Replacement of missing teeth in the posterior maxilla is always a challenge for the treating implant surgeon as the posterior maxilla has several obstacles in the form of quality, quantity, the anatomy of the maxillary sinus, and inaccessibility. To overcome these deficiencies, several surgical procedures such as sinus lift, bone augmentation, tilted implants, short implants, and zygomatic implants were tried. Since these procedures have their own limitations, pterygomaxillary region provides us an excellent place for placement of implant and rehabilitation of posterior maxilla. This case repo

This technique is an adaptation of the double-flask technique that is well-known for the fabrication of the hollow bulb portion of a maxillary obturator. It can be used only for patients with both an atrophic maxillary alveolar ridge and a greater than usual interocclusal distance. Anything less will not permit fabrication of a hollow denture.

INTRODUCTION: Extreme maxillary atrophy results from partial or total maxillary bone defect, affecting the alveolar and basal segments. The residual bone is only a few millimeters high and does not allow retention of dental prostheses (complete edentulation), or placing implants. Bone reconstruction with cranial bone grafts usually allows obtaining enough bone volume in a single step to place implants for prosthetic rehabilitation, 3 months later.
TECHNICAL NOTE: Reconstruction begins by inserting bone grafts on the sinus floor. The pre-maxilla is then rebuilt with bone grafts placed on the n

We have evaluated the survival of dental implants in extremely atrophic edentulous maxillas after autogenous bone grafting. We compared two techniques: the Le Fort I down-grafting procedure and conventional raising of the sinus floor with onlay bone grafting. Twenty-seven consecutive patients had their atrophic maxillas reconstructed, 10 of whom were treated with a Le Fort I osteotomy with down-grafting and interpositioning of bone, and 17 by raising the sinus floor and onlay grafts. All implants were placed 5-6 months after grafting. There was no difference in the survival of implants between

The purpose of this article is to review the literature published and to assess the success of treatment of patients with atrophic posterior maxilla with pterygoid implants. Studies from 1992 to 2009 on patients with atrophic posterior maxilla rehabilitated with pterygoid implants were reviewed. Those reporting clinical series of at least 5 patients with atrophic posterior maxilla (Class IV and V of Cawood and Howell), rehabilitated with pterygoid implants and fixed prosthesis, and with 12 months minimum follow-up were included. In each study the following were assessed: number of patients, nu

Reconstruction of the atrophic maxilla in a young adult presents unique challenges. This article describes reconstruction in a 30-year-old patient using a two-stage procedure. Bilateral maxillary sinus augmentations with simultaneous corticocancellous grafting to the anterior maxilla and alveolar ridge were performed. Eight endosseous implant fixtures were subsequently placed into the maxilla after a five-month healing phase. Six months later the implants were uncovered, healing abutments placed and a maxillary vestibuloplasty performed. The rationale behind this treatment and a review of the

Restoring the dentition of an edentulous patient is often challenging. Endosseous dental implants have allowed for far more versatility in this area but still require adequate maxillary and mandibular alveolar bone. Often, unless significant bone grafting techniques are used, true restoration of the dentition can be impossible with traditional endosseous implants. The advent of zygomatic implants, however, may provide a viable, predictable, and stable alternative for the restoration of the dentition in patients with severe maxillary alveolar bone loss.

INTRODUCTION: Zygomatic implants are a good rehabilitation alternative for upper maxilla with severe bone reabsorption. These implants reduce the need for onlay-type bone grafting in the posterior sectors and for maxillary sinus lift procedures - limiting the use of bone grafts to the anterior zone of the upper jaw in those cases where grafting is considered necessary.
OBJECTIVE: To evaluate the survival of 101 zygomatic implants placed in upper maxilla presenting important bone reabsorption, with a follow-up of 1-72 months.
PATIENTS AND METHODS: A retrospective study was made of 101 Zygoma(

Postoperative cosmetic results in soft tissue after combined surgical and prosthetic treatment of the extremely atrophic edentulous maxilla by maxillary advancement, sinus lift, and insertion of enosseous implants are rarely considered during preoperative planning. In a prospective study that began in 1993 in 23 patients, the treatment concept was determined by a medical rapid prototyping model and video imaging to predict the soft tissue profile, taking into consideration the appearance of the front teeth. In eight patients, the surgical and/or the prosthetic concept was modified according to

