[Aplastic anemia].

Aplastic anemia is a rare disease with a large differential diagnosis, including neoplastic origin as well as congenital bone marrow failure syndromes. Investigations must be quick and precise. Treatment depends on the patient's age and consists of immunosuppression treatment or allogeneic bone marrow transplantation. Because of the risk of progression to other hematological diseases, a close specialized follow-up is recommended.

Acquired aplastic anemia.

In aplastic anemia, hematopoiesis fails: Blood cell counts are extremely low, and the bone marrow appears empty. The pathophysiology of aplastic anemia is now believed to be immune-mediated, with active destruction of blood-forming cells by lymphocytes. The aberrant immune response may be triggered by environmental exposures, such as to chemicals and drugs or viral infections and, perhaps, endogenous antigens generated by genetically altered bone marrow cells. In patients with post-hepatitis aplastic anemia, antibodies to the known hepatitis viruses are absent; the unknown infectious agent may

[Severe aplastic anemia].

Severe aplastic anemia is a rare disorder which is most often fatal when bone marrow transplantation or an immunosuppressive regimen is not applied. The cause of the disease in individual patients is not identifiable by laboratory investigations. The decision for a therapeutic modality is based on patient age and availability of an HLA-identical bone marrow donor. Early application of bone marrow transplantation may cure up to 80% of patients. Many patients may also be rescued with combined, intensive immunosuppression. Each of these treatment modalities, however, require early application, pr

[Somatic and immune profiling of chemotherapy-associated aplastic anemia: a comparison with primary aplastic anemia and cancer without aplastic anemia].

This study aimed to characterize the somatic variant candidate gene profile of patients with chemotherapy-associated aplastic anemia (CAA) and compare it with that of patients with cancer without aplastic anemia (non-AA) and primary aplastic anemia (PAA). This study included 24 patients with CAA diagnosed at Peking Union Medical College Hospital from September 2019 to May 2023 (male-to-female ratio of 3∶5; median age, 60 years). Peripheral blood samples were collected for whole-exome sequencing, and the results were compared with publicly available data of patients with non-AA and PAA. A total

Hepatitis-associated Aplastic Anemia.

Hepatitis-associated aplastic anemia (HAAA) accounts for 4% of autoimmune hepatitis in children. An episode of seronegative autoimmune hepatitis is followed a few days or months later by aplastic anemia or full aplasia. This autoimmune disease could be due to a regulation defect in the immune response to a viral trigger, in a genetically predisposed individual. Other causes of hepatitis or aplastic anemia have to be ruled out. Steroids and azathioprine usually control the liver damage but do not prevent the development of aplastic anemia. Aplastic anemia is treated with either hematopoietic st

Treatment of aplastic anemia.

Survival of patients with aplastic anemia after immunosuppressive therapy with ATG/ALG ranges from 35% to 60%. However, long-term follow-up on these patients has indicated a high frequency of hematologic complications, including PNH, myelodysplasia, ANL, and recurrent aplasia. In contrast to immunosuppressive therapy, allogeneic marrow transplantation results in cure of aplasia. Problems initially limiting the success of HLA-matched allogeneic marrow transplants included graft rejection and complications associated with acute and chronic GVHD. Infusion of donor buffy coat cells along with marr

[Congenital aplastic anemia-Fanconi].

Results of the investigation of 3 children aged from 2 to 7,5 with diagnosed aplastic anemia of Fanconi type are presented. Two children were females and one child was a male. Their illness symptoms appeared at the age of one, three and four respectively. All three patients had similar symptomes: palenes, tiredness, epistaxis, appearance of hemorrhagic syndrome and delayed growth. The following anomalies were also present: small size, microcephaly, mandibular hypoplasia, high palate and malformation of the urinary tract. In one child ductus Botalli persistens was also revealed. The laboratory

[Pharmacotherapy for aplastic anemia].

Immunosuppressive therapy for aplastic anemia in Japan has changed dramatically over the past few years. Combination with eltrombopag has been shown to improve the outcome of immunosuppressive therapy, and in the 2022 edition of the reference guide, anti-thymocyte globulin plus cyclosporine and eltrombopag became the standard treatment for severe cases. This also changed the role of immunosuppressive therapy in young patients. Further improvement in treatment results is expected now that equine-derived anti-thymocyte globulin has been approved and the indication of romiplostim has been expande

Aplastic Anemia, Pediatric Aspects.

Inherited bone marrow failure syndromes (BMFs) comprise at least one-fourth of children with aplastic anemia, and perhaps up to 10% of adults. The most common syndrome is Fanconi's anemia (FA), with more than 1,000 reported cases. FA is autosomal recessive, with birth defects in approximately 75% of patients. It is a DNA repair syndrome, diagnosed by finding chromosomal aberrations in cells treated with clastogenic agents. The major problems in FA are, in order, aplastic anemia, leukemia, and other cancers. There are at least five complementation groups; the gene for Group C has been cloned. C

Special Education: Aplastic Anemia.

