Anaphylaxis to general anesthetics.

The incidence of hypersensitivity reactions to anesthetics is estimated 1 in 13,000 anesthetics up to 1 in 3,180. The rate of mortality ranges between 3 and 9%. 90% of reactions appear at anesthesia induction. Cardiovascular collapse and bronchospasm are more frequent in IgE-dependent reactions. The leading causes are neuromuscular blocking agents (50-70% of cases). IgE-dependent reactions are predominant. Previous sensitization by other compounds containing quaternary ions is suspected. Cross-reactions are frequent. Latex allergy is the second cause, followed by antibiotics and beta-lactams i

[Skin tests for general anesthetics].

During a meeting between anaesthetists and allergists, skin tests were presented to show their value and the difficulties to which they give rise when they are weak. General principles, techniques and readings are described first. As usual, it is sensitivity, specificity and negative predictive value that are at the centre of attention. Selected clinical cases are given as examples. The high prevalence of cases who show a latent sensitisation to curare indicates an increase in allergological tests to patients who present risk factors.

[Complications induced by general anesthetics].

We reported 38 observations with adverse reactions during anesthesia. These observations have collected over about one year. For each observation we studied some parameters like sex, aspect of the adverse reaction, number of general anesthesia before the accident, atopy, age etc. Among the parameters, we noticed the great frequency of reaction in the female group (81.5%) and an increased level of total IgE (greater than or equal to 150 Ul/ml) in 38%. For each investigated drug we realized skin test (Prick-test and when negative, intra-dermal test) and Lymphoblastic Transformation Test L.T.T.).

General anesthesia/general anesthetics: Journal of Anesthesia symposium 2025.

Key interventions for anesthesia patients include administering drugs, infusing fluids and blood, and providing artificial ventilation. As general anesthetics are always administered to patients undergoing general anesthesia, anesthetists need to be familiar with the fundamentals and the latest issues of general anesthesia/anesthetics. The Journal of Anesthesia symposium 2025 aimed to discuss fundamental topics and environmental issues related to general anesthesia and anesthetics. In this symposium, we had following topics: (1) patient outcomes of general anesthesia observed outside the opera

[Blood-brain barrier and general anesthetics].

The transport of drugs into the brain is prevented in most cases by the presence of the blood brain barrier (BBB). Endothelial cells in the brain of vertebrates are fused together by tight junctions that eliminate the normal pathways of free diffusion existing in the capillary beds of organs other than the brain or spinal cord. Therefore, a molecule that has a molecular weight less than a 500 Dalton can cross the BBB in proportion to lipid solubility. Recent molecular biological studies have contributed to the isolation and functional analysis of transporter proteins at the BBB. There are thre

Mechanism and Development of Modern General Anesthetics.

BACKGROUND: Before October 1846, surgery and pain were synonymous but not thereafter. Conquering pain must be one of the very few strategies that has potentially affected every human being in the world of all milestones in medicine. METHODS: This review article describes how various general anesthetics were discovered historically and how they work in the brain to induce sedative, hypnosis and immobility. Their advantages and disadvantages will also be discussed. RESULTS: Anesthesia is a relatively young field but is rapidly evolving. Currently used general anesthetics are almost invariably

General intravenous anesthetics - pharmacodynamics, pharmacokinetics and chiral properties.

In continuation of our published review on general inhalational anesthetics, the current article presents a survey of intravenous agents for general anaesthesia. From chemical point of view these compounds belong to structurally diverse categories, such as barbiturates - thiopental (Sodium pentothal®, Trapanal®, Pentothal®), methohexital (Brevital®), and hexobarbital (Evipan®, Hexenal®, Citopan®, Tobinal®); non-barbiturate derivatives - ketamine (Ketalar® Ketaset®), esketamine (Ketanest®), and etomidate (Amidate®, Hypnomidate®), phenolic derivatives - propofol (Diprivan®); steroid derivatives

The Effects of General Anesthetics on Synaptic Transmission.

General anesthetics are a class of drugs that target the central nervous system and are widely used for various medical procedures. General anesthetics produce many behavioral changes required for clinical intervention, including amnesia, hypnosis, analgesia, and immobility; while they may also induce side effects like respiration and cardiovascular depressions. Understanding the mechanism of general anesthesia is essential for the development of selective general anesthetics which can preserve wanted pharmacological actions and exclude the side effects and underlying neural toxicities. Inhibition of learning and memory by general anesthetics.

