglobulinaemia.

BackgroundWaldenstrom's macroglobulinaemia (WM) is an uncommon B-cell lymphoproliferative disorder characterized by bone marrow infiltration and production of monoclonal immunoglobulin. Uncertainty remains if alkylating agents, such as chlorambucil, melphalan or cyclophosphamide, are an effective form of management.ObjectivesTo assess the effects and safety of the alkylating agents on Waldenstrom's macroglobulinaemia (WM).Search strategyWe searched the Cochrane Central Register of Controlled Trials (Issue 1, 2008), MEDLINE (1966 to 2008), EMBASE (1980 to 2008), the Chinese Biomedical Base (198

Secondary 3-Chloropiperidines: Powerful Alkylating Agents.

In previous works, we demonstrated that tertiary 3-chloropiperidines are potent chemotherapeutics, alkylating the DNA through the formation of bicyclic aziridinium ions. Herein, we report the synthesis of novel secondary 3-chloropiperidine analogues. The synthesis incorporates a new procedure to monochlorinate unsaturated primary amines utilizing N-chlorosuccinimide, while carefully monitoring the temperature to prevent dichlorination. Furthermore, we successfully isolated highly strained bicyclic aziridines by treating the secondary 3-chloropiperidines with a sufficient amount of base. We con

Alkylating agents stronger than alkyl triflates.

A new class of potent electrophilic "R(+)" alkylating agents has been developed using weakly nucleophilic carborane anions as leaving groups. These reagents, R(CHB(11)Me(5)X(6)) (R = Me, Et, and i-Pr; X = Cl, Br), are prepared via metathesis reactions with conventional alkylating agents such as alkyl triflates, using the high oxophilicity of silylium ion-like species, Et(3)Si(carborane), as the driving force to obtain increased alkyl electrophilicity. The crystal structure of the isopropyl reagent, i-Pr(CHB(11)Me(5)Br(6)), has been determined, revealing covalence in the alkyl-carborane bonding

Alkylating anticancer agents and their relations to microRNAs.

Alkylating agents represent an important class of anticancer drugs. The occurrence and emergence of tumor resistance to the treatment with alkylating agents denotes a severe problem in the clinics. A detailed understanding of the mechanisms of activity of alkylating drugs is essential in order to overcome drug resistance. In particular, the role of non-coding microRNAs concerning alkylating drug activity and resistance in various cancers is highlighted in this review. Both synthetic and natural alkylating agents, which are approved for cancer therapy, are discussed concerning their interplay w

Response of generalized granuloma annulare to alkylating agents.

The conditions of three patients who had generalized granuloma annulare responded favorably to treatment with alkylating agents. This mode of therapy in two cases was chosen on the basis of recent evidence that implicates delayed hypersensitivity mechanisms as forming part of the pathogenesis of granuloma annulare. Two patients had rapid clearing of their lesions. The response in one of these patients was observed during therapy for a concomitant plasma cell dyscrasia. The third patient had an unusual form of granuloma annulare that had evolved during a period of 28 years, with hand deformitie

[Plasmocytoma, alkylating agents, and acute myeloid leukemia (author's transl)].

Two cases of the development of acute myeloid leukemia (AML) after treatment with alkylating agents are reported. In Case 1, melphalan and then cyclophosphamide had been given for multiple myeloma. 46 months after onset of cytostatic treatment AML occurred, as confirmed cytochemically and by qualitative determination of urinary lysozyme. In Case 2, cyclophosphamide had been given for rheumatoid arthritis. After a latency of 34 months 'smouldering leukaemia' developed with an atypical monocytic leukaemic cell population. In a third case, multiple myeloma and monocytic leukaemia developed synchr

An Update on Alkylating Agents in Breast Cancer Therapy.

Alkylating agents, characterized by their ability to bind to and modify DNA, have shown promising impacts on breast cancer patients in clinical trials across various stages and phases. This review, utilizing data from the National Library of Medicine's clinicaltrials.gov, investigates the efficacy of these drugs in breast cancer treatment. The report focuses on cyclophosphamide, an alkylating agent that prevents cancer cell DNA replication, and its synergistic effects when combined with other medications such as docetaxel, a taxane that suppresses cell division. Results indicate that these com

Ovarian reserve after treatment with alkylating agents during childhood.

STUDY QUESTION: What is the effect of different alkylating agents used without pelvic radiation to treat childhood cancer in girls on the ovarian reserve in survivors? SUMMARY ANSWER: Ovarian reserve seems to be particularly reduced in survivors who received procarbazine (in most cases for Hodgkin lymphoma) or high-dose chemotherapy; procarbazine but not cyclophosphamide dose is associated with diminished ovarian reserve. WHAT IS KNOWN ALREADY: A few studies have demonstrated diminished ovarian reserve in survivors after various combination therapies, but the individual role of each treatmen

DNA damage induced by alkylating agents and repair pathways.

