[Pleural amyloidosis attesting to generalised amyloidosis].

During the course of generalised amyloidosis, pulmonary diseases are mainly represented by tracheobronchial involvement and diffuse or nodular parenchymal localizations. The authors report the case of a 61-year-old woman presenting exudative pleural effusion. The diagnosis of generalised amyloidosis in association with multiple myeloma with cardiac and renal involvement was established. A thoracoscopy performed during the investigation of the pleural effusion and the biopsy revealed amyloid deposits in the pleura. The patient died although melphalan/prednisone treatment was initiated. Pleural

Amyloidosis.

Amyloidosis is an uncommon plasma-cell dyscrasia with an incidence of eight patients per million per year. It is often difficult to recognize because of the myriad symptoms and vague nature of the clinical presentation. Symptoms include fatigue, dyspnea, edema, paresthesias, and weight loss. Clinical syndromes at presentation include nephrotic-range proteinuria with or without renal insufficiency, cardiomyopathy, hepatomegaly, symptomatic peripheral neuropathy, and autonomic failure. Recent advances have occurred in evaluation of patients by using the free light chain assay and new prognostic

[Amyloidosis].

Amyloidosis are rare protein misfolding and deposition diseases which, with very few exceptions, are treatable easily nowadays. The prognosis depends on the form of amyloidosis, which is particularly unfavorable for heart involvement that is diagnosed too late. Patients die within months to a few years or suffer irreversible loss of function of the affected organs. Once the diagnosis has been made, treatment should be started without delay. Amyloidosis centers offer support in the diagnosis and development as well as clinical trials of an optimal therapy concept.

[Amyloidosis].

The term amyloidosis refers to the extracellular deposition of fibrils composed of different types of plasma proteins. Various clinical symptoms are caused by the tissue damage related to the deposited fibrillary material. Except of the brain, all organs can be affected: kidney, liver, spleen, lung, gastrointestinal tract, endocrine organs, skin, heart and autonomous nervous system. Diagnosis is confirmed by specific histological methods (congo red stain, polarized and electron microscopy, immunohistochemistry) and genetic testing. Scintigraphy with radioisotope labeled serum amyloid P-compone

Amyloidosis.

Amyloidosis is not a single disease but a series of diseases in which there is extracellular deposition of a protein which, although it may be derived from different and unrelated sources, folds into a beta pleated sheet. There have recently been significant advances in elucidating the pathogenesis and in the treatment of this group of disorders. By identifying the source of precursor protein, treatment is aimed at eliminating or reducing the extent of deposition and is tailored for each patient. Early diagnosis is required for the optimal effect of treatment on patient survival and quality of

Amyloidosis.

Amyloidosis is considered rare but has an incidence similar to that of Hodgkin's disease and chronic granulocytic leukemia. The diagnosis should be considered in any patient with unexplained nephrotic-range proteinuria, heart failure, peripheral neuropathy, or hepatomegaly. If a monoclonal protein is found in a patient with any of these clinical presentations, a biopsy should be performed and the specimen stained with Congo red. The simplest source of diagnostic material is subcutaneous fat tissue. Treatment usually consists of chemotherapy, which may be oral and low dose or high dose with ste

Peritoneal amyloidosis: unusual localization of gastrointestinal amyloidosis.

Amyloidosis is a rare disease that results from the extracellular deposition of amorphous fibrillar protein. It is usually observed in a systemic form. Gastrointestinal involvement is frequent but peritoneal localization is unusual. A 43-year-old male was investigated for nephritic colic. Morphologic explorations revealed small intestine agglomerans in the periumbilical region, infiltration of peritoneal fat and multiple coelio-mesenteric lymph nodes. There were no clinical or biological abnormalities and endoscopic examinations were normal. The patient then underwent an exploratory laparoscop

Portuguese-type amyloidosis (transthyretin amyloidosis, ATTR V30M).

Portuguese-type amyloidosis (transthyretin amyloidosis, ATTR V30M) is the most common form of systemic hereditary amyloidosis, inherited in autosomal dominant mode. The disease, also called familial amyloid polyneuropathy type I (FAP-I), is caused by a mutant transthyretin (TTR) protein, which is synthesized by the liver. A single amino acid substitution of methionine for valine at position 30 of the TTR molecule (TTR V30M) was found in Portuguese patients. The clinical disease usually manifests as a peripheral sensory, motor and autonomic neuropathy starting in the 3rd or 4th decade of life.

