Bacteroides infections in children.

From 1974 to 1990, 336 Bacteroides isolates were obtained from 312 specimens from 274 patients. They comprised 180 (54%) B. fragilis isolates, 55 (16%) B. theta-iotaomicron, 36 (11%) B. vulgatus, 34 (10%) B. distasonis, 21 (6%) B. ovatus and 10 (3%) B. uniformis. Infections in 253 (92%) patients were polymicrobial, but in 21 (8%) children, a Bacteroides sp. was isolated in pure culture. Most Bacteroides isolates were from peritoneal fluid (114), abscesses (110), wound infections (20), blood cultures (10) and from patients with pneumonia (14) or chronic otitis media (8). Predisposing conditions

Metronidazole treatment of Bacteroides fragilis infections.

Seven patients with Bacteroides fragilis infections were treated with intravenous and/or oral metronidazole. Infections treated included endocarditis, osteomyelitis, lung abscess, empyema, peritonitis, septicemia, and pelvic infection. Some patients had failed to respond to therapy with chloramphenicol or clindamycin or both. Metronidazole was used alone or in combination with aminoglycosides. Serum levels of metronidazole several times in excess of the minimal inhibitory concentrations for the organisms were easily achieved and in one patient the CSF metronidazole level was equal to that of t

Significance of encapsulated Bacteroides melaninogenicus and Bacteroides fragilis groups in mixed infections.

Organisms of the Bacteroides melaninogenicus and Bacteroides fragilis groups are often found mixed with facultatively anaerobic organisms in infections. The relative importance of these Bacteroides groups and facultative anaerobic pathogens in mixed infections was investigated in a subcutaneous abscess model in mice. This was determined by observing the effect of antimicrobial therapy directed against one or both organisms present in the abscess. Clindamycin or metronidazole was used for treatment of infections caused by Bacteroides species, and either gentamicin, penicillin, ampicillin, or ox

Porphyromonas (Bacteroides) endodontalis: its role in endodontal infections.

Porphyromonas endodontalis (formerly Bacteroides endodontalis) is a black-pigmented anaerobic Gram-negative rod which is associated with endodontal infections. It has been isolated from infected dental root canals and submucous abscesses of endodontal origin. The presence of P. endodontalis in infected dental root canals has been correlated with symptoms of an acute infection. It is occasionally found on oral mucous membranes and periodontal pockets. P. endodontalis has shown relatively low virulence in experimental monoinfections. In anaerobic mixed infections it can play an essential role. D

Isolation of Bacteroides corrodens from infections in children.

Six cases of infections in children with Bacteroides corrodens are reported. In five the infection was mixed but in one fatal extradural and subdural empyema the organism was found in pure culture. The bacteriological findings are presented. It is suggested that Bacteroides corrodens may assume a pathogenic role and the rarity of its isolation may be due to delay in the appearance of the colony.

Wound infections due to Bacteroides fragilis following intestinal surgery.

Ninety-six wound infections due to Bacteroides fragilis occurring after intestinal surgery are described. Most infections followed appendicectomy or colectomy for diverticulitis. B. fragilis was isolated in pure growth from 47 per cent of infections, and in mixed culture the other common bacteria were Klebsiella spp., Escherichia coli and Enterobacter spp. Fifty-three per cent of the infections responded rapidly to either non-specific measures or antibiotic therapy, but 47 per cent of patients developed complications, usually abscess formation. The successful isolation of bacteroides from clin

Anaerobic urinary infections: Bacteroides fragilis bacteremia from the urinary tract.

A case is reported of Bacteroides fragilis bacteremia from an obstructed upper urinary tract. Although anaerobic bacteria rarely cause significatn urinary tract infection it may occur, particularly in the presence of obstruction. The urinary tract must be considered a possible site for anaerobic infections.

[Bacteroides infection in surgical patients (author's transl)].

