Back Injuries.

When searching for evidence-based answers about treating athletes with low back injury/pain, there are some difficulties. The first is defining who is an athlete. The second problem is that the lifetime prevalence of low back pain in the general population in our country approaches 100. Last, most studies published only deal with a narrow population of athletes, often performing very different types of physical activity. We searched the literature for studies that specifically evaluated athletes longitudinally. This article reviews the demographics, diagnostic challenges, history and physical

Back injuries in gymnastics.

The complaint of low back pain in the adolescent must never be taken lightly. A high index of suspicion should be particularly entertained in a child participating in gymnastic training or competition. As noted in this article, steps can now be taken, particularly if a specific diagnosis is made early, to institute specific treatment with a high likelihood of success. Young gymnasts complaining of back pain must never be passed off as having sustained a back strain or "muscle spasms" and treated symptomatically. Persistent back pain beyond two weeks warrants, in our opinion, a complete evaluat

Back injuries in dancers.

Back injuries may occur at any age or level of skill and are slightly more prevalent in male dancers. The risk factors that may be involved in a given injury include training errors, musculotendinous imbalance, anatomic malalignment of the lower extremity, shoe wear, and floor surface. Assessment of these risk factors provides guides for management and prevention.

Back injuries in the athlete.

In summary, the approach to the athlete with low back pain must include an emphasis on aggressive nonoperative intervention, education, and rehabilitation. A diligent attempt must be made to establish a correct diagnosis, though this may be difficult at first. A firm diagnosis allows individualized treatment that meets the strenuous needs of the athlete. Work-up should be standardized to avoid missing what will appear obvious in retrospect. Unlike most of the general patient population, athletes are unlikely to tolerate a long period of "wait and see" therapy. It is necessary to have qualified

Back injuries in the young athlete.

The diagnosis of back pain in the young athlete should be specific and not attributed to nonspecific, mechanical causes. Risk factor identification and intervention are required. Treatment is then initiated in a specific pattern, addressing flexibility and muscular imbalances. Bracing is often used to allow healing of growth tissue. The lumbosacral orthosis may be molded in a lordotic posture to unload the disc or antilordotic posture to relieve the posterior column; however, customizing the lordosis to the individual biomechanics may be required. Spinal stabilization is initiated with therapy

Back injuries in the pediatric athlete.

Pediatric and adolescent athletes presenting with back pain are much more likely to have a pathologic cause for their symptoms than are adult patients. For this reason, it is important for those caring for younger athletes to maintain a high index of suspicion for some of the more common pathologic causes of back pain in this population. Diagnostic evaluation should be undertaken more quickly in pediatric and adolescent athletes. Sports-related diagnoses that must be considered include disc-related back pain, atypical Scheuermann's kyphosis, spondylolysis, and spondylolisthesis. Patients of th

Lower back and elbow injuries in golf.

Golf injuries to the lower back and elbow are common problems in both the professional and amateur player, and any information regarding the successful treatment of these injuries has important implications for the medical practitioner. This paper presents the successful management and outcome of two case studies associated with low back pain and lateral epicondylitis in golf. Exercise therapy and conditioning has been shown to be an effective treatment modality for these two types of injury. In particular, a dynamic exercise programme which incorporates golf functional rehabilitation, is a mo

Rehabilitation of neck and low back injuries.

Sports-related mechanisms of cervical and lumbar spine injury and their clinical presentations have been discussed. The keys to treatment of these injuries in the athlete include rehabilitation of the injured area, recognition of the mechanism of injury, and correction of any technical faults that may have contributed thereto. Criteria for return to sport are a critical part of the rehabilitation of the injured athlete. Most importantly, the practitioner can anticipate potential areas of injury and can recommend prehabilitation exercises to avert many of these injuries.

The recovery patterns of back pain among workers with compensated occupational back injuries.

OBJECTIVES: To investigate the longitudinal patterns of recovery among workers with compensated occupational back injuries. METHODS: A longitudinal cohort study, with one-year follow-up via structured telephone interviews, among respondents off work because of "new" back injuries. Self-reported pain intensity was recorded at baseline and at four follow-up time points over the course of one year. Workers who answered the questionnaire on at least three occasions (n = 678) were classified into clusters according to their patterns of pain intensity over time using a two-step cluster analysis. Low back injury.

