The global rise in anticoagulant use has increased the number of major bleeding events that warrant timely and effective pharmacologic reversal. Reversal strategies should be informed by the pharmacodynamic and pharmacokinetic features of the anticoagulant and antidote, regulatory indications, quality of evidence, patient-specific factors, and availability of treatment options. Protamine sulfate neutralizes unfractionated heparin, whereas no specific antidotes exist for low-molecular-weight heparins or fondaparinux. Four-factor prothrombin complex concentrates effectively reverse vitamin K ant

More patients than ever are presenting for urgent or emergent procedures while therapeutically anticoagulated for various medical indications. Medications including warfarin, antiplatelet agents such as clopidogrel, direct oral anticoagulants such as apixaban, and even heparin or heparinoids may be present. Each of these medication classes presents its own challenges when coagulopathy needs to be quickly corrected. This review article presents evidence-based discussions of monitoring and reversal of these medication-induced coagulopathies. In addition, there will be a brief discussion of other

Since percutaneous transluminal coronary angioplasty was first described and the breakthrough studies of the role of stents were reported, the evolution in anticoagulation and antiplatelet therapy used during percutaneous coronary intervention (PCI) has reduced periprocedural ischemic events and stent thrombosis. Although greater combinations and doses of anticoagulation with antiplatelets seem to provide the best protection against thrombogenic and embolic events, there is a significant trade-off with a higher risk of major and minor bleeding episodes. This review article expands on each of t

In an exciting era with many alternatives to the old anticoagulants heparin and warfarin emerging on the scene, awareness of the possibility to reverse their effect is mandatory. In this review, the traditional antidotes for warfarin (vitamin K, plasma, and prothrombin complex concentrate) and for heparin (protamine) are described together with the newer alternatives (recombinant activated factor VII, concatameric peptides, and recombinant platelet factor 4). For some of the newer anticoagulants, possible antidotes have been identified, whereas other alternatives have been discarded. There is

Bleeding is the most important adverse effect of antithrombotic treatment and may be a major cause of morbidity, longstanding debilitation, and even mortality. In the case of severe hemorrhage in a patient who uses anticoagulant agents, it may be crucial to reverse anticoagulant treatment. Conventional anticoagulants such as vitamin K antagonists can be neutralized by the administration of vitamin K or prothrombin complex concentrates, whereas heparin and heparin derivatives can be counteracted by protamine sulfate. The antihemostatic effect of aspirin and other antiplatelet strategies can be

IMPORTANCE: Intracerebral hemorrhage (ICH) is the deadliest stroke subtype, and mortality rates are especially high in anticoagulation-associated ICH. Recently, specific anticoagulation reversal strategies have been developed, but it is not clear whether there is a time-dependent treatment effect for door-to-treatment (DTT) times in clinical practice.
OBJECTIVE: To evaluate whether DTT time is associated with outcome among patients with anticoagulation-associated ICH treated with reversal interventions.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from the American Heart As

In light of an aging population with increased cardiovascular comorbidity, the use of oral anticoagulation (OAC) is steadily expanding. A variety of pharmacological alternatives to vitamin K antagonists (VKA) have emerged over recent years (direct oral anticoagulants, DOAC, i.e., dabigatran, rivaroxaban, apixaban, and edoxaban) which show a reduced risk for the occurrence of intracerebral hemorrhage (ICH). Yet, in the event of ICH under OAC (OAC-ICH), hematoma characteristics are similarly severe and clinical outcomes likewise substantially limited in both patients with VKA- and DOAC-ICH, whic

BACKGROUND: Intracerebral hemorrhage is a well-recognized complication of anticoagulation therapy. However, there are only a few reports that address the management of aneurysmal subarachnoid hemorrhage (aSAH) in anticoagulated patients.
OBJECTIVE: We report on our experiences with the use of prothrombin complex concentrate (PCC) for rapid anticoagulation reversal in aSAH.
METHODS: We retrospectively analyzed our institutional database of consecutive patients who received PCC between February 2006 and August 2014 (n > 1000). Data from all anticoagulated patients referred to our hospital for

BACKGROUND: Andexanet alfa is a reversal agent for factor Xa (FXa) inhibitors (rivaroxaban and apixaban) and is available for the treatment of severe FXa inhibitor-associated bleeding. However, an increase in thrombotic events after administration has been reported.
OBJECTIVES: This multicenter Dutch observational study aimed to provide insights into the characteristics, usage patterns, and clinical outcomes of the Dutch patient population treated with andexanet alfa.
METHODS: We included all patients treated with on- and off-label andexanet alfa from 6 of the 11 hospitals in the Netherlands

