Aneurysms: thoracic aortic aneurysms.

Thoracic aortic aneurysms (TAAs) have many possible etiologies, including congenital heart defects (eg, bicuspid aortic valves, coarctation of the aorta), inherited connective tissue disorders (eg, Marfan, Ehlers-Danlos, Loeys-Dietz syndromes), and degenerative conditions (eg, medial necrosis, atherosclerosis of the aortic wall). Symptoms of rupture include a severe tearing pain in the chest, back, or neck, sometimes associated with cardiovascular collapse. Before rupture, TAAs may exert pressure on other thoracic structures, leading to a variety of symptoms. However, most TAAs are asymptomati

[Thoracic aortic aneurysms].

The various pathological types of aneurysm are reviewed and their natural history discussed in relation to morphology, etiology and site of the aneurysm. For diagnostic purposes, computed tomography is a reliable method in the detection of aortic aneurysm; for evaluation of the ascending segment, ultrasonography may also be considered a useful procedure. The two non-invasive methods are particularly valuable in monitoring patients with thoracic aneurysms not (yet) suitable for surgery. In the light of the literature and our own experience, practical guidelines for adequate treatment of non-dis

Thoracic endovascular aortic repair for ruptured descending thoracic aortic aneurysm.

BACKGROUND: We evaluated clinical outcomes of thoracic endovascular aortic repair (TEVAR) for ruptured descending thoracic aortic aneurysm (rDTAA). METHODS: Twenty-three patients with rDTAA (mean age, 76.8 ± 8.8 years) underwent TEVAR at our center between January 2008 and April 2013. RESULTS: In twenty-three patients, five patients (21.7%) were in shock before surgery. Technical success was achieved in 21 patients. After TEVAR, retrograde Type A aortic dissection occurred in one patient, Type I endoleak in one patient, and Type II endoleak in three patients. The 30-day mortality rate was 4.

Thoracic aortic aneurysm and dissection.

Aortic dissection is the most devastating complication of thoracic aortic disease. In the more than 250 years since thoracic aortic dissection was first described, much has been learned about diseases of the thoracic aorta. In this review, we describe normal thoracic aortic size; risk factors for dissection, including genetic and inflammatory conditions; the underpinnings of genetic diseases associated with aneurysm and dissection, including Marfan syndrome and the role of transforming growth factor beta signaling; data on the role for medical therapies in aneurysmal disease, including beta-bl

Stable Ruptured Thoracic Aortic Aneurysm.

A ruptured aortic aneurysm is a life-threatening condition that should be dealt with promptly and requires early management by a concerned specialist team. A ruptured aortic aneurysm can present with a wide array of symptoms, ranging from shortness of breath, chest pain, to sudden collapse. This is a case of a female in her 70s who presented to the emergency department after an unwitnessed collapse at home. Following early tests, it was determined that the patient had a ruptured thoracic aortic aneurysm (TAA), which was deemed to be stable. She was then referred to the vascular team for additi

Thoracic aortic aneurysm gene dictionary.

Thoracic aortic aneurysm is typically clinically silent, with a natural history of progressive enlargement until a potentially lethal complication such as rupture or dissection occurs. Underlying genetic predisposition strongly influences the risk of thoracic aortic aneurysm and dissection. Familial cases are more virulent, have a higher rate of aneurysm growth, and occur earlier in life. To date, over 30 genes have been associated with syndromic and non-syndromic thoracic aortic aneurysm and dissection. The causative genes and their specific variants help to predict the disease phenotype, inc

Thoracic Endovascular Aortic Repair in "Shaggy Thoracic Aortic Aneurysms".

OBJECTIVES: To report the outcomes of thoracic endovascular aortic repair (TEVAR) for shaggy thoracic aortic aneurysms (STA). METHODS: It is a single center, retrospective, observational, cohort study. Data were collected prospectively between January 2005 and May 2019. STA was defined, based on computed tomography angiography findings, as the presence of an irregular/ulcerated atheroma protruding and/or thrombus thickness ≥ 5 mm protruding into the aortic lumen, and/or occupying more than two thirds of the circumference of the aortic diameter axially. Primary outcomes were early (≤ 30 days)

Hypoxemia after thoracic aortic aneurysm repair.

