Obstructive azoospermia.

Obstructive azoospermia accounts for 40% of azoospermia and results from obstruction of the excurrent ducts (due to many causes) at any location between the rete testis and the ejaculatory ducts. The diagnosis of obstructive azoospermia (OA) requires a stepwise approach to differentiate it from nonobstructive OA and to formulate management options. Localization of the site of obstruction relies on history, physical examination, and possibly laboratory, genetic, imaging tests, and intraoperative findings. The prospects for patients with OA are excellent given recent advances in microsurgical ap

Differentiation between nonobstructive azoospermia and obstructive azoospermia: then and now.

Male infertility has seen an increase in prevalence with cases of azoospermia estimated to affect 10%-15% of infertile men. Confirmation of azoospermia subsequently necessitates an early causal differentiation between obstructive azoospermia (OA) and nonobstructive azoospermia (NOA). Although less common when compared to NOA, OA can represent upward 20%-40% of cases of azoospermia. While there are a multitude of etiologies responsible for causing NOA and OA, correctly distinguishing between the two types of azoospermia has profound implications in managing the infertile male. Genetics of Azoospermia.

Azoospermia affects 1% of men, and it can be due to: (i) hypothalamic-pituitary dysfunction, (ii) primary quantitative spermatogenic disturbances, (iii) urogenital duct obstruction. Known genetic factors contribute to all these categories, and genetic testing is part of the routine diagnostic workup of azoospermic men. The diagnostic yield of genetic tests in azoospermia is different in the different etiological categories, with the highest in Congenital Bilateral Absence of Vas Deferens (90%) and the lowest in Non-Obstructive Azoospermia (NOA) due to primary testicular failure (~30%). Azoospermia. Surgical prospectives.

For a long period of time, azoospermia has been considered a condition with poor therapeutical possibilities. Even in successful cases the number of pregnancies has been very low. With the new techniques in artificial insemination and fertilization there is new interest in treating azoospermic patients. A complete diagnostic procedure should be performed. The use of bilateral scrotal exploration and testicular biopsy allow better chances of diagnosis and treatment. More than 25% of azoospermic patients observed in an Andrological Centre could be surgically treated. Positive results were obtain

Endocrine studies of azoospermia. II. Serum steroid levels in obstructive azoospermia.

Serum estrone, estradiol, estriol, progesterone, 17 alpha-hydroxyprogesterone, and testosterone were determined in the sera of 25 normal males and 25 patients with obstructive azoospermia. An increase in testosterone and estriol levels, and a decrease in other hormones were demonstrated in patients with obstructive azoospermia. These long-term effects of obstruction of the vas apparently also occur in comparable situations, for example, after vasectomy.

The significance of karyotyping and azoospermia factor analysis in patients with nonobstructive azoospermia or oligozoospermia.

OBJECTIVE: We present our study about the significance of karyotyping and azoospermia factor(AZF) analysis in patients with azoospermia or oligozoospermia. MATERIALS AND METHODS: We retrospectively reviewed 141 Taiwanese patients with nonobstructive azoospermia and 45 Taiwanese patients with oligozoospermia at MacKay Memorial Hospital, Taiwan, from 2010 to 2021 to determine the significance of karyotyping and azoospermia factor analysis. The karyotyping was analyzed using the Giemsa banding method. The AZF microdeletions were determined using multiplex polymerase chain reaction using primers

The epidemiology and etiology of azoospermia.

The misconception that infertility is typically associated with the female is commonly faced in the management of infertile men. It is uncommon for a patient to present for an infertility evaluation with an abnormal semen analysis report before an extensive female partner workup has been performed. Additionally, a man is usually considered fertile based only on seminal parameters without a physical exam. This behavior may lead to a delay in both the exact diagnosis and in possible specific infertility treatment. Moreover, male factor infertility can result from an underlying medical condition

Clinical study of azoospermia.