Anticoagulant drugs are taken by millions of patients throughout the world. Warfarin has been the most widely prescribed anticoagulant for decades. In recent years, new oral anticoagulants have been approved for use, are being positioned as alternatives to warfarin, and represent an enormous market opportunity for pharmaceutical companies. Requests for urgent reversal of anticoagulants are not uncommon especially in the setting of critical bleeding. This review summarizes information on reversal of warfarin by vitamin K, plasma, prothrombin complex concentrates, and recombinant VIIa. In additi

Oral anticoagulants (OACs), including vitamin K antagonists and direct-acting OACs, are commonly used for the prevention and treatment of thrombosis. OAC-treated patients may require urgent anticoagulation reversal in circumstances of life-threatening bleeding or emergent surgery. Despite the availability of Food and Drug Administration-approved OAC reversal agents, published treatment guidelines, and institutional protocols, use of OAC reversal products and dosing vary considerably and off-label use is common. We review key factors influencing current clinical practice and examine barriers to

OBJECTIVE: Gastrointestinal bleeding (GIB) is a common complication associated with warfarin use. However, the optimal approach for anticoagulation reversal-whether with prothrombin complex concentrate (PCC), fresh frozen plasma (FFP), vitamin K (intravenous or oral), or no reversal-remains unclear as current literature and society guidelines do not provide definitive recommendations.
METHODS: A retrospective analysis of emergency department patients with warfarin-associated GIB who presented to an academic health system was performed, comparing reversal with PCC, FFP, vitamin K only, or no r

Anticoagulant therapy has undergone substantial evolution over the past century, progressing from early parenteral agents such as heparin to vitamin K antagonists (VKAs) and, more recently, to direct oral anticoagulants (DOACs). This review provides an overview of the major classes of anticoagulants, including unfractionated heparin (UFH), low molecular weight heparin (LMWH), fondaparinux, direct thrombin inhibitors, VKAs, and DOACs, with emphasis on their mechanisms of action, clinical applications, laboratory monitoring, and reversal strategies. While UFH remains widely used in acute setting

UNLABELLED: There has seen an increase in anticoagulant consumption worldwide over the past few decades. With this widespread utilization of anticoagulants, clinicians are increasingly likely to encounter situations where anticoagulants would need to be withheld. This includes emergency and elective procedures or surgeries as well as major or minor bleeding as a direct result of over anticoagulation or consequent to other intercurrent illnesses such as sepsis or trauma with multiorgan failure, where the anticoagulant may contribute to coagulation abnormalities. Clinicians are required to have

PURPOSE: To evaluate the impact of electronic health record-integrated voice alerts on turnaround time for anticoagulation reversal agents across 6 hospitals within a health system.
METHODS: This was a quasi-experimental pre-post study evaluating implementation of voice alerts for 4 reversal agents. Voice alerts were triggered when an eligible order entered the pharmacist verification queue. Six hospitals transitioned to the system on a staggered schedule. Orders were categorized into pre- and postimplementation periods, each spanning 90 days. Time from order signing to verification and admin

BACKGROUND: Intracranial hemorrhage (ICH) is a complication of oral anticoagulation and is associated with significant morbidity and mortality. Clinical need exists for biomarkers to measure anticoagulation in patients with factor Xa inhibitor-associated ICH to assess the hemostatic effect of reversal agents. This study explored the utility of thromboelastography (TEG) to assess anticoagulation in emergency department (ED) patients who received activated prothrombin complex concentrate (aPCC) reversal for factor Xa-inhibitor-associated ICH.
METHODS: This was a prospective, single-center, coho

Bleeding events are common in patients prescribed anticoagulants and can have devastating consequences. Several specific and nonspecific agents have been developed to reverse the effects of anticoagulant drugs or toxins. Vitamin K, as the oldest of these antidotes, specifically counteracts the effects of pharmaceuticals and rodenticides designed to deplete stores of vitamin K-dependent factors. In cases of life-threatening bleeding, the addition of prothrombin complex concentrates (PCCs) allows for the immediate replacement of coagulation factors. While the use of PCCs has been extended to the