OBJECTIVE: To characterize early postoperative hypoxemia (POH) after proximal aortic aneurysm repair, evaluate its association with outcomes, and determine whether circulatory arrest (CA) contributes to the development of POH. METHODS: We performed a single-center retrospective study of 790 adults undergoing proximal aortic aneurysm repair via sternotomy (2012-2024). POH was defined as an arterial oxygen tension/inspired oxygen fraction ratio <300 mm Hg at 6 hours and categorized as no hypoxemia (>300), mild (200-300), or moderate/severe (<200). Baseline and operative characteristics were bal

[Ruptured Non-dissecting Thoracic Aortic Aneurysm].

Although the operative results for non-dissecting thoracic aneurysm has been improving markedly, that of the ruptured one is still poor. Once aneurysmal rupture is occurred, a patient can almost never survive. Only few patients could arrive to an emergency hospital. It is reported that about 60% of the patients are died suddenly and the remaining 40% of the patients may undergo surgery or endovascular intervention. In the annual report in 2012 published by "Journal of General Thoracic and Cardiovascular Surgery", the mortality rates during postoperative 30 days of the non-ruptured and the rupt

[Thoracic aortic aneurysm in Marfan syndrome].

Thoracic aortic aneurysms are the main cardiovascular complication of Marfan's syndrome. Elastic tissue dystrophy of the ascending aorta is responsible for appearances of cystic medial necrosis in the major forms. Dilatation of the ascending aorta is progressive with time. The risk of dissection and rupture, an acute complication of Marfan's syndrome, is very high when the aortic dilatation reaches 60 mm, although dissection may be observed with dilatation of 50 mm. Aortic regurgitation is found in half the cases. Echocardiography, CT scan and MRI provide accurate assessment of the anatomy of

Chylothorax complicating a thoracic aortic aneurysm.

Chylothorax is an uncommon condition, usually associated with advanced malignant disease or trauma to the thoracic duct. Idiopathic chylothorax and other diverse causes have been anecdotally reported. We describe a rare case of chylothorax, developing as a result of damage to the thoracic duct by a thoracic aortic aneurysm. Tetracycline pleurodesis was successful in controlling the chylous effusion.

The Prevalence of Thoracic Aortic Aneurysms in Patients with Abdominal Aortic Aneurysms.

BACKGROUND: The aim of this study was to quantify the prevalence of thoracic aortic aneurysms (TAAs) in patients with a known abdominal aortic aneurysm (AAA) within the Northwest of England and understand the characteristics of this subpopulation. METHODS: A retrospective review of data from all patients diagnosed with an AAA from 2010 to 2023 and entered onto AAA surveillance within the National Health Service (NHS) Manchester University Foundation Trust. Of these, patients with a thoracic computed tomography (CT) available were reviewed for diagnosis of TAA. RESULTS: Out of 250 patients wi

Mechanisms driving thoracic aortic aneurysm instability.

Thoracic aortic aneurysms arise from a combination of biological and mechanical factors. Current clinical guidelines use size and rate of expansion to stratify risk, but such metrics do not necessarily predict if an aneurysm will stabilize, grow, dissect, or rupture. Computational biomechanical models can provide additional insights into mechanisms of aneurysm behavior that would be difficult or impossible to capture in vivo. Here, we use a constrained mixture theory of growth and remodeling to simulate lesion progression while covarying rate-dependent parameters that contribute to the natural

Thoracic Aortic Aneurysm: A Clinical Review.

Thoracic aortic aneurysms are common. Most thoracic aortic aneurysms are degenerative. However, some are associated with connective tissue disorders, bicuspid aortic valves, or familial/genetic predisposition. Most are asymptomatic, discovered incidentally on imaging. Aortic diameter is the best predictor of the natural history and risk of complications. Treating hypertension and smoking cessation can slow their growth. Surveillance imaging and referral for prophylactic aortic repair based on absolute aneurysm diameter is the primary means to decrease mortality from thoracic aortic aneurysm. W
