Patellofemoral arthritis is a relatively common condition, affecting up to 24% of women and 11% of men over the age of fifty-five years who have symptomatic osteoarthritis of the knee. Isolated patellofemoral arthritis is not as prevalent and has been reported to occur in 9% of radiographs of symptomatic knees in individuals over the age of forty years. Approximately 75% of all patellofemoral arthroplasties are done in women, and the increased frequency of patellofemoral disease may be related to the frequency of knee malalignment and dysplasia in women, although some studies have noted no sex

Revision knee arthroplasty is often complicated by the presence of bone loss, making the use of conventional revision implants impossible, due to inadequate bone stock and ligamentous stability. Distal femoral replacement, although traditionally was developed for salvage surgeries for bone tumours, plays an important role in revision surgeries of elderly low-demand adults with marked bone loss and ligamentous instability. Here, we report the case of a 66-year-old female, with a history of bilateral knee replacement 5 years before the presentation, who developed left knee pain, with X-ray featu

There is increasing interest in the use of patellofemoral joint replacements and the cohort receiving them are the youngest of any of the groups of patients undergoing a knee arthroplasty. With more contemporary prostheses, progression of arthritis in other parts of the knee joint is becoming the predominant mechanism of failure. We conducted a multicenter retrospective comparative cohort study to discover whether the outcome of total knee replacement is compromised by prior patellofemoral joint arthroplasty. A total of 21 patients with a mean age of 61 years, who were revised from a patellofe

OBJECTIVE: The aim of this study was to determine which type of knee arthroplasty is easier to forget by comparing levels of joint awareness evaluated with the Forgotten Joint Score (FJS-12) after unicondylar versus total knee arthroplasty.
METHODS: Patients who underwent either unicondylar or total knee arthroplasty due to primary gonarthrosis were retrospectively identified and then divided into 2 groups: the TKA group (218 patients; mean age = 68.93 ± 7.14 years) and the UKA group (131 patients; mean age = 60.39 ± 7.03 years). The status of joint awareness after knee replacement surgery wa

OBJECTIVE: To evaluate the influence of patellar replacement on total knee arthroplasty by comparing with non patellar replacement.
METHODS: Between September 2010 and November 2010, 63 patients (63 knees) with osteoarthritis who met the selection criteria and underwent total knee arthroplasty, were randomly divided into 2 groups: patellar replacement in 32 cases (replacement group), non patellar replacement in 31 cases (non patellar replacement group). There was no significant difference in gender, age, disease duration, osteoarthritis grading, the clinical and functional scores of American

BACKGROUND: Bilateral total knee arthroplasty (BTKA) under the same anesthesia (simultaneous) or staged are options for patients with end-stage arthritis of the knee that carries advantages and limitations. Not all patients are candidates for simultaneous BTKA, and therefore, surgeons prefer to stage the two TKAs. The optimal safe interval between two TKAs is not known. The present systematic review aimed to determine the optimal time interval between the two stages of BTKA.
METHODS: Pubmed and Scopus databases were searched to identify publications from January 1979 to November 2017 in Engli

BACKGROUND: Failure of knee arthroplasty can lead to significant pain and functional disability. In a small number of patients with fracture, sepsis or massive bone loss they are considered for arthrodesis or above knee amputation. The purpose of this study was to investigate the results and complications of using customised hinged knee replacements in their place.
METHODS: Since 1993, 30 of the 280 (10.7%) revision knee replacements in the Avon Orthopaedic Centre, Bristol have used Endo customised hinged knee prostheses (21 rotating, nine fixed) for salvage of limb threatening situations in

INTRODUCTION: Distal femoral replacement (DFR) revision surgery represents significant cost to the healthcare systems as well as significant patient morbidity/mortality. A retrospective review of revision knee surgery with DFR would elucidate a treatment algorithm for patients with complex reconstructive issues. In this study, we review patients' health metrics to better understand indications and potential complications for DFR revision. The data can then provide insight to specific patient factors that may put patients at higher risk for revision surgery.
METHODS: Seventy-one patients who u

INTRODUCTION: Total knee arthroplasty is a common orthopaedic procedure in the UK; consequently, revision surgery and periprosthetic fractures are increasing in incidence. Strategies for management of these cases include non-operative strategies, internal plate fixation and revision of the distal femoral component. One under-reported practice is to perform distal femoral replacement in cases with poor distal femoral bone stock.
MATERIALS AND METHODS: The department's electronic database was searched for all patients undergoing revision of total knee arthroplasty. From these, all patients havi

Total knee arthroplasty, or replacement (TKR), is now the most commonly performed surgical procedure performed in adults with haemophilia. It is indicated when end-stage haemophilic arthropathy results in intractable pain and reduced function. In patients with haemophilia, however, there has always been a concern about the high risk of infection, which carries with it potentially catastrophic consequences. The aims of this study were to review the case series of TKR for haemophilic arthropathy published in the medical literature, comparing the published infection rates and the differing clotti

