Tibial artery pseudoaneurysms: delayed complication of balloon catheter embolectomy.

Although complications of balloon catheter embolectomy are infrequent, the injury potential of these catheters is well recognized. This article describes a case of multiple tibial artery pseudoaneurysms that appeared 4 years after embolectomy in a 42-year-old man with otherwise normal arteries. The patient was treated by internal aneurysmorrhaphy without sequelae. A literature review of balloon catheter injuries yielded 46 cases categorized as arterial disruption (29), intimal injury (12), or catheter malfunction (5). These resulted in hemorrhage (13 cases), arteriovenous fistula (12), pseudoa

Balloon-Atrioseptostomy in Small Children Using an Embolectomy Catheter: Preliminary Data.

Interventional treatment of restrictive atrial septal defects in complex heart disease is considered state-of-the-art therapy up to date. Nevertheless, dedicated balloons are lacking so far, as several products have been withdrawn from the market. We report on off-label use of a balloon embolectomy catheter used successfully in a preterm patient and discuss whether this device might be used in other patients as well as it seems to be promising due to its shape and versatility.

[Nonsurgical recanalization of tibial arteries. Balloon dilatation, local fibrinolysis and aspiration embolectomy].

To date, percutaneous angioplasty has only rarely been employed to recanalize arteries in the lower leg. In almost all such patients, the limb has been at risk of being amputated. Since the introduction of special thin-caliber balloon catheters and concomitant drug therapy to prevent vascular spasms and thromboses, results have been markedly improved. In the case of fresh occlusions, local fibrinolysis with percutaneous aspiration embolectomy is employed.

Balloon protection of the mesenteric-renal vessels during embolectomy of acute paravisceral aortic occlusion.

Acute thrombosis of the aorta is an uncommon, but potentially devastating, vascular emergency. The absence of pathognomonic signs or symptoms makes the diagnosis challenging and, often, time consuming. No optimal treatment has been established, with greater challenge present when the thrombus involves the mesenteric-renal vessels. In this case report, we present an innovative, hybrid treatment of a nearly occluded paravisceral aorta with endovascular aortic embolectomy through a surgically accessed right femoral artery. We protected the superior mesenteric, renal, and left common iliac arterie

[Curative effects of extrusive and Fogarty balloon catheter embolectomy in acute femoral vein thrombosis: experiment with rabbits].

OBJECTIVE: To compare the influence extrusive and Fogarty balloon catheter embolectomy on the patency rate of bloodstream and the damage of the wall of vein in acute femoral vein thrombosis. METHODS: Eighty rabbits were randomly divided into 4 groups: Group A (n = 25) undergoing ligation of unilateral femoral to establish acute femoral vein thrombosis model and treated by extrusion of the hind leg muscles 24 h after the operation, Group B (n = 25) treated by Fogarty balloon catheter embolectomy 24 h after establishment of the thrombosis model, Group C (n = 25) undergoing sham operation, and G

Emergency embolectomy for embolic occlusion of the middle cerebral artery after internal carotid artery balloon test occlusion. Case report.

Balloon test occlusion of the internal carotid artery (ICA) is useful in preoperatively assessing the risk of temporary occlusion or permanent sacrifice of the carotid artery. The incidence of symptomatic complications from this procedure is 1.7%. The case is reported of a 57-year-old woman in whom a balloon test occlusion of the left ICA was attempted. She developed a left ICA dissection/occlusion with subsequent embolization to the left middle cerebral artery, leading to right-sided hemiplegia and expressive aphasia. She was successfully treated by an emergency embolectomy followed by surgic

Innovative Use of Interventional Hardware in Surgical Embolectomy in Small Infants.

Surgical embolectomy for thrombosis and ischemia following femoral arterial cannulation in small infants is rare. The nonavailability of embolectomy balloons in small sizes and the small size of the native vessels pose a challenge in repair. We describe the use of a coronary angioplasty balloon for the purpose of embolectomy in a 2.5-kg infant with femoral arterial occlusion and limb ischemia.

The technology of balloon catheters in interventional angiography.

Balloon catheters with expansion capabilities from 1 mm to 6 cm have been developed. They feature torque control and can be selectively positioned within vessels as easily as the ordinary diagnostic angiographic catheters. The balloons can be utilized for hemorrhage control, conventional embolectomy, operative angiography, vena cava occlusion, and purposeful vascular thrombosis or dearterialization. In the event of unsuccessful control, the balloon can be readily deflated and removed within minutes.

Risk factors in selected patients undergoing femoral embolectomy.