As life expectancy increases, a larger number of elderly people require dental health care attention for implant-supported rehabilitation, with the aim of restoring the function and aesthetics of the oral cavity. Most of these patients have lost their teeth long time ago, causing a severe bone resorption and maxillary sinus pneumatization. Therefore, the current study aims to demonstrate, through the description of the clinical case, the treatment with zygomatic implants as an option for treating severely atrophic maxillas. In this clinical study, the patient presented, in the clinical and ima

BACKGROUND: Pre-implant augmentative surgery is a prerequisite in many cases in the anterior maxilla to achieve a stable, long-term esthetic final result.
PURPOSE: The aim of the present study was to evaluate the outcome of ridge augmentation with cancellous freeze-dried block bone allografts in the anterior atrophic maxilla followed by placement of dental implants.
MATERIALS AND METHODS: Thirty-one consecutive patients were included in the study. A bony deficiency of at least 3 mm horizontally and up to 3 mm vertically according to computerized tomography (CT) served as inclusion criteria.

PURPOSE: This study aimed to describe the clinical case of implant placement in the nasopalatine canal as an aid for atrophic maxilla rehabilitation.
CASE REPORT: The surgical procedure was carried out in 2 stages, the first part consisted of a surgery to lift the maxillary sinus membrane associated with the xenogeneic bone placement and the second part consisted of the surgery for the implants placement, both under local anesthesia. A number of seven osseointegrated implants were placed in the maxilla, one was anchored in the nasopalatine canal region after having its neurovascular content e

BACKGROUND: Mandibular bone grafts are used for alveolar reconstruction in partially edentulous patients. Few reports describe the use of mandibular bone for total maxillary reconstruction.
AIM: To describe indications to the use of extensive mandibular bone grafts for maxillary reconstructions in edentulous patients.
METHODS: A retrospective evaluation of a group of patients with edentulous resorbed maxillae reconstructed with mandibular bone grafts in preparation for implant placement was performed. The studied cohort consisted of 15 patients who were treated with mandibular onlay blocks a

OBJECTIVES: To evaluate the volumetric changes in bone grafts to the severely atrophic edentulous maxilla over 6 months.
METHODS: Ten edentulous patients received autogenous bone as buccal onlay grafts to the maxilla and bilaterally particulate bone grafts to the maxillary sinus. Two mm contiguous axial CT scans were obtained 1 to 2 weeks and 6 to 7 months postoperatively. The area of each graft was measured and the volume calculated according to the formula V(graft)=Sigma of areas x the thickness of each section.
RESULTS: The volume of the inlay and onlay grafts was reduced by an average of

The reconstruction of the severely resorbed maxilla requires complex surgical treatment sequencing. Often, multiple grafting procedures are required either before or in conjunction with implant placement. Regardless of the surgical modality, the grafting procedures and the placement of implants in poor quality bone require undisturbed healing during which no pressure is placed on the grafted implant ridge. The use of transitional implants allows the surgeon to provide stable temporary prostheses throughout the healing phase, while preventing pressure from being placed on the grafted or implant

A new method is described (Sailer, 1988) whereby endosseous implants are inserted in the totally atrophic maxilla, the intermaxillary relationship and vertical dimension corrected and a vestibuloplasty performed during one surgical procedure. The method can also be used to treat minor degrees of maxillary alveolar atrophy. The procedure enables 20 mm-long Titanium screws to be used in totally atrophic maxillae. The following surgical procedures are carried out during one operation: A Le Fort I osteotomy, removal of the mucous membrane of the sinus floor, obturation of the denuded maxillary sin

BACKGROUND: Ectrodactyly ectodermal dysplasia-cleft syndrome is a rare genetic syndrome with an incidence of 1/90,000 live births, characterized by cleft lip and palate, severely hypoplastic maxilla, and hypodontia. Patients diagnosed with ectrodactyly ectodermal dysplasia-cleft syndrome suffer from a severely hypoplastic maxilla that is highly difficult to treat using traditional orthognathic methods. In this study, we propose using distraction osteogenesis to achieve a major advancement while maintaining good stability and minimal relapse. To our knowledge, this is the first description of p
