Association of emergency intensive care unit occupancy due to brain-dead organ donors with ambulance diversion.

Our study aims to explore how intensive care unit (ICU) occupancy by brain-dead organ donors affects emergency ambulance diversions. In this retrospective, single-center study at an emergency ICU (EICU), brain-dead organ donors were managed until organ procurement. We classified each day between August 1, 2021, and July 31, 2023, as either an exposure day (any day with a brain-dead organ donor in the EICU from admission to organ procurement) or a control day (all other days). The study compared these days and used multiple logistic regression analysis to assess the impact of EICU occupancy by

Percutaneous fetoscopic spina bifida repair: effect on ambulation and need for postnatal cerebrospinal fluid diversion and bladder catheterization.

OBJECTIVE: A trial comparing prenatal with postnatal open spina bifida (OSB) repair established that prenatal surgery was associated with better postnatal outcome. However, in the trial, fetal surgery was carried out through hysterotomy. Minimally invasive approaches are being developed to mitigate the risks of open maternal-fetal surgery. The objective of this study was to investigate the impact of a novel neurosurgical technique for percutaneous fetoscopic repair of fetal OSB, the skin-over-biocellulose for antenatal fetoscopic repair (SAFER) technique, on long-term postnatal outcome. Quantile regression: a statistical tool for out-of-hospital research.

The performance of out-of-hospital systems is frequently evaluated based on the times taken to respond to emergency requests and to transport patients to hospital. The 90th percentile is a common statistic used to measure these indicators, since they reflect performance for most patients. Traditional regression models, which assess how the mean of a distribution varies with changes in patient or system characteristics, are thus of limited use to researchers in out-of-hospital care. In contrast, quantile regression models estimate how specified quantiles (or percentiles) of the distribution of

Trends in quetiapine use and non-fatal quetiapine-related ambulance attendances.

INTRODUCTION AND AIMS: Concern about the non-medical use of quetiapine and related acute harms is growing. Case series cite quetiapine as a potential drug of misuse, while recent research questions its relative safety in comparison with other atypical antipsychotic preparations. This paper explores population-level patterns of quetiapine-related ambulance attendances over time, identifying associated risk factors and potential subpopulations at-risk of acute harms. DESIGN AND METHODS: A retrospective analysis of quetiapine-, olanzapine- and risperidone-related ambulance attendances in metropo

New and emerging drivers of ambulance demand.

BACKGROUND: The number of calls for ambulances in Australia has been rising faster than population growth, but it is unclear what is driving this demand. This study aims to quantify and compare the drivers of emergency ambulance demand in Victoria, Australia, before and after the COVID-19 pandemic, including demographic and clinical factors. METHODS: We conducted a retrospective cohort study of all emergency ambulance calls in Victoria from January 1, 2016, to December 31, 2023. Using negative binomial segmented time series regression models, adjusted for population growth and seasonality, we

Factors associated with ambulance use among patients with low-acuity conditions.

BACKGROUND: The use of ambulances for low-acuity medical complaints depletes emergency medical services (EMS) resources that could be used for higher-acuity conditions and contributes to emergency department (ED) overcrowding and ambulance diversion. Objective. We sought to understand the characteristics of patients who use ambulances for low-acuity conditions. We hypothesized that patients who arrive to the ED by ambulance for low-acuity conditions are more likely to be members of vulnerable populations. METHODS: A secondary analysis was performed on the National Hospital Ambulatory Medical

The Head Injury Transportation Straight to Neurosurgery (HITS-NS) randomised trial: a feasibility study.

BACKGROUND: Reconfiguration of trauma services, with direct transport of traumatic brain injury (TBI) patients to neuroscience centres (NCs), bypassing non-specialist acute hospitals (NSAHs), could potentially improve outcomes. However, delays in stabilisation of airway, breathing and circulation (ABC) and the difficulties in reliably identifying TBI at scene may make this practice deleterious compared with selective secondary transfer from nearest NSAH to NC. National Institute for Health and Care Excellence guidance and systematic reviews suggested equipoise and poor-quality evidence - with

Patient Ambulations Predict Hospital Readmission.

