Accuracy of the Subchondral Smile and Surface Referencing Techniques in Reverse Shoulder Arthroplasty.

Inferior glenoid baseplate tilt relative to the coronal axis of the scapular body has been associated with improved results and fewer postoperative complications in reverse shoulder arthroplasty. However, the native glenoid surface is not always a reliable reference for the true scapular axis. Digital preoperative planning software and advanced imaging now allow surgeons to more precisely determine optimal glenoid placement. The purpose of this study was to evaluate the accuracy of the subchondral smile and cannulated surface guide techniques in achieving inferior glenoid baseplate tilt by usi

Cementless Hip Arthroplasty in Patients with Subchondral Insufficiency Fracture of the Femoral Head.

BACKGROUND: Subchondral insufficiency fracture of the femoral head (SIFFH) occurs in elderly patients and might be confused with osteonecrosis of the femoral head (ONFH). Subchondral insufficiency fracture of the femoral head is an insufficiency fracture at the dome of the femoral head and has been known to be associated with osteoporosis, hip dysplasia, and posterior pelvic tilt. This study's aims were to evaluate (1) surgical complications, (2) radiological changes, (3) clinical results, and (4) survivorship of THA in patients with SIFFH. METHODS: From November 2010 to June 2017, 21 patient

Functional improvement of unicompartmental knee arthroplasty compared with total knee arthroplasty for subchondral insufficiency fracture of the knee.

Subchondral insufficiency fracture of the knee (SIFK) causes acute knee pain in adults and often requires surgical management. Unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA) are the two most common surgical treatments for SIFK. While both UKA and TKA have their advantages, there is no consensus for SIFK localized on the medial compartment. We hypothesized that patients with SIFK treated with UKA would show superior patient-reported outcomes compared to those who underwent TKA. A total of 90 patients with SIFK located medially were included in the TKA (n = 45) and UK

Post-arthroscopic Subchondral Insufficiency Fractures of the Knee Yield High Rate of Conversion to Arthroplasty.

PURPOSE: To evaluate both the potential causes and resultant outcomes in patients in whom subchondral insufficiency fracture of the knee (SIFK) develops after arthroscopy. METHODS: We performed a retrospective review of all patients with a magnetic resonance imaging diagnosis of SIFK after arthroscopic meniscectomy and chondroplasty over a 12-year period. RESULTS: A total of 28 patients were included, with a mean age of 61 years and mean follow-up period of 5.7 years. SIFK showed a predilection for the medial compartment (n = 25, 89%), specifically the medial femoral condyle (n = 21, 75%). I

Subchondral insufficiency fracture of the femoral head after total knee arthroplasty in a patient with rheumatoid arthritis.

We report an 80-year-old woman with rheumatoid arthritis (RA) who was found to have subchondral insufficiency fracture of the right femoral head after total knee arthroplasty (TKA). Initially, plain radiographs showed no obvious changes, but magnetic resonance imaging (MRI) revealed an irregular, discontinuous, low-intensity band on T1-weighted images of the right hip. She underwent hemiarthroplasty of the hip. This report describes a rare case of subchondral insufficiency fracture of the femoral head after TKA in a patient with RA.

Subchondral Insufficiency Fracture of the Femoral Head in a Terminal Cancer Patient Treated by Total Hip Arthroplasty.

INTRODUCTION: Proper fracture management is crucial for maintaining quality of life in cancer patients with metastatic bone lesions. Subchondral insufficiency fracture (SIF) of the femoral head can lead to severe pain and functional decline. When accompanied by metastatic acetabular lesions, implant stability becomes difficult to achieve, making surgical decision-making highly complex, particularly in terminal cancer patients with limited life expectancy. CASE REPORT: We report an 80-year-old woman with terminal lung adenocarcinoma who developed a SIF of the femoral head and an acetabular met

Total Hip Arthroplasty is the Most Effective Treatment for Atraumatic Subchondral Insufficiency Fractures of the Femoral Head.

BACKGROUND: Subchondral insufficiency fracture of the femoral head (SIFH) is a relatively newly used diagnosisfollowing an insidious onset of hip pain in individuals without apparent antecedent trauma. The aim of this study was to investigate the outcomes of a consecutive series of patients with SIFH treated at one institution. METHODS: A total of 93 patients aged 23 to 91 years who were treated for SIFH in the outpatient clinics at one academic, urban medical center between August 2012 and August 2019 were identified. The diagnosis was made by magnetic resonance imaging or radiograph of the

Spontaneous Osteonecrosis/Subchondral Insufficiency Fractures of the Knee: High Rates of Conversion to Surgical Treatment and Arthroplasty.

BACKGROUND: Spontaneous osteonecrosis of the knee has recently been termed subchondral insufficiency fracture of the knee (SIFK) to appropriately recognize the etiology of mechanical overloading of the subchondral bone. The purpose of this study was to assess clinical outcomes of SIFK based on progression to surgical treatment and arthroplasty, and to evaluate the risk factors that increase the progression to arthroplasty. METHODS: A retrospective review was performed on patients with a diagnosis of SIFK, as confirmed with use of magnetic resonance images (MRIs). Baseline and final radiograph

Subchondral stem cell therapy versus contralateral total knee arthroplasty for osteoarthritis following secondary osteonecrosis of the knee.

