Antipsychotic agents: a review.

Antipsychotic agents are useful in the treatment of psychosis due to both functional disorders (i.e., idiopathic disorders that are usually treated by psychiatrists) and organic mental disorders. These drugs are classified as low- and high-potency agents. Low-potency agents such as chlorpromazine block muscarinic and alpha 1-adrenergic receptors. Consequently, they produce anticholinergic side effects and orthostatic hypotension. High-potency antipsychotic agents have a higher affinity for dopamine receptors and a relatively negligible affinity for muscarinic and alpha 1 receptors. The high-po

Antipsychotic agents: a clinical update.

Antipsychotic agents have many indications and are frequently used. All physicians, regardless of their subspecialty, will most likely treat patients who receive these drugs. This article is designed to help nonpsychiatric physicians use antipsychotics appropriately. The indications, recommended dosages, side effects, and drug interactions of antipsychotic medications are reviewed. Receptor binding data are used to help predict the side-effect profile of these agents. Knowledge of these side effects and of the patient's medical condition helps the physician select a drug that the patient can t

Antipsychotic agents and QT changes.

Recently, antipsychotic medications of the novel or atypical classes have received increased attention because of concerns with respect to potential lengthening of the QT interval, yet the currently available and commonly prescribed conventional antipsychotics are significantly more cardiotoxic, particularly agents in the butyrophenone and phenothiazine classes. Lengthening of the QT interval can be associated with a fatal paroxysmal ventricular arrhythmia known as torsades de pointes. The specific duration of the QT interval at which the risk of an adverse cardiac event is greatest, is not es

Using antipsychotic agents in older patients.

OBJECTIVES: Antipsychotics are widely used in geriatric psychiatric disorders. A growing number of atypical antipsychotics are available, expanding clinical options but complicating decision-making. Many questions about use of antipsychotics in older patients remain unanswered by available clinical literature. We therefore surveyed expert opinion on antipsychotic use in older patients (65 years of age or older) for recommendations concerning indications for antipsychotics, choice of antipsychotics for different conditions (e.g., delirium, dementia, schizophrenia, delusional disorder, psychotic

[Antipsychotic agents and pharmacotherapy of psychoses].

Antipsychotic drugs have been used clinically for approximately 50 years. Reserpine and chlorpromazine were first drugs found be useful in schizophrenia. The term "antipsychotic" is used to describe a group of drugs that has been used mainly for treating of schizophrenia, but also is effective in some other psychoses and agitated states. The dopamine hypothesis is the most fully developed of several hypothesis and is the basis for rational drug therapy. However, the dopamine hypothesis is far from complete. Typical antipsychotic drugs have a wide spectrum of central nervous system, autonomic a

Atypical antipsychotic agents: a critical review.

The pharmacology, efficacy, and adverse effects of atypical antipsychotic agents when used to treat schizophrenia and other disorders are reviewed. Atypical antipsychotic agents were developed in response to problems with typical agents, including lack of efficacy in some patients, lack of improvement in negative symptoms, and troublesome adverse effects, especially extrapyramidal symptoms (EPSs) and tardive dyskinesia CTD). Atypical antipsychotics differ from typical psychotics in their "limbic-specific" dopamine type 2 (D2)-receptor binding and high ratio of serotonin type 2 (5-HT2)-receptor

Overdose profiles of new antipsychotic agents.

Although the more recently introduced antipsychotic drugs are increasing in popularity, the pattern of symptomatology when taken in overdose is not well defined. We monitored all enquiries to the National Poisons Information Service, London (NPIS, London) concerning antipsychotic drugs over a 9-month period in 1997 and report our findings concerning four drugs (olanzapine, clozapine, risperidone and sulpiride). All overdoses involving a single agent were followed up by a letter to the enquirer requesting details and outcome of the case. Although a total of 574 enquiries involving the selected

Adverse effects of antipsychotic agents. Do newer agents offer advantages?

Although antipsychotic drugs are effective in treating the so-called positive features of schizophrenia, between one-quarter and one-third of patients respond poorly. Furthermore, the incidence of adverse effects is high, especially those reflecting disruption of extrapyramidal function, and is a major source of non-compliance. There is a clear need for new compounds that are more efficacious and/or better tolerated. Until recently, the classical dopamine hypothesis, with its emphasis on D2 blockade as the key mechanism of antipsychotic action, dominated drug development, though the emphasis i

Sexual dysfunction with antihypertensive and antipsychotic agents.

