BACKGROUND: Scapho-trapezio-trapezoid (STT)/triscaphe fusion is used to stabilize the radial column of the wrist. However the reported results are controversial.
MATERIAL AND METHODS: 111 patients were treated with STT fusion from 1992 to 1997. Indications were chronic dissociation of the scapholunate joint (n = 15), idiopathic arthrosis of the scaphotrapeziotrapezoid joint (n = 11), Kienböck's disease in advanced stage (n = 84) and dislocation of the trapezium (n = 1). This study reviews the results after an average follow-up period of four years (range, two to eight years).
RESULTS: Patien

Arthrodesis of the first metatarsophalangeal joint is a powerful procedure that can improve the load-bearing capabilities of the forefoot and assist in medial arch stability. It is the mainstay of treatment for patients with severe arthritic deformity of the great toe joint, because it addresses the importance first ray weight-bearing has on the rest of the foot. In select individuals, fusion can also be effective as a primary procedure in the treatment of hallux valgus. Rather than cause detrimental effects to the function of the foot, this article suggests that first metatarsophalangeal arth

BACKGROUND: Hallux interphalangeal (IP) arthritis can occur after first metatarsophalangeal (MTP) arthrodesis. IP arthrodesis is a standard treatment, but in the setting of prior MTP surgery there will be increased stress on the IP joint. This may result in diminished potential for bone healing. This investigation assessed the outcomes of hallux IP arthrodesis after first MTP arthrodesis.
METHODS: Charts were retrospectively reviewed for patients who underwent interphalangeal arthrodesis between January 1, 2007, and April 3, 2017, and who had a minimum of 12 weeks of follow-up. We compared pa

Naviculocuneiform (NC) arthrodesis is considered a challenging procedure compared to arthrodesis of other medial column joints and/or hindfoot fusions. The objectives of this study were to 1) report the incidence of nonunion and complications after NC arthrodesis with or without adjunct joint arthrodesis, and 2) report incidences in which a fixation method was used and report how many cuneiform joints were fused. A retrospective review of medical charts and radiographs were performed on patients from 1/2013 through 11/2023 at a single institution in the Midwest region. 137 cases included and 9

Non-unions after fracture dislocation of the ankle joint are extremely rare with predominantly operative treatment. In contrast, after fractures of the tibial plafond (pilon fractures) infections are seen in the literature in 37 % and non-unions are seen in 27 % after open reduction and internal fixation, requiring secondary ankle arthrodesis in about one quarter of all cases. In contrast to aseptic non-union or arthrosis, which can be salvaged with screw arthrodesis, with prevailing infection and severe osteoporosis external fixation (either one- or two-sided) is the treatment of choice. A triple arthrodesis is comprised of subtalar, talonavicular, and calcaneocuboid joints arthrodesis. A pantalar arthrodesis is triple arthrodesis combined with tibiotalar arthrodesis. The goal of the procedure is to obtain a correction of deformity and achieve a plantigrade, functional, painless, stable, weightbearing foot that can be used to ambulate. This is done by creating an osseous continuity across the ankle, subtalar, and talonavicular, and calcaneocuboid joints. There are several approaches and fixation strategies that result in successful clinical union and should be chosen to match

Shoulder arthrodesis is an end-stage, salvage procedure for the glenohumeral joint and can provide a pain-free, stable shoulder with varying levels of function. Common indications include brachial plexus injury, chronic instability with rotator cuff and deltoid dysfunction, and failed shoulder arthroplasty. Multiple techniques are described, including intra-articular and extra-articular arthrodeses. Fusion can be accomplished with screw fixation, plate fixation, external fixation, and arthroscopic-assisted techniques. The optimal position of the arm is heavily debated in the literature, but th

The tarso-metatarsal 1 joint (TMT-I) arthrodesis is a treatment option or moderate to severe hallux valgus (HV) deformities. Instability of the TMT1 joint is still a debatable indication. Using stable osteosynthesis techniques allows early postoperative weight bearing. Plantar plating combined with a lag screw is the biomechanical most stable construct. An additional intermetatarsal screw can improve the horizontal stability. Clinical results are good and radiological parameters stay constant, even in the long term.

