Association of Multimorbidity in Children Undergoing Ambulatory Surgical Procedures With Same-Day Admissions.

BACKGROUND: Chronic health conditions affect approximately 27% of children in the United States and about 1 in 15 has multimorbidity (MM), defined as the coexistence of two or more chronic conditions. Despite its prevalence, the effect of MM on outcomes after ambulatory surgery remains poorly understood. Hospital admission following a scheduled outpatient procedure is an important measure of perioperative quality and safety. We therefore aimed to estimate the prevalence of MM among children scheduled for elective ambulatory surgery and to evaluate its association with same-day hospital admissi

A Comparative Study between Hip Arthroscopy Procedures Performed in Hospitals and Ambulatory Surgical Centers.

PURPOSE: Hip arthroscopy is commonly performed on an outpatient basis; however, many are still performed in hospital operating rooms (HOR) over ambulatory surgery centers (ASC). Therefore, this study aimed to compare patient demographics and complications between hip arthroscopies performed in HOR and ASC. MATERIALS AND METHODS: This was a retrospective cohort study (level III evidence) of 832 patients who underwent hip arthroscopy between 2014-2022 at a multi-hospital academic health system. Nine hundred four hip arthroscopies were performed, 72 of which were staged bilateral. Demographics,

An ambulatory surgical procedure under local anesthesia for treatment of female urinary incontinence.

The object was to study prospectively the results of a modified intravaginal slingplasty for the surgical treatment of female stress incontinence, carried out under local anesthesia as a day procedure. Seventy five patients with genuine stress incontinence were operated upon and followed for a 2-year period. All patients were diagnosed urodynamically to have genuine stress incontinence. Pad tests and quality of life assessments were carried out in all patients both pre- and postoperatively. There were no intra- or postoperative complications and 63 patients (84%) were completely cured througho

Intravaginal slingplasty (IVS): an ambulatory surgical procedure for treatment of female urinary incontinence.

A new ambulatory procedure for treatment of female urinary incontinence (intravaginal slingplasty, IVS) was performed on 50 patients. In all patients the surgical procedure was carried out under local anaesthesia and without postoperative urinary catheterization. The technique has been elaborated from previous experimental and clinical studies and aims at restoration of the pubourethral ligament and the suburethral vaginal hammock. Thirty-eight patients suffered from genuine stress incontinence as objectively verified and 12 patients had symptoms and signs of both urge and stress incontinence.

HRS/ACC scientific statement: Guiding principles on the performance of intracardiac ablation procedures in ambulatory surgical centers.

Ambulatory surgical centers (ASC) in the United States have emerged as an innovative model in health care delivery, potentially serving as a cost-effective, patient-centric alternative to traditional hospital-based care. Supported by Centers for Medicare & Medicaid Services policy expansions, ASCs accommodate procedures across specialties from orthopedics to interventional cardiology. By leveraging technological/procedural advancements and same-day discharge protocols, appropriately selected procedures in the field of cardiac electrophysiology (such as intracardiac ablation procedures and card

Outcomes and Safety Among Patients With Obstructive Sleep Apnea Undergoing Cancer Surgery Procedures in a Freestanding Ambulatory Surgical Facility.

BACKGROUND: Patients with obstructive sleep apnea (OSA) may be at increased risk for serious perioperative complications. The suitability of ambulatory surgery for patients with OSA remains controversial, and several national guidelines call for more evidence that assesses clinically significant outcomes. In this study, we investigate the association between OSA status (STOP-BANG risk, or previously diagnosed) and short-term outcomes and safety for patients undergoing cancer surgery at a freestanding ambulatory surgery facility. METHODS: We conducted a retrospective analysis of all patients h

Less is More: A Cost and Environmental Waste Analysis in Nonadherence to Antibiotic Prophylaxis Guidelines in Clean Ambulatory Surgical Procedures.

BackgroundAntibiotic misuse extends beyond clinical outcomes, imposing significant financial, operational, and environmental burdens on health care systems. Despite guidelines advising against antibiotic prophylaxis for elective low-risk laparoscopic operations, clean head and neck, and soft tissue procedures, many surgeons continue to use them. We performed a cost and environmental waste analysis at our institution resulting from nonadherence to guidelines.MethodsA retrospective chart review was performed at a single institution between January 2022 and March 2023. Development of an appropriate list of surgical procedures of a specified maximum anesthetic complexity to be performed at a new ambulatory surgery facility.

UNLABELLED: A common but difficult task for a hospital when it decides to open a freestanding ambulatory surgery facility is how to decide which surgical procedures should be done at the new facility. This is necessary in order to determine how many operating rooms to plan for the new facility and which ancillary services are needed on-site. In this case study, we describe a novel methodology that we used to develop a comprehensive list of procedures to be done at a new ambulatory facility. The level of anesthetic complexity of a procedure was defined by its number of ASA Relative Value Guide

Anomalous origin of the left coronary artery: discovery during an ambulatory surgical procedure in a 3-month old, previously healthy infant.

We report a rare coronary anomaly in a 3-month old symptomless infant with an anomalous origin of the left coronary artery, that became manifest during anaesthesia for an ambulatory procedure, leading to circulatory arrest. Precordial stethoscope and pulse oximeter unequivocally showed a circulatory arrest. Even in ambulatory procedures, a presumed healthy patient may quickly develop a life-threatening condition because of occult disease. The successful management of such cases depends on efficient and coordinated teamwork.

[Contribution of ambulatory digestive surgery].

In France, ambulatory surgery is controlled by specific regulations which outline the organization of the facilities. It is practiced less in France than in other countries, but specific governmental incentive policies have been instituted. The characteristic feature of digestive surgery is the high occurrence of post-operative nausea and vomiting due to the peritoneal incision. New surgical procedures and anesthetic regimens allow ambulatory care in children and adults. But the choice of ambulatory care is based on the patient's characteristics more than on surgical procedure and follows well
