Group A Streptococcus-infected Aortic Aneurysm.

A 64-year-old man presented with abdominal bloating and a fever. Initial imaging revealed an abdominal aneurysm suspected to be an infected aortic aneurysm (IAA); therefore, antibiotic treatment was initiated. Blood culture confirmed group A Streptococcus (GAS). Follow-up computed tomography revealed aneurysm expansion, confirming the diagnosis of IAA. The patient survived the emergency surgery. IAA is a potentially fatal disease with a high risk of rupture. IAA is rarely caused by GAS and is a diagnostic challenge owing to non-specific symptoms and imaging findings. We herein review reports o

Infected Aneurysm after Endoscopic Submucosal Dissection.

A 79-year-old man on hemodialysis was hospitalized for further investigation. Early gastric cancer was diagnosed by gastrointestinal endoscopy and endoscopic submucosal dissection (ESD) was performed. Fever and abdominal pain thereafter developed, and a severe inflammatory response was observed on a blood test. Contrast computed tomography (CT) showed ulcer-like projections and soft tissue surrounding the aorta, from the celiac to left renal artery. An infected aneurysm was diagnosed. Although infected aneurysms developing after laparoscopic cholecystectomy or biopsy of contiguous esophageal d

Surgical treatment of infected aortic aneurysm.

BACKGROUND: We report results of infected aortic aneurysms treated by a single group over 20 years. METHODS: Retrospective review. RESULTS: Seventeen patients were treated, 10 with infrarenal and 7 suprarenal infections. All had abdominal/back pain, 88% were febrile, 71% had leukocytosis, and 24% were hemodynamically unstable. The most common responsible organism was Staphylococcus aureus (29%) followed by Salmonella organisms (24%). All suprarenal infections were gram-positive organisms. Infrarenal infections were treated with preliminary axillofemoral bypass followed by aortic resection. S

Brucella-infected abdominal aortic aneurysm: management strategies for an uncommon aneurysm.

OBJECTIVE: The occurrence of Brucella-induced abdominal aortic aneurysms is an exceedingly rare phenomenon, yet it stands as one of the most severe complications within this context. The combined utilization of serological testing and imaging diagnostics has been validated as an effective approach for the identification of Brucella-induced abdominal aortic aneurysms. Presently, the predominant therapeutic strategies encompass antibiotic treatment and surgical intervention. Nonetheless, ongoing controversies persist concerning the establishment of diagnostic criteria, the optimal timing and sel

Infected Coronary Artery Aneurysm Within Chronically Occluded Stents.

BACKGROUND: Infected coronary artery aneurysm (ICAA) is a rare, fatal complication. While usually associated with virulent pathogens, ICAA involving chronic stent occlusion and atypical organisms remains poorly understood. CASE PRESENTATION: An 85-year-old immunocompromised woman receiving sarilumab presented with sepsis of unknown focus. The 12-year-old stents in the right coronary artery had been documented as occluded 3 weeks before admission. Despite therapy for Corynebacterium species bacteremia, the patient died of progressive septic shock. Autopsy identified the cause of death as aorti

Infected Paravisceral Aneurysm Repair with Parallel Stent Grafts.

An infected aortic aneurysm (IAA) is a rare but potentially life-threatening pathology characterized by rapid growth and a substantial risk of rupture compared to non-infected aneurysms. Reports on the endovscular treatment of infected paravisceral aneurysms are limited in the literature. This article describes our experience with endovascular repair of IAAs involving the visceral arteries and includes a literature review. We present two cases of symptomatic IAAs located at the celiac trunk ostium in high-risk surgical patients. Both cases were successfully treated with the parallel stent graf

Endovascular management of ruptured infected popliteal artery aneurysm.

Infected popliteal aneurysm is a rare high-risk condition that can present as an emergency with acute rupture and sepsis. Management of acute ischemia in the presence of local and systemic sepsis is challenging. Open surgery is not always possible and carries a high risk of morbidity and death. An endovascular approach has been advocated in infected aneurysms elsewhere in the body, with good short-term and medium-term outcomes encouraging such approach in the popliteal artery. We report a case of successful endovascular treatment of an infected ruptured popliteal aneurysm with favorable outcom

Surgery for infected aneurysm of the aortic arch.

