The double anaerobic threshold in heart failure.

BACKGROUND: Cardiopulmonary exercise test (CPET) has an important role in assessing heart failure (HF) patients. Among CPET parameters, a pivotal role is attributed to the anaerobic threshold (AT), normally determined by V-slope, ventilatory equivalent and end-tidal methods. In about 10% of healthy subjects, a lack of concordance between these methods has been reported. This event was named double AT (DT). We hypothesized that DT was due to a delay in chemoreflex response. METHODS: We reanalyzed CPET data of two cross-over studies in which we compared CPET in stable HF patients treated for tw

Aerobic threshold, anaerobic threshold, and maximal oxygen uptake of Japanese speed-skaters.

The purpose of this study was to investigate the physiologic and metabolic parameters of speed-skaters with different training regimes and performance level and examine some physiologic prerequisites for speed-skating. The subjects were 25 male speed-skaters including members of the 1984 Japanese National Speed Skating Team whose ages ranged from 19 to 25 years. Aerobic threshold (AerT), anaerobic threshold (AnT), and VO2max were determined during a progressive bicycle ergometer exercise. The power was increased by 12.25 W every 3 min to exhaustion. AerT was determined using gas exchange varia

Effort syndrome: hyperventilation and reduction of anaerobic threshold.

Effort syndrome is an entity in danger of being subsumed into "chronic fatigue syndrome" and lost to sight. Its distinctive feature is the reduction of the anaerobic threshold for work by depletion of the body's alkaline buffering systems through hyperventilation. This article describes the history and clinical features of effort syndrome and reports a study in which capnography is used to identify the anaerobic threshold by registering the respiratory response to the onset of metabolic acidosis. The patients' thresholds are low, and provide a goal for rehabilitation. In other forms of chronic

[Relationship between anaerobic threshold and breathlessness during exercise].

We studied the possibility that during incremental exercise anaerobic threshold (AT) is associated with an increase in breathlessness in subjects with increased impedance or respiratory system. Six patients with chronic lung diseases (CLD) and six healthy subjects performed cycle ergometer exercise under two conditions; with free resistive loading and with 9 cmH2O/l/sec of inspiratory resistive loading (IRL). Ventilation, respiratory pattern, VO2, VCO2, were measured every 15 sec by a metabolic cart, Minato Respiromonitor RM 300. AT was determined by V-slope method. Mouth occlusion pressure (P

Time-based V-slope for determination of anaerobic threshold.

BACKGROUND: The standard method for determining the noninvasive anaerobic threshold (AT) is the V-slope (VS) method. It is often supplemented by the ventilatory equivalent (VEQ) and end-tidal (ET) methods. However, a comparison issue arises: VS plots VCO2 against VO2 (x-axis), whereas VEQ and ET conventionally plot each variable against elapsed exercise time. This study aimed to develop a time-based VS graph to align all three methods on the same x-axis. METHODS: V-slope is drawn as a square with equal sides, with the diagonal line representing the respiratory exchange ratio (R) = 1. V-slope

Evidence of a double anaerobic threshold in healthy subjects.

AIMS: The anaerobic threshold (AT) is an important cardiopulmonary exercise test (CPET) parameter both in healthy and in patients. It is normally determined with three approaches: V-slope method, ventilatory equivalent method, and end-tidal method. The finding of different AT values with these methods is only anecdotic. We defined the presence of a double threshold (DT) when a ΔVO2 > 15 mL/min was observed between the V-slope method (met AT) and the other two methods (vent AT). The aim was to identify whether there is a DT in healthy subjects. METHODS AND RESULTS: We retrospectively analysed

[Anaerobic threshold and oxygen uptake in patients with mitral stenosis].

The exercise stress test with a bicycle ergometer was performed for 31 patients with mitral stenosis and for 10 normal subjects. The patients were categorized in two groups. Group 1 consisted of 16 patients having indication for surgical intervention and Group 2 consisted of 15 patients without such surgical indications. Oxygen uptakes at the anaerobic threshold and at peak exercise (MAX) were assessed by percent attainment of the predicted normal value from Posner's equation. Heart rates during exercise did not differ between the two groups. However, Group 1 had significantly smaller values o

Exercise-entrained breathing and non-invasive determination of anaerobic threshold.

Incremental cycle exercise was performed by 17 subjects under two breathing conditions: spontaneous breathing (Non-ENT run) and deliberate entrainment of breathing frequency (f) to cycling rate at preferred coupling ratios up to possibly high work rates (ENT run). The purpose was to investigate the influence of entrainment on two non-invasive determinations of anaerobic threshold (AT): the break points of VE/VO2 (VAT) and VCO2 (V-slope AT) changes against VO2. Compared to the Non-ENT run, VAT in the ENT run was greater in 12 subjects entrained at higher f accompanied by relative hyperventilati

Plasma osmolality, volume, and renin activity at the "anaerobic threshold".

Plasma renin activity (PRA), volume (PV), osmolality, and hemodynamic parameters were examined in relation to the anaerobic threshold (AT) during progressive cycle ergometry (PE) and repetitive bouts of unilateral isokinetic knee extension-flexion (LE) at 50% maximum voluntary contractions in eight normotensive males. During PE, the observed rise in PRA paralleled that of lactate with abrupt increases occurring at the AT. Correlation of % delta lactate (La), % delta osmolality, and % delta PV with % delta PRA were r = 0.65, 0.36, and -0.51, respectively (all P less than 0.01). In addition, whe

Dangerous curves. A perspective on exercise, lactate, and the anaerobic threshold.

