Acta radiologica (Stockholm, Sweden : 1987)1993eng
The ligaments of the proximal row of carpal bones and the triangular fibrocartilage (TFC) strongly influence the function and stability of the wrist. Injury to the ligaments may result in chronic wrist pain or instability. Wrist arthrography is valuable in the investigation of such damage when surgical intervention is considered and plain radiography is unrewarding. There are also several technical modifications of the standard radiocarpal arthrography available. Owing to the possibility of congenital perforations and degenerative changes in these ligaments the arthrographic findings should be

Arthrography of the elbow is performed infrequently, but may be quite useful in demonstrating the presence, number, and location of intra-articular loose bodies, and in evaluating articular cartilaginous surfaces. Such information may be valuable in the selection of an operative versus a nonoperative course of treatment, and, if surgery is elected, it may be useful to the surgeon in planning the operative approach and in locating the position of all of the abnormalities present. Elbow arthrography is also of occasional value in demonstrating synovial disease, evaluating certain traumatic disor

Wrist arthrography can be helpful in the evaluation of the chronically painful wrist and, more specifically, in visualization of the integrity of the triangular fibrocartilage and interosseous ligaments. To be meaningful, arthrographic findings, i.e., compartment communication, synovial irregularity, tendon sheath and lymphatic visualization, loose bodies, and cartilage abnormalities, must be correlated with clinical history and physical examination.

Ankle arthrography allows radiologists to assess accurately the degree of damage caused by ankle sprains. Ankle sprains are a significant problem to a large number of people, particularly those who participate in sports. Proper care of these ankle injuries requires the combined expertise of many disciplines. Radiologists should be aware of the significance of these lesions and utilize all modalities, including plain films, stress films, and ankle arthrograms, when indicated, to obtain the exact pathologic diagnosis of severe sprains. The primary role of arthrography is to separate those patien

For decades, fluoroscopic arthrography was the only method available to image a joint with contrast enhancement. Advances in CT led to the natural development of CT arthrography. Development of MRI and its capability for multiplanar imaging led to direct magnetic resonance arthrography (MRA). This technique has been performed since 1987 and has surpassed CT arthrography in popularity in the United States. Indirect MRA developed subsequently to offer a less invasive alternative. This article presents an overview of direct MRA and addresses joint-specific issues regarding direct MRA. An overview

The purpose of this study was to determine the accuracy and the specificity of an optimum technique of posterior talocalcaneal/posterior subtalar (PST) joint arthrography and anesthetic injection in patients with hindfoot pain. Fifty-five PST joints were studied in 55 patients. The posteromedial approach was used in the first 24 patients, followed by an anterolateral approach in 31 patients. The ease of performance, success of confirming PST needle position, and adverse effects were noted. After contrast injection, a combination of 1% lidocaine and 0.5% bupivacaine was injected. Results consis

Arthrography is a proven and well-established diagnostic aid in the evaluation and treatment of lesions in several joints; it provides useful information in the treatment of shoulder and wrist lesions. The procedure can be used whenever the cause of shoulder pain is in doubt. Arthrography of the shoulder can aid in the diagnosis of a variety of lesions: rotator cuff tear, subluxation of the shoulder, recurrent dislocation, subluxation and tear of the biceps tendon, the frozen shoulder, anterior capsular derangements, and combinations of these abnormalities. Arthrography has occasional, but def

OBJECTIVE: To compare the diagnostic value of indirect magnetic resonance arthrography (I-MRA) with that of direct MR arthrography (D-MRA) for labral tears, rotator cuff tears, and long head of biceps tendon (LHBT) tears using a 3-T MR unit.
MATERIALS AND METHODS: Institutional review board approval was given; written informed consent was obtained from all patients. From November 2005 to June 2006, 19 patients (eight men and 11 women; mean age, 51 years) who had undergone both I-MRA and D-MRA underwent arthroscopic surgery. Both methods were performed in fat-saturated axial, coronal oblique,

PURPOSE: In glenohumeral instability, CT arthrography and MR arthrography of the shoulder joint were compared to assess accuracy in diagnosis of labral lesions and other internal derangements of the joint, and to evaluate relevance of both imaging methods for therapy.
METHODS: 38 patients with symptoms of shoulder instability were examined clinically, arthrographically with CT and MRI, and arthroscopically. Arthrography with CT and MRI was performed in a double-contrast technique after single puncture and simultaneous injection of the contrast agents for both imaging methods. Type and extent

Magnetic resonance (MR) arthrography has gained increasing popularity as a diagnostic tool in the assessment of intra-articular derangements. Its role has been studied extensively in the shoulder, but it also has been explored in the hip, elbow, knee, wrist and ankle. This article reviews the current role of direct MR arthrography in several major joints, with consideration of pertinent anatomy, techniques and applications.