Lisfranc fracture-dislocations are complex injuries that require a high skill set from foot and ankle surgeons to diagnose and treat. Conservative treatment is seldom an option for treatment of Lisfranc injuries. The authors believe that even subtle injuries require surgical intervention. The comparison between open reduction internal fixation, partial arthrodesis, and complete arthrodesis is discussed, as well as various fixation techniques to accomplish these procedures when approaching a Lisfranc injury.

PURPOSE OF REVIEW: Arthrodesis of the thumb carpometacarpal, metacarpophalangeal, and interphalangeal joints are well-established options in the management of arthritis, instability, and/or deformity. This review includes a general review of the indications and technique of arthrodesis and recent review of the literature.
RECENT FINDINGS: New literature has focused on the role of arthrodesis in managing carpometacarpal arthritis and compared its results with ligament reconstruction and tendon interposition. However, no conclusive evidence demonstrates the superiority of arthrodesis. Several n

Wrist arthrodesis results in a high degree of patient satisfaction and predictable pain relief in most patients. Most patients are able to return to gainful employment, many without impairment. Some patients require restrictions and employment in a less strenuous occupation. Successful fusion rates have been reported in the vast majority of patients overall. Although the functional outcome is acceptable for most patients, some adaptation is necessary, because certain activities such as perineal care and manipulating the hand in tight spaces are difficult. Activities that require forceful grippi

NC arthrodesis is a useful procedure to alleviate pain secondary to arthritis or collapse of the medial longitudinal arch. Fusion of this joint provides stabilization of the medial column and is often used in conjunction with other flatfoot-correcting procedures to restore normal function to the foot. Because the NC joint is the center of the lever arm of the foot and is therefore subject to bending-moment stress, concomitant gastrocnemius recession or tendo-Achillis lengthening is recommended.

Twenty-five adult patients with 32 fused hind-feet were evaluated at an average of 4.3 years after triple arthrodesis. All patients had Stage 3 or 4 deformity secondary to adult acquired flatfoot. All operations were performed using a standardized technique using rigid internal fixation. Postoperative American Orthopaedic Foot and Ankle Society hindfoot scores improved an average of 36 points. There was one nonunion and two residual varus malunions in patients who underwent additional procedures to obtain satisfactory position and fusion. All but one patient were satisfied with the result of t

Arthrodesis of the hindfoot is indicated in congenital foot deformity of young adults, in posttraumatic arthrosis of the subtalar joint following calcaneus fractures, in idiopathic, isolated arthroses of hindfoot joints, in rheumatoid arthritis and in deformities of the longitudinal arch of the foot. Common and useful combinations are the triple-arthrodesis and the arthrodesis of the subtalar and the calcaneocuboid joint, in particular if both joints are affected by calcaneus fractures. Arthrodesis of the subtalar and the talonavicular joint are often performed as isolated procedures. The thre

Lunotriquetral arthrodesis has been recommended for the treatment of disabling pain at the lunotriquetral joint after more conservative measures have failed. We retrospectively analyzed a series of 22 patients treated with a lunotriquetral arthrodesis for presence of fusion, method of fixation, duration of immobilization, carpal alignment, pain relief, and ability to return to work. Fixation with a Herbert screw supplemented with a Kirschner wire (K-wire) was superior to fixation with K-wires alone. Immobilization longer than 6 weeks was superior to immobilization less than 6 weeks. Combined u

Twenty consecutive patients were treated with wrist arthrodesis. Nine patients had rheumatoid arthritis, and eleven patients had a variety of other arthritic conditions. The average follow-up time was 34 months. Clinical examination and roentgenograms showed that eighteen patients had solid fusion of their wrists, with an average of 11 weeks of immobilization. Two patients had delayed union--one of them removed his cast after the operation. No reason for the delayed union was found in the second patient, who had rheumatoid disease. Ultimately, both patients had solid fusions after a total immo

There has been significant enhancement in surgical management of hallux valgus deformity. Recognition of the role of medial column hypermobility has resulted in better functional outcomes with decreased risk of recurrence. Modern techniques have evolved to include enhanced fixation in a move toward minimal postoperative downtime. Evolution to include true triplane correction, including frontal plane derotation of the first ray, has resulted in optimal functional outcomes. The addition of anatomic triplane restoration, enhanced internal fixation, and early return to weight-bearing activities ar
