Sudden cardiac death (SCD) accounts for 15%-60% of mortality in patients with heart disease. Generally, this has been attributed to ventricular tachyarrhythmia. However, ventricular tachyarrhythmia has been documented or strongly suspected on clinical grounds in a relatively small proportion of SCD patients (8%-50%). Attempted prophylaxis of SCD by implantation of cardioverter-defibrillator is associated with variable success in different subsets of high-risk cardiac patients (30%-70%). A significant number of SCD, therefore, appear to be due to catastrophic circulatory failure. Multiple inter

The complex nature of immune system behavior in both autoimmune diseases and transplant rejection can be understood through the lens of avalanche dynamics in critical-point systems. This paper introduces the concept of the "immunological avalanche" as a framework for understanding unpredictable patterns of immune activity in both contexts. Just as avalanches represent sudden releases of accumulated potential energy, immune responses exhibit periods of apparent stability followed by explosive flares triggered by seemingly minor stimuli. The model presented here draws parallels between immune sy

Granular packings undergo intermittent rearrangements, known as precursors, before avalanche onset. Using grain-scale tracking in an inclined granular layer, we show experimentally that the number and recurrence of precursors change with the compatibility between the anisotropic structure inherited from preparation and the subsequent loading direction. Preparation also modifies the propagation of the mobility front, defined as the region in which the packing undergoes measurable displacement: frequent precursors produce small progressive advances, whereas rare precursors involve larger extensi

ConspectusAvalanches within nanoparticles seem like science fiction, but if they are avalanches of photons, they open up real-world innovations in imaging, sensing, optical computing, and other unexplored light-driven technologies. Avalanches are outsized events arising from the integration of many smaller inputs, and photon avalanching (PA) was first reported in bulk crystals in 1979 as an unexpectedly large jump in luminescence as excitation intensity was slowly increased. It would be 41 years before PA would be observed at the nanoscale in photon avalanching nanoparticles (ANPs), Tm3+-doped

AIM: Asphyxia is the primary cause of death among avalanche victims. Avalanche airbags can lower mortality by directly reducing grade of burial, the single most important factor for survival. This study aims to provide an updated perspective on the effectiveness of this safety device.
METHODS: A retrospective analysis of avalanche accidents involving at least one airbag user between 1994 and 2012 in Austria, Canada, France, Norway, Slovakia, Switzerland and the United States. A multivariate analysis was used to calculate adjusted absolute risk reduction and estimate the effectiveness of airba

Avalanche accidents are frequently lethal events with an overall mortality of 23%. Mortality increases dramatically to 50% in instances of complete burial. With modern day dense networks of ambulance services and rescue helicopters, health workers often become involved during the early stages of avalanche rescue. Historically, some of the most devastating avalanche accidents have involved military personnel. Armed forces are frequently deployed to mountain regions in order to train for mountain warfare or as part of ongoing conflicts. Furthermore, military units are frequently called to assist

OBJECTIVE: Avalanches pose a life-threatening risk to participants of outdoor winter activities. Determining the causes of death in avalanche fatalities can aid rescue and resuscitation strategies and hopefully improve survival.
METHODS: The study population included all avalanche fatalities in Utah from the 1989-90 to 2005-06 winter seasons. The Utah Avalanche Center and Medical Examiner records were reviewed to identify accident circumstances, autopsy findings, and causes of death.
RESULTS: Fifty-six avalanche deaths were identified during the study period. Most deaths occurred while parti

The autopsies of 12 victims from two snow avalanches in North-Norway are reported. Supportive evidence from non-autopsied and surviving victims is included. Consistent autopsy findings were prominent lung oedema, moderate cerebral oedema, extreme contraction of the left ventricle, petechiae in the superior vena cava drainage area, and acute congestion in lungs and kidneys. In four cases in whom no resuscitation was attempted, aortic oxygen pressure was in the range expected in pure asphyxial-type deaths in one and in pure cardiac-type deaths in three. No air pocket was seen in front of the mou

