Rehabilitation of anterior cruciate ligament injuries.

The previously outlined exercise program for the knee is for the instability that results from the loss of the anterior cruciate ligament and/or capsular ligaments laterally. Its success depends on communication between surgeon, therapist, and trainer. Each must understand what the other is doing and must follow the biomechanical and healing restraints of the surgery. The understanding of soft tissue healing is ongoing. Research in this field will greatly enhance the rehabilitation programs of the future.

Orthobiologics in anterior cruciate ligament injuries.

UNLABELLED: Anterior cruciate ligament (ACL) injury remains a major challenge in sports medicine because it affects a young and active population and is associated with instability, secondary meniscal and chondral damage, delayed return to sport and post-traumatic osteoarthritis. This narrative review provides an ACL-specific overview of orthobiologics, with an emphasis on the biologic rationale, current clinical applications and the strength of the available evidence. The biologic limitations of the native ACL, particularly its poor intrinsic healing capacity in the intra-articular environmen

Anterior Cruciate Ligament Injuries in Female Athletes.

Female athletes are at a significantly greater risk of anterior cruciate ligament injury when compared to males. The cause of this is multifactorial, due to interactions of extrinsic, anatomic, genetic, hormonal, and neuromuscular factors. Careful preoperative consideration of the patient's anatomy, biomechanics, and goals is crucial when discussing graft type and adjunct procedures. Female athletes are also significantly less likely to return to sport than males, often due to fear of re-injury and/or distrust of their knee. To optimize success in female athletes, it is important to take a mul

Anterior Cruciate Ligament Injuries in Baseball Players.

PURPOSE: To determine common mechanisms of anterior cruciate ligament (ACL) injury in baseball players and to quantify the rate of return to play after primary surgical reconstruction and review intermediate clinical outcomes. METHODS: Surgical injuries involving the ACL in youth, high school, collegiate, and professional baseball players were queried for an 11-year period (2001 to 2011). Over the study period, 42 baseball players were identified who had undergone arthroscopically assisted primary ACL reconstruction by 1 of 3 attending surgeons. Retrospective chart review was performed for al

Anterior cruciate ligament injuries in female athletes.

The anterior cruciate ligament (ACL) is frequently injured in elite athletes, with females up to eight times more likely to suffer an ACL tear than males. Biomechanical and hormonal factors have been thoroughly investigated; however, there remain unknown factors that need investigation. The mechanism of injury differs between males and females, and anatomical differences contribute significantly to the increased risk in females. Hormonal factors, both endogenous and exogenous, play a role in ACL laxity and may modify the risk of injury. However, data are still limited, and research involving o

Arthroscopic Anterior-Row Fixation for Anterior Cruciate Ligament Avulsion Injuries.

Anterior cruciate ligament avulsion fractures are more commonly seen in children with open physes than in adults. Arthroscopic fixation is considered the gold standard in the management of such injuries. Our technique of anterior-row fixation for these injuries provides various advantages in the form of physeal-sparing, complete anatomic reduction with no anterior beaking, no arthrofibrosis, no residual instability, no intra-articular hardware, no need for a second operation to remove implants, and finally, a full range of movement with no loss of extension. Compared with other methods of fixa

Mechanisms of anterior cruciate ligament injury.

This study examined the mechanisms of anterior cruciate ligament (ACL) injury. In the first part of the study, using a comprehensive, standardized questionnaire, 89 athletes (100 knees) were interviewed about the events surrounding their ACL injury. A noncontact mechanism was reported in 71 (72%) knees and a contact injury in 28 (28%) knees; one patient was unsure if there was any contact. Most of the injuries were sustained at footstrike with the knee close to full extension. Noncontact mechanisms were classified as sudden deceleration prior to a change of direction or landing motion, while c

Treatment of acute and chronic anterior cruciate ligament-posterior cruciate ligament-lateral side knee injuries.

