Autonomic control of the eye.

The autonomic nervous system influences numerous ocular functions. It does this by way of parasympathetic innervation from postganglionic fibers that originate from neurons in the ciliary and pterygopalatine ganglia, and by way of sympathetic innervation from postganglionic fibers that originate from neurons in the superior cervical ganglion. Ciliary ganglion neurons project to the ciliary body and the sphincter pupillae muscle of the iris to control ocular accommodation and pupil constriction, respectively. Superior cervical ganglion neurons project to the dilator pupillae muscle of the iris

Adenosine 3',5'-monophosphate in nervous tissue: increase associated with synaptic transmission.

Brief periods of stimulation of the preganglionic nerve fibers produced a severalfold increase in the content of adenosine 3',5'-monophosphate in superior cervical sympathetic ganglia, whereas postganglionic stimulation did not. These and other experiments indicated that the increased concentrations of adenosine 3'5'-monophosphate were closely associated with the process of synaptic transmission. This increase occurred primarily in postsynaptic cells.

[Riley-Day syndrome (familial dysautonomia)].

Riley-Day syndrome (RDS, familial dysautonomia) is reviewed from a viewpoint of autonomic disturbance. RDS shows pandysautonomia, including alacrima, orthostatic hypotension, gastrointestinal paresis, and paroxysmal hyperautonomic state, such as hypertension, vomiting crisis, and blotchy erythema. Sensory disturbances, including absence of taste and pain sensation, are common. Fungiform papillae on the tongue are sparse. Tests of autonomic function reveal postganglionic dysfunction. Sural nerve biopsy reveals depletion of small myelinated fibers and unmyelinated fibers, which corresponds well

Anhidrosis in multiple system atrophy: a preganglionic sudomotor dysfunction?

Anhidrosis occurs in the majority of multiple system atrophy (MSA) patients but the underlying site of lesion is not well established. We describe three patients with long-standing MSA and anhidrosis diagnosed on the basis of a thermoregulatory sweating test. In biopsies of anhidrotic skin, immunofluorescence analysis disclosed a well preserved postganglionic sudomotor innervation in all three patients supporting the hypothesis of a preganglionic nerve fiber lesion underlying their anhidrosis. Postganglionic sudomotor fiber integrity was also confirmed by normal electrodermal responses in one

Autonomic Component of Carpal Tunnel Syndrome.

Carpal tunnel syndrome (CTS) is the most common compression neuropathy. The median nerve contains sensory, motor, and sympathetic fibers. Involvement of the different fibers of the median nerve in CTS may vary; hence, one of the sensory, motor, or autonomic dysfunctions may be dominant. Autonomic fibers of the median nerve consist of postganglionic sympathetic vasomotor and sudomotor fibers. An incomplete interruption of the axoplasmic flow may provoke a reflex that increases the sympathetic activity, leading to sweating, Raynaud phenomenon, and paleness of fingers. A complete sympathetic axop

[Acute pandysautonomia].

39 years old woman with onset of faint, numbness of the extremities was reported. Special test of autonomic functions, endocrinology, biochemistry and nerve biopsy were carried out. The diagnosis of acute pandysautonomia was made. Clinically it is characterized by dilated pupils, non-reactive pupils, orthostatic hypotension, positive noradrenaline infusion test and negative sweating test with atropine. These changes suggested the disturbance of generalized sympathetic post-ganglionic nerve. In other hand defective salivation and lacrimation, lower values of the ratio between the R-R interval,

Innervation of sweat glands in the forehead. A study in patients with Horner's syndrome.

The amount of sweating in lateral and medial sites in the forehead was investigated with quantitative evaporimetry in 18 patients with Horner's syndrome: eight cases with a central (1st), five with a preganglionic (2nd), and five with a postganglionic (3rd) neurone lesion. The amount of sweating was measured after body heating, and, at another occasion, after intracutaneous injection of the cholinergic drug pilocarpine. The two sites were at the root of the nose (medial position) and at the lateral angle of the eye (lateral position). Generally, there was a reduced level of sweating on the sym

Localization of cholecystokininlike immunoreactivity in the rat spinal cord, with particular reference to the autonomic innervation of the pelvic organs.

The distribution of cholecystokinin in the spinal cord was investigated by immunohistochemistry. Throughout the length of the spinal cord cholecystokinin immunoreactivity was found in laminae I and II, in the spinal reticular nucleus, and in the surroundings of the central canal. On the basis of the cholecystokinin pattern lamina II could be divided into a dorsal and ventral part. In the lumbar and sacral spinal cord additional terminal fields of cholecystokinin immunoreactive boutons unique to these levels were found. They corresponded to the intermediolateral nucleus and to the medial lumbar

The spectrum of clinicopathological features in pure autonomic neuropathy.

We assessed the clinicopathological features of nine patients with pure autonomic neuropathy, that is, neuropathy without sensory or motor deficits. The duration from symptom onset to diagnosis ranged from 1 month to 13 years. Of eight patients in whom serum antiganglionic acetylcholine receptor antibody was determined, four were positive. All patients who tested positive for this antibody manifested widespread autonomic dysfunction, with the exception of one patient who only experienced orthostatic hypotension. However, patients who were negative for the antiganglionic acetylcholine receptor

Sympathetic neural outflow directly recorded in patients with primary autonomic failure: clinical observations, microneurography, and histopathology.

