[Comparison of the incidence of enterobacterial R plasmids in hospitals and ambulatory care facilities].

Bacteriological examination of patients in hospital and ambulatory facilities revealed that in hospitals the investigated diseases of patients are twice as frequently caused by pathogenic enterobacteria which contain R plasmids than pathogenic bacteria detected in subjects who had ambulatory treatment. Comparison of the number of isolated autonomously transmissible R plasmids revealed an even greater disproportion because some bacterial strains, in particular in hospitals, contained several compatible R plasmids. The author assumes that the almost treble number of R plasmids in strains from ho

CASE STUDY OF CONGENITAL ANOMALIES OF THE UPPER LIMB IN REFERENCE AMBULATORY CARE FACILITY.

OBJECTIVE: The epidemiological profile of congenital anomalies of the upper limbs (CAULs) is of major relevance to monitoring and planning. A study of this profile may reveal if there is prevalence of some specific type of malformation in comparison to a more comprehensive epidemiological sample. The Latin American Collaborative Study of Congenital Malformations (ECLAMC) has an extensive database, providing an excellent source of comparison. This study aims to evaluate the epidemiological profile of CAULs at the hand surgery department of the Hospital Federal da Lagoa (HFL) in Brazil, and comp

Ambulatory independence is associated with higher incidence of latent tuberculosis infection in long-term care facilities in Taiwan.

BACKGROUND/PURPOSE: Tuberculosis (TB) in the elderly population remains a major challenge in areas with intermediate disease burden like Taiwan. Despite the increasing burden and high risks of TB in the elderly population, particularly those living in long-term care facilities (LTCFs), diagnostic testing for latent tuberculosis infection (LTBI) has not been carefully evaluated in this group. This study aimed to investigate the prevalence and predictors of LTBI in older adults living in LTCFs. METHODS: Older adults living in seven LTCFs in Taiwan were prospectively enrolled between January and

Association of sugammadex reversal of neuromuscular block and postoperative length of stay in the ambulatory care facility: a multicentre hospital registry study.

BACKGROUND: Encapsulation of rocuronium or vecuronium with sugammadex can reverse neuromuscular block faster than neostigmine reversal. This pharmacodynamic profile might facilitate patient discharge after ambulatory surgery. METHODS: We included patients who underwent ambulatory surgery with general anaesthesia and neuromuscular block between 2016 and 2021 from hospital registries at two large academic healthcare networks in the USA. The primary outcome was postoperative length of stay in the ambulatory care facility (PLOS-ACF). We examined post hoc whether the type of reversal affects posto

Special article: Creation of a guide for the transfer of care of the malignant hyperthermia patient from ambulatory surgery centers to receiving hospital facilities.

CLINICAL PROBLEM: Volatile anesthetics and/or succinylcholine may trigger a potentially lethal malignant hyperthermia (MH) event requiring critical care crisis management. If the MH triggering anesthetic is given in an ambulatory surgical center (ASC), then the patient will need to be transferred to a receiving hospital. Before May 2010, there was no clinical guide regarding the development of a specific transfer plan for MH patients in an ASC. MECHANISM BY WHICH THE STATEMENT WAS GENERATED: A consensual process lasting 18 months among 13 representatives of the Malignant Hyperthermia Associati

Vaccine Effectiveness against Influenza in 2015/16 in Hospital and Ambulatory Medical Care Facilities: Polish Results of the European I-MOVE+ Multicenter Study.

Influenza vaccination is the best measure available to prevent seasonal influenza infection. The majority of studies on vaccine effectiveness in the 2015/16 season conducted in the European I-MOVE+ Project, show that a match between the circulating influenza strains in the general public and those included in the vaccine for the Northern Hemisphere was low to moderate. As part of I-MOVE+, Poland has implemented a case control negative study design and molecular biology methods, such as real time RT-PCR, to assess the vaccine match and effectiveness. The research described herein consisted of t

Data in ambulatory care logistics: What modelers need and what practice can offer.

Ambulatory care facilities play a critical role in many healthcare systems worldwide. To ensure efficient care provision, we must match care demand with care supply. To support provider decision-making, this article reviews Operations Research planning problems, the corresponding planning and control decisions that must be made when opening up or running an ambulatory care facility, and their data requirements. We give an overview of demand and supply-related data that an ambulatory care facility can collect and comment on the consequences for decision-making if some of that data is missing. W…

[Taking stock after 400 days: the effects on ambulatory care from a medical point of view].

After 400 days of integrated care the number and content of existing contracts are not yet known. This nationwide anonymity might be the result of a competition between present health insurers and healthcare providers. Anyway, it does not contribute to the quality of healthcare. Taking the contract signed with the AOK Bavaria and comprising the integrated care of patients with prostate carcinoma, prostate adenoma and prostatitis as an example the contents and consequences of this agreement will be described. Advantages and disadvantages for the parties involved are explained. Integrated care m

An interdisciplinary mixed-methods systematic review of multidimensional factors shaping the outpatient waiting experience.

BACKGROUND: Patient satisfaction is a central indicator of healthcare quality and a key outcome in outpatient care. The waiting room, as the first point of contact, strongly shapes patients' comfort, expectations, and perceptions of service quality. Yet most existing studies remain confined within single disciplinary domains rather than integrated relationships among them. This lack of cross-disciplinary synthesis limits theoretical understanding and practical guidance. A systematic review is therefore needed to develop a comprehensive framework for improving outpatient experiences and support

Original Research: Is Your Outpatient Office Prepared to Manage an Opioid Overdose?

BACKGROUND: Outpatient facilities, such as family and adult practice offices, psychiatric offices, and substance use treatment centers, should be equipped to manage medical emergencies and facilitate hospital transfers. Clinics that treat patients with opioid use disorder must be especially prepared to address respiratory arrest due to opioid overdose. PURPOSE: The objective of this integrative review was to identify emergency response initiatives already investigated or developed that could be adapted to address opioid-related medical emergencies in the outpatient adult treatment setting.
