The neuroanatomy of Broca's aphasia.

INTRODUCTION: Broca's aphasia, a condition characterized by nonfluent speech and difficulty with language production, results from focal brain damage and is most often caused by stroke. Although traditionally linked to lesions in Broca's area (Brodmann areas 44 and 45 in the left inferior frontal gyrus), recent evidence suggests that the neuroanatomy of Broca's aphasia is far more complex, implicating a broader network of cortical and subcortical regions. This study aimed to delineate the specific cortical and white matter features that, when damaged, lead to persistent Broca's aphasia. METHO

Confusion vs Broca Aphasia: A Case Report.

INTRODUCTION: Broca aphasia presents with impaired expression of spoken and/or written language and is often caused by infarction in the Broca area in the frontal lobe. We present a case of Broca aphasia that was initially interpreted as confusion. CASE PRESENTATION: A 76-year-old woman was brought to the Emergency Department because of confusion and slurred speech that began in the morning. The patient had an extensive history of alcohol abuse, hyperlipidemia, and hypertension and had recently quit drinking 5 days earlier. The patient appeared confused, answering questions with "I don't know

Etiology of stroke in Broca's aphasia.

We reviewed clinical and diagnostic data from 36 patients with Broca's aphasia due to stroke who were evaluated at the University of Iowa Hospitals and Clinics between 1982 and 1989. The group consisted of 20 women and 16 men, aged 28-80 (median, 63.5; mean, 57.5). The presumptive cause of cerebral infarction was embolism in 15 patients (42%), atherothrombosis in 9 (25%), undetermined in 9 (25%), and "other" in 3 (8%). The ratios of women to men were 8:7, 3:6, 7:2, and 2:1, respectively. A majority of emboli were of presumed cardiac origin: atrial fibrillation, 7; prosthetic cardiac valve, 3;

Compensatory articulation in Broca's aphasia.

Thirteen patients clinically diagnosed as Broca's aphasics, were administered a vowel bite block task to ascertain their on-line compensatory articulation ability. Formant frequencies for F1 and F2 were obtained from a pitch-synchronous spectral analysis of digitized vowel waveforms. Bite block/i/productions were consistently characterized by higher F1 and lower F2 frequencies compared to normal vowel productions. Undershooting of tongue elevation and fronting explains this result. No consistent pattern of "off-target" productions were found for/a/tokens. A multiple correlation analysis among

[Broca's area and Broca's aphasia: based on the observations of two cases with the lesions involving Broca's area].

Recently, the relation between Broca's area and Broca's aphasia has come into notice again. We investigated two right-handed patients without Broca's aphasia in spite of the presence of lesions involving Broca's area after hemorrhagic infarction. In addition, the clinical findings, the clinical courses and the sites of lesions of two cases were quite similar as follows. They showed no motor deficits and no other abnormal neurological findings from the beginning of the strokes. From the near muteness in the initial stage, speech began to improve gradually. And then, they demonstrated so called

The efficacy of cueing techniques in Broca's aphasia.

Twenty Broca's aphasia patients were stimulated with four cues in a picture-naming task. Among the severe aphasics in the group, presentation of a word to be imitated was the most effective cue and presentation of the initial syllable of the word ranked second. Sentence completion and printed word cues were equally effective and ranked third. Mild aphasic patients responded equally well to all four classes of cues. Reliability measures indicated that the order of potency of cues for the severe group was stable over time. Oral apraxia did not appear to contribute significantly to the severity o

[Evaluation, treatment and evolution of Broca's aphasia].

This paper presents the evaluation treatment and improvement of a patient with Broca's aphasia. A specific form of speech therapy, based on the peculiarities of the clinical picture of Broca's aphasia, was applied during one year. Our results have shown that this form of therapy leads to a significant improvement of the communicative abilities, even in the cases with severe form of aphasia. The importance of application of the specific methods and therapies for the rehabilitation of aphasic patients is pointed out.

Dissociation of Broca's area from Broca's aphasia in patients undergoing neurosurgical resections.

OBJECTIVE: Broca's aphasia is a syndrome of impaired fluency with retained comprehension. The authors used an unbiased algorithm to examine which neuroanatomical areas are most likely to result in Broca's aphasia following surgical lesions. METHODS: Patients were prospectively evaluated with standardized language batteries before and after surgery. Broca's area was defined anatomically as the pars opercularis and triangularis of the inferior frontal gyrus. Broca's aphasia was defined by the Western Aphasia Battery language assessment. Resections were outlined from MRI scans to construct 3D vo

[Broca's aphasia and apraxia of speech (author's transl)].

The paper shows that the aphasic syndrome originally described by Paul Broca corresponds to what is now called anarthria or apraxia of speech rather than to the nosological entity which nowadays goes by the name of Broca's aphasia. The paper further endeavours to demonstrate that the output difficulties of anarthric patients are different from those of patients with Broca's aphasia, and it suggests a number of criteria to discriminate between anarthria and Broca's aphasia (Acta neurol belg., 1982, 82, 80-90).

Improved naming in patients with Broca's aphasia with tDCS.