A number of general observations can be made from these recent studies. Lactate is a ubiquitous substance that is produced and removed from the body at all times, even at rest, both with and without the availability of oxygen. It is now recognized that lactate accumulates in the blood for several reasons, not just the fact that oxygen supply to the muscle is inadequate. Lactate production and removal is a continuous process; it is a change in the rate of one or the other that determines the blood lactate level. Rather than a specific threshold, there is most likely a period of time during whic

Lactate accumulation relative to the anaerobic and respiratory compensation thresholds.

Anaerobic thresholds of five male subjects were determined invasively (ATi), from a marked increase in plasma lactate above resting levels (delta La), and noninvasively (ATn), from a nonlinear increase in minute ventilation (VE) during incremental work (IW) leg cycling tests; work rate was increased 30 W every 2 min. Each subject also performed four constant-load work (CLW) tasks just above and just below their ATn and respiratory compensation threshold (RCT), i.e., the point expressed as O2 consumption (VO2) or work rate, at which VE increases disproportionally to CO2 output during IW. Pick Your Threshold: A Comparison Among Different Methods of Anaerobic Threshold Evaluation in Heart Failure Prognostic Assessment.

BACKGROUND: In clinical practice, anaerobic threshold (AT) is used to guide training and rehabilitation programs, to define risk of major thoracic or abdominal surgery, and to assess prognosis in heart failure (HF). AT of oxygen uptake (V.O2; V.O2AT) has been reported as an absolute value (V.O2ATabs), as a percentage of predicted peak V.O2 (V.O2AT%peak_pred), or as a percentage of observed peak V.O2 (V.O2AT%peak_obs). A direct comparison of the prognostic power among these different ways to report AT is missing. RESEARCH QUESTION: What is the prognostic power of these different ways to report

Oxygen Consumption at Anaerobic Threshold Predicts Cardiac Events After Heart Transplantation.

OBJECTIVES: The ventilatory efficiency and functional capacity measured by the cardiopulmonary exercise test (CPET) have been used as important prognostic variables in congestive heart failure. This study sought to identify whether these predictors before heart transplantation (HTX) play a key role in predicting adverse events in patients with heart failure after HTX. METHODS: This was a retrospective cohort study design. HTX recipients were included for analysis. Ventilation to carbon dioxide production slope (VE/VCO2 slope) and oxygen consumption (VO2) during exercise were collected by CPET

[The merit of prescribed exercise using anaerobic threshold for myocardial infarction].

The merit of using anaerobic threshold (AT) for prescribed exercise was evaluated in 55 out-patients with old myocardial infarction (OMI) one to 3 months after the attacks. Comparisons of optimum exercise intensities using 2 methods; either heart rate at AT (ATHR) or heart rate by Karvonen's method (KHR), were conducted. Treadmill stress test was performed in all subjects. ATHR was determined by ramp protocol and KHR by Bruce's protocol. Items measured were AT, peak oxygen consumption (peak VO2), ATHR and KHR. The results were as follows: 1. Mean peak VO2 and AT in patients with OMI were 25.8

Critical analysis of the "anaerobic threshold" during exercise at constant workloads.

The method described by Wasserman for anaerobic threshold (AT) determination, based on the recording of ventilatory parameters, was supported by the simultaneous appearance of hyperlactacidemia and hyperventilation during a standardized incremental work test. Our study aimed at testing the AT in another profile of exercise, viz., during exercises at constant workloads. A homogenous population of 66 healthy subjects performed on a treadmill a total of 100 exercises of 20 min duration at constant workloads (43, 48, 52, 57, 63, and 71% VO2 max). The VO2, V, and venous plasma lactic acid (LA) were

Impact of physical function on indeterminable anaerobic threshold in patients with heart failure.

BACKGROUND: Anaerobic threshold (AT) during cardiopulmonary exercise testing (CPET) is not always determinable in patients with heart failure (HF). However, little is known about the clinical features of patients with HF who have indeterminable AT. Therefore, the present study aimed to clarify the clinical features of such patients. METHODS: A total of 70 patients with HF (58 males; age: 68±12 years) who underwent CPET during hospitalization were divided into two groups: determinable AT (n=50) and indeterminable AT (n=20). Physical function, echocardiographic results, and laboratory findings

Assessing Aerobic and Anaerobic Thresholds with Emphasis on Isocapnic Buffering in Endurance Runners.

Accurate determination of metabolic thresholds is essential for designing effective endurance training. This study aimed to apply a multi-visit Step-Ramp-Step (SRS) protocol to identify aerobic (VT1) and anaerobic (VT2/RCP) thresholds in trained endurance runners, with a particular focus on delineating the isocapnic buffering region-the ventilatory phase between VT1 and VT2 where carbon dioxide (PetCO2) remains stable despite rising ventilation. Twelve trained male runners (mean age: 27.1 ± 1.9 years; VO2max: 60.5 ± 2.1 ml·kg-1·min-1) completed the SRS protocol across separate lab visits. Each

Assessment of the chronotropic response at the anaerobic threshold: an objective measure of chronotropic function.

The evaluation of the heart rate response to exercise is important for the diagnosis of chronotropic incompetence and the assessment of a rate responsive algorithm of sensor-controlled pacemakers. The aim of the present study was to examine a classification of the chronotropic response at an individually moderate exercise level. Sixteen pacemaker patients (patient group, age 62.9 +/- 7.6 years) with sick sinus syndrome and 15 age-matched healthy subjects (control group, age 57.6 +/- 9.4 years) underwent a maximum cardiopulmonary exercise test on a treadmill after a protocol with individually s
