Noncoronary angioplasty.

PTA is an established method of revascularization in a variety of medical conditions. It is performed for specific morphologic and clinical indications. PTA is the procedure of choice in Fontaine stage IIB through IV lower extremity ischemia due to iliac and/or femoropopliteal stenosis or short occlusion. Its role is less certain in infrapopliteal disease, although current studies have begun to establish long-term effectiveness. PTA is the procedure of choice for renal revascularization in renovascular hypertension due to fibromuscular disease or non-ostial atherosclerosis, selected cases of r

[Angioplasty in unstable chest angina. Angioplasty in unstable angina].

Angioplasty in acute coronary syndromes without ST elevation (angina + non Q wave infarction) is traditionally associated with an increased risk of major cardiac complications compared with angioplasty in stable angina. The recent development of instrumental techniques, in particular the introduction of stenting and pharmacological support for angioplasty have transformed the immediate prognosis in unstable angina. Consequently, angioplasty has become better tolerated and more easily recommended. Angioplasty cannot be considered without active medical therapy, including aspirin, betablockers,

Reporting standards for angioplasty and stent-assisted angioplasty for intracranial atherosclerosis.

BACKGROUND AND PURPOSE: Intracranial cerebral atherosclerosis causes ischemic stroke in a significant number of patients. Technological advances over the past 10 years have enabled endovascular treatment of intracranial atherosclerotic stenosis. The number of patients treated with angioplasty or stent-assisted angioplasty for this condition is increasing. Given the lack of universally accepted definitions, the goal of this document is to provide consensus recommendations for reporting standards, terminology, and written definitions when reporting clinical and radiological evaluation, technique

[Percutaneous transluminal angioplasty].

Percutaneous transluminal angioplasty (PTA) was done in a 58-year-old man with hypertension due to a left-sided renal artery stenosis. Hypertension was cured dramatically and plasma renin activity was within the normal range after the operation. This non-traumatic procedure seems to be excellent and recommendable as a first treatment of renovascular hypertension.

Dissection complicating angioplasty.

Extensive arterial dissection producing significant arterial obstruction or occlusion after technically uncomplicated percutaneous transluminal angioplasty in three patients is described. Despite the initially ominous arteriographic appearance, short-term (4-8 months) follow-up demonstrated complete resolution without surgical intervention. A trial of conservative management is recommended for this complication.

Percutaneous transluminal angioplasty.

Percutaneous transluminal angioplasty is a relatively new procedure; its technology is still evolving and experience with it is increasing rapidly. There are only limited follow-up data on patients who have undergone this technique. Rates of late patency of the dilated arteries and potential long-term complications in patients treated with percutaneous transluminal angioplasty are unknown. Therefore, physicians performing this technique should collect and report long-term follow-up data on their patients, using clinical, hemodynamic, and angiographic determinations. Reduction of thrombogenicity with argon laser angioplasty. Comparison with balloon angioplasty.

Direct argon or thermal laser angioplasty (LA) was evaluated for thrombogenicity using angioscopy, and compared with balloon angioplasty (BA). In each of 8 dogs, 4 segments (both proximal and distal iliac arteries) were treated by laser-thermal and/or balloon angioplasty. One segment was treated by balloon angioplasty and 3 other segments were treated with either thermal LA with 7 W using a "Hot-Tip" laser probe (2.0 mm), or BA and thermal LA, or a special optical probe which emits a 3 W argon laser beam. Mean percent area stenosis by thrombus was 44 +/- 23 in balloon-dilated, 23 +/- 21 in the

In the 59 hypertensive patients submitted to percutaneous transluminal angioplasty (PTA) of the renal artery, there was an immediate success in the blood pressure in 91.5% and a later one of 79.6%. In these patients we obtained better results: 81.4% in the unilateral lesions, more than in the bilateral ones--72.7%; 82.5% in the renal artery trunk lesions, more than in the ostium ones--71.4%; 88.9% in the lesions of fibromuscular origin, more than in the aterosclerotic ones--75%; 84.4% in up to 55 years old patients, more than in older ones--71.4%. These differences were not significant. The re

Reduction of vasoreactivity and thrombogenicity with laser-thermal angioplasty: comparison with balloon angioplasty.

UNLABELLED: The vasoreactivity and thrombogenicity of laser-thermal angioplasty were examined and compared with those of balloon angioplasty in an atherosclerotic rabbit iliac artery. Eight rabbits underwent laser-thermal angioplasty with a 1.7-mm hot-tip probe activated at 7 W with a probe temperature of 126 +/- 19 degrees C in one iliac artery. The other iliac artery was treated with balloon angioplasty using a 2.0-mm balloon. Angiographic luminal diameter increased from 0.19 +/- 0.15 to 1.54 +/- 0.35 mm by laser and from 0.29 +/- 0.22 to 1.84 +/- 0.20 mm by balloon (P less than 0.0001, resp

Percutaneous transluminal mesenteric angioplasty.