Low back injuries occur commonly in tennis, but the pathophysiologic, biomechanical, and clinical characteristics are not well defined. Tennis players may be at an increased risk of lumbar disc pathology from rotational and hyperextension shearing effects. Treatment of low back injury at present is empiric, but sport-specific lumbar stabilization and unloading of the lumbar disc should be developed. Prospective longitudinal research protocols are needed to study the lumbar spine in tennis players.

Back injuries and the fast bowler in cricket.

Here, I review research that has investigated the aetiology of injuries experienced by adolescent and adult fast bowlers. Mechanical factors play an important role in the aetiology of degenerative processes and injuries to the lumbar spine. This is particularly so in fast bowling, where a player must absorb vertical and horizontal components of the ground reaction force that are approximately five and two times body weight at front-foot and rear-foot impact, respectively. Attenuated forces are transmitted to the spine through the lower limb, while additional forces at the lumbo-sacral junction

Pediatric injuries in the back of pickup trucks.

Travel in the back of pickup trucks has not been adequately addressed as an occupant protection issue. This study compares injuries sustained by children riding in the back of pickup trucks with those of children riding in the cab. Data were obtained from a multihospital monitoring system and the coroner in a single urban county. The series of injured children consisted of 290 children 0 through 14 years of age, 201 of whom had been riding in the cab and 89 in the back. Age distribution of the children demonstrated that it is most frequently the 10- to 14-year-olds who travel in the back. Chil

Injury-induced kinematic compensations within the lower back: impact of non-lower back injuries.

With the number of musculoskeletal disorders increasing in the workplace, the potential exists for multiple injuries due to compensations. The objective of this study was to quantify the impact of non-lower back injuries on the trunk motions adopted by the individual during typical lifting tasks. A total of 32 injured subjects (eight for each injury group--shoulder, hand/wrist, knee and foot/ankle) and 32 matched (gender, height and weight) healthy subjects performed laboratory lifting tasks. The independent variables were task asymmetry (clockwise, sagittally symmetric and counter-clockwise),

Work-related low back injuries caused by unusual circumstances.

Expanding the knowledge of issues that surround work-related injuries allows for the development of more successful work accident prevention policies, treatment and rehabilitation protocols, and education programs. Specifically, clinical observation indicated that many patients did not perceive or report the circumstances of the injury as being part of their regular duties. The objective of this study was to investigate whether unusual activities or circumstances at work played any significant role in the rehabilitation of patients with disabling low back pain. Four hundred thirty-seven patien

Low back injury in sport.

Low back injuries are shown to present a serious hazard to sportsmen and women, and may lead to long-term disability in relation to their sport. Training methods, particularly weight training, led to many of the injuries and it is recommended that personnel involved in training become more aware of these dangers. Spondylolysis was found in 18% of cases and it is suggested that the full investigation of low back pain in a sportsman is incomplete until this lesion has been excluded.

Is There a Relationship Between Workload and Occurrence of Back Pain and Back Injuries in Athletes?

The back is subjected to a great deal of strain in many sports. Up to 20% of all sports injuries involve an injury to the lower back or neck. Repetitive or high impact loads (e.g., running, gymnastics, skiing) and weight loading (e.g., weightlifting) affect the lower back. Rotation of the torso (e.g., golf, tennis) causes damage to both, the lumbar and thoracic spine. The cervical spine is most commonly injured in contact sports (e.g., boxing, football). One of the factors that increases the odds of injuries in athletes is excessive and rapid increases in training loads. In spite of currently

Back injuries to fast bowlers in cricket: a prospective study.

Eighty-two high performance young male fast bowlers (mean age 16.8 years) were tested immediately prior to the season for selected kinanthropometric and physiological data. Subjects were also filmed both laterally (200 Hz) and from above (100 Hz) while bowling so that their front foot impacted a force platform during the delivery stride. The players then completed a log book over the ensuing season that detailed their training and playing programmes. All cricket related injuries over this season were assessed by a sports physician who used computerized tomography to assist in the diagnosis of
