Predictors of functional impairment in residents of assisted-living facilities: the Maryland Assisted Living study.

BACKGROUND: Assisted living is a popular residential option for older individuals, yet little research has been done on people choosing this option. This study examines predictors of functional impairment in assisted living residents in the domains of cognition, mood, and health. METHODS: An experienced team of neuropsychiatrists, nurses, and technicians using a number of cognitive, behavioral, health, and functional status tests and a cross-sectional study design assessed 198 residents of 22 assisted living facilities in Maryland. Data from these evaluations were used in univariate and multi

Prevalence and persistency of deficiency citations in Florida assisted living facilities.

BACKGROUND: Assisted living facilities (ALFs) are plagued with quality issues but there are limited studies on the quality of ALFs. In Florida, state surveyors conduct inspections of ALFs and cite and/or fine facilities that are in violation of regulations. Yet, we do not know the types of quality problems identified and the extent to which facilities repeat such problems. In this study, we begin to fill this gap in our understanding of ALFs quality by summarizing the deficiency citations in Florida ALFs. METHODS: We obtained inspection citation data on 957 large ALFs (bed size ≥ 25) in Flori

Predictors of caregiver unawareness and nontreatment of dementia among residents of assisted living facilities: the Maryland Assisted Living Study.

OBJECTIVE: Assisted living (AL) is a rapidly expanding residential option for the senior population. With increased utilization, it becomes important to understand the detection and treatment of dementia in this setting, but little is known. The objective of this study was to identify and evaluate factors associated with caregiver unawareness of dementia and failure to treat dementia in AL. METHODS: The setting was a cross-sectional study of a random sample of AL facilities in central Maryland (The Maryland Assisted Living Study). Geriatric psychiatrists evaluated 198 participants and assigne

The public health nursing role in elder neglect in assisted living facilities.

OBJECTIVE: Assisted living facilities (ALFs) are a highly unregulated segment of health care serving a large group of vulnerable elders. The purpose of this study was to examine the scope of neglect and neglect-related outcomes in ALFs and to determine whether citations given by state inspectors for certain institutional practices and staff inadequacies were associated with neglect. DESIGN AND SAMPLE: Exploratory descriptive. Citations given during routine inspections and narrative reports of complaint investigations written by Arizona state surveyors in 165 ALFs. MEASURES: Database that inc

Conflict resolution styles: a comparison of assisted living and nursing home facilities.

In this exploratory study, the authors investigated how interpersonal conflict is resolved in assisted living and nursing home facilities. In particular, the authors examined whether conflict resolution styles differed between type of facility and between residents and staff in each type of facility. Four focus groups were conducted--two with residents and two with staff from each type of facility. The focus groups centered on discussing the occurrence of conflict and how each participant handled it. Discourse analysis was employed to identify participants' use of three styles of conflict reso

Loneliness, social inactivity and social isolation of older adults in assisted living facilities.

OBJECTIVE: Health and well-being of older adults living in long-term care are affected by loneliness, social inactivity, and social isolation. We 1) investigated the prevalence and overlapping of concepts of loneliness, social inactivity, and/or social isolation in assisted living facilities (ALFs) and 2) compared residents' characteristics and psychological well-being (PWB) in three groups (Gr): Gr0: not lonely, socially inactive, or socially isolated; Gr1: lonely; and Gr2: not lonely, but socially inactive and/or socially isolated. METHODS: A cross-sectional survey including older adults (n

Examining family and facility characteristics associated with family satisfaction in assisted living: the Minnesota Assisted Living Report Card project.

BACKGROUND AND OBJECTIVES: Family members play an essential role in monitoring and advocating for quality of care in assisted living (AL) communities. Despite their importance, few population-based studies have examined how family and facility characteristics influence family satisfaction in AL. We evaluated family and facility characteristics associated with family satisfaction in AL. METHODS: Using cross-sectional survey data from the Minnesota Assisted Living Report Card project (2024), we identified resident- and facility-level predictors of AL resident-reported quality of life in 8 domai

Preliminary evaluation of medication therapy management services in assisted living facilities in rural Minnesota.

OBJECTIVES: While pharmacists are providing medication therapy management (MTM) services, few programs have been assessed for outcomes, particularly those in rural areas. The objectives are to: 1) categorize the number and type of drug-therapy problems (DTPs) that were identified by consultant pharmacists in assisted living facilities, 2) assess resident and administrator satisfaction of consultant pharmacist MTM services, and 3) assess the direct costs of providing MTM services and make some preliminary inferences about the program's economic viability. DESIGN: We conducted a cross-sectional

Infection prevention and control standards in assisted living facilities: are residents' needs being met?

BACKGROUND: Assisted living facilities (ALFs) provide housing and care to persons unable to live independently, and who often have increasing medical needs. Disease outbreaks illustrate challenges of maintaining adequate resident protections in these facilities. OBJECTIVES: Describe current state laws on assisted living admissions criteria, medical oversight, medication administration, vaccination requirements, and standards for infection control training. METHODS: We abstracted laws and regulations governing assisted living facilities for the 50 states using a structured abstraction tool. S

Efficacy and safety of donepezil in patients with Alzheimer's disease in assisted living facilities.

The aim of this 12-week, open-label study was to determine the safety and efficacy of donepezil in participants with Alzheimer's disease (AD) residing in assisted living facilities (ALFs). Participants received 5 mg donepezil daily for 6 weeks followed by 10 mg daily for 6 weeks. Primary and secondary outcomes were change from baseline in Mini-Mental State Examination (MMSE) and Neuropsychiatric Inventory 8 (NPI-8) scores, respectively. Safety was assessed by adverse events (AEs) and laboratory tests. Of the 97 participants, 76 completed the study. Mean MMSE score (18.7 at baseline) improved 1

The Embedded Model for Bridging Essential Delivery of Care in Assisted Living Facilities-EMBED-ALF.

Older adults residing in assisted living facilities (ALFs) often face fragmented medical care, especially when managing complex chronic conditions, cognitive impairment, or end-of-life needs. This article describes an interprofessional provider-led, integrated care model embedded within an ALF in a major metropolitan area that includes memory care and home hospice. The model integrates geriatrics and palliative medicine-trained providers into routine care, supported by structured communication, tele-emergency services, and collaboration with hospice and hospital teams. While formal outcomes ar

Sleep in assisted living facility residents versus home-dwelling older adults.

BACKGROUND: Sleep problems among assisted living facility (ALF) residents are not well understood, and sleep-related differences between ALF residents and home-dwelling older adults have not been examined. METHODS: We compared sleep patterns in 19 ALF residents to sleep patterns in 19 matched home-dwelling older people (age > or =65 years). All were participating in the follow-up portion of a longitudinal study of sleep and functional outcomes following post-acute rehabilitation. Sleep was assessed with the Pittsburgh Sleep Quality Index and 1 week of wrist actigraphy. RESULTS: By actigraphy

Associated factors with antipsychotic use in assisted living facilities: a cross-sectional study of 4367 residents.

BACKGROUND: antipsychotics are widely used in assisted living (AL facilities). Even more, the prescription of these drugs is gradually increasing since the availability of second-generation atypical antipsychotics. More knowledge is needed on prescription reasons to understand this increasing prevalence. METHODS: cross-sectional analysis of 4,367 residents. Data were obtained from medical records assessed by geriatricians from the AL facility. A multiple logistic regression model (backward stepwise) was used to assess the independent associated factors with antipsychotic use. RESULTS: antips
