Ankle arthroscopy for ankle fractures.

In many patients who undergo open reduction-internal fixation of ankle fractures, there is a failure to achieve good clinical outcomes despite radiographic evidence of anatomic reduction. One possible reason for this is the high incidence of concomitant intra-articular pathology associated with ankle fractures that may go unrecognized using traditional open approaches. Arthroscopy in the setting of acute operative management of ankle fractures provides a means to completely assess intra-articular pathology, as well as provide direct therapeutic intervention in many instances. Arthroscopic mana

Neuropathic ankle fractures.

A 65-year-old diabetic female presented with a 3-week history of a left swollen foot after a minor inversion injury and was found to have a minimally displaced fibular fracture. Despite casting and strict instructions to remain non-weight bearing, the patient continued to bear weight and later developed a significantly more displaced fracture with a draining ulcer. This injury eventually required a tibiotalocalcaneal arthrodesis using a retrograde hindfoot nail. Neuropathy and neuropathic fractures can be devastating complications of diabetes and thus require early diagnosis and intervention b

[Treatment of ankle fractures].

Fractures of the ankle joint are among the most frequent fractures encountered in any accident and emergency department. The traditional classification systems, i.e. the Lauge-Hansen- and the Weber classifications, have proven inadequate, when it comes to deciding, whether treatment should be surgical or conservative. About ten years ago, a stability-based classification was proposed in view of the fact, that the stabilisation of osteosynthesis depends on the stability of the ankle joint. The literature shows excellent long-term results for conservatively treated stable ankle joint fractures.

Ankle fractures in children.

Computed tomography is useful for preoperative planning and postreduction assessment for intra-articular pediatric ankle fractures. Nondisplaced pediatric ankle fractures can be effectively managed with cast immobilization and close radiographic follow-up evaluation. Physeal ankle injuries in younger children with considerable growth remaining should be followed closely for at least one year after injury as growth arrest may result in substantial angular deformity. Open reduction and internal fixation should be strongly considered when an articular step-off of <2 mm cannot be maintained by clo

[Ankle fractures in children].

In the present work we address the issue of rare childhood bone fractures of the talus. In the study, we retrospectively analyzed a group of pediatric patients with fractures of the talus treated in the space of 10 years at two workplaces of trauma surgery. Patients were evaluated clinically and radiographically with at least one year follow-up. Fourteen patients with a mean age of 12 years met the methodological criteria. All suffered low-energy trauma. According to Marti-Weber classification six of them suffered central fracture of the talus. Of these, two were treated by closed reduction an

Ankle fractures in children.

Ankle fractures are common in children, and they have specific implications in that patient population due to frequent involvement of the physis in a bone that has growth potential and unique biomechanical properties.Characteristic patterns are typically evident in relation to the state of osseous development of the segment, and to an extent these are age-dependent.In a specific type known as transitional fractures - which occur in children who are progressing to a mature skeleton -a partial physeal closure is evident, which produces multiplanar fracture patterns.Computed tomography should be

Posterior malleolar ankle fractures.

AIMS: The primary aim of this study was to address the hypothesis that fracture morphology might be more important than posterior malleolar fragment size in rotational type posterior malleolar ankle fractures (PMAFs). The secondary aim was to identify clinically important predictors of outcome for each respective PMAF-type, to challenge the current dogma that surgical decision-making should be based on fragment size. METHODS: This observational prospective cohort study included 70 patients with operatively treated rotational type PMAFs, respectively: 23 Haraguchi Type I (large posterolateral-

Ankle fractures in diabetics.

Treatment of the diabetic patient with ankle fracture presents a unique set of challenges to the surgeon. The care of these patients should follow a multidisciplinary approach with a team of orthopedic and vascular surgeons, internists, anesthesiologists, nurses, and diabetic educators. Meticulous preoperative planning, intraoperative technique, and postoperative care can be decrease potential limb-threatening complications; however, complications will occur despite excellent care. Early recognition and treatment of preoperative complications is imperative. These patients require close attenti

Supination adduction ankle fractures: Ankle fracture or pilon variant?

