Perioperative care requires a comprehensive assessment of the risks of bleeding and thrombosis. The 2022 CHEST Guidelines offer an empirical foundation for customized treatment using clinical judgment and risk assessment based on CHA2DS2-VASC and HAS-BLED scores. Despite the ease of access to anticoagulation guidelines, decision-making remains challenging, particularly when determining the necessity of bridging therapy with unfractionated or low-molecular-weight heparin. To facilitate this decision, we provide a mnemonic, STREAM, that highlights high-risk situations that require bridging. Thes

BACKGROUND: It is uncertain whether bridging anticoagulation is necessary for patients with atrial fibrillation who need an interruption in warfarin treatment for an elective operation or other elective invasive procedure. We hypothesized that forgoing bridging anticoagulation would be noninferior to bridging with low-molecular-weight heparin for the prevention of perioperative arterial thromboembolism and would be superior to bridging with respect to major bleeding.
METHODS: We performed a randomized, double-blind, placebo-controlled trial in which, after perioperative interruption of warfar

BACKGROUND: NAION, a multifactorial disorder with an unclear mechanism has limited insight on its treatment option. Despite conflicting results in its relation to hypercoagulation, many metabolic risk factors associated with NAION are intertwined with hypercoagulation. We aim to investigate the impact of anticoagulant therapy in the vascular and visual function of NAION patients.
METHODS: A prospective interventional study was conducted in the Neuro-Ophthalmology Division, Department of Ophthalmology at FKUI-RSCM Kirana from October 2020 to April 2022, involving two groups of NAION subjects:

BACKGROUND: Current guidelines recommend bridging anticoagulation in patients undergoing cardiac rhythm device surgery with a "moderate to high risk" of thromboembolism. Patients at "low risk" are advised to stop oral anticoagulation without bridging to the procedure. This study examines real world adherence to accepted guidelines and the clinical sequelae of nonadherence.
METHODS: We performed a review of all patients undergoing device surgery receiving chronic anticoagulation over a prespecified time period of 14 months. Patients were classified per American College of Chest Physician guide

When Bridge anticoagulant neutralizing agent, prepared by column fractionation, was infused with factor VIII concentrate into a hemophilic dog, the apparent level of circulating factor VIII was markedly increased. Material isolated from 30 ml donor plasma raised the level of factor VIII nearly three-fold, during the first post-transfusion hour. This was achieved without shortening the period during which a hemostatically-effective level of factor VIII was maintained. The same dose also reduced the amount of factor VIII required for prompt and successful hemostasis after standardized template g

Demonstrating the efficacy of new treatments in any condition may be a challenging endeavor, and is particularly the case in sepsis. In the early 21st century, recombinant activated protein C showed a survival benefit in severe sepsis; however, subsequent studies could not replicate these results, leading to the discontinuation of this agent. Several potential reasons have been proposed for the unfavorable results of trials, including choosing an inappropriate outcome target. Concerning anticoagulant therapies, some studies have targeted sepsis with disseminated intravascular coagulation (DIC)

BACKGROUND: In preparation for an invasive procedure with a high bleeding risk, patients with a mechanical heart valve temporarily have to discontinue their anticoagulant therapy and are usually bridged with either intravenous unfractionated heparin (UFH) or subcutaneous low-molecular-weight heparin (LMWH). In this study we retrospectively analyzed the safety of UFH versus LMWH as bridging strategy in left-sided mechanical heart valve patients.
METHODS: We performed a retrospective multicenter study in four surgical centers in The Netherlands. Patients with a mechanical heart valve implantati

OBJECTIVE: To evaluate bleeding rates among patients who received a mechanical aortic valve replacement (mAVR) treated with warfarin monotherapy versus warfarin with a therapeutic parenteral anticoagulant bridge.
METHODS: This retrospective, single-center, observational study included patients at least 18 years old who received an mAVR and had anticoagulation ordered by the end of postoperative day 2. Exclusion criteria included an INR goal other than 2-3, a hypercoagulable state, postoperative mechanical circulatory support, delayed sternal closure, pregnancy or incarceration. The primary ou

Limited evidence exists to guide the use of early parenteral anticoagulation following mechanical heart valve replacement (MVR). The purpose of this study was to compare the 30-day rates of thrombotic and bleeding complications for MVR patients receiving therapeutic versus prophylactic dose bridging regimens. In this retrospective cohort study we reviewed anticoagulation management and outcomes of all patients undergoing MVR at five Canadian hospitals between 2003 and 2010. The primary efficacy outcome was thromboembolism (stroke, transient ischaemic attack, systemic embolism or valve thrombos

Prosthetic valve thrombosis (PVT) represents a serious and potentially lethal complication. It can be attributed more frequently to inadequate anticoagulant therapy. We present a case of acute aortic mechanical valve thrombosis six months after implantation. The patient discontinued oral anticoagulation after being discharged following the primary operation. Two days after reinitiating warfarin as an outpatient, he developed acute valve thrombosis presenting with symptoms and signs of cardiac failure. He was managed with intravenous thrombolysis with a recombinant plasminogen activator which r