PURPOSE: Today's general anesthetics were developed empirically according to their ability to produce memory blockade, analgesia, immobility, and unconsciousness. Thus, a major outstanding question remains: How do anesthetics produce their desirable behavioural end points at the molecular level? Understanding the mechanisms underlying memory blockade is of particular importance, because some patients experience the unexpected recall of events during anesthesia while others experience persistent memory deficits in the postoperative period. This review provides a brief summary of the acute memor

General Anesthetics: Aspects of Chirality, Pharmacodynamics, and Pharmacokinetics.

The introduction of general anesthetics in the mid-19th century is considered one of the greatest contributions to medical practice. It was the first time that complicated surgical interventions became feasible, without putting an excessive strain on the patient. The first general anesthetics-diethyl ether, chloroform, and nitrous oxide-were limited by often severe adverse reactions and a narrow therapeutic window. They were later succeeded by modern anesthetics, with high anesthetic effect along with diminished toxicity. As with other medical drugs, many anesthetic compounds contain chiral ce

General inhalational anesthetics - pharmacodynamics, pharmacokinetics and chiral properties.

Since the advent of nitric oxide, diethyl ether, chloroform and cyclopropane, the greatest advancement in the area of general inhalational anesthetics has been achieved by the introduction of fluorinated anesthetics and the relevant chiral techniques. This progress led to marked decrease in mortality rates in anesthesia. In the group of chiral fluorinated compounds, halothane (Fluotan®), isoflurane (Foran®), desflurane (Supran®) and enflurane (Ehran®) are deployed as volatile anesthetics. Chiral anesthetics possess a stereogenic center in their molecules and thus exist as two enantiomers (S)-(

Disruption of Hippocampal Multisynaptic Networks by General Anesthetics.

BACKGROUND: Previous studies showed that synaptic transmission is affected by general anesthetics, but an anesthetic dose response in freely moving animals has not been done. The hippocampus provides a neural network for the evaluation of isoflurane and pentobarbital on multisynaptic transmission that is relevant to memory function. METHODS: Male Long-Evans rats were implanted with multichannel and single electrodes in the hippocampus. Spontaneous local field potentials and evoked field potentials were recorded in freely behaving rats before (baseline) and after various doses of isoflurane (0

Similar destabilization of neural dynamics under different general anesthetics.

Different classes of anesthetics induce unconsciousness despite acting through distinct molecular mechanisms, raising the possibility that their convergent effects arise at the level of the dynamics of neural population activity. To explore this, we analyze intracortical electrophysiological recordings during infusions of propofol, ketamine, and dexmedetomidine, applying a rigorous method to estimate the stability of population dynamics during anesthesia. We find that all three anesthetics, despite their molecular differences, similarly affect cortical states by reducing dynamic stability. The

Malignant hyperthermia: responses of skeletal muscles to general anesthetics.

Anesthetic-induced malignant hyperthermia (MH) originally attracted scientific interest because of the associated high fatality rate. Recently, the introduction of treatment with the muscle relaxant dantrolene sodium has dramatically reduced the frequency of death from MH. Nonetheless, diagnosing MH susceptibility remains a problem because it necessitates a muscle biopsy specimen that must be tested in a specially equipped laboratory. Thus, diagnosis is both expensive and invasive. Development of simpler and less invasive methods would be aided by identification of the primary defect underlyin

[Is fluidization of the phospholipid bilayer by general anesthetics a reason for general anesthesia?].

It was found that chlorpromazine and pipolfene potentiating general anaesthesia increase viscosity of phospholipid membrane in the region of polar headgroups and in the hydrophobic area near glycerol sceleton. These results (independently of potentiating mechanism) confuse a mechanism of anaesthetics action as increased fluidity of membrane phospholipids.

From Bench to Bedside: Understanding General Anesthetics' Impact on PTSD.

Post-traumatic stress disorder (PTSD) is a multifaceted mental health disorder arising from exposure to traumatic events, characterized by persistent and distressing symptoms. Recent years have seen growing recognition of anesthesia's dual role in PTSD. While countless patients receive anesthetics during traumatic medical care, their long-term psychological impacts remain unclear, raising urgent clinical questions. This review provides an in-depth analysis of both clinical and animal studies examining the effects of commonly used inhaled and intravenous anesthetics, such as sevoflurane/isoflur

[Processed EEG for personalized dosing of anesthetics during general anesthesia].

Electroencephalogram (EEG)-guided anesthesia is indispensable in modern operating rooms and has become established as the standard form of monitoring. Many anesthesiologists rely on processed EEG indices in the hope of averting anesthesia-related complications, such as intraoperative awareness, postoperative delirium and other cognitive complications in their patients. This educational review aims to provide information on the five most prevalent monitors used to guide depth of sedation during general anesthesia. This article elucidates the principles underpinning the application of these moni