WHAT IS HYPOPLASTIC ANEMIA? Aplastic anemia is a hematological disease characterized by pancytopenia and bone marrow hypoplasia. Acquired cases of aplastic anemia are almost all idiopathic and arise from unknown causes. Other cases of aplastic anemia are secondary and are caused by radiation, chemicals or viruses. PATHOPHYSIOLOGY: Aplastic anemia is manifested as a marked reduction in the number of pluripotent hematopoietic stem cells, but why this occurs is still uncertain. Some of the proposed causes include abnormalities of the hematopoietic stem cells, abnormalities in the hematopoietic mi

Aplastic and hypoplastic anemia.

Aplastic anemia is an acquired or constitutional trilineage bone marrow failure with peripheral pancytopenia, evidence of cellular hypoplasia of marrow, and replacement of normal cellular elements by fat. In some cases the marrow may be hypoplastic and the peripheral cytopenia relatively mild. The condition may persist for years and result in acute leukemia. In other cases, hypoplasia may involve only a single cell line. Basic research is directed toward understanding the multitude of cells, with a view toward selective manipulation of the cells themselves, interacting hormones, or cellular re

Aplastic anemia in systemic lupus erythematosus: a distinct presentation of acquired aplastic anemia?

Aplastic anemia is a rare but serious complication of systemic lupus erythematosus (SLE) with an often dramatic and unanticipated onset. The peripheral destruction of formed blood elements, which frequently accompanies the syndrome, may obscure or delay the diagnosis of bone marrow suppression, and the number of published cases may be an underestimate of the actual incidence of the disease. Furthermore, the disease course may differ significantly from other forms of acquired aplastic anemia and seems to carry a more favorable prognosis once effectively diagnosed and treated. In addition, aplas

[Congenital aplastic anemias].

Congenital (constitutional) aplastic anemias in children are relatively rare compared to other diseases but as their outcome is nearly always fatal they are given full attention in pediatric-hematological literature. Although in many patients the number of white cells and thrombocytes as well as of red cells is diminished even in the first phase of the disease (or later in the course of the disease) so that these diseases are really pancytopenias, the term most frequently used to describe these clinical entities is aplastic anemia. The clinical and hematological characteristics of some aplasti

Regional patterns in the incidence of aplastic anemia in Thailand. The Aplastic Anemia Study Group.

The annual incidence of aplastic anemia has been determined in a rigorous and standardized epidemiologic study conducted in Thailand. A total of 374 cases were identified over a period of 3-6 years in three geographically defined and distinct regions of the country; Bangkok, Khonkaen in the northeast, and Songkla in the south. The incidence was 3.9 cases per million persons in Bangkok, 3.0 per million in Songkla, and 5.0 per million in Khonkaen. These rates are as high or higher than in any region of Europe or Israel as reported in the International Agranulocytosis and Aplastic Anemia Study, i

Immunosuppressive therapy for pediatric aplastic anemia: a North American Pediatric Aplastic Anemia Consortium study.

Quality of response to immunosuppressive therapy and long-term outcomes for pediatric severe aplastic anemia remain incompletely characterized. Contemporary evidence to inform treatment of relapsed or refractory severe aplastic anemia for pediatric patients is also limited. The clinical features and outcomes for 314 children treated from 2002 to 2014 with immunosuppressive therapy for acquired severe aplastic anemia were analyzed retrospectively from 25 institutions in the North American Pediatric Aplastic Anemia Consortium. The majority of subjects (n=264) received horse anti-thymocyte globul

Pediatric Hepatitis-Associated Aplastic Anemia.

Hepatitis-associated aplastic anemia (HAAA) is a rare but life-threatening disorder that requires early recognition and intervention. Pediatricians play a critical role in identifying cases where acute hepatitis transitions to severe bone marrow failure. This review presents the diagnostic and therapeutic complexities associated with HAAA, emphasizing the importance of a multidisciplinary approach. Key topics include the pathophysiology of immune-mediated marrow suppression, diagnostic strategies (eg, immunophenotyping, bone marrow biopsy), and management approaches (eg, immunosuppressive ther

[Exclusive diagnosis of aplastic anemia].

As an independent disease, aplastic anemia (AA) has been recognized for more than a century. When AA is diagnosed, other non-AA bone marrow failures should be excluded. It is termed as exclusive diagnosis of AA. The exclusive diagnosis of AA is helplessly based on that there is no parameter by which AA can be sensitively and specifically diagnosed now. So further searching for the meaningful diagnostic parameters of AA should be carried on to establish a direct diagnostic protocol of this disease and make it possible to differentiate it clearly from other bone marrow failure disease such as co

Aplastic anemia: pathophysiology and treatment.

An immune basis for most patients with aplastic anemia (AA) provides a rationale for immunosuppressive therapy (IST), using antithmyocyte globulin and cyclosporine as one therapeutic modality; hematologic response is observed in up to 75% of patients. Recent advances in understanding the pathogenesis of AA have identified defective telomere maintenance as an important explanation for the onset of marrow failure, relapse and clonal evolution after IST, in some patients with AA. The finding of inherited mutations in the telomerase gene complex in patients with apparent acquired AA has important
