Androgen insensitivity syndrome.

Androgen insensitivity syndrome (AIS) is a disorder caused by a mutation of the gene encoding the androgen receptor (AR; Xq11-q12). The prevalence of AIS has been estimated to be one case in every 20,000 to 64,000 newborn males for the complete syndrome (CAIS), and the prevalence is unknown for the partial syndrome (PAIS). The symptoms range from phenotypically normal males with impaired spermatogenesis to phenotypically normal women with primary amenorrhea. Various forms of ambiguous genitalia have been observed at birth. The diagnosis is confirmed by determining the exact mutation in the AR

Androgen insensitivity syndrome.

The androgen insensitivity syndromes (AIS) fall within the generic category of 46,XY DSD (disorder of sex development) and present as phenotypes associated with complete or partial resistance to the action of androgens. Three categories are recognized: complete androgen insensitivity syndrome (CAIS), partial androgen insensitivity syndrome (PAIS), mild androgen insensitivity syndrome (MAIS). The androgen receptor (AR) is encoded by an 8 exon gene on the X chromosome long arm. More than 800 mutations in the AR gene have been reported in AIS patients (www.androgendb.mcgill.ca/). They are distrib

Androgen insensitivity syndrome.

In a relatively short period of time, understanding of the fundamental causes of androgen insensitivity syndromes has improved dramatically. This has been brought about by the combination of several disciplines, including endocrinology, genetics, developmental and molecular biology. Mutations can be identified in the androgen receptor gene in suspected cases of AIS, and their functional consequences examined in various in-vitro systems. This information can then be correlated with the clinical presentation of the patient, and is beginning to provide an explanation for the highly variable clini

Androgen insensitivity syndrome.

Androgen insensitivity syndrome in its complete form is a disorder of hormone resistance characterised by a female phenotype in an individual with an XY karyotype and testes producing age-appropriate normal concentrations of androgens. Pathogenesis is the result of mutations in the X-linked androgen receptor gene, which encodes for the ligand-activated androgen receptor--a transcription factor and member of the nuclear receptor superfamily. This Seminar describes the clinical manifestations of androgen insensitivity syndrome from infancy to adulthood, reviews the mechanism of androgen action,

[Partial androgen insensitivity syndrome].

There were studied 13 individuals with partial androgen insensitivity syndrome (PAIS) and we observed hypertrophy of clitoris in 92.3%, female pubic hair in 69.2%, complete absence of vagina in 69.2% and normal female development of breasts in 38.5%. In the partial form of AIS more frequent than in the complete form surgery for inguinal hernia, for hypertrophy of clitoris and for artificial vagina was necessary. The discussed form of this male hermaphroditism is caused by rare and untypical mutations in the androgen receptor gene and a differential diagnosis must be performed by exclusion abov

[Complete androgen insensitivity syndrome].

SUMMARY INTRODUCTION: Androgen insensitivity syndrome (AIS) belongs to disorders of sex development, resulting from complete or partial resistance to the biological actions of androgens in persons who are genetically males (XY) with normally developed testes and age-appropriate for males of serum testosterone concentration. CASE OUTLINE: A 21-year-old female patient was admitted at our Clinic further evaluation and treatment of testicular feminization syndrome, which was diagnosed at the age of 16 years. The patient had never menstruated. On physical examination, her external genitalia and bre

[Androgen insensitivity syndromes].

The syndromes of androgen resistance illustrate a special field of endocrinology, that is pathology of target-tissues. These syndromes are responsible for male pseudohermaphroditism and provoke in XY subjects, with a normal testicular androgen secretion, abnormalities in the phenotype with all the possibilities from an "idealistically" female phenotype in the testicular feminization syndrome to an almost normal male phenotype in the mildest forms of the incomplete syndrome. Understanding of these syndromes has strongly benefited of progress in biochemistry and molecular biology concerning the

Androgen insensitivity syndrome: a review.

PURPOSE: Androgen insensitivity syndrome (AIS) is a disorder characterized by peripheral androgen resistance due to androgen receptor mutations in subjects with 46 XY karyotype. The severity of hormone resistance (complete, partial or mild) determines the wide spectrum of phenotypes. METHODS: We performed a literature review on Pubmed focusing on etiopathogenesis, molecular alterations, and diagnostic-therapeutic management. RESULTS: AIS is determined by a large variety of X-linked mutations that account for the wide phenotypic spectrum of subjects; it represents one of the most frequent dis

MRI analysis of androgen insensitivity syndrome.

BACKGROUND: Androgen insensitivity syndrome (AIS) is a rare disorder of sex development, and accurate preoperative imaging assessment is crucial for clinical decision-making. This study aimed to describe the magnetic resonance imaging (MRI) features of AIS, correlate imaging findings with surgical and pathological results, and evaluate the preoperative value of MRI in AIS. METHODS: This retrospective study analyzed the clinical data and preoperative MRI images of 12 patients with androgen insensitivity syndrome diagnosed through chromosomal karyotyping and confirmed by surgical exploration at

The challenges of androgen insensitivity syndrome.

