Arterio-appendiceal fistula after arterial reconstruction with synthetic graft.

A patient who 5 years previously had an aortoiliac reconstruction with a dacron graft developed intestinal bleeding. At laparotomy this was found to be due to a fistula between a pseudoaneurysm at the right iliac artery anastomosis and the appendix. Resection of the aneurysm and adjacent parts of artery and graft with additional appendectomy prevented further intestinal bleeding. A search of the literature produced only one previously reported case similar to this.

Arterio-venous fistula with pseudoaneurysm of renal artery.

INTRODUCTION: The authors describe a rare but potentially severe, sometimes even life-threatening complication occurring after various invasive renal interventions. The cause that gives rise to a fistula formation between close branches of renal artery and vein is an injury of these vessels during resection of a localised renal tumour, renal biopsy, percutaneous nephrostomy, and even blunt injury of the kidney. Blood flows under pressure from renal artery, pushes the surrounding renal parenchyma open, and creates a cavity varying in size and shape. This pseudoaneurysm may communicate with the

Arterio-oesophageal fistula caused by aberrant right subclavian artery aneurysm.

A 63-year old man had dysphagia for 4 months and was admitted to our hospital with sudden haematemesis. Computed tomography revealed an aberrant right subclavian artery (ARSA) aneurysm and free air inside the aneurysm. Arterio-oesophageal fistula was diagnosed, and an emergency operation was performed. Before thoracotomy, a percutaneous transluminal angioplasty (PTA) balloon was inserted from the right brachial artery and placed at the orifice of the ARSA to control bleeding. Through a left thoracotomy, the aorta was excised and the orifice of the ARSA was exposed. There was no sign of infecti

[Arterial transcatheter embolization in arterio-venous fistulae therapy (aithor's transl)].

In arterio-venous malformations, arterial trans-catheter embolization can be considered as a definitive treatment, a pre-operative devascularizing technique, and a palliative treatment of those not surgically operable malformations. A very important point is choosing the correct way where to introduce the catheter and the embolizing devices. The most known devices are: synthetic fibrin foam (Gelfoam, Spongostan), Gianturco coils, and among the non reabsorbable devices a polyvinylic alcohol foam (Ivalon), the isobutyl-2-cyanoacrylate and silicone. Furthermore detachable balloons and bristle-bru

Coil embolisation of intercostal artery for operative localisation of spinal dural arterio-venous fistulae.

We describe a new method of intra-operative localisation of spinal dural arterio-venous fistulae, involving pre-operative embolisation of the appropriate intercostal artery. This method has significant advantages over other forms of spinal level localisation and has not previously been described.

Jugular venous reflux may mimic type I dural arterio-venous fistula on arterial spin labeling magnetic resonance images.

PURPOSE: Previous studies have shown that arterial spin-labeling (ASL) has high sensitivity and specificity for detecting dural arteriovenous fistulas (DAVFs). However, in case of jugular venous reflux (JVR), the labeled protons in the jugular vein may lead to a venous hypersignal in the jugular vein, sigmoid, and transverse sinus on ASL images and mimic DAVF. METHODS: To ascertain this hypothesis, two blinded senior neuroradiologists independently and retrospectively reviewed randomized ASL images and graded the likelihood of DAVF on a 5-point Likert scale in 2 groups of patients: (i) 13 pat

Detailed ultrasonographic assessment of calcified arteries before arterio-venous fistula construction and fistula outcomes - a prospective observational study.

BACKGROUND: Arterial calcifications (mainly mediocalcinosis) are highly prevalent in patients on hemodialysis. Severe calcifications reduce the possibility of a forearm arterio-venous fistula (AVF) construction, reduce maturation rates and increase the risk of hemodialysis access-induced distal ischemia (HAIDI). We report a prospective observational study on detailed ultrasound assessment of calcified arteries and outcomes of AVFs constructed in those conditions. METHODS: In patients referred for vascular mapping, distal, mid and proximal radial artery (RA) as well as brachial artery were asse

Three Extremely Rare Findings in the Same Patient: Harlequin Syndrome, Thyrocervical Trunk Aneurysm, and Systemic-Pulmonary Arterio-Arterial Fistula.