This study evaluated how many patients with azoospermia might have fertility potential using assisted conception techniques. A total of 102 male patients with azoospermia were included in the study. Thirteen patients had sex chromosomal abnormalities. Testicular biopsy performed in the other 89 patients showed incomplete spermatogenesis in 47 of them whereas 42 had complete spermatogenesis. In the latter 42 patients, distal vasography demonstrated bilateral obstruction of the excurrent ducts in 14 patients whereas no distal obstruction of the ducts was found in 28. The 89 patients were divided

Oligo/azoospermia in Nigeria.

Oligo/azoospermia contributes significantly to infertility in male Nigerians, being responsible for most of the problem. By definition, it would appear that the criteria for the diagnosis of this problem in Nigerians should be sperm density below 10 million/ml, total sperm ejaculate below 25 million, motility below 40%, and normal forms below 40% in agreement with more recent findings in other parts of the world. This reinforces the already generally accepted that the WHO may need to review its criteria for diagnosing oligo/azoospermia. Preventable causes of oligo/azoospermia in Nigeria includ

In cases of azoospermia especially after vasectomy the investigation of the seminal sediment investigation is necessary. Exclusive on this way it is possible to find some side dispersed spermatozoa. The described case deals with a pregnancy about 48 days following vasectomy. Postoperative the sperm sediment investigation was not performed. In the following judicial hearing the doctor justified himself, that there do'nt exist any special direction in different textbooks of andrology as well as of urology. But this case demonstrates the necessary of the seminal sediment, which is the most import

Causes of azoospermia and their management.

Azoospermia may occur because of reproductive tract obstruction (obstructive azoospermia) or inadequate production of spermatozoa, such that spermatozoa do not appear in the ejaculate (non-obstructive azoospermia). Azoospermia is diagnosed based on the absence of spermatozoa after centrifugation of complete semen specimens using microscopic analysis. History and physical examination and hormonal analysis (FSH, testosterone) are undertaken to define the cause of azoospermia. Together, these factors provide a >90% prediction of the type of azoospermia (obstructive v. non-obstructive). Full defin

Azoospermia: diagnosis and management.

By using a detailed history and physical examination, endocrinologic studies, and, in some cases, testicular biopsy, we assign azoospermic men to three pathogenic groups: pretesticular, testicular, and posttesticular. This and further subclassification allow physicians to provide prognostic information. A few patients will have lesions amenable to surgical correction, or endocrinologic abnormalities that can be treated by replacement therapy or, as in the case of prolactin elevation, by surgery, irradiation, or bromocriptine therapy. Patients afflicted with irreversible sterility should be adv

Varicocelectomy in patients with azoospermia.

The results of surgical treatment of 33 men with azoospermia and varicocele are presented. Microscopic examination of a testicular biospy revealed the Sertoli-cells-only syndrome in nine subjects while degenerative changes in the seminiferous tubules of one or both testes were present in another six persons. In 15 cases decreased spermatogenesis was noted in the testis on the side ipsilateral to the varicocele. In the remaining three subjects the biopsy revealed active spermatogenesis but with absent spermiogenesis. In all cases the changes were equal or more advanced on the affected side. Imp

The effect of azoospermia factor microdeletions on intracytoplasmic sperm injection results in azoospermia patients.

BACKGROUND &OBJECTIVE: Y chromosome abnormalities are common in male patients with severe oligo-azoospermia. In studies with karyotype analysis and cytogenetic methods, the importance of the Y chromosome in spermatogenesis has been well understood. Deletions in the azoospermia factor (AZF) localized at the distal end of the Y chromosome adversely affect the spermatogenesis process. Our objective was to determine the frequency of AZF microdeletion in azoospermia patients who underwent microTESE. METHODS: In this retrospective cohort study, 806 azoospermic men attending the In Vitro Fertilizati

Azoospermia factor microdeletions: occurrence in infertile men with azoospermia and severe oligozoospermia from China.

Azoospermia factor (AZF) microdeletions are the most frequent genetic cause of male infertility after Klinefelter's syndrome. Although some assisted reproductive techniques such as intracytoplasmic sperm injection (ICSI) have been successfully introduced to clinical treatment for infertile males, the AZF microdeletions might be transmitted from infertile fathers to their male offspring during these procedures. Thus, it is important to carefully evaluate AZF microdeletions in infertile males before assisted reproductive techniques are performed. In this article, we aimed to investigate the freq
