Glenn circulation causes early and progressive shunting in a surgical model of pulmonary arteriovenous malformations.

BACKGROUND: Pulmonary arteriovenous malformations (PAVMs) universally develop in patients with single ventricle congenital heart disease (CHD). Single ventricle PAVMs have been recognized for over 50 years, yet they are poorly understood, and we lack any medical therapies. To improve our understanding of single ventricle PAVM initiation and progression, we developed a surgical rat model of Glenn circulation and characterized PAVM physiology over multiple time points. METHODS: Using adult rats, we performed a left thoracotomy and end-to-end anastomosis of the left superior vena cava to the lef

Glenn circulation causes early and progressive shunting in a surgical model of pulmonary arteriovenous malformations.

Pulmonary arteriovenous malformations (PAVMs) universally develop in patients with single ventricle congenital heart disease. Single ventricle PAVMs have been recognized for over 50 years but remain poorly understood. To improve our understanding, we developed a surgical rat model of Glenn circulation and characterized PAVM physiology over multiple time points. We performed a left thoracotomy and end-to-end anastomosis of the left superior vena cava to the left pulmonary artery (unilateral Glenn), or sham surgical control. To assess PAVM physiology, we quantified intrapulmonary shunting using

Cranial dural arteriovenous shunts: selection of the ideal lesion for surgical occlusion according to the classification system.

BACKGROUND: The types of cranial dural arteriovenous fistulae (cDAVFs) that constitute good surgical candidates are unclear despite the use of classifications. We aimed to compare the DES classification with other classification schemes in identifying "ideal lesions for surgery." The DES scheme is based on two features: the level of the shunt (BVS, bridging vein shunt; DSS, dural sinus shunt; ISS, isolated sinus shunt; EVS, emissary vein shunt) and the type of leptomeningeal venous reflux (LVR) (direct, exclusive, strained). METHODS: In this observational cohort study, the angiographies of 20

Rapid onset of intrapulmonary arteriovenous shunting after surgical repair of tetralogy of Fallot with pulmonary atresia.

We describe a 2-year-old girl with tetralogy of Fallot and pulmonary atresia, palliated as a neonate with a right modified Blalock Taussig shunt, who developed severe cyanosis following total correction in the absence of corresponding evidence of parenchymal lung disease on the chest X-ray. Selective pulmonary angiography showed new intrapulmonary shunting involving only the right middle and lower lobes only. The cyanosis resolved rapidly subsequent to inhalation of nitric oxide. To our knowledge, this is the first documented case of rapid onset of localised intrapulmonary right-to-left shunti

Middle meningeal artery arteriovenous shunting after surgical evacuation of chronic subdural hematoma: a retrospective angiographic cohort study.

Arteriovenous shunting involving the middle meningeal artery (MMA) has been reported after cranial trauma, neurosurgical procedures, and endovascular interventions. However, its angiographic frequency and characteristics after chronic subdural hematoma (cSDH) surgery remain poorly defined. This study aimed to assess the frequency and angiographic features of MMA arteriovenous shunting in patients undergoing MMA embolization after surgical evacuation of cSDH. We retrospectively analyzed consecutive patients who underwent selective and superselective MMA angiography during MMA embolization after

Risk assessment of arteriovenous fistulas focusing on the relationships between the properties of shunted blood flow sounds and a medical/surgical history of hemodialysis patients.

The global prevalence of end-stage kidney failure is increasing, with hemodialysis as the primary treatment. An arteriovenous fistula serves as a critical lifeline for patients undergoing hemodialysis, yet its function often deteriorates due to complications such as narrowing or blockage of the blood vessels. This study aimed to clarify the relationships between the sound properties of blood flow through arteriovenous fistulas and a medical/surgical history of patients undergoing hemodialysis by analyzing the distribution of sound frequencies from 100 to 4,000 Hz. Data were collected from 53 p

Epicardial 15-MHz Echocardiography for Effective Repair of Coronary Arteriovenous Fistula.