A retired oncologist describes the unexpected death of his wife, Claudia, who spent 25 years as head of the geriatrics service of a major VA hospital. The couple drew comfort in their orchestration of a "good death"; nevertheless they understood that her death was hastened by a number of missteps. In the 2 months following a hip fracture, a chain of complications that required five surgical interventions led to massive hemorrhage from necrotizing esophagitis. Claudia lacked resilience to recover from a sequence of traumas sustainable by a younger person-but even the most experienced geriatrici

Over the last decade, a proliferation of interest has emerged in the area of avalanche survival, yielding both an improved understanding of the pathophysiology of death after avalanche burial and technological advances in the development of survival equipment. The dismal survival statistics born out of the modern era of winter recreation unmistakably reveal that elapsed time and depth of burial are the most critical variables of survival and the focus of newer survival devices on the market. Although blunt trauma may kill up to one third of avalanche victims, early asphyxiation is the predomin

Neuromodulatory levels in the brain change moment to moment, yet are typically ignored in studies of neural criticality, where empirical support remains varied. Here we show in a biophysical network of bursting neurons that arousal acts as a mechanistic control parameter for a directed percolation phase transition. At intermediate arousal, neuronal activity exhibits peaked susceptibility, power-law avalanche size and duration distributions, universal shape collapse, and anomalous diffusion, jointly satisfying the hyperscaling relation of the 2+1-dimensional directed percolation universality cl

As winter backcountry activity increases, so does exposure to avalanche danger. A complicated situation arises when multiple victims are caught in an avalanche and where medical and other rescue demands overwhelm resources in the field. These mass casualty incidents carry a high risk of morbidity and mortality, and there is no recommended approach to patient care specific to this setting other than basic first aid principles. The literature is limited with regard to triaging systems applicable to avalanche incidents. In conjunction with the development of an electronic avalanche rescue trainin

In avalanche accidents the cause of cardio-respiratory arrest (asphyxia, hypothermia, trauma) is difficult to determine in the field but may be important (high number of victims, limited number of rewarming places). 32 avalanche accidents (16 survivors/16 deaths) were therefore reviewed retrospectively. In 19 patients with cardiorespiratory arrest, asphyxia was frequent (17 patients, 89%) hypothermia was rare (two patients, 11%). The cooling rate between the accident and the arrival at the hospital was 3.0 degrees C/h. (range 0.75-5.8). The core temperature at arrival in the hospital was depen

On 30 Jan 2015, two avalanche accidents happened in the Black Forest (at the foot of the 1493 m high Feldberg and the Herzogenhorn situated next to it), in which experienced ski tourers--a 58-year-old woman and a 20-year-old man--were completely buried by snow masses. Both victims were recovered dead after nearly 2 hours under the snow. The avalanches were promoted by strong snowfalls, snowdrift by the wind and steep downwind slopes. One of the victims, the 20-year-old man, underwent a forensic autopsy. The findings suggested death by protracted asphyxiation with agonal hypothermia. A mechanic

In North America and Europe, approximately 150 people die of avalanches per year, and fatalities are presumed to be many times higher in developing countries. Four factors are decisive for survival: grade of burial, duration of burial, presence of an air pocket and a free airway, and severity of trauma. According to Swiss data, the overall mortality rate with avalanche burial is 23%, but it largely depends on the grade of burial. While the mortality rate is 52.4% in completely buried (head below the snow) victims in the Swiss population, it is only 4.2% in partially buried (head free) victims.

OBJECTIVE: Increasing numbers of avalanche victims with polytrauma have highlighted their need for radiological injury characterisation, which this study examines.
METHODS: People in Tirol injured by avalanche during 1994-2005 and admitted to the Innsbruck Medical University Hospital were included. Data for this retrospective study were obtained from the Austrian avalanche register and local electronic patient files archive.
RESULTS: During the observation period 94 avalanche victims in Tirol were admitted to our hospital, and a mean of 1.5 radiological methods were used per person at presen

This study estimates the proportions of the three major causes of avalanche death globally, and reviews potential factors influencing the proportions of causes of avalanche-related deaths (PCAD). By searching databases and consulting experts, we retrieved studies and registries in multiple languages, which examined PCAD. As a result, we retrieved 1,415 reports and included 37 for the study (22 for meta-analysis). We performed a meta-analysis to estimate pooled proportions. Between-study heterogeneity was assessed jointly by [Formula: see text] and 95% prediction interval of pooled estimates. P