OBJECTIVE: Infected aneurysm of the aortic arch is rare and can be fatal without surgery. We report our surgical experience with infected aneurysms of the aortic arch. METHOD: We conducted a retrospective chart review. RESULTS: Between 1995 and 2006, 10 patients with infected aneurysms of the aortic arch were treated at our hospital. There were 8 men with a median age of 67.5 years (range, 50-79 years). The most common pathogen was nontyphoid Salmonella in 5 (50%) patients, followed by Staphylococcus aureus in 2 patients. The site of infection was the aortic arch at the level of the brachioc

Infected internal iliac artery aneurysm: a case report.

Isolated internal iliac aneurysms are rare, and although most are of atherosclerotic origin the cause may also be congenital, traumatic, associated with pregnancy or infectious. A 56-year-old man presented with a swollen, painful left lower limb. Within a few days, weakness of the limb developed with fever and an acute abdomen with free air on x-ray. At emergency laparotomy a small perforation was found in the ascending colon. Examination of the left iliac fossa revealed a ruptured left internal iliac artery aneurysm. Extra-anatomic cross-femoral bypass grafting was done to revascularize the l

Infected intracranial aneurysm in an infant: case report.

The case of a ruptured, infected intracranial aneurysm occurring in a 34-day-old child is reported. The child was brought for examination after a sudden onset of screaming and pallor, followed by focal seizures. Blood cultures grew Staphylococcus aureus, and a computed tomographic scan revealed a large hematoma in the region of the left sylvian fissure. An angiogram demonstrated a 17-mm aneurysm arising from a small branch of the middle cerebral artery. At craniotomy, the hematoma was evacuated and an infected aneurysm was removed. The etiology, pathogenesis, and management of infected intracr

Successfully treated infected aneurysm caused by Listeria monocytogenes.

Infected aneurysms caused by Listeria monocytogenes are extremely rare. Therefore, there is no standard procedure for their diagnosis and treatment. A 76-year-old Japanese man with diabetes and hypertension was diagnosed with a left common iliac aneurysm caused by L. monocytogenes, using multidetector computed tomographic angiography and rapid diagnostic testing of the positive blood culture. He was successfully treated with a combination of ampicillin administration, timely surgical debridement, and in-situ Y-graft placement with revascularization and omental implantation. Infected abdominal aortic aneurysm due to Escherichia coli.

Early diagnosis of infected abdominal aortic aneurysm (IAAA) is still a medical challenge due to its diverse and non-specific symptoms and signs. The most common responsible pathogens are Salmonella, Staphylococcus, Campylobacter and Streptococcus species. The authors report the case of a 67-year-old man, admitted for high fever and finally diagnosed with Escherichia coli (E.coli)-related IAAA. The IAAA ruptured during the general anaesthesia induction, leading to an emergency surgery. The authors successfully proceeded to an open aneurysmectomy with extensive debridement and in situ graft rep

[Infected aneurysm of abdominal aorta: early CT finding].

Infected abdominal aortic aneurysm is an uncommon but life-threatening disease, especially in case of salmonella infection. Early CT findings should be well known in order to allow immediate diagnosis and accurate management. The authors present an early CT finding of a salmonella infected aneurysm of abdominal aorta in an HIV-infected patient. This pattern consists in a slight-enhancing focal densification of periaortic soft-tissue, while aorta remains of normal size. Within two weeks, infection progressed to the constitution of an infected aneurysm. This CT finding seems to be initial to pre

Infectious endocarditis complicated with intracranial infected aneurysm rupture and sinus of valsalva aneurysm rupture.

BACKGROUND: Infectious endocarditis (IE) is an infectious disease caused by direct invasion of the heart valve, endocardium, or adjacent large artery endocardium by pathogenic microorganisms. Despite its relatively low incidence, it has a poor prognosis and a high mortality. Intracranial infectious aneurysms (IIA) and ruptured sinus of Valsalva aneurysm (RSVA) are rare complications of IE. CASE PRESENTATION: We report a young male patient with symptoms of respiratory tract infection, heart murmurs and other symptoms and signs. The patient also had kidney function impairment and poor response

An Infected Popliteal Aneurysm after Plain Old Balloon Angioplasty.

The case was a 76-year-old man with chronic limb-threatening ischemia. Plain old balloon angioplasty (POBA) was performed on the popliteal artery. Subsequently, he suffered from cellulitis around the POBA site, followed by reocclusion. Staphylococcus aureus was detected in a blood culture. After re-revascularization with POBA, both purulent gonitis and an infected popliteal aneurysm were observed to occur. We performed aneurysmectomy and bypass grafting with the saphenous vein and then continued antibiotic therapy. Although treatment consisted of endovascular therapy (EVT) with nothing left be
