Brain stem responses evoked by stimulation of the mature cochlear nucleus with an auditory brain stem implant.

OBJECTIVES: The Nucleus auditory brain stem implant (ABI) has been used in the hearing rehabilitation of totally deaf individuals for whom a cochlear implant is not an option such as in the case of neurofibromatosis type 2 (NF2). Intraoperative electrically evoked auditory brain stem responses (EABRs) are recorded to assist in the placement of the electrode array over the dorsal and ventral cochlear nuclei in the lateral recess of the IVth ventricle of the brain stem. This study had four objectives: (1) to characterize EABRs evoked by stimulation with an ABI in adolescents and adults with NF2,

The multichannel auditory brain stem implant: performance in twenty patients.

The auditory brain stem implant has been used effectively to provide hearing sensations to individuals deafened by bilateral auditory nerve tumors (neurofibromatosis type 2). During tumor removal, the auditory brain stem implant is implanted into the lateral recess of the fourth ventricle by a translabyrinthine approach and is intended to stimulate auditory neurons of the cochlear nucleus complex. A new eight-electrode multichannel auditory brain stem implant was developed and evaluated in 20 patients who had at least 3 months' experience with the device. Mild nonauditory sensations (primarily

Auditory brain-stem responses in guinea pigs following middle ear implantation of Ceravital.

The effects on auditory function caused by implantation of the bioactive glass ceramic Ceravital in the middle ear of guinea pigs was investigated. Auditory brain-stem responses (ABR) were used to measure hearing function. A threshold shift due to a conductive hearing loss was observed immediately post-operatively. This threshold shift increased towards the 20th post-operative day and improved thereafter. Surgical manipulation of the middle ear and biochemical reactions between the Ceravital granules and the middle ear wall may have contributed respectively to the immediate and late temporary

Brain stem responses evoked by stimulation with an auditory brain stem implant in children with cochlear nerve aplasia or hypoplasia.

OBJECTIVES: The inclusion criteria for an auditory brain stem implant (ABI) have been extended beyond the traditional, postlingually deafened adult with Neurofibromatosis type 2, to include children who are born deaf due to cochlear nerve aplasia or hypoplasia and for whom a cochlear implant is not an option. Fitting the ABI for these new candidates presents a challenge, and intraoperative electrically evoked auditory brain stem responses (EABRs) may assist in the surgical placement of the electrode array over the dorsal and ventral cochlear nucleus in the brain stem and in the postoperative p

The prognostic value of promontory electric auditory brain stem response in pediatric cochlear implantation.

OBJECTIVE: To test the hypothesis that children with clear promontory electrically evoked auditory brain stem responses (prom-EABRs) would outperform, after cochlear implantation, children who had no prom-EABR preoperatively. DESIGN: A prospective study was undertaken on 47 implanted children assigning them to two groups (group A: 35 children with a clear wave e-V in the preoperative prom-EABR and group B: 12 children with no prom-EABR). Speech perception and speech intelligibility were assessed annually up to 3 yr after implantation with the IOWA sentence test (level A and level B), Connecte

Multichannel auditory brain stem implant: case studies comparing fitting strategies and results.

A new 8-electrode prosthesis has been developed for individuals deafened by bilateral auditory nerve tumors (neurofibromatosis-2). Twelve patients have received the multichannel auditory brain stem implant at House Ear Institute since 1992, 11 of whom receive useful auditory sensations. Processor fitting includes scaling and ranking of electrode-specific pitch sensations, and determination of any nonauditory sensations. Two representative patients differing in experience and number of usable electrodes are compared in terms of range of auditory sensations and speech perception performance. An

[Use of auditory brain stem implant in a patient with bilateral vestibular schwannoma (type 2 neurofibromatosis].

Surgical technique and functional results of implantation of the acoustic implant of the brain stem (BS) are described for a 16-year-old patient with bilateral vestibular shvannoma (neurofibromatosis of type 2). Initially, a large (IV degree) tumor was removed from the left cerebellopontile angle using the translabyrinthine-transcochlear approach. The function of the facial nerve recovered completely. The BS implant was implanted a year later in the course of removing shvannoma from the right cerebellopontile angle using the above approach. The patient received 22-channel implant Nucleus-CI22M

Nucleus 20-channel and 21-channel auditory brain stem implants: first European experiences.