In the era of catheter intervention, the surgical repair of coronary arteriovenous fistula needs to be free from residual shunt. Intraoperative epicardial 15-MHz echocardiography helps to find the residual shunt after ligation or obliteration of anomalous vessels. Here we report our method of the echo-assisted surgical repair of coronary arteriovenous fistula in 7 adult patients. The method made the operation for coronary arteriovenous fistula free from residual shunt, but care should be taken to the development of new vessels after the operation.

Intraoperative real-time identification of abnormal vessels within the bright field by superselective arterial injection of saline and its slow-motion recording using a high frame rate digital camera during surgical treatment of spinal arteriovenous shunts: technical note.

The authors describe a novel method of observing blood flow in abnormal vessels with slow-motion video during surgical treatment of spinal arteriovenous shunts. The method is based on the use of superselective angiography with saline for visualizing abnormal vessels in bright field and commercially available high frame rate digital camera for recording slow-motion video.

A case of craniocervical junction pial arteriovenous fistula causing postoperative medullary and spinal cord edema.

OBJECTIVES: Pial arteriovenous fistulas (pAVFs) are direct connections between the pial artery and vein without an intervening nidus. We report a rare case of craniocervical junction (CCJ) pAVF causing medullary and spinal cord edema resulting from surgical removal of the varix with remnant shunt after coil embolization. CASE DESCRIPTION: A 16-year-old man presented with subarachnoid hemorrhage. Digital subtraction angiography revealed a CCJ pAVF with multiple fistulas at the 2 varices (varix A and varix B), which was fed by the bilateral lateral spinal arteries and anterior spinal artery (AS

Systemic arteriovenous fistulae for end-stage cyanosis after cavopulmonary connection: a useful bridge to transplantation.

OBJECTIVE: Intractable cyanosis after partial or complete cavopulmonary connection may rarely be managed by creating a systemic arteriovenous fistula. We investigated the long-term performance of arteriovenous fistulae. METHODS: All 21 patients who received an arteriovenous fistula at The Hospital for Sick Children since the 1950s were investigated using parametric competing risk techniques. Primary arteriovenous fistula indication was (1) suboptimal pulmonary blood flow (N = 15) or (2) pulmonary shunting via pulmonary arteriovenous malformations (N = 6). Arteriovenous fistula longevity was d

Surgical Ligation of Bilateral C2 Dural Arteriovenous Fistulae.

Spinal arteriovenous fistulae (AVFs) of the craniocervical junction are rare lesions that have variable angioarchitecture. These lesions are generally characterized by arteriovenous shunting at a single fistulous point, the disconnection of which is generally curative.1 We present a case of bilateral dural AVF at the C2 level treated with surgical ligation (Video 1). Our patient was an elderly woman who presented after cervical magnetic resonance imaging was performed as part of work-up for right-sided shoulder tingling. This study demonstrated prominent flow voids along the dorsal and ventral

[Surgical treatment of intrarcranial dural arteriovenous fistulas].

Surgical treatment is indicated in patients who present clinical symptoms for intracranial dural arteriovenous fistula and retrograde cortical venous filling on cerebral angiography. Further, surgery is indicated in those patients that cannot be cured by endovascular procedures. For dural arteriovenous fistulas in the anterior fossa, tentorium, craniovertebral junction or convex, surgery was considered as the first option. Morphologically, non-sinus dural arteriovenous fistulas and dural arteriovenous fistulas with isolated sinus should be treated with surgery. The surgical procedure for intra

Intra-uterine use of a Foley balloon catheter to tamponade an actively bleeding post-traumatic uterine arteriovenous shunt.

