The efficacy and toxicity of antineoplastic antimetabolites: Role of gut microbiota.

The incidence and mortality of cancer are rapidly growing all over the world. Nowadays, antineoplastic antimetabolites still play a key role in the chemotherapy of cancer. However, the interindividual variations in the efficacy and toxicity of antineoplastic antimetabolites are nonnegligible challenges to their clinical applications. Although many studies have focused on genetic variation, the reasons for these interindividual variations have still not been fully understood. Gut microbiota is reported to be associated with the efficacy and toxicity of antineoplastic antimetabolites. In this re

[5'-deoxy-5-fluoruridine, a new antineoplastic pyrimidine antimetabolite].

5'-deoxy-5-fluorouridine (5-DFUR) exerts a marked cytostatic activity, both intraperitoneally and orally, against experimental tumors. It has a strong antitumor effect on the Crocker sarcoma S 180, the Lewis lung carcinoma and a chemically induced squamous cell carcinoma of the skin. Its ratio of antitumor activity toxicity is more favourable than that of 5-fluorouracil, 2'-deoxy-5-fluorouridine and 1-(2-tetra-hydrofuryl)-5-fluorouracil. Mode of action and reasons for the favourable therapeutic ratio are discussed.

Intercalator-induced, topoisomerase II-mediated DNA cleavage and its modification by antineoplastic antimetabolites.

Defining specific biochemical targets of active antineoplastic agents could aid in discovering better anticancer therapy and more thoroughly understanding the biochemical basis of malignancy. Through a series of cellular and biochemical studies, we and others have identified the nuclear enzyme topoisomerase II as the target of several active agents, including 4'-(9-acridinylamino) methanesulfon-m-anisidide (m-AMSA). The interference with topoisomerase II produced by m-AMSA can be quantified in whole cells exposed to m-AMSA by using the alkaline elution technique to measure DNA cleavage. Antime

Protective Effect of Silver Nanoparticles Against Cytosine Arabinoside Genotoxicity: An In Vivo Micronucleus Assay.

Cancer treatments have harmful side effects, including genotoxic ones. Our previous research discovered that a specific silver nanoparticle (AgNPs) formulation could reduce the genotoxic effects of an alkylating agent, cyclophosphamide. This study aims to evaluate if this protective effect is observed against an antimetabolite anticancer agent, cytosine arabinoside (Ara-C). An erythrocyte micronucleus assay was conducted on BALB/c mice. A most significant effect was observed after the application scheme, including three doses of Ara-C and three subsequent doses of AgNPs, resulting in a 3.7 and

Bleomycin compatibility with selected intravenous medications.

Three separate in vitro admixture experiments were accomplished with bleomycin and other common parenteral medications. Bleomycin was assayed by a radioimmunoassay procedure. Bleomycin was found to be compatible with a majority of drugs including the aminoglycoside antibiotics, the vinca alkaloid antineoplastics, the antimetabolite fluorouracil, several phosphate salt corticosteroids, diphenhydramine and the anticoagulant heparin sulfate. Incompatible admixtures include the acidic cephalosporin and penicillin-derived antibiotics, the antineoplastics, methotrexate, and mitomycin C, the sodium s

Antineoplastic Agents: Environmental Prevalence and Adverse Outcomes in Aquatic Organisms.

Cancer is the second leading cause of death worldwide, with 9.6 million cancer-related deaths in 2018. Cancer incidence has increased over time, and so has the prescription rate of chemotherapeutic drugs. These pharmaceuticals, known as antineoplastic agents, enter the aquatic environment via human excretion and wastewater. The objectives of the present critical review were to investigate the risk of antineoplastics to aquatic species and to summarize the current state of knowledge regarding their levels in the environment, because many antineoplastics are not adequately removed during wastewa

Postoperative instillation of mitomycin eye drops in the treatment of primary pterygium.

1. Pterygia, triangular sheets of fibrovascular tissue that invade the cornea, have recurrence rates of 30% to 50% with currently available surgical procedures. 2. Mitomycin, an antimetabolite-antibiotic agent, has been reported to be effective in preventing recurrences of surgically excised pterygia when used adjunctively as eye drops. 3. Because mitomycin-C is an antineoplastic agent, appropriate policies and proper patient education are important.

Real-world incidence of nausea and vomiting induced by antineoplastic agents: insights and future directions from the Japanese adverse drug event report database.