BACKGROUND: The treatment of patients at increased risk for arterial thromboembolism who require temporary interruption of warfarin sodium therapy is a common clinical problem. We investigated the efficacy and safety of a standardized periprocedural anticoagulation regimen with low-molecular-weight heparin.
METHODS: We studied 650 consecutive patients with a mechanical heart valve, chronic atrial fibrillation, or embolic stroke who required interruption of warfarin therapy because of an invasive procedure. Warfarin was stopped 5 or 6 days before the procedure, and patients received subcutaneo

Patients with atrial fibrillation, venous thrombosis, and mechanical heart valve (MHV) regularly undergo procedures on a daily basis, for which they require bridging anticoagulation, but this poses significant challenges. Bridging anticoagulation involves temporary interruption of long-term anticoagulation therapy for procedures and continued overlap with short-acting anticoagulants during perioperative period. Heparin-based agents are often used for overlapping in perioperative period to reduce the risk of thromboembolism, but the evidence for benefit particularly in patients with MHV remains

For patients with mechanical heart valves, oral vitamin K antagonists effectively reduce the risk of valve thrombosis. Bridging strategies that use intravenous unfractionated heparin or subcutaneous low molecular weight heparin (LMWH) are required when reversal of anticoagulation is needed for invasive procedures or bleeding complications. There is limited data comparing anticoagulation efficacy between subtypes of LMWH and dosing regimens in this context. This report describes the case of a 45-year-old man with acute mechanical mitral valve thrombosis and suggests that the use of once daily d

BACKGROUND AND AIM OF THE STUDY: Following mechanical heart valve replacement, patients may require a form of 'bridging' anticoagulation to prevent valve-associated thromboembolism until oral vitamin K antagonists take effect. In 2000, the present authors changed their bridging protocol to a fixed dose of 40 mg enoxaparin twice daily (b.i.d., subcutaneous), regardless of the patient's body weight and renal function. The study aim was to evaluate the feasibility of this protocol with regards to thromboembolism, hemorrhage and other valve-associated adverse effects.
METHODS: Between April 2000

BACKGROUND: In patients receiving chronic oral anticoagulation with vitamin K antagonists (VKAs) it may be necessary to temporarily discontinue VKA therapy to allow surgery or other invasive procedures to be performed, as maintaining treatment may increase the risk of bleeding during the procedure. This, however, creates a clinical dilemma, since discontinuing VKAs may place the patient at risk of thromboembolism.
SCOPE: We undertook a systematic narrative review of patients on chronic oral anticoagulation, requiring a periprocedural bridging therapy with heparin during invasive procedures.

BACKGROUND: This retrospective single-center study evaluates differences in bleeding and thrombotic events between a homogenous group of patients undergoing mechanical aortic valve replacement who either received or did not receive intravenous unfractionated heparin or subcutaneous low-molecular weight heparin as bridging strategy to warfarin therapy.
METHODS: Clinical data on a total of 158 patients undergoing mechanical aortic valve replacement at our center between 2001 and 2014 were collected. Patients were grouped according to postoperative anticoagulation strategy: warfarin only (n = 53

A combination of excellent anticoagulation and endothelium-friendly properties is highly imperative for blood-contacting implants in clinics. Herein, bioinspired by cell membrane and catecholamine chemistry, a zwitterionic lubrication copolymer with bridge architecture was designed and synthesized for fabricating a versatile and stable platform to construct an antithrombotic/endothelium-friendly surface, which was prepared by a facile and flexible dip-coating method. Importantly, the zwitterionic coating greatly improved the lubrication and antithrombotic performances based on the tenacious hy

For warfarin-treated patients with atrial fibrillation (AF) at low thromboembolic risk, recent studies have shown harm associated with periprocedural bridging using low-molecular-weight heparin. Clinician surveys have indicated a preference toward excessive bridging, especially among noncardiologists; however, little is known about actual practice patterns in these patients. We performed a retrospective evaluation of bridging in the setting of gastrointestinal endoscopy. We identified 938 patients with AF on warfarin who underwent esophagogastroduodenoscopy or colonoscopy between 2012 and 2016

BACKGROUND: Periprocedural heparin bridging therapy aims to reduce the risk of thromboembolic events in patients requiring an interruption in their anticoagulation therapy for the purpose of an elective procedure. The efficacy and safety of heparin bridging therapy has not been well established.
OBJECTIVES: To compare through meta-analysis the effects of heparin bridging therapy on the risk of major bleeding and thromboembolic events of clinical significance among patients taking oral anticoagulants.
METHODS: We searched PubMed, EMBASE and the Cochrane library from January 2005 to July 2016.