Improved functional ability and physical activity are strongly associated with a broad range of positive health outcomes including reduced risk of hospital readmission. This study presents an algorithm for detecting ambulations from time-resolved step counts gathered from remote monitoring of patients receiving hospital care in their homes. It examines the statistical power of these ambulations in predicting hospital readmission. A diverse demographic cohort of 233 patients of age 70.5±16.8 years are evaluated in a retrospective analysis. Eleven statistical features are derived from raw time s

Spinal dysraphism: a cross-sectional and retrospective multidisciplinary clinic-based study.

BACKGROUND: Spinal dysraphism is a common birth defect that causes different kinds of secondary impairments, including joint deformities, reduced mobility, and bowel/bladder dysfunction. Due to the diversity in terminology, cultural/ethnic differences, and medical policies, prior study results cannot be generalized to all populations. Therefore, we performed this study to define the characteristics of patients in Taiwan with spinal dysraphism. METHODS: Patients diagnosed with a myelomeningocele or lipomyelomeningocele were identified from the database of our spinal dysraphism multidisciplinar

Infection stones in patients with myelomeningocele and ileal conduit urinary tract diversion.

Infection renal calculi are serious complications of urinary tract diversion in patients with myelomeningocele. During a 12-month period, 10 renal units containing calculi were managed surgically. The kidneys were in patients with multiple previous operations and medical problems. Nine of the ten stones had a staghorn configuration, filling the renal pelvis with calyceal extensions. Nephrectomy was necessary for a nonfunctional kidney and a conventional nephrolithotomy was done in one case. Endourologic techniques were used for treatment of eight renal units. Endourologic methods depended on p

Powered prosthetic feet do not decrease contralateral knee loads relative to energy storing and returning feet during walking in a functionally diverse cohort of individuals with unilateral transtibial limb loss.

Among individuals with unilateral transtibial limb loss (UTTLL), contralateral knee osteoarthritis is a prevalent musculoskeletal condition that can greatly reduce long-term mobility and quality of life. While powered (POW) prosthetic feet may decrease contralateral knee loads during walking, a biomechanical risk factor for osteoarthritis, previous studies comparing POW to energy storing and returning (ESR) feet have been limited to relatively small (N=6-10), homogenous samples. Specifically, prior studies have included either younger, more novice ambulators (age: 25-38 years, 9-37 months ambu

Gait as a quantitative indicator of the severity of chemotherapy-induced peripheral neuropathy: a pilot study.

Chemotherapy-induced peripheral neuropathy (CIPN) is a prevalent and debilitating side effect of neurotoxic chemotherapeutic agents. Patient-reported outcome (PRO) measures are widely utilized to assess CIPN severity; however, they are subject to bias and variability. This prospective case series investigates whether gait parameters provide an objective measure of CIPN severity. This pilot study consisted of 27 patients: 9 active CIPN, 9 CIPN resolved, and 9 healthy controls. Gait parameters including velocity, ambulation time, cadence, distance, and number of steps were assessed using GAITRit

A qualitative exploration of behaviours and lifestyle factors impacting levels of vitamin D within a UK ambulance service workforce (EVOLVED).

INTRODUCTION: Vitamin D deficiency can impact health and well-being and may affect workplace performance. Shift, indoor and night working, alongside variable awareness of vitamin D, likely puts ambulance staff at an increased risk of deficiency. Screening in one ambulance service detected that 46% of staff had insufficient or deficient vitamin D levels (i.e. 50.0 nmol/L or less, as defined by NICE). The aim of the EVOLVED study was to explore the behaviours and lifestyle factors of ambulance service staff with a range of vitamin D levels and understand the impacts on their work and personal li