Elastosis Amyloidosis as a Histologic Pattern in Cutaneous Amyloidosis.

Elastosis amyloidosis is an exceptionally rare form of cutaneous amyloidosis characterized by amyloid deposition around dermal elastic fibers. To date, only 11 cases have been documented in the literature, some of them with associated systemic involvement. We report a case of a 60-year-old man presenting with an asymptomatic umbilical nodule. Histopathological analysis revealed amyloid deposits in the upper reticular dermis with characteristic distribution around the elastic fibers, confirmed by positive staining with PAS and Congo red. Immunohistochemical staining was positive for cytokeratin

Autonomic involvement in hereditary transthyretin amyloidosis (hATTR amyloidosis).

PURPOSE: Hereditary transthyretin amyloidosis (hATTR amyloidosis) is a progressive disease primarily characterized by adult-onset sensory, motor, and autonomic neuropathy. In this article, we discuss the pathophysiology and principal findings of autonomic neuropathy in hATTR amyloidosis, the most common methods of assessment and progression, and its relation as a predictive risk factor or a measure of progression in the natural history of the disease. METHODS: A literature search was performed using the terms "autonomic neuropathy," "dysautonomia," and "autonomic symptoms" in patients with he

Amyloidosis in subcutaneous heroin abusers ("skin poppers' amyloidosis").

Systemic amyloidosis has recently emerged as a major cause of nephropathy among heroin abusers in New York City. Although focal glomerulosclerosis is typically seen in intravenous drug abusers who present with the nephrotic syndrome, those who escape this complication are at risk for the later development of amyloidosis related to their use of the subcutaneous route. Twenty such addicts identified between 1981 and 1984 are described. Patients typically present with chronic suppurative skin infections, edema, the nephrotic syndrome, benign urinary sediment, and normal-sized or enlarged kidneys.

[AL Amyloidosis].

AL amyloidosis is a systemic disease caused by misfolded immunoglobulin light chains deposited as amyloid fibrils in multiple organs. Peripheral neuropathy, which often presents with sensory dissociation and autonomic dysfunction, is a key initial manifestation that can occasionally be misdiagnosed as a chronic inflammatory demyelinating polyneuropathy. This review outlines clinical features and diagnostic strategies, including electrophysiological studies, nerve biopsy findings, and recent advances in treatment. Particular focus was given to the pathological mechanisms underlying neuropathy i

Pulmonary Amyloidosis.

Amyloidosis is a heterogeneous group of rare diseases characterized by the deposition of misfolded and insoluble proteins in tissues. Lung involvement, airways or parenchyma, is relatively common, but usually mild. Some patients may develop chronic progressive forms or experience life-threatening complications. In this review, we summarize the most recent advances in the comprehension of molecular mechanisms and classification of amyloidosis. We then illustrate the different forms of lung amyloidosis and the current treatment options. Increased awareness in the medical community and the creati

Cardiac amyloidosis.

BACKGROUND: Cardiac amyloidosis is a rare condition often recognised as restrictive cardiomyopathy caused by extracellular accumulation of proteins in the myocardium. A more seldom form is amyloidoma, a solitary, localised tumour-like deposition of amyloid. CASE PRESENTATION: A man in his forties presented with lethargy, chest discomfort and fever over the preceding three weeks. Transoesophageal echocardiography (TEE) revealed an intracardiac tumour causing severe obstruction and secondary increased diastolic pressure gradient between the left atrium and ventricle. Emergency surgery revealed

Cardiac Amyloidosis.

Amyloidosis is a pathologic and clinical condition resulting from the accumulation of insoluble aggregates of misfolded proteins in tissues. Extracellular deposition of amyloid fibrils in the myocardium leads to cardiac amyloidosis, which is often overlooked as a cause of diastolic heart failure. Although cardiac amyloidosis was previously believed to have a poor prognosis, recent advances in diagnosis and treatment have emphasized the importance of early recognition and changed management of this condition. This article provides an overview of cardiac amyloidosis and summarizes current screen