PURPOSE: Antineoplastic treatment commonly leads to nausea and vomiting (NV), which considerably affects the quality of life and adherence to therapy of patients. This study aimed to analyze the incidence of NV induced by antineoplastic agents using the Japanese Adverse Drug Event Report (JADER) database and to explore novel antiemetic therapies for oral antineoplastic agents. METHODS: Data from April 2004 to March 2023 were extracted from the JADER database. The analysis considered guideline-recommended antiemetics and involved the temporal evaluation of cases with NV. The onset patterns of

[Cardiovascular complications of chemotherapy].

In the last decades, oncological therapy has changed the natural history of many types of cancer, which can now be considered curable or as chronic or slowly progressive diseases. It is well known that chemotherapy and radiotherapy may induce cardiotoxicity. Anthracyclines are among the most active antineoplastic agents, and their cardiac effects have been known for a long time. In addition, there is an emerging role for potential cardiovascular effects of other chemotherapy drugs such as taxanes, antimetabolites (capecitabine) as well as targeted agents (monoclonal antibody trastuzumab and be

Medication Error- A Case Report of Misadventure with Methotrexate.

Methotrexate is an antimetabolite drug with antineoplastic and immunomodulatory properties, useful as an antineoplastic agent in various haematological and solid tumours. MTX toxicity can occur because of accidental ingestion/overdose by the patient or because of prescription error. The toxic effects manifest as severe mucositis or as organ damage (bone marrow depression, renal/hepatic injury). The toxicity usually results from parenteral overdose or repeated chronic drug ingestion. Acute high dose ingestion does not result in MTX toxicity because of saturable absorption kinetics. We present a

Selected nucleos(t)ide-based prescribed drugs and their multi-target activity.

Nucleos(t)ide analogues play pivotal roles as antiviral, cytotoxic or immunosuppressive agents. Here, we review recent reports of nucleoside analogues that exhibit broad-spectrum activity towards multiple life-threatening RNA and DNA viruses. We also present a discussion about nucleoside antimetabolites-approved antineoplastic agents-that have recently been shown to have antiviral and/or antibacterial activity. The approved drugs and drug combinations, as well as recently identified candidates for investigation and/or experimentation, are discussed. Several examples of repurposed drugs that ha

5-Fluorouracil-Induced Acute Toxic Leukoencephalopathy: A Rare Adverse Effect and Its Early Detection on MRI.

This case report discusses a rare occurrence of 5-fluorouracil (5-FU)-induced encephalopathy, an adverse effect of this commonly used antimetabolite and antineoplastic agent. While 5-FU is primarily associated with systemic side effects such as gastrointestinal issues and neutropenia, its cerebral involvement is infrequently documented. Diagnostic imaging techniques, such as computed tomography (CT) and magnetic resonance imaging (MRI), are essential for diagnosing and monitoring this condition. Early recognition and treatment are crucial for a favorable prognosis. We present a case of 5-FU-in

Raltegravir plus two nucleoside analogues as combination antiretroviral therapy in HIV-infected patients who require cancer chemotherapy.

BACKGROUND: There are few data on the best combination antiretroviral therapy in patients with HIV infection who need cancer chemotherapy because of drug-drug interactions and increased risk of toxic effects. METHODS: We evaluated the safety, efficacy and interactions of a raltegravir (RAL)-based regimen in 30 HIV-infected patients who received antineoplastic agents. RESULTS: A total of 17 patients had a non-AIDS-defining malignancy (7 with Hodgkin disease) and 13 had an HIV-related cancer (9 non-Hodgkin lymphoma, 2 Kaposi sarcoma and 2 anal cancer). Overall, they received 49 cycles of chemo

Methotrexate enhances the antianabolic and antiproliferative effects of 5-aminoimidazole-4-carboxamide riboside.

Because of its ability to mimic a low energy status of the cell, the cell-permeable nucleoside 5-aminoimidazole-4-carboxamide (AICA) riboside was proposed as an antineoplastic agent switching off major energy-consuming processes associated with the malignant phenotype (lipid production, DNA synthesis, cell proliferation, cell migration, etc.). Key to the antineoplastic action of AICA riboside is its conversion to ZMP, an AMP mimetic that at high concentrations activates the AMP-activated protein kinase (AMPK). Here, in an attempt to increase the efficacy of AICA riboside, we pretreated cancer

Neurotoxicity associated with systemic high-dose cytosine arabinoside.

High doses of cytosine arabinoside (ara-C) have been used in the treatment of refractory forms of acute nonlymphoblastic leukemia (ANLL) and ANLL occurring after previous antineoplastic therapy. In addition to the usual toxicities associated with antimetabolites, neurotoxicity, mainly in the form of cerebellar dysfunction, develops in a significant proportion of patients receiving high-dose cytosine arabinoside HDara-C. This study was performed to determine the incidence of cerebellar dysfunction in our patients and to determine any factors that predict its development. In this series of 30 co