BACKGROUND: Supination adduction (SAD) fractures are rotational ankle fractures with a characteristic vertical medial malleolus fracture and tension failure fibula fracture. While these fractures are considered rotational injuries, they can have joint impaction that could lead to early joint degeneration. The purpose of this study was to characterize SAD ankle fractures and compare these injuries with partial articular pilon fractures. METHODS: Following IRB approval, we retrospectively reviewed ankle and pilon fractures (OTA 43 & 44) treated at two academic level-1 trauma centers from 2008-2

Ankle Fracture Classification: An Innovative System for Describing Ankle Fractures.

A good classification system is important for clinical handoffs, research, and clinical treatment guidelines. A reliable classification system shows good interobserver and intraobserver agreement. This study analyzed the interobserver and intraobserver agreement of a descriptive system for ankle fractures and the Lauge-Hansen classification. Three groups of observers (experts, semiexperts, and novices) scored a total of 20 ankle radiographs. All ankle radiographs were classified according to the Lauge-Hansen and Danis-Weber classifications. The ankle fractures were subsequently reviewed in a d

Ankle fracture spur sign is pathognomonic for a variant ankle fracture.

BACKGROUND: The hyperplantarflexion variant ankle fracture is composed of a posterior tibial lip fracture with posterolateral and posteromedial fracture fragments separated by a vertical fracture line. This infrequently reported injury pattern often includes an associated "spur sign" or double cortical density at the inferomedial tibial metaphysis. The objective of this study was to quantitatively establish the association of the ankle fracture spur sign with the hyperplantarflexion variant ankle fracture. METHODS: Our clinical database of operative ankle fractures was retrospectively reviewe

Should we consider ankle fractures as osteoporotic fractures?

UNLABELLED: Since ankle fractures are common fractures in the lower limb, it is worth exploring their potential association with osteoporosis. As part of the research, patients with ankle fractures were screened for osteoporosis to assess its potential for identifying patients with osteoporosis (osteoporosis cases). Consequently, screening should be included as part of fracture management. Ankle fractures are part of the orthopaedic surgeon's daily routine. They are the most common lower limb fractures in the elderly. The absence of an age peak, such as that seen in distal radius fractures, ma

Ipsilateral tibia and ankle fractures.

This study reports on 18 cases of diaphyseal fractures of the tibia associated with ipsilateral but not contiguous ankle fractures. Thirteen of these complex fractures resulted from low-energy torsional forces, while five were caused by high-energy impact. Definitive treatment consisted solely of cast immobilization in four; 14 had operative stabilization of each fracture component. Of these 14, four patients were treated initially in long leg casts and one in a plaster splint, but each required surgical intervention to control fracture alignment. The average follow-up time was 21.2 months. In

Management of Geriatric Ankle Fractures.

PURPOSE OF REVIEW: This review evaluates current management options for managing ankle fractures in geriatric patients with osteoporosis. It addresses the challenges posed by compromised bone integrity and examines operative and non-operative approaches to promote early mobility and functional recovery in elderly individuals. RECENT FINDINGS: Recent studies show that both surgical and non-surgical management can yield comparable functional outcomes, but complication profiles differ. Studies have assessed advanced fixation options such as standard and extended ORIF, hindfoot nailing, and fibul

Current concepts in ankle fractures.

Ankle fractures are among the most common orthopaedic injuries. Operative management is performed in unstable ankle fracture patterns to restore the stability and native kinematics of the ankle mortise and minimize the risk of post-traumatic degenerative changes. In this study, we review current concepts in ankle fracture management, including posterior malleolus fixation, syndesmosis fixation, deltoid ligament repair, fibular nailing, and early weightbearing, from both a biomechanical and clinical perspective.

Ankle fractures in diabetic patients.

Surgical complications are more common in patients with complicated diabetes (presence of inner organ failure, neuropathy).Of all patients undergoing ankle fracture fixation, approximately 13% are diabetic and 2% have complicated diabetes mellitus.Non-operative management of ankle fractures in patients with complicated diabetes results in an even higher rate of complications.Insufficient stability of ankle fractures (treated operatively, or non-operatively) can trigger Charcot neuroarthropathy, and result in bone loss, deformity, ulceration, and the need for amputation.Rigid fixation is recomm
