Necrotic steroid-dependent erythema nodosum leprosum mimicking vasculitis in multibacillary leprosy with negative bacilloscopy: a diagnostic challenge.

Necrotic erythema nodosum leprosum (ENL) is a rare, severe and atypical variant of type 2 reaction characterized skin lesions mimicking vasculitis. Bacilloscopy and histopathological features may not be suggestive, in patients with repeated cycles of multidrug therapy (MDT) and concomitant use of corticosteroids. A 20-year-old man previously diagnosed with multibacillary leprosy received repeated cycles of MDT, at which time he developed recurrent painful nodules that worsened to widespread necrotic ulcers. Although previous bacilloscopy was positive, repeated test during severe ulcerative pre

Comparison of Three Polymerase Chain Reaction Protocols for the Diagnosis of Leprosy in Ecuador.

Leprosy or Hansen's disease remains a public health concern in Ecuador, where diagnosis still relies on clinical evaluation and bacilloscopy. This pilot study evaluated the performance of three polymerase chain reaction (PCR) protocols for detection of Mycobacterium leprae in 31 clinical specimens from patients clinically suspected of leprosy. Two real-time PCR assays (quantitative polymerase chain reaction [qPCR]) and one conventional PCR were compared with bacilloscopy. The two qPCR assays showed identical positivity rates of 32.3% (n = 10/31), whereas the conventional PCR detected only 3.2%

Use of Fourier transform infrared spectroscopy and chemometric analysis to predict the bacilloscopic index in the saliva of leprosy patients.

Leprosy is an infectious disease caused by the Mycobacterium leprae complex. Although the diagnosis of this disease is clinical, additional tests are often needed. One of the most common tests is intradermal smear bacilloscopy, which detects the patient's bacillary load. However, bacilloscopy is an invasive test and its results are subjective. The aim of this study was to propose a methodology based on Fourier transform infrared (FTIR) spectroscopy to predict the bacilloscopic index in the saliva of leprosy patients. A total of 88 saliva samples were included, and the bacilloscopic index of th

Pathogenesis of Leprosy: An Insight Into B Lymphocytes and Plasma Cells.

The pathogenesis of leprosy is still not fully understood. Several studies have been performed on the involvement of T cells in leprosy and more recently have focused on genetic factors and innate immune response. There are still only few reports about the role of B cells in active leprosy lesions in different spectral forms of the disease. The literature on tuberculosis suggests that B cells play an important role in the regulation of the granulomas, in cytokine production, T-cell response, and antigen presentation. Only few studies investigated the role of B cell in leprosy. We investigated

Active search for leprosy cases in Midwestern Brazil: a serological evaluation of asymptomatic household contacts before and after prophylaxis with bacillus Calmette-Guérin.

Leprosy is a disease caused by Mycobacterium leprae that carries a high risk of disability, making early diagnosis mandatory. This study aimed to determine the applicability of anti-PGL-1 IgM antibody detection, using the ML FLOW technique, as an assistant tool for the detection of leprosy infection in asymptomatic household contacts (AHHC) of multibacillary leprosy index cases from Midwest Brazil. Serological changes induced by the prophylaxis of these household contacts with Bacillus Calmette-Guérin (BCG) were also verified. A total of 91 AHHC were assessed, among which, 18.68% (n = 17) pres

Accuracy of the qPCR method in intradermal scraping samples from individuals with suspected leprosy in a Brazilian macroregion.

Leprosy is an infectious and chronic dermatoneurological disease, considered the most frequent cause of physical disabilities worldwide. It remains a major challenge for public health in Brazil, which is classified as a country with a high burden of the disease and ranks second in number of cases among all nations. Laboratory tests are important tools to complement the clinical-epidemiological evaluation, and molecular techniques emerged in the hope of reducing the limitations existing in the laboratory diagnosis of leprosy. We aimed to evaluate the accuracy of real-time polymerase chain react

[Atypical presentations of leprosy: apropos of 2 cases].

This report describes two atypical cases of leprosy. A 48 year old male patient presented laryngeal dyspnea with adhesions of the oropharynx of which the biopsies were inconclusive. The patient was cachectic with hyperesthesia of the extremities and two subcutaneous nodules. The biopsy of one nodule evoked thesaurismosis or dyslipoidosis while the bacilloscopy was positive in nasal smears. A 14 year old female patient suffered from bullae which appeared spontaneously on erythematous skin on the legs and upper arms. Upon examination those areas were found to be hypoaesthetic, as was a very larg

[Is urogenital tuberculosis a current disease still?].

In 1991, the epidemiology service of the health department of La Rioja announced that the number of deaths and new cases were higher than the national average (45/100,000 in 1991). Our service detected 16 new cases of urogenital tuberculosis during 1990-1991. The most common clinical symptoms being urgency and frequency. The bacteriological diagnosis using the Lowenstein-Jensen culture medium was positive in 87.50% of the cases. Anomalies were detected in 87.50% of the urographic analyses, the more frequently observed being infundibular involvement. Polychemotherapy consisting of rifampicin, e

Evaluation of circulating immune complexes in leprosy.

Circulating immune complexes (CIC) were evaluated in leprosy by 4 methods: the 125I-C1q binding assay (C1q), the platelet aggregation test (PAT), the 3.5% polyethylene glycol (PEG) precipitation test and the 2.5% PEG precipitation assay. Serum samples belonged to lepromatous leprosy bacilloscopy positive (LL+), lepromatous leprosy bacilloscopy negative (LL-), tuberculoid (TT) and first grade contact group (Co). Studies performed by the 3 first methods showed higher CIC levels in LL+ group (p less than 0.01) and lower values in the 3 others, all of them when compared to normals. On the contrary

qPCR detection of Mycobacterium leprae in biopsies and slit skin smear of different leprosy clinical forms.

Leprosy, whose etiological agent is Mycobacterium leprae, is a chronic infectious disease that mainly affects the skin and peripheral nervous system. The diagnosis of leprosy is based on clinical evaluation, whereas histopathological analysis and bacilloscopy are complementary diagnostic tools. Quantitative PCR (qPCR), a current useful tool for diagnosis of infectious diseases, has been used to detect several pathogens including Mycobacterium leprae. The validation of this technique in a robust set of samples comprising the different clinical forms of leprosy is still necessary. Thus, in this

A Multicenter evaluation of leprosy rapid test Fast ML Flow Hanseníase in Brazil.

PURPOSE: Leprosy represents a public health concern; its diagnosis remains clinical. Recently, the Brazilian public health system/SUS incorporated the leprosy-specific IgM anti-PGL-I rapid test Fast ML Flow Hanseníase (MLFH/Bioclin, Brazil; originally ML Flow) to aid leprosy contacts surveillance and diagnosis. Anti-PGL-I antibody levels correlate with bacterial load. This multicenter study evaluated MLFH performance in confirmed leprosy cases from four Brazilian reference centers. METHODS: MLFH results were compared to anti-PGL-I ELISA, bacilloscopy/BI and analyzed in indeterminate/I, pure n

Giant cells lepromatous leprosy. Diffuse dermatitis with exuberant foreign body giant cells in treated lepromatous leprosy.

Patients with lepromatous leprosy that have received treatment for many years usually get follow up biopsies for persistent skin lesions or positive bacilloscopy even if the values are lower than in the initial bacilloscopy. We report the case of a 48-year old woman with long-standing lepromatous leprosy of 15 years of evolution, with a bacterial index of 4 in the direct smear and the initial skin biopsy. The patient was treated with multidrug therapy for 32 months although the treatment recommended by the World Health Organization (WHO) is only for 12 months. A skin biopsy was taken to determ