Central electrical stimulation of the auditory pathway can allow hearing in patients suffering from deafness localized in the auditory nerve. Developments in a multichannel auditory brain stem implant based on the Nucleus Mini 22 cochlear implant with transcutaneous signal transmission is discussed. The device has been implanted in seven European patients who suffered from neurofibromatosis type 2. Preliminary speech perception results and patient satisfaction are encouraging, and the data presented include some limited open-set speech recognition.

[Oto-neuro-surgical approach and accessibility to the cochlear nuclei. Significance in auditory brain stem implant].

The auditory brainstem implant (ABI) is now used to stimulate the cochlear nucleus to obtain auditory perception in patients with type 2 neurofibromatosis. Only electrical stimulation of the cochlear nucleus complex, in the lateral recess of the fourth ventricle, can achieve auditory rehabilitation of these profound bilateral retrocochlear deafness. With 20 standard translabyrinthine approaches, our personal anatomical study propose to describe surgical landmarks of the cochlear nuclear complex and surgical accessibility of the lateral recess of the fourth ventricle. Three-Dimensional Surface Reconstruction of the Human Cochlear Nucleus: Implications for Auditory Brain Stem Implant Design.

Objective  The auditory brain stem implant (ABI) is a neuroprosthesis placed on the surface of the cochlear nucleus (CN) to provide hearing sensations in children and adults who are not candidates for cochlear implantation. Contemporary ABI arrays are stiff and do not conform to the curved brain stem surface. Recent advancements in microfabrication techniques have enabled the development of flexible surface arrays, but these have only been applied in animal models. Herein, we measure the surface curvature of the human CN and adjoining regions to assist in the design and placement of next-gener

[Auditory brain stem potentials evoked by electrical stimulation of cochlear nuclei during central auditive implant setting].

INTRODUCTION: The implantation of a stimulator over the surface of the cochlear nuclei can provide a partial auditive capability to patients deaf due to a bilateral lesion of the 8th nerve. Intraoperative monitoring of short latency electrically-evoked potentials is useful to asses the correct positioning of the implant, specially when there are anatomical distortions. PATIENTS AND METHODS: Evoked potentials from two patients with type II neurofibromatosis were recorded during implantation surgery, using a blanker system to eliminate the stimulus artifact. RESULTS: In both cases, two-peak ve

Paediatric auditory brainstem implant: How we do it.

BACKGROUND: The auditory brainstem implant (ABI) is a surgically implanted neuroprosthetic that may be used in the management of profound hearing loss in paediatric patients that are ineligible for cochlear implantation, primarily due to anatomical constraints. METHOD: We describe our technique for the implantation of a paediatric ABI. Surgical steps include post auricular incision, the creation of a periosteal flap, subperiosteal pocket and bony recess for the device, retrosigmoid craniotomy, placement of the electrodes adjacent to the cochlear nucleus and multi-layer closure. We highlight n

Subtonsillar placement of auditory brainstem implant.

A 26-year-old female with NF2 underwent removal of an acoustic schwannoma via extended retrosigmoid approach with subtonsillar placement of the auditory brainstem implant. The patient had already shown palsy of the lower cranial nerves on the opposite side due to previous surgery. Differing from conventional routes such as the translabyrinthine and the retrosigmoid, the subtonsillar approach enabled us to observe the entire cochlear nucleus and avoid injury to the 9th and 10th cranial nerves when applying the electrode. Hearing in pure tone average was maintained almost at the preoperative lev

[Auditory brainstem implantation].

In this review, we discuss the auditory brainstem implant (ABI), which is a neuroprosthetic device being an advanced hearing aid in cases of bilateral, profound or complete hearing impairment due to a non-functional or absent cochlear nerve, or an inner ear malformation precluding cochlear implantation. Originally indicated in patients with bilateral vestibular schwannomas caused by neurofibromatosis type 2, the ABI has in recent years seen an increase in the aural rehabilitation of congenitally deaf children. Outcomes for patients are mixed, but generally the device leads to a reasonable impr