Mortality rates after femoral embolectomy (FE) in patients with an acutely ischaemic leg vary from 20-40%. In the last 3 years we have adopted a policy of proceeding directly to femoral embolectomy in those patients with a strong clinical suspicion of an embolus. Where doubt exists about the diagnosis, arteriography is performed in combination with local streptokinase, balloon dilatation and/or reconstruction. In a prospective study between September 1984 and March 1987, 43 patients underwent femoral embolectomy with a limb salvage rate of 87%. The early mortality (within 30 days) was 16%, the

Inflation volume-balloon diameter and inflation pressure-balloon diameter characteristics of commonly used bronchial blocker balloons for single-lung ventilation in children.

BACKGROUND: Balloon-tipped bronchial blocker catheters are widely used in pediatric thoracic anesthesia to establish single-lung ventilation. In clinical practice, their balloons demonstrate sudden expansion when inflated with air. In addition, there are concerns related to the high inflation pressures required to expand the balloons. METHODS: This in vitro study assessed inflation volume- and inflation pressure-balloon diameter characteristics of the Fogarty arterial embolectomy catheters and Arndt endobronchial blockers. Balloon diameters were photographically assessed during unrestricted v

Managing thrombosis of arteriovenous fistula accompanied by aneurysm and single outflow of elbow perforating vein: a case report.

BACKGROUND: We report a case of a patient who suffered thrombosis of a radial artery-cephalic vein fistula accompanied by aneurysm and a single outflow path of the elbow perforating vein. We performed open surgery combined with Fogarty balloon catheter embolectomy, anastomotic reconstruction and forearm median vein transposition. CASE PRESENTATION: The patient presented with an arteriovenous fistula (AVF) after haemodialysis 5 years ago. In the process of dialysis, the fistula vein was punctured, resulting in aneurysm, high pressure and difficult haemostasis after needle extraction. AVF occlu

The injury potential of Fogarty balloon catheters.

The problem of arterial injury following embolectomy using a balloon catheter is reviewed. Three illustrative clinical cases are presented. The cause of the arterial injury in these cases was due to rupture of the vessel. The hypotheses were that either the vessel was ruptured by direct penetration or bursting by overdistension of the balloon. The linear appearances of the arterial tear in two patients and a false aneurysm in the third suggested that bursting was the correct explanation. Laboratory investigation was carried out on animals and human cadavers and these confirmed that arterial in

Percutaneous aspiration thromboembolectomy (PAT): an alternative to surgical balloon techniques for clot retrieval.

Percutaneous aspiration thromboembolectomy (PAT) is an angiographic technique that can be used to remove thromboembolic debris from the distal lower extremity circulation. This procedure employs a specially designed catheter-sheath system, which can be used alone or in combination with balloon angioplasty or thrombolytic drugs (streptokinase 10,000 U/hr or urokinase 100,000 U/hr for 6 hours) to remove thromboembolic material. PAT is best suited for treating iatrogenic emboli resulting from intra-arterial catheterization or balloon angioplasty but can be used as a supplement to Fogarty embolect

A new use of Fogarty catheter: chest tube clearance.

Chest tubes are commonly used for patients who have undergone a cardiothoracic procedure to avoid the complications related to the accumulation of blood and serous fluid in the chest. Although the traditional methods such as milking, stripping or active chest tube clearance devices are used to establish patency of the chest tubes, they can become clogged at any time after their placement. Our technique may re-establish the tube patency with utilising Fogarty catheter and without any detriment to tissues.

Biliary ascariasis: percutaneous transhepatic management.

In a 60-year-old woman who underwent cholecystectomy and T-tube drainage for biliary ascariasis, postoperative bilirubin levels continued to increase. T-tube cholangiography revealed multiple filling defects in the bile ducts, which suggested roundworm reinfestation. Because the patient could not tolerate endoscopy, a percutaneous transhepatic approach with balloon dilatation of the ampulla of Vater and subsequent advancement of roundworms into the duodenum with an embolectomy balloon was used. The procedure was successful, with no major complications. The use of a percutaneous transhepatic tr

Arterial thromboembolism. A 20-year perspective.

Our experience with 739 patients with lower extremity thromboembolism since the advent of the balloon catheter has led us to several important observations: As the etiology has shifted from rheumatic to atherosclerotic, we treat a more complex group of patients, one fourth of whom have severe, preexisting peripheral occlusive disease. Early diagnosis and treatment is essential to decrease the mortality and morbidity, which has ranged about 25% +/- 10%. Anticoagulation must be continued in the postoperative period, accepting wound hematomas as a fair "trade-off" to prevent recurrent embolus and

We describe a case of successful pulmonary embolectomy that required lobectomy to treat for endobronchial bleeding. Sixty-nine-year-old female got into shock required CPR during rehabilitation after replacement of femur head. Her enhanced computed tomography (CT) showed massive embolism in pulmonary artery. Emergency embolectomy was performed under cardiopulmonary bypass (CPB). A videoscope was employed to see inside the pulmonary artery. At the end of the CPB, endobronchial bleeding occurred. The right bronchus was occluded by balloon and CPB was weaned. The right lower lobectomy was done on