The cytotoxic effects of alkylating agents are strongly attenuated by cellular DNA repair processes, necessitating a clear understanding of the repair mechanisms. Simple methylating agents form adducts at N- and O-atoms. N-methylations are removed by base excision repair, AlkB homologues, or nucleotide excision repair (NER). O(6)-methylguanine (MeG), which can eventually become cytotoxic and mutagenic, is repaired by O(6)-methylguanine-DNA methyltransferase, and O(6)MeG:T mispairs are recognized by the mismatch repair system (MMR). MMR cannot repair the O(6)MeG/T mispairs, which eventually lea

Sensitive and selective photoinduced-electron-transfer-based sensing of alkylating agents.

Photoinduced-electron-transfer (PET)-based chemosensing is a very elegant way of reporting the presence of a guest species in solution. This method was successfully applied for the detection of different ionic species, such as cations, anions, and protons. Herein, we report on the application of the PET chemosensing concept for the efficient and selective detection of different alkylating agents. 2-(2-Dimethylaminoethyl)benzo[de]isoquinoline-1,3-dione (1) was found to be a highly selective and effective PET chemosensor that turns luminescent upon reacting with different alkylating agents. Polyphenols enhance the activity of alkylating agents in leukaemia cell lines.

Polyphenols have been shown to sensitize solid tumours to alkylating agents such as cisplatin, and induce apoptosis and/or cell-cycle arrest. Here, we assess the effects of five polyphenols alone and in combination with three alkylating agents: cisplatin, cyclophosphamide and chlorambucil in lymphoid and myeloid leukaemia cells lines, and non-tumour control cells. In lymphoid leukaemia cell lines there was a synergistic reduction in ATP and glutathione levels, an induction of cell cycle arrest, DNA damage and apoptosis when quercetin, apigenin, emodin and rhein were combined with cisplatin and

Radiotherapy, alkylating agents, and risk of bone cancer after childhood cancer.

BACKGROUND: Individuals who had cancer in childhood are at higher risk of developing bone cancer than any other type of second primary cancer. PURPOSE: Using the population-based National Registry of Childhood Tumours in Britain, we investigated the incidence and etiology of second primary bone cancer after childhood cancer in a cohort study and in a case-control study. METHODS: A cohort study of 13,175 3-year survivors of childhood cancer diagnosed in Britain between 1940 and 1983 revealed 55 subsequent bone cancers. A largely nested case-control study comprised 59 case subjects developing

Bladder urotoxicity pathophysiology induced by the oxazaphosphorine alkylating agents and its chemoprevention .

The use of oxazaphosphorines (cyclophosphamide, ifosfamide) in the treatment of numerous neoplastic disorders is associated with their essential adverse effect in the form of hemorrhagic cystitis, which considerably limits the safety and efficacy of their pharmacotherapy. HC is a complex inflammatory response, induced by toxic oxazaphosphorines metabolite - acrolein with subsequent immunocompetetive cells activation and release of many proinflammatory agents. However, there are some chemoprotectant agents which help reduce the HC exacerbation. The article briefly discuses the mechanism of acti

Multidrug resistance to alkylating agents and platinum compounds: state of our knowledge.

Resistance to platinum-containing drugs and alkylating agents is multifactorial. Relapsed patients and those who have an incomplete response to induction chemotherapy are unlikely to achieve a complete response to other treatments. This pattern of resistance to therapy is very broad in that patients not only fail to respond to drugs from the same pharmacologic classes as used in primary treatment, but also do not respond to other available drugs or irradiation. Using ovarian cancer as a prototype tumor, researchers have initiated laboratory studies to determine the mechanisms responsible for t

Susceptibility of Fanconi's anemia lymphoblasts to DNA-cross-linking and alkylating agents.

In order to develop the usefulness of Fanconi's anemia (FA) lymphoblast lines for biochemical and genetic studies, we have determined their sensitivity to a variety of DNA-damaging chemicals. We have adapted a growth inhibiton protocol in which the sensitivity of a cell line is characterized by the drug concentration yielding a 50% inhibiton of growth (EC50). The DNA-cross-linking agents, mitomycin C, nitrogen mustard, melphalan, 1,3-butadiene diepoxide, cis-diaminedichloroplatinum(II), and cyclophosphamide, were all more toxic to four FA cell lines than to five normal lines. Three lines, HSC
