Percutaneous transluminal laser angioplasty (PTLA) has been carried out on 63 peripheral arterial occlusive lesions in 43 patients. Most patients complained of intermittent claudication. The sites of lesions are the iliac artery (28 lesions or 44.4%), the femoropopliteal artery (30 lesions or 47.6%), and the below knee artery (5 lesions or 8.0%). There are 54 lesions (85.7%) that showed over 70% stenosis, with 19 of them complete occlusions (30.2%). Out of 63 lesions, 16 lesions or 25.4% had occlusions of over 10 cm. A Nd:YAG laser (1.06 microns, continuous wave) was used for the vaporization

Recent developments in technology have stimulated interest in the use of catheter-directed laser energy for removal of atherosclerotic plaques. Over the past three years, intensive research efforts by a number of investigators have defined the potential therapeutic benefit of laser angioplasty, as well as its limitations. Unlike balloon angioplasty, the laser has the capacity to remove totally atheromatous plaque or thrombus by photovaporization. Unique properties of the laser allow light energy to be concentrated for plaque removal. Arterial damage adjacent to the plaque is prevented by certa

Laser energy transmitted through fiberoptic systems can recanalize totally occluded peripheral arteries and improve extremity perfusion in selected patients with peripheral vascular disease. Such a technique is obviously appealing in that it (1) reduces the morbidity currently associated with the surgical treatment of symptomatic peripheral atherosclerosis, and (2) allows treatment of patients currently excluded from therapy by the presence of other severe medical problems or relatively mild symptoms. Unfortunately, current delivery systems allow recanalization of only a small channel by laser

The relative safety and thrombogenicity of pulsed excimer and thermal laser angioplasty systems were compared in 20 normal coronary artery segments in a total of seven pigs. Using similar over the wire catheter systems and laser delivery periods of 3 to 5 s, thermal laser angioplasty was achieved with a 1.3 mm metal probe heated with 10 W of continuous argon laser energy and excimer laser angioplasty was performed with a 4.5F excimer laser catheter consisting of 13 concentrically arranged 200 microns fiber optics delivering 35 to 40 mJ/mm2 of xenon chloride (308 nm) excimer laser irradiation a

Endoscopic laser ablation of atheroma using continuous wave lasers is limited by imprecise control of thermal ablation, resulting in a crater that expands in width and depth, with thermal damage to adjacent normal tissue. We compared the gross and histologic effects of pulsed 308 mm excimer irradiation to continuous-wave Nd:YAG and Argon Ion laser irradiation, and pulsed 1,060 nm, 532 nm, 355 nm, and 266 nm laser irradiation in 205 atherosclerotic aortic segments. In contrast to the continuous-wave Nd: YAG, Argon Ion, and pulsed 1,060 nm, 532 nm, and 355 nm laser irradiation, which produced gr

Direct argon or thermal laser angioplasty (LA) was evaluated for thrombogenicity using angioscopy, and compared with balloon angioplasty (BA). In each of 8 dogs, 4 segments (both proximal and distal iliac arteries) were treated by laser-thermal and/or balloon angioplasty. One segment was treated by balloon angioplasty and 3 other segments were treated with either thermal LA with 7 W using a "Hot-Tip" laser probe (2.0 mm), or BA and thermal LA, or a special optical probe which emits a 3 W argon laser beam. Mean percent area stenosis by thrombus was 44 +/- 23 in balloon-dilated, 23 +/- 21 in the

Coronary laser balloon angioplasty (LBA) is a new technique which permits application of heat (generated by the laser source) and pressure (by balloon inflation) to thermally weld tissue during coronary angioplasty (PTCA). The goal of LBA is to achieve a large, smooth arterial lumen, by thermal welding of dissection flaps, elimination of elastic recoil, elimination of vasospasm, reduction in platelet activation, desiccation of thrombus, and inhibition of smooth muscle cell proliferation (Table 1). The LBA system consists of a 50 watt continuous wave Nd:YAG laser with a wavelength of 1060 nm, a

BACKGROUND AND AIMS: We devised a technique to treat peripheral arterial disease (PAD) with laser, i.e. percutaneous transluminal laser angioplasty (PTLA). Considerable good results were obtained with PTLA, but it is apparently considered obsolete as a technique to treat occlusive arterial disease of peripheral arteries, perhaps because of the development and improvement of stents and the ease of their use compared to the somewhat intricate technique required for PTLA. Although the author admits the usefulness of stents, they are foreign to a human body. PTLA does not use a foreign body and co

Laser-assisted angioplasty is rapidly evolving into a promising adjunct to or replacement for standard vascular procedures. A protocol was devised to evaluate the technique in a non-selected, consecutive patient population to define the applications and limitations of the technique. In a 12-month period, 358 lower-limb atherosclerotic lesions were treated with laser/balloon angioplasty [percutaneously (52%) or open (48%)] in 206 consecutive patients. Overall, the laser/balloon technique recanalised 234 lesions (65% laser success), judged clinically effective by a greater than 0.15 improvement