Lumbar apophyseal (facet) joint arthropathy is an important cause of low back pain and sciatica. Apophyseal joint arthrography sometimes shows "leakage" of contrast medium from the joint capsule. A histological explanation for this "leakage" at the lumbo-sacral apophyseal joint is suggested.

Injuries to the lateral ankle ligaments are extremely common. Many patients with acute injuries to the lateral ligament complex develop chronic ankle instability that requires surgical intervention. MR arthrography of the ankle plays a valuable role in identifying those patients who are most at risk for the development of chronic instability. MR arthrography of the ankle also aids in preoperative planning and in differentiating among other entities that can present as ankle instability.

Magnetic resonance imaging (MRI) of the hip has been valuable in the diagnosis of occult osseous abnormalities and of periarticular soft tissue disorders. MRI has been less useful in the evaluation of acetabular labral tears and other intra-articular abnormalities. Image optimization is more technically challenging in the hip than in smaller joints because the overlying soft tissues are thicker, resulting in decreased spatial and contrast resolutions that may not be adequate to distinguish the acetabular labrum from subchondral bone, articular cartilage, and joint capsule. MR arthrography (MRA

Magnetic resonance (MR) arthrography is possible in any joint in which standard arthrography is performed. The diagnostic potential of this technique is optimized in the assessment of complex anatomical structures and intra-articular abnormalities that are difficult to visualize on conventional MR images. This paper presents the most common applications of MR arthrography in the shoulder, hip, knee, ankle, elbow and wrist, and explores the clinical indications in which MR arthrography adds the greatest benefit compared to standard arthrography and conventional MR imaging. Arthrography of the wrist and hand is easy to perform, but accurate diagnosis requires meticulous technique and thorough knowledge of anatomy, pathology, and imaging principles. Wrist arthrography is usually performed to assess pain or instability after trauma; a complete examination usually requires injection of the RC, midcarpal, and distal radioulnar joints. Abnormalities that can be detected include interosseous ligament tears, capsular tears, triangular fibrocartilage perforations and separations, cartilaginous defects, loose bodies, and synovial abnormalities including adhesive capsuliti

The role of knee arthrography today is demonstrated and technical problems are discussed. Among many variants the position of the patient and the choice of contrast media play a great part in the result of the examination. Mild complications occur in 0.25% of the examinations, severe and life threatening complications are extremely rare. Diagnosis of meniscal lesions is most important for knee arthrography; arthroscopy and arthrography are complementary examinations and not mutually exclusive: combined they achieve an accuracy of 97-98%. In the same way arthrography is able to evaluate condrop

Sources of error in performing and interpreting knee arthrograms are discussed. Normal anatomic structures and technical factors can simulate tears in most areas of both the medial and lateral menisci. These pseudotears, as well as unrecognized true tears, occur more commonly in the posterior horn of the medial meniscus. Inferior meniscal recesses and coronary ligament irregularities of the posterior medial meniscus often represent incomplete tears. Diagnostic, etiologic, and therapeutic aspects of these recesses, or pseudorecesses, are discussed. Excessive intra-articular positive contrast me

OBJECTIVE: We prospectively compared MR imaging, MR arthrography, and CT arthrography to determine the sensitivity of each technique in detecting glenoid labral tears and in determining whether the labrum is detached or degenerated.
SUBJECTS AND METHODS: Thirty patients 19-39 years old (mean, 27 years old) who had either signs and symptoms of shoulder instability or shoulder pain of unexplained origin were referred for diagnostic imaging. Each patient underwent MR imaging, followed by MR arthrography after intraarticular injection of 25 ml of a dilute solution of gadopentetate dimeglumine. Tw