Managing the patella and balancing the patellofemoral joint space is one of the most difficult aspects of performing a primary total knee replacement (TKR). The situation is compounded in the revision situation. Unfortunately, an otherwise well-performed TKR will fail because of problems with the patella or the extensor mechanism. This article discusses various aspects of the management of the patella and extensor mechanism during revision TKR with an objective of minimizing complications and maximizing functional outcomes. The topics covered include exposure of the patella and extensor mechan

BACKGROUND AND PURPOSE: Lateral knee replacement is not commonly performed. Possible explanations are the lower incidence of lateral knee osteoarthritis, relative technical complexity, and different biomechanics, which may affect failure modes and survival rates. We aimed to analyze the difference in risk of revision between lateral and total knee replacements (TKRs) using the Dutch Arthroplasty Register data.
METHODS: From 2007 to 2021, procedures were selected from the Dutch Arthroplasty Register. Unique patients based on their first procedure were identified. Procedures performed between 2

Fifty-eight osteoarthritic and thirty-one rheumatoid patients underwent modular total knee-replacement arthroplasty. The major indication for the operation was relief of pain. Contraindications to this resurfacing arthroplasty included varus-valgus instability of over 20 degrees, combined varus-valgus instability with flexion contracture of over 40 degrees, marked recurvatum, and predominant patellofemoral symptoms. In 59 per cent of the osteoarthritic and 58 per cent of the rheumatoid patients, complete relief of pain was evident when they were evaluated twenty-four months after surgery, whil

Catastrophic implant failure is a rare complication after total knee arthroplasty (TKA) that can lead to metallosis and pseudotumour formation requiring complex revision surgery. This case report describes a man in his 60s who presented with progressive knee pain, a squeaking implant and ligamentous instability 20 years post-primary TKA. Imaging demonstrated polyethylene failure, tibial baseplate breakage, extensive osteolysis and metallosis. The patient underwent a revision procedure with a distal femur replacement (DFR). Postoperatively he had no complications, re-admissions or revision surg

UNLABELLED: Revision total knee arthroplasty can be considerably more difficult in cases involving severe bone loss, complete absence of collateral ligaments, and persistent periprosthetic supracondylar femoral fractures. Modular segmental endoprosthetic distal femur replacement is a limb salvage option when other surgical options are unfeasible. The clinical performance of rotating hinge knee prostheses has greatly improved with the evolution of second and third generation designs. The increased freedom of rotation decreases the prosthetic bone stresses and the longevity. While designs have i

Tibial post failure is a rare but serious complication of posterior-stabilized total knee arthroplasty that requires revision surgery. Although tibial post fracture has previously been reported, this case involves an implant with a design feature that may predispose patients to the complication. The fracture also occurred later than observed in most other reports. A 72-year-old male who had undergone a posterior stabilized total knee arthroplasty seven years prior presented with knee pain and instability after a fall from standing. Although plain radiographs were not diagnostic, history and ph

BACKGROUND: Distal femoral replacement (DFR) is a salvage option for complex primary and revision total knee arthroplasty (TKA). Although excellent immediate fixation and weight-bearing are achieved, there is a paucity of data on long-term outcomes of TKA with DFR. The purpose of the present study was to determine implant survivorship, clinical outcomes, and radiographic results of TKAs with contemporary DFR components in a large series.
METHODS: We identified 144 consecutive TKAs performed with DFR for non-oncologic indications from 2000 to 2015 at a single academic institution. Indications

A multi-centre clinical trial of ICLH (Freeman-Swanson) arthroplasty has been in progress since 1971. In this paper the results up to two years after operation are reported in seventy-one knees displaying at least 30 degrees of fixed flexion, 25 degrees of valgus or 20 degrees of varus, before operation. It has been found that knees displaying 70 degrees of fixed flexion, 70 degrees of valgus, 30 degrees of varus or 50 degrees of valgus/varus instability can be satisfactorily aligned and stabilised with acceptable function. Three knees required revision. The other complications are listed and

BACKGROUND: The primary aim of this study is to identify independent preoperative predictors of outcome and patient satisfaction for the second total knee arthroplasty (TKA).
METHODS: A retrospective cohort of 454 patients undergoing an asynchronous (6 weeks or more apart) bilateral primary TKA were identified from an arthroplasty database. Patient demographics, comorbidities, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and Short Form-12 scores were collected preoperatively and 1 year postoperatively. Overall patient satisfaction was assessed at 1 year.
RESULTS: T

Use of testosterone replacement therapy (TRT) has increased significantly in the last few years and has been linked to tendon ruptures after a number of orthopedic procedures. Knee extensor mechanism disruption (EMD) after total knee arthroplasty (TKA) leads to significant morbidity and a decline in patients' quality of life. However, its association with TRT use remains unclear. We aimed to determine the association between TRT and the risk of EMD in patients undergoing primary TKA. This retrospective cohort study utilized the Merative MarketScan database to identify adults aged ≥ 18 years wh