The endovascular treatment of chronic mesenteric ischemia remains in its infancy. This state is most certainly related to the small patient population affected by this devastating condition and the surgeon's limited desire to pursue percutaneous options for this group. Surgical trepidation is not unwarranted because in many facets mesenteric angioplasty is similar to percutaneous carotid intervention. Interventions in these vessels represent manipulation of terminal and unduplicated arterial systems. Complications compromising flow to either vascular bed can yield rapid grave outcomes. Despite

Carotid stent-assisted angioplasty.

We present the results of the first 33 patients who underwent carotid stent-assisted angioplasty at the Heart Unit of Panorama Medi-Clinic between March 1995 and June 1996. Intravascular stents (48 Palmaz-Schatz and 1 Wall stent) were deployed in the common carotid arteries (5) and internal carotid arteries (38) of both symptomatic (28 patients) and asymptomatic patients (5 patients) with > 60% stenosis (35 with > 70% and 6 with 60% stenosis). Thirteen patients underwent bilateral stent implantation, and had percutaneous transluminal coronary angioplasty or a peripheral and/or coronary stent i

European carotid angioplasty trial.

The Carotid and Vertebral Artery Transluminal Angioplasty Study (CAVATAS) is an international multicenter randomized trial, the primary aims of which are: (1) to determine the risks and benefits of carotid and vertebral artery transluminal angioplasty; and (2) to compare these with surgical treatment (carotid endarterectomy) or best medical treatment. Through the end of October 1995, 193 patients have been randomized (90 at our center). In this article, the rationale for the trial, its design and protocol, and the experience of our institution within the trial will be discussed.

Restenosis after coronary angioplasty.

The major disadvantage of using percutaneous transluminal coronary angioplasty to treat patients with atherosclerotic coronary disease is the frequent occurrence of restenosis after an initially successful procedure. Studies in animals and histological observations in man have demonstrated that restenosis is characterized by neointimal hyperplasia due to smooth muscle cell proliferation and to the synthesis of extracellular matrix. Improvements in technology or pharmacological interventions have had no significant impact on the rate of restenosis. In spite of our increased understanding of the

[Percutaneous transluminal laser angioplasty].

Percutaneous transluminal laser angioplasty (PTLA) has been carried out on 63 peripheral arterial occlusive lesions in 43 patients. Most patients complained of intermittent claudication. The sites of lesions are the iliac artery (28 lesions or 44.4%), the femoropopliteal artery (30 lesions or 47.6%), and the below knee artery (5 lesions or 8.0%). There are 54 lesions (85.7%) that showed over 70% stenosis, with 19 of them complete occlusions (30.2%). Out of 63 lesions, 16 lesions or 25.4% had occlusions of over 10 cm. Renal angioplasty: current status.

Percutaneous transluminal angioplasty has attracted a great deal of attention in recent years and is being used with increasing frequency for the treatment of renovascular hypertension from renal artery stenosis. One hundred forty dilatation procedures have been performed in 90 patients with 119 renal artery stenoses. Ten of the patients were treated for renal insufficiency alone. An initial success rate of 95% was achieved. The blood pressure response has been followed for 1-52 months (mean, 22 months). The mean diastolic pressure decreased by 36.7 mm Hg in response to the treatment. Of the 8

Percutaneous transluminal renal angioplasty.

One to 2 percent of the 25,000,000 hypertensive patients in the United States have renovascular hypertension. Until recently, the treatment of choice for a patient with an ischemic, renin-producing kidney that caused hypertension involved a major abdominal operation. Percutaneous catheter techniques are now available that permit correction of a renal artery stenosis under fluoroscopic guidance. This percutaneous transluminal angioplasty procedure has received much attention, but to date no long-term follow-up studies are available. Because of the limited life-expectancy of patients with a gene

Standard balloon angioplasty versus angioplasty with paclitaxel-eluting balloons for femoropopliteal artery stenosis.

AIM: The aim of this article was to review the available literature on the use of drug-coated balloons (DCB) for endovascular treatment of femoropopliteal arterial stenosis. METHODS: Manual searches of articles, presentations, and clinical trials were performed. Selected references were reviewed and summarized. RESULTS: Due to the high morbidity associated with femoropopliteal bypass, endovascular approaches, such as balloon angioplasty and stent placement, have become the first line of therapy for isolated, de novo femoral atherosclerosis. However, percutaneous interventions have been limit