More than 80 cases of bacteroides infections are reported. 54 cases of these infections were postoperative complications. Relationships between bacteroides infection and diseases of the colon are significant. Not very impressive local symptoms made the diagnoses of abscesses difficult. In most cases bacteroides are susceptible to chloramphenicol and tetracycline. Patients develop agglutinable antibodies after bacteroides infection.

nim gene-independent metronidazole-resistant Bacteroides fragilis in surgical site infections.

Background:Bacteroides fragilis is the most common anaerobic pathogen isolated from surgical site infections (SSIs). Metronidazole resistance is increasing and the mechanisms of resistance are not clear in some isolates. The aim of the present study was to investigate the metronidazole susceptibility prevalence, and detect nim genes in B. fragilis isolates from SSIs. Methods: This study included 100 surgery patients with signs and symptoms indicative of SSIs. Syringe aspiration of the infected site was used to collect specimens. All specimens were cultured on BBA (Brucella blood agar), KVLB (k

Carbenicillin for treatment of Bacteroides fragilis infections: why not penicillin G?

Carbenicillin has been advocated for treatment of infections caused by Bacteroides fragilis and other anaerobic bacteria. Wide-scale use of the drug in this setting could result in a substantial increase in carbenicillin-resistant Pseudomonas aeruginosa, an effect that would have serious implications. Thirty-four strains of B. fragilis, one-half from bacteremic infections, were tested in vitro, and penicillin G was found to be twice as active as carbenicillin on an equal weight basis; 94% of the strains were inhibited by 32 microgram of penicillin/ml, a level easily achieved therapeutically. Genetic determinants for cfxA expression in Bacteroides strains isolated from human infections.

OBJECTIVES: To identify genetic determinants that determine beta-lactamase expression in Bacteroides strains isolated from human infections. METHODS: Beta-lactam susceptibility and beta-lactamase enzyme expression were characterized in selected strains. Beta-lactamase genes and surrounding regions were analysed by PCR, inverse PCR and Southern hybridization. RESULTS: High resistance to penicillins and cephalosporins was found among most isolated strains, in which all known beta-lactamase genes from Bacteroides are represented, but differences were found in their expression of enzyme activity

Infection with Bacteroides fragilis: pathogenesis and immunoprophylaxis in an animal model.

Organisms of the genus Bacterioides represent the major group of obligate anaerobes involved in human infections. Bacteroides usually cause either bacteremia or localized abscesses. B. fragilis and Bacteroides melaninogenicus have chemically incomplete lipopolysaccharides as compared with the lipopolysaccharides (endotoxins) of aerobic bacteria, and the lipopolysaccharides of Bacteroides lack of biologic potency characteristic of endotoxin. Isolation and characterization of seven phages infecting gut Bacteroides.

Despite the importance of the gut microbiota in human health, isolation and studies of phages infecting abundant gut bacteria have increased only in the past few years. The majority of phages isolated and characterized thus far infect facultative or strict aerobic bacteria. The aim of this study is to isolate and sequence new phages that infect Bacteroides species, which are abundant and prevalent members of the human gut microbiota across populations. Here, we report the isolation and genomes of seven phages infecting three human gut Bacteroides species from the United States. These phages ar

First case of abdominal infection caused by bacteroides fluxus.

Bacteroides fluxus is a Gram-negative anaerobic bacillus isolated from human faeces in healthy individuals. Until now, this bacterium had not been involved in human diseases. We report the first case of abdominal infection due to this microorganism in an elderly patient. A 76-year-old man with a history of chronic pulmonary obstructive disease presented with dyspnea, orthopnea and cough. The clinical evolution worsened with both a colonic ischemia and further diffuse peritonitis of pancreatic origin. Peritoneal fluid was obtained and the culture yielded B. fluxus in pure culture. Sera from eight patients with either septicemia or pyothorax caused by Bacteroides fragilis were assayed for specific antibody by the agglutination, immunofluorescence, and agar gel diffusion techniques. Antibodies were detected in the sera of all eight patients by all three serological tests employed; titers as high as 1:160 and 1:320 were detected by the agglutination test and the immunofluorescence technique, respectively, and all patients had precipitating antibody. Failure of B. fragilis sub-species thetaiotaomicron to react with any heterologous antisera from the other seven patients inf