We evaluated 2 patients with primary autonomic failure, without clinical peripheral neuropathy. One had primary autonomic failure alone (PAF), and the other had autonomic failure and multiple system atrophy (MSA). Direct intraneural recordings demonstrated a marked reduction of sympathetic efferent nerve impulse activity in the PAF patient. The patient with MSA had spontaneous bursts of sympathetic nerve impulses that confirmed the functional integrity of post-ganglionic sympathetic efferent neurons. Neurosecretory activity of these neurons correlated with the electrophysiologic findings. The

Postganglionic cholinergic dysautonomia with incomplete recovery: a clinical, neurophysiological and immunological case study.

A 26-year-old man presented with signs and symptoms of marked postganglionic cholinergic autonomic dysfunction manifested by non-reacting dilated pupils, paresis of accommodation, decreased salivation, dry skin, atony of the bladder, erectile impotence and complete gastrointestinal paresis. Standard neurophysiological tests for myelinated sensory and motor fibre function and quantitative methods to examine unmyelinated parasympathetic, sympathetic and afferent fibres were performed: parasympathetic function was measured by heart rate variation tests. Thermoregulation in neuropathies.

Peripheral neuropathy affecting autonomic and small sensory fibers can cause abnormalities of both autonomic and behavioral thermoregulation. Quantitative autonomic and sensory neurophysiologic tests and quantification of the linear density of intraepidermal nerve fibers potentially can stratify those at risk of impaired thermoregulation during cold and heat challenges. New data relating to thermoregulatory sweating impairment in neuropathy are presented in this chapter. Of 516 neuropathy patients analyzed, 345 were found to have thermoregulatory sweat test (TST) abnormalities with a mean perc

[Causalgia in the upper limbs following neck trauma: existence of widespread dysfunction of sympathetic postganglionic fibers].

The functions of autonomic nervous system were systematically evaluated in a case of causalgia in the upper limbs after neck trauma. A 14-year-old boy had had hard blow on his nucha in a rough fight. After one week, a sustained burning pain, swelling and skin color change developed in the left upper limb. These symptoms also appeared in the right upper limb after 6 weeks. The physical examinations disclosed edema, reddish moist skin, and atrophic nail in the upper limbs. The neurological examinations showed a radiating pain to the upper limbs caused by the neck movement or pressure on the supr

Cryosurgery on postganglionic fibers (posterior nasal branches) of the pterygopalatine ganglion for vasomotor rhinitis.

Clinical experience with a newly designed device for cryosurgery on autonomic nerves in cases of vasomotor rhinitis is reported. The device consists of a handy spray of liquid nitrogen and a slender probe 2.5 mm in diameter. The posterior nasal branches of the pterygopalatine ganglion were destructed by freezing for 5-8 sec with a jet of supercooled nitrogen gas delivered onto the pterygopalatine foramen under anesthesia with 4% lidocaine. Satisfactory results were obtained by this procedure in 77 of 102 cases of vasomotor rhinitis which had failed to respond to antihistaminics. This original

Auxiliary Tests of Autonomic Functions.

The autonomic nervous system is a complex neural network that controls several organ systems. Its assessment includes a detailed history of autonomic functions, clinical examination, and autonomic tests. Most widely used is a battery of tests that assess cardiovascular reflex autonomic and sudomotor tests, which include deep breathing (assesses parasympathetic function), Valsalva maneuver, tilt test (both assess parasympathetic and adrenergic functions), and sudomotor testing for the evaluation of postganglionic sudomotor fibers. These basic tests represent a foundation of autonomic testing. N

Careful Dissection of the Distal Ureter Is Highly Important in Nerve-sparing Radical Pelvic Surgery: A 3D Reconstruction and Immunohistochemical Characterization of the Vesical Plexus.

OBJECTIVE: Radical hysterectomy with pelvic lymphadenectomy (RHL) is the preferred treatment for early-stage cervical cancer. Although oncological outcome is good with regard to recurrence and survival rates, it is well known that RHL might result in postoperative bladder impairments due to autonomic nerve disruption. The pelvic autonomic network has been extensively studied, but the anatomy of nerve fibers branching off the inferior hypogastric plexus to innervate the bladder is less known. Besides, the pathogenesis of bladder dysfunction after RHL is multifactorial but remains unclear. We st

Autonomic dysfunction in chronic inflammatory demyelinating polyradiculoneuropathy.

OBJECTIVES: Autonomic deficits in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) have not been adequately quantitated. The Composite Autonomic Severity Score (CASS) is a validated instrument for laboratory quantitation of autonomic failure derived from standard autonomic reflex tests. We characterized dysautonomia in CIDP using CASS. METHODS: Autonomic function was retrospectively analyzed in 47 patients meeting CIDP criteria. CASS ranges from 0 (normal) to 10 (pandysautonomia), reflecting summation of sudomotor (0-3), cardiovagal (0-3), and adrenergic (0-4) subscores. Sever