The physiology of the normal sexual response, epidemiology of sexual dysfunction, and the pharmacologic mechanisms involved in antihypertensive- and antipsychotic-induced problems with sexual function are discussed, with recommendations for patient management. The physiologic mechanisms involved in the normal sexual response include neurogenic, psychogenic, vascular, and hormonal factors that are coordinated by centers in the hypothalamus, limbic system, and cerebral cortex. Sexual dysfunction is frequently attributed to antihypertensive and antipsychotic agents and is a cause of noncompliance

Role of Antipsychotic Agents for Adjunct Sedation.

Antipsychotics are an attractive option for adjunctive sedation in critically ill adults given their sedating effects, short duration of action, lack of effect on respiratory drive, and multiple routes of administration but high-quality evidence from RCTs is not yet available to guide the use of these agents as adjunct sedatives in the intensive care unit. The purpose of this article is to explore the role of antipsychotics as adjunctive sedatives in mechanically ventilated adults receiving continuous sedation.

Antipsychotic agents: efficacy and safety in schizophrenia.

Antipsychotics have provided a great improvement in the management of people with schizophrenia. The first generation antipsychotics could establish the possibility of managing many psychotic subjects in an outpatient setting. With the advent of the second (SGA) and third generation antipsychotics (TGA), other psychiatric disorders such as bipolar depression, bipolar mania, autism, and major depressive disorder have now been approved for the use of these drugs for their treatment. Also, the administration of more specific assessment tools has allowed for better delineation of the repercussions

Priapism associated with two atypical antipsychotic agents.

Priapism has been associated with many antipsychotic agents, including clozapine, risperidone, and olanzapine. This prolonged, usually painful, penile erection rarely results from the alpha-adrenergic blocking action of antipsychotics. A 22-year-old African-American man developed priapism during treatment with risperidone and, on a later occasion, during treatment with ziprasidone. The problem resolved only with substitution of other drugs for these antipsychotics. Certain patients may be more vulnerable than others to this adverse effect. Clinicians must be aware of such complications and use

Clozapine: a novel antipsychotic agent.

Clozapine is an antipsychotic without the extra-pyramidal adverse effects associated with currently marketed antipsychotics. In animals, this drug has not been shown to induce catalepsy and only weakly antagonizes the stereotypic movements induced by apomorphine and the amphetamines. Clozapine is rapidly absorbed after both single and repeated oral doses, with steady-state concentrations attained within eight to ten days after beginning therapy. It is metabolized to N-oxideclozapine and N-desmethylclozapine, which have less pharmacological activity than the parent compound and are excreted in

Clozapine: an atypical antipsychotic agent.

The pharmacology, pharmacokinetics, clinical efficacy, adverse effects, dosage, and cost of the atypical antipsychotic drug clozapine are reviewed. Clozapine is a dibenzazepine compound chemically similar to loxapine but with a distinct pharmacologic profile. Unlike currently available medications, clozapine has a low potential for causing extrapyramidal symptoms and does not induce dopamine type 2 receptor hypersensitivity. It shows affinity in vitro not only for dopamine type 1 and 2 receptors but also for histamine type 1, alpha-adrenergic type 1 and 2, serotonin type 2, and muscarinic rece

Antipsychotic agents in the treatment of bipolar mania.

OBJECTIVES: Antipsychotics have been widely used in the treatment of bipolar mania. The purpose of this manuscript was to briefly review the evidence of typical and atypical antipsychotics for the treatment of bipolar mania. METHODS: A detailed literature review was conducted on the use of typical and atypical antipsychotics in the treatment of bipolar mania using standard search engines. A summary of the published literature on each agent is described followed by a discussion on the overall comparison of the different agents. RESULTS: For typical antipsychotics, up until recently, there was

Neurosyphilis, malaria, and the discovery of antipsychotic agents.

Four of the most disabling human diseases are syphilis, malaria, schizophrenia, and manic-depressive illness. The history of the development of treatments for these seemingly unrelated disorders intersects at several points. Treatment of tertiary cerebral syphilis (general paresis) by inducing fever with malaria led to a Nobel Prize. Although attempts to synthesize quinine, a plant product effective against malaria, failed, these efforts encouraged industrial organic chemists to synthesize many useful substances, including dyes, antibiotics, and antihistamines. The aniline-derived dye methylen