Knee dislocations are rare but devastating injuries. The ACL-PCL-lateral side injury combination is representative of the challenges these injuries present. Early management is focused on vascular integrity. When possible, acute repair and reconstruction within 3 weeks from injury is preferred. Chronically deficient knees generally will require lateral side reconstruction rather than repair and may require limb realignment. Addressing all injured structures is imperative to afford the best chance at a reasonable outcome. Good results with surgery are possible, but some degree of persistent dis

Treatment of combined anterior cruciate ligament-posterior cruciate ligament-lateral side injuries of the knee.

ACL-PCL-posterolateral corner injuries most frequently are seen in multiple trauma patients but do occur in the athletic injury population. Acute three-ligament-injured knees may have been tibiofemoral dislocations with spontaneous reduction in the field. Careful documentation of the neurovascular status is essential in these cases to avoid the complications associated with limb ischemia. Systematic evaluation of these patients with history, physical examination, imaging studies, examination under anesthesia, and diagnostic arthroscopy will aid in the correct diagnosis and treatment plan formu

Knee Anterior Cruciate Ligament Injuries: Common Problems and Solutions.

Anterior cruciate ligament (ACL) reconstruction is one of the most commonly performed procedures in the United States. Although complications are rare in ACL surgery, failure to appreciate them can lead to significant patient morbidity in the short and long terms. More common complications in ACL reconstruction include tunnel malposition, infection, tunnel osteolysis, fixation failure, fracture, arthrofibrosis, graft site morbidity, and deep vein thrombosis or pulmonary embolism. Tunnel malposition is the most common technical error in ACL reconstruction leading to failure. Proper planning dur

Intrasubstance Anterior Cruciate Ligament Injuries in the Pediatric Population.

Pediatric intrasubstance anterior cruciate ligament (ACL) tears have a significant epidemiologic impact as their numbers continue to grow globally. This review focuses on true pediatric intrasubstance ACL tears, which occur >400,000 times annually. Modifiable and non-modifiable risk factors include intercondylar notch width, ACL size, gender, landing mechanisms, and hormonal variations. The proposed mechanisms of injury include anterior tibial shear and dynamic valgus collapse. ACL tears can be associated with soft tissue and chondral defects. History and physical examination are the most impo

Anterior cruciate ligament injuries in patients with open physes.

From July 1988 to August 1989, six children with open physes and injuries to the anterior cruciate ligament were treated operatively. All injuries involved twisting episodes during sports activities. All six patients had meniscal abnormalities, and, additionally, one patient sustained a Grade III medical ligament tear and had a lateral patellar dislocation. Operative reconstruction used hamstring tendons and place a groove over the front of the tibia and a groove over the top of the femur without violation of the growth plates. The anterior cruciate ligament was primarily repaired (three patie

Osteotomy combined with anterior cruciate ligament reconstruction for anterior cruciate ligament injury and biplanar deformity.

BACKGROUND: It has been confirmed that the increased posterior tibial slope over 12 degrees is a risk factor for anterior cruciate ligament injury, and varus deformity can aggravate the progression of medial osteoarthritis. AIM: To evaluate the efficacy of modified high tibial osteotomy (HTO) and anterior cruciate ligament reconstruction (ACLR) in the treatment of anterior cruciate ligament (ACL) injuries with varus deformities and increased posterior tibial slope (PTS) based on clinical and imaging data. METHODS: The patient data in this retrospective study were collected from 2019 to 2021.

Combined Anterior Cruciate Ligament Tensioned Remnant Suspension Repair and Autograft Reconstruction for Sherman I-II Anterior Cruciate Ligament Injuries.

Arthroscopic anterior cruciate ligament reconstruction with tensioned remnant preservation is a simple, reproducible strategy designed to enhance graft biology and stability in appropriately selected patients while maintaining anatomic tunnel accuracy. Prioritized for acute cases with functional remnants (Sherman I-II), the approach leverages the remnant's "biologic scaffold" and "fluid-seal" effects to promote graft synovialization, revascularization, and reinnervation, reduce synovial fluid washout into the tunnels, and potentially improve rotational control. When applied under strict indica