Arteriovenous shunts (AV-shunts) whilst rare, can complicate intrauterine surgical procedures leading to severe vaginal bleeding. We present the case of a 29-year-old woman who experienced sudden, torrential vaginal bleeding 10 days following a hysteroscopic resection and curettage of a placental remnant after vaginal delivery. Ultrasonographic evaluation with power doppler demonstrated a post-traumatic AV-shunt. Tamponading the shunt for 36 hours utilising an intra-uterine inflated Foley catheter successfully controlled the bleeding and led to complete resolution of the AV-shunt. Accurately d

Paraplegia due to posttraumatic pelvic arteriovenous fistula treated by surgery and embolization. Case report.

A case is presented in which a posttraumatic pelvic arteriovenous fistula caused progressive paraplegia because of voluminous shunting into the epidural venous system. Surgical ligation and transcatheter embolization of major and minor arterial feeders decreased shunt flow sufficiently to permit direct embolization of the fistula by an injectable plastic. This combined approach may allow obliteration of unresectable acquired or congenital arteriovenous malformations.

Filum terminale arteriovenous shunt with nidus structure: a report of rare condition and treatment consideration.

BACKGROUND: In the literature, filum terminale arteriovenous shunts (FTAVSs) always feature a single shunt point. Nidus-type FTAVSs have rarely been reported, and the best treatment strategy is unclear. This is a report of one exceptional case of a nidus-type FTAVS and surgical treatment of the lesion. CASE DESCRIPTION: The patient suffered from cauda equina syndrome for 9 months. Magnetic resonance imaging and spinal angiography revealed a nidus-type FTAVF at the L2 level. Surgical resection was performed in the hybrid operating room, and the nidus was completely resected with the assistance

Arteriovenous fistulae of the filum terminale.

Filum terminale arteriovenous fistulae are a rare type of arteriovenous shunt generally characterized by a single direct communication between the artery of the filum terminale and a single draining vein. These intradural arteriovenous shunts are three times more common in men than women (mean age 55 years). Symptoms are related to venous congestion, vascular hypertension, and a putative chronic steal phenomenon which result in spinal cord ischemia and myelopathy. Interestingly, hemorrhage has never been reported as a mode of presentation. MRI demonstrates increased flow voids and T2 changes i

Arterioveneuze Aneurysmata Van De Long.

A case history is presented of a 12 year old girl with pulmonary arteriovenous fistula. The diagnosis was based on clinical and radiological signs and on physiological evidence for a large arteriovenous shunting of blood in the lungs. As an unexpected other finding, diffusing capacity was markedly decreased. The largest radiologically apparent fistula, situated in the right lower lobe, was resected; peroperative clamping of pulmonary arterial blood supply to the affected lobe substantiated the presence of an arteriovenous fistula. Following surgery, arteriovenous shunting was found to be only

Occult Brain Arteriovenous Malformation Superimposed on a Pial Arteriovenous Fistula: Case Report.

INTRODUCTION: Pial arteriovenous fistulas are characterized by an abnormal connection between an intracranial artery and vein without an intervening nidus. Their predominant symptoms largely arise from mass effect, shunting, or hemorrhage. Most conservatively managed cases progress to death, but endovascular and/or surgical intervention is often successful. CASE PRESENTATION: We present the unique case of a 15-year-old girl with spontaneous intracranial hemorrhage from a single-vessel arteriovenous fistula. Although preoperative imaging failed to show a distinct nidus, intraoperative indocyan

Giant Inferior Mesenteric Vein Aneurysm Secondary to Iatrogenic Inferior Mesenteric Arteriovenous Fistula.

PURPOSE: Visceral venous aneurysms are rare, especially in the inferior mesenteric vein (IMV). We report a giant IMV aneurysm secondary to an iatrogenic arteriovenous fistula (AVF). CASE REPORT: A woman presented with an incidental finding of a 7 cm large IMV aneurysm and an inferior mesenteric arteriovenous shunt. The patient underwent successful endovascular occlusion of the shunt to avoid aneurysm rupture and portal hypertension. CONCLUSION: Embolization is a possible treatment strategy for mesenteric venous aneurysms with an AVF. CLINICAL IMPACT: We describe an unusual mesenteric AV-shu
