Aprepitant: Review on Solubility Enhancement.

INTRODUCTION: Substance P and its neurokinin (NK-1) receptors are upregulated in different pathophysiological conditions. Overexpression of the NK-1 receptor in cancer conditions has provided a promising pathway for cancer treatment. Clinically, Aprepitant (APT) is used as the only NK-1 antagonist in chemotherapy-induced nausea and vomiting (CINV). Currently, investigations into using APT as a synergistic combination with radiation or standard chemotherapeutic drugs are underway. However, APT is categorised as a BCS Class IV drug, and therefore, solubility is one of the challenges when it must

Aprepitant for refractory nivolumab-induced pruritus.

Although substantial progress has been made in the treatment of non-small-cell lung cancer (NSCLC) patients with immune checkpoint inhibitors (ICIs), severe immune-related adverse events (irAEs) sometimes occur. Here, we report a case of severe refractory pruritus after Stevens-Johnson syndrome (SJS) in a patient with NSCLC treated with nivolumab. The patient was a 76-year-old Japanese woman with advanced NSCLC treated with nivolumab. After the second dose, she experienced severe rash with mucous involvement. We diagnosed SJS and started 50mg of oral prednisolone (1mg/kg). The rash completely

Emerging Role of Aprepitant in Cannabis Hyperemesis Syndrome.

Cannabis hyperemesis syndrome (CHS) is a clinical syndrome associated with prolonged and regular cannabis use. CHS is characterized by recurrent episodes of intractable nausea and vomiting. Given the overlap with other medical conditions and the frequent delay in diagnosis, finding an effective anti-emetic regimen for symptomatic control of CHS can be challenging. We report a case study where aprepitant (Emend) was successfully used as an anti-emetic in the treatment of CHS when all other common anti-emetics failed.

A pharmacological overview of aprepitant for the prevention of postoperative nausea and vomiting.

INTRODUCTION: Post-operative nausea and vomiting (PONV) affects 30% of all patients undergoing surgery and up to 80% of high-risk patients. Antiemetics for PONV prophylaxis target a variety of receptor systems, with varying degrees of efficacy and side effect profile. Neurokinin -1 receptor antagonists are the most recent class of compounds investigated for PONV prophylaxis, with aprepitant being the only one currently approved for this indication. AREAS COVERED: This review covers the pathophysiology of PONV, current recommendations for PONV prophylaxis, pharmacokinetics, and pharmacodynamic

Clinical interaction between dexamethasone and aprepitant in chemotherapy for lymphoma.

Aprepitant (Apr) is an effective antiemetic agent for chemotherapy-induced nausea and vomiting (CINV). Current CINV guidelines recommend the antiemetic combination of a 5-HT3 receptor antagonist, Apr, and dexamethasone (Dex) for highly emetogenic chemotherapies. Apr inhibits CYP3A4 dose-dependently. Since Dex is metabolized by CYP3A4, the combined use of Apr and Dex inhibits Dex metabolism. CINV guidelines therefore recommend dose-reduction of Dex when Apr and Dex are used together. However, there is some controversy over whether or not Dex should be reduced when administered as an antitumor a

Aprepitant is a novel, selective activator of the K2P channel TRAAK.

TRAAK (KCNK4, K2P4.1) is a mechanosensitive two-pore domain potassium (K2P) channel. Due to its expression within sensory neurons and genetic link to neuropathic pain it represents a promising potential target for novel analgesics. In common with many other channels in the wider K2P sub-family, there remains a paucity of small molecule pharmacological tools. Specifically, there is a lack of molecules selective for TRAAK over the other members of the TREK subfamily of K2P channels. We developed a thallium flux assay to allow high throughput screening of compounds and facilitate the identificati

Aprepitant and fosaprepitant: a 10-year review of efficacy and safety.

Chemotherapy-induced nausea and vomiting (CINV) is a common adverse event associated with anticancer treatment that can have a significant adverse impact on patient health-related quality of life and that can potentially undermine the effectiveness of chemotherapy. Traditional regimens to prevent CINV generally involved a combination of a corticosteroid plus a 5-hydroxytryptamine (5HT3) receptor antagonist (RA). In the past 10 years, antiemetic treatment has greatly advanced with the availability of the neurokinin-1 receptor antagonist (NK1 RA) aprepitant and its prodrug fosaprepitant. NK1 RAs

Spray-freeze-drying aprepitant with hydroxypropyl cellulose increases nasal bioavailability.

The design of free-flowing lyophilized powders by spray-freeze-drying (SFD) enables novel therapeutic applications, including nasal administration. In this study, a non-aqueous SFD approach using dimethyl sulfoxide (DMSO) as the spray solvent was investigated. Low-viscosity hydroxypropyl cellulose grades (HPC-SSL and HPC-UL) were employed as excipients to produce rapidly dissolving, porous, amorphous aprepitant powders at different drug-to-polymer ratios. The resulting spherical particles contained amorphous aprepitant and exhibited sizes between 250 μm and 500 μm. In vitro dissolution at pH 7

Does Preoperative Aprepitant Reduce Nausea and Emesis After Orthognathic Surgery?

Orthognathic surgery has a high rate of postoperative nausea and vomiting (PONV), which amplifies discomfort and delays recovery. Aprepitant, a neurokinin-1 receptor antagonist, has been associated with reduced PONV when used prophylactically, complementing the use of intraoperative and postoperative antiemetics. This retrospective cohort study examines the effect of a single preoperative dose of aprepitant on postorthognathic PONV. The primary predictor was aprepitant usage. The primary outcome was postoperative nausea, with emesis and rescue antiemetic use measured as secondary outcomes. Cov