PURPOSE: Total knee arthroplasty (TKA) implanted in patients with secondary osteonecrosis (ON) related to corticosteroids have relatively poor outcome (20% revision rate) at a mean follow-up of only eight years. With the hypothesis that subchondral bone marrow injection might improve knees in these patients, we evaluated 30 patients who had bilateral knee osteoarthritis with severe joint space narrowing and received TKA in one knee and subchondral bone marrow concentrate injection in the contralateral knee. MATERIAL AND METHODS: A prospective randomized controlled clinical trial was carried o

Free subchondral screws in tibial plateau fractures: do they hinder a total knee arthroplasty? Strategies to prevent complications.

Open reduction and internal fixation of extended lateral column tibial plateau fractures through a tibial condyle osteotomy and limited arthrotomy with the use of free subchondral locking screws is a straightforward and safe technique. However, these free subchondral screws are enclosed in the subchondral bone and therefore virtually impossible to remove after bone healing. The question arises whether these free subchondral screws might hinder a future total knee arthroplasty. In order to refute this, we retrospectively reviewed all surgically managed tibial plateau fractures in our tertiary c

Moderate-to-Severe Varus Deformity Is Associated With Conversion to Arthroplasty in Patients Who Have Subchondral Insufficiency Fracture of the Knee.

BACKGROUND: Although untreated subchondral insufficiency fractures of the knee may result in progression of osteoarthritis, leading to arthroplasty in approximately 30% of cases, the factors associated with the requirement for arthroplasty remain unknown. We hypothesized that varus deformity would be associated with increased risk of nonoperative treatment failure and conversion to arthroplasty in patients who have subchondral insufficiency fractures with early osteoarthritis. METHODS: A retrospective cohort study was conducted on 162 patients aged greater than 60 years diagnosed with subchon

Risk of Arthroplasty in Patients with Subchondral Insufficiency Fractures of the Knee: A Matched Study of the Implantable Shock Absorber using a Validated Predictive Model.

Subchondral insufficiency fractures of the knee (SIFK) can result in high rates of osteoarthritis and arthroplasty. The implantable shock absorber (ISA) implant is a titanium and polycarbonate urethane device which reduces the load on the medial compartment of the knee by acting as an extra-articular load absorber while preserving the joint itself. The purpose of this study was to evaluate whether partially unloading the knee with the ISA altered the likelihood of progression to arthroplasty utilizing a validated predictive risk model (SIFK score). A retrospective case-control (2:1) study was

Subchondral drilling for chondral flaps reduces the risk of total hip arthroplasty in femoroacetabular impingement surgery at minimum five years follow-up.

INTRODUCTION:: The best treatment of acetabular chondral flaps during surgery for femoroacetabular impingement (FAI) is unknown. We asked if subchondral drilling improves clinical and radiographic outcome and if there are factors predicting failure. METHODS:: We treated 79 patients with symptomatic FAI and acetabular chondral flaps with surgical hip dislocation between January 2000 and December 2007. Exclusion of all patients with previous hip pathology or trauma resulted in 62 patients (80 hips). The chondral flap was slightly debrided in 43 patients/51 hips (control group). In 28 patients/2

Subchondral Vertebral Body Bone Marrow-Derived Mesenchymal Stem Cells Injections Combined With Lumbar Disc Arthroplasty for Vertebrogenic and Discogenic Pain: A Novel Case Report.

BACKGROUND: Chronic axial low back pain is often multifactorial, involving overlapping facetogenic, discogenic and vertebrogenic pain generators. Although several treatment modalities exist for clear-cut cases in which the primary pain generator is well defined, optimal management for patients with mixed pathology remains unclear. CASE REPORT: In this case, a 27-year-old man presented with refractory axial low back pain and severely limited range of motion associated with Modic type 1 endplate changes and discogenic pain at L3-4 and L5-S1 confirmed by provocative discography. Distraction arthroplasty combined with autologous bone grafting for diffuse-type tenosynovial giant cell tumour with articular cartilage defect and subchondral bone cysts: A case report.

Tenosynovial giant cell tumour (TGCT) encompasses a group of lesions that present with synovial differentiation and most commonly occur in the joint synovium, bursae, and tendon sheaths. Diffuse-type TGCT (Dt-TGCT), previously known as pigmented villonodular synovitis, is one of the most common benign soft-tissue tumours of the foot and ankle and usually affects young adults. The differential diagnosis of Dt-TGCTs remains a clinical problem because their clinical symptoms are similar to those of inflammatory arthritis, including rheumatoid arthritis. Moreover, persistent Dt-TGCTs can lead to a

Positive effect of removal of subchondral bone plate for cemented acetabular component fixation in total hip arthroplasty: a randomised RSA study with ten-year follow-up.

We hypothesised that the removal of the subchondral bone plate (SCBP) for cemented acetabular component fixation in total hip arthroplasty (THA) offers advantages over retention by improving the cement-bone interface, without jeopardising implant stability. We have previously published two-year follow-up data of a randomised controlled trial (RCT), in which 50 patients with primary osteoarthritis were randomised to either retention or removal of the SCBP. The mean age of the retention group (n = 25, 13 males) was 70.0 years (sd 6.8). The mean age in the removal group (n = 25, 16 males) was 70.