BACKGROUND: Language impairment (aphasia) is a common neurological deficit after strokes. For individuals with chronic aphasia (beyond 6 months after the stroke), language improvements with speech therapy (ST) are often limited. Transcranial direct current stimulation (tDCS) is a promising approach to complement language recovery but interindividual variability in treatment response is common after tDCS, suggesting a possible relationship between tDCS and type of linguistic impairment (aphasia type). METHODS: This current study is a subgroup analysis of a randomised controlled phase II futili

Voice rehabilitation of Broca's aphasia following total laryngectomy.

Total laryngectomy, as a consequence of carcinoma of the larynx, results in loss of speech function. Cerebrovascular stroke is the leading cause of reduced speech production ability, and thereby communication difficulties. The case is presented of a 60-year-old male patient who suffered stroke five years after a total laryngectomy. Speech rehabilitation was hampered due to the depressive state of the patient. Although contraindicated, the secondary voice prosthesis was implanted. Only at that moment the patient showed willingness and motivation for speech rehabilitation. The aim of this presen

Acquired dyslexia in Serbian speakers with Broca's and Wernicke's aphasia.

This study examined patterns of acquired dyslexia in Serbian aphasic speakers, comparing profiles of groups with Broca's versus Wernicke's aphasia. The study also looked at the relationship of reading and auditory comprehension and between reading comprehension and reading aloud in these groups. Participants were 20 people with Broca's and 20 with Wernicke's aphasia. They were asked to read aloud and to understand written material from the Serbian adaptation of the Boston Diagnostic Aphasia Examination. A Serbian Word Reading Aloud Test was also used. The people with Broca's aphasia achieved b

Exploring the efficacy of melodic intonation therapy with Broca's aphasia in Arabic.

Even though the efficacy of melodic intonation therapy (MIT) with persons with aphasia (PWA) has been explored in different languages, the efficacy of MIT with Arabic-speaking PWA has never been explored. Aims: To explore the efficacy of MIT, adapted to Arabic, in promoting the expressive abilities of a 70-year-old Jordanian Arabic-speaking male subject with severe Broca's aphasia 3 months post-onset. Methods: An 8-week MIT therapy programme with tapping (1.5 h daily, 6 days a week) was used in a multiple baseline design across two types of trained phrases (i.e. automatic and self-generated ph

Impaired grammar with normal fluency and phonology. Implications for Broca's aphasia.

Extensive testing of grammatical function, including assessment of spontaneous language, inflectional morphology, ability to produce grammatical structures, syntactic comprehension and grammatical judgement, was carried out in 2 patients with large dominant frontal lobe lesions, including but not confined to, the third frontal convolution. Both patients were fluent and had normal articulation and phonological production and neither was agrammatic, suggesting that even very large frontal lesions do not produce Broca's aphasia and that language cortex proper is confined to the postcentral perisy

Electromagnetic articulography treatment for an adult with Broca's aphasia and apraxia of speech.

Electromagnetic articulography (EMA) was explored as a means of remediating [s]/[symbol in text] articulation deficits in the speech of an adult with Broca's aphasia and apraxia of speech. Over a 1-month period, the subject was provided with 2 different treatments in a counterbalanced procedure: (1) visually guided biofeedback concerning tongue-tip position and (2) a foil treatment in which a computer program delivered voicing-contrast stimuli for simple repetition. Kinematic and perceptual data suggest improvement resulting from visually guided biofeedback, both for nonspeech oral and, to a l

[Jargon agraphia in a case with Broca aphasia: dissociation between language and writing dominancy].

A 69-year-old left-handed man developed Broca aphasia and jargon agraphia following a right cerebral infarction. He learned to write with his right hand. There is no family history of left-handedness. Neurological examination revealed hemiparesis, sensory impairment, and hyperreflexia with pathological reflexes on the left side. An MRI demonstrated lesions in the area including the pre- and post-central gyri, the posterior part of the middle and inferior frontal gyri, and inferior parietal lobule on the right side. Neuropsychologically he was alert and cooperative. He demonstrated severe Broca

105 Inappropriate ICD Shocks in a Patient with Dilated Cardiomyopathy and Broca's Aphasia.

With a growing number of ICD recipients, device complications are seen more frequently in the clinical setting and outpatient departments. Among the most severe are ICD infections and inappropriate therapies caused by oversensing of atrial tachycardias or lead fracture. We report on a 76-year-old female patient with dilative cardiomyopathy and Broca's aphasia after stroke, who experienced 105 consecutive inappropriate ICD shocks due to cluster missensing of her fractured ICD lead. The diagnosis was complicated and delayed by patient's aphasia emphasizing the need for intensified remote monitor

Verbal short-term memory and motor speech processes in Broca's aphasia.

The present study investigated the relationship between verbal short-term memory and motor speech processes in healthy control subjects and five patients suffering from Broca's aphasia. Control subjects showed a phonological similarity effect, a word length effect and an articulatory suppression effect, supporting the hypothesis of a phonological store and an articulatory loop component of short-term memory. A similar effect of phonological similarity was observed in the aphasic patients, while the effects of word length and articulatory suppression were reduced. In control subjects, measures