UNLABELLED: The vasoreactivity and thrombogenicity of laser-thermal angioplasty were examined and compared with those of balloon angioplasty in an atherosclerotic rabbit iliac artery. Eight rabbits underwent laser-thermal angioplasty with a 1.7-mm hot-tip probe activated at 7 W with a probe temperature of 126 +/- 19 degrees C in one iliac artery. The other iliac artery was treated with balloon angioplasty using a 2.0-mm balloon. Angiographic luminal diameter increased from 0.19 +/- 0.15 to 1.54 +/- 0.35 mm by laser and from 0.29 +/- 0.22 to 1.84 +/- 0.20 mm by balloon (P less than 0.0001, resp

The ability to ablate atheroma without generating heat makes the excimer laser wavelength a promising intraluminal technique for the treatment of arterial occlusive disease. This series reviews a preliminary experience treating patients with superficial femoral arterial disease admitted with limb-threatening ischemia or claudication. Twenty-six diseased superficial femoral arteries (5 stenotic and 21 occluded) were treated in 23 consecutive patients. Patients with claudication (18) reluctant to undergo bypass or with limb-threatening ischemia (8) at extremely high risk for surgery were include

The effects of balloon angioplasty (BA) and laser-assisted balloon angioplasty (LABA) on arteries were compared. Atherosclerosis was induced in the iliac arteries of New Zealand White rabbits by means of balloon denudation and a diet supplemented with 1% cholesterol and 3% peanut oil. Six weeks later, one iliac artery was dilated with a 2.5- or 3.0-mm-diameter balloon. The contralateral iliac artery was treated with a 1.5-mm-diameter laser probe heated with 6 W of argon laser energy, and then BA was performed. Four weeks later, the mean luminal diameter of the LABA-treated arteries was smaller

A new laser probe that converts argon laser energy to heat in a metallic cap on a fiberoptic fiber was compared with a bare argon laser fiberoptic catheter system for the dissolution of atherosclerotic obstructions under fluoroscopic guidance in 24 atherosclerotic rabbits. Twelve animals were randomly assigned to receive treatment with either a bare fiberoptic system or a laser probe coupled to an argon laser. Both fibers had similar outer (0.9 mm) and core (400 mu) diameters. Pulses of 1 watt for 1 second's duration were delivered from the tip of the fiberoptic system, while pulses of 6 watts

Acute coronary syndromes such as unstable angina and myocardial infarction are attributed to a pathophysiologic process that involves rupture of atherosclerotic plaque and subsequent thrombosis. Percutaneous intervention of anomalous coronary arteries in patients who present with acute coronary syndromes impose unique technical challenges related to the specific anatomic course and morphology of these vessels. Selection of appropriate guiding catheter configuration, choice of supportive guidewire, and proper delivery and activation of debulking devices and stents are important steps toward ach

After nearly 2 decades of research and experimentation with laser-assisted angioplasty, the xenon-hydrogen chloride excimer laser emerged as the laser device best suited for the treatment of peripheral artery disease. Emitting light at a wavelength of 308 nm, this laser utilizes a nonthermal mechanism of action to ablate plaque and thrombus in powerful discrete pulses. The excimer laser is particularly useful for the treatment of complex conditions, such as long chronic occlusions in the superficial femoral artery and in those patients with below-the-knee disease and critical limb ischemia who

A case is described in which an excimer laser is used to assist angioplasty of a severe central venous lesion that is refractory to conventional techniques. Modern laser technology uses the ultraviolet (UV) spectral region to generate nanosecond pulses of energy. This results in photoablation, which is the process by which energy photons cause molecular bond disruption, while minimizing thermal damage to the surrounding vascular tissues. Further investigation of excimer laser assisted angioplasty in the setting of hemodialysis access intervention is needed to rigorously define its potential ro

A non-thermal Xenon-Chloride excimer laser was used for peripheral arterial recanalization in eight patients with obliterating atherosclerosis of the superficial femoral artery. All patients had a total occlusion with severe claudication or pain at rest. There were three complications, which all led to thrombosis of the irradiated artery. In one, the puncture site in the artery had to be closed operatively. All vessels were followed up radiographically three months after the procedure. In seven patients the vessel was patent. Several investigators have warranted caution in using thermal lasers

The advent of balloon angioplasty as a clinical device crystallized the concept of nonsurgical revascularization. The problems of restenosis, diffuse disease, and total occlusions persist despite the demonstrated efficacy of balloon angioplasty. During the past 5 years, a variety of laser devices and catheter designs have demonstrated usefulness in the treatment of peripheral vascular disease. Initial success rates of 70-90% have been reported in occluded femoropopliteal arteries. Further clinical trials are warranted to compare the relative efficacy of these devices with each other and conven

Laser thermal recanalization has been used clinically as an adjunct to balloon angioplasty in the treatment of peripheral arteriosclerotic disease, with improved initial success rates in total peripheral occlusions and greater 1 yr vessel patency suggested, as compared to balloon angioplasty alone. However, the morphological effects of laser-assisted balloon angioplasty are unknown. Therefore, the goals of the present study were to evaluate 1) the angiographic and histologic effects of laser thermal recanalization followed by balloon angioplasty and 2) the hypothesis that balloon-catheter-indu