Androgen insensitivity syndrome (AIS) is an X-linked recessive genetic syndrome that occurs as result of an androgen receptor mutation; it affects the normal masculinization process in chromosomal male patients. More than 900 androgen receptor mutations that can lead to AIS have been identified. The complete androgen insensitivity is characterized by a total lack of response to androgens, usually in patients with 46XY karyotype but with feminine phenotype. Primary amenorrhoea and inguinal swellings in female patients are the main signs that could raise suspicion for this syndrome. Patients wit

Androgen insensitivity syndrome: A case report.

RATIONALE: 46, XY disorder of sex development (46, XY DSD) is relatively rare and frequently missed in clinical practice, necessitating early standardized diagnosis and treatment. Androgen insensitivity syndrome (AIS), a major subtype, should be considered in phenotypic females with suggestive clinical clues. PATIENT CONCERNS: A 29-year-old phenotypic female presented with primary amenorrhea and intermittent abdominal pain. Examination revealed normal breast development, sparse pubic hair, and female external genitalia. Pelvic ultrasonography and CT showed an absent uterus and ovaries, with b

Androgen insensitivity syndrome: Can cytology help?

Androgen insensitivity syndrome (AIS) is described as a patient's clinical (phenotypical) presentation as a female with male karyotyping. Classically, patients are normal looking females with complaints of primary amenorrhea. The gonads may be found as extra-genital swellings; rarely, the testes may undergo malignant transformation. Thus, gonadectomy is indicated in these patients on attaining puberty. A rare and interesting case of clinically unsuspected AIS in a young female who presented with primary amenorrhea and inguinal swelling is reported. The initial diagnosis was suggested on fine n

Imaging characteristics of androgen insensitivity syndrome.

Androgen insensitivity syndrome (AIS), also known as testicular feminization, is a genetic disorder which leads to lack of response to androgens caused by a defect in the androgen receptor. It is relatively uncommon and is usually diagnosed through clinical symptoms, laboratory findings, physical exam, radiological imaging, and genetic analysis. Our case is a middle-aged woman with complete AIS and demonstrates the importance of the various imaging modalities that are implemented in initially diagnosing and assisting in surgical management.

Androgen insensitivity syndrome: a case report.

This report refers to a rare case of complete androgen insensitivity syndrome that had presented at the age of 35 years with complaint of abdominal mass, primary amenorrhea and infertility to Jimma University Hospital. A well-developed breast with absence of axillary and pubic hair was seen on examination. There was also an abdominal mass arising from the pelvis occupying the hypogastric and right iliac region. Additionally, there was a reducible mass of 5 by 4 c.m at the left inguinal region. She had a normal female external genitalia with blindly ending vagina of about 4 c.m long. Laporotomy

Androgen insensitivity syndrome: Presentation, diagnosis, and management.

Androgen insensitivity syndrome (AIS) is a genetic disorder impacting 46,XY individuals. It occurs in a complete form, producing a 46,XY female with female genitalia but no internal female reproductive organs; a partial form, producing a 46,XY newborn with genitalia intermediate between the classic male and female types; and a mild form, producing a 46,XY male with male genitalia but oligospermia. This review examines the pathophysiology, presentation, diagnosis, and management of AIS. Its goal is to prepare the advanced practice registered nurse to recognize AIS, facilitate its diagnosis, and

Long-term consequences of androgen insensitivity syndrome.

Androgen insensitivity syndrome (AIS) is one of the most common sexual developmental disorders. According to the grade of the remaining androgen receptor (AR) function, AIS is classified as complete (CAIS), partial (PAIS) or mild (MAIS). In CAIS, the prevalence of germ cell tumours is increased compared with the general population. Although patients with CAIS used to undergo gonadectomy before puberty, nowadays a gonadectomy is recommended after spontaneous puberty, and up to 15% of patients retain their gonads. Nevertheless, the risk of germ cell tumour increases gradually after puberty. Annu

Partial androgen insensitivity: the Reifenstein syndrome revisited.

We investigated eight patients with the Reifenstein syndrome to define the hormonal basis for this condition. The patients had normal or elevated concentrations of plasma androgens, normal production rates of testosterone and dihydrotestosterone, elevated serum levels of luteinizing hormone and normal 5alpha-reductase activity in skin fibroblasts. These findings indicate that the syndrome results from defective androgen action rather than from decreased androgen synthesis. The term "partial androgen insensitivity syndrome" describes this condition more accurately than a term based on clinical

Rare Case of Complete Androgen Insensitivity Syndrome.

Androgen insensitivity syndrome is a rare X-linked recessive condition in which patients present a female phenotype. After complete androgen insensitivity syndrome (CAIS) diagnosis, the timing of gonadectomy should be evaluated, considering the risks and benefits of this procedure. This paper reports an uncommon case of complete androgen insensitivity syndrome diagnosed belatedly in an adult patient. Surgical treatment was deemed necessary due to the elevated risk of gonadal malignancy.

Mild Androgen Insensitivity Syndrome: The Current Landscape.

OBJECTIVE: Mild androgen insensitivity syndrome (MAIS) belongs to the androgen insensitivity syndrome (AIS) spectrum, an X-linked genetic disease that is the most common cause of differences in sex development. Unfortunately, AIS studies mainly focus on the partial and complete phenotypes, and the mild phenotype (MAIS) has been barely reported. Our purpose is to explore the MAIS facets, clinical features, and molecular aspects. METHODS: We collected all reported MAIS cases in the medical literature and presented them based on the phenotype and molecular diagnosis. RESULTS: We identified 49 d