Harlequin syndrome is a rare autonomic disorder characterized by unilateral diminished sweating and flushing of the face in response to heat or exercise. Extrinsic ganglion compressions, most of the times by neoplasms, can induce the syndrome. During investigation of a 27-year-old woman presenting Harlequin syndrome with diminished sweating on the left side and flushing of the right half of her face, a left thyrocervical trunk aneurysm was detected by angio-magnetic resonance imaging. Thyrocervical trunk aneurysms are rare, and only a few cases have been reported. Treatment is advised due to p

[High flow arterial priapism secondary to post-traumatic bilateral arterio-cavernous fistula. Highly selective embolization with autologous blood clot].

High flow arterial priapism is rare and characterized to a prolonged non-painful partial erection. Arteriocavernous fistula, rarely bilateral, is associated with penile or perineal trauma (genitoperineal area). Review of the literature, in only eleven cases this fistula is bilateral. Different treatments with non common norm has been used. We report a case of a 27 years old male who presented high flow priapism secondary to bilateral postraumatic arterio-cavernous fistula. Autologous blood clot was used for fistulas embolization to minimize the risk of impotence in this young patient. No compl

Arterio-bronchial fistula as a complication of post-traumatic subclavian artery pseudoaneurysm - radiological findings and outcome.

Post-traumatic arterio-bronchial fistula is a rare cause of life-threatening hemorrhage. We present a case of a ten-year-old patient with a post-traumatic subclavian artery pseudoaneurysm complicated by an arterio-bronchial fistula and occlusion of the subclavian artery. The patient was treated with a stent-graft. We present radiological images of the fistula and the evolution of the changes in subsequent follow-up examinations.

The brachial artery-basilic vein arterio-venous fistula in vascular access for haemodialysis--a review paper.

AIMS: To review the available literature regarding patency rates and complications of the brachial-basilic arterio-venous fistula (BBAVF) and to discuss this with relation to the current dialysis outcomes quality initiative guidelines. METHODS: An internet based literature search was performed using Pubmed, Medline and Medscape databases to identify all published reports of the BBAVF in the English language from which the full articles were retrieved and cross-referenced. RESULTS: Of 136 papers identified, 28 were directly relevant to this review including four prospective studies (one rando

Coil embolization of iatrogenic arterio-venous fistula from pulmonary collateral artery to subclavian vein.

A young woman with surgically treated tetralogy of Fallot with pulmonary atresia, with multiple aorto-pulmonary collateral arteries, developed an arteriovenous fistula subsequent to transvenous insertion of an automatic implantable defibrillator via the left subclavian vein. The fistula extended between this vein and a systemic-to-pulmonary collateral artery arising from the subclavian artery. Unilateral peau d'orange, and painful congestion of the left arm and breast, ensued. These were cured by coil embolization of a fistula-related aneurysm.

Congenital coronary arterio-ventricular fistulae, associated with absence of proximal coronary artery from aorta.

A male newborn expired 5 days after birth, because of multiple congenital cardiac anomalies. The postmortem examination demonstrated tricuspid stenosis, pulmonary atresia, hypoplastic right ventricle, persistent venous valve, and so on. In addition, he had coronary arterio-ventricular fistulous connection, associated with absence of the proximal connection of the coronary artery to the aorta or the pulmonary artery. As the result, the coronary blood was totally supplied via the right ventricular-coronary channels. Histologic examination revealed that these arterio-luminal channels were charact

A case of spontaneous haematoma from vertebral artery arterio-venous fistula in a patient with neurofibromatosis type 1.

Neurofibromatosis type 1 (NF1) is an autosomal dominant condition caused by a mutation on chromosome 17. Vascular abnormalities are recognised complications of NF1. These include aneurysms, stenoses, arteriovenous malformations, fistulae, etc. We report the case of a young gentleman with NF1 with a spontaneous cervical bleed from an arteriovenous fistula arising from the left vertebral artery, and illustrate the various management options and difficulties arising from rapid fistula formation.
