Bariatric surgery or medicine for type 2 diabetes?

Diet and medical treatment are the standard treatment for type 2 diabetes. In obese subjects with type 2 diabetes, bariatric surgery is effective in resolving diabetes. Two clinical trials comparing bariatric surgery to medical treatment were evaluated. Both the Surgical Treatment And Medications Potentially Eradicate Diabetes Efficiently (STAMPEDE) trial (laparoscopic Roux-En Y gastric bypass and sleeve gastrectomy) and the DIet and medical therapy versus BAriatric SurgerY in type 2 diabetes (DIBASY) trial (laparoscopic gastric bypass and biliopancreatic-diversion) showed that surgery was mor

Bariatric endoscopy, care-curative medicine and legal conflicts. Spanish Bariatric Endoscopy Group (Gettemo-SEED) Positioning.

Bariatric endoscopy treats obesity as a disease, in addition to its multiple associated comorbidities, so it should be considered in the "care-curative" field and not as "satisfying, voluntary or outcoming" medicine. Insufficient weight loss cases, or complications may occur. This, in parallel with the greater diffusion of these techniques, results an increase in the risk of complaints and judicial claims, which will presumably grow during next years. In this sense, we consider that all Bariatric Endoscopic Units working with medical-scientific rigor, must be able to be accredited and have leg

Bariatric surgery and medicines: from first principles to practice.

Obesity is a major public health issue with significant health and financial costs. Almost one in three Australian adults are living with obesity Bariatric surgery can have a role in the management of obesity. There is evidence for its effectiveness in preventing or reversing chronic health conditions The type of bariatric surgery can significantly impact the absorption, distribution, metabolism or elimination of orally administered drugs. Some changes can be predicted from pharmacokinetic and physiological effects, but management should be individualised The effect of weight loss itself after

Cardiovascular benefits of obesity therapies: an overview of obesity medicines and metabolic bariatric surgery.

Obesity is an independent driver of cardiovascular disease (CVD), mediated through adverse haemodynamic loading, insulin resistance, systemic inflammation, endothelial dysfunction and prothrombotic pathways. Contemporary obesity therapies show cardiovascular (CV) benefits beyond improvements in traditional risk factors. Across large CV outcome trials, glucagon-like peptide 1 receptor agonists consistently reduce three-point major adverse CV events (MACE) in patients with overweight, obesity and established CVD with and without diabetes. In obesity-related heart failure of preserved ejection fr

[Bariatric endoscopy-Possibilities and value in obesity medicine and surgery-A separate special field].

Endoscopy has become established as an important component in all preoperative, intraoperative and postoperative areas of bariatric surgery. As guidelines are not yet available, relevant studies and international position papers have been collected in this article. Preoperatively, endoscopy should be performed before sleeve gastrectomy (SG, detection of reflux status) and before bypass procedures (status survey of the postoperatively no longer accessible distal stomach). Routine endoscopic monitoring should be performed 2-3 years after SG and a OAGB (one anastomosis gastric bypass) for recordi

Using lifestyle medicine in U.S. health care to treat obesity: too many bariatric surgeries?

More than one-third of Americans are classified as obese. Many clinicians perform bariatric surgery (BSx) when it is said that lifestyle intervention failed. However, BSx is medically complex, with extremely variable success, certain failures, major complications, and sometimes death. Although many studies declare BSx as more effective for producing weight loss than nonsurgical lifestyle management, these conclusions are flawed when lifestyle management between cohorts are not identical. Lifestyle behavior change is essential to success for both surgical and nonsurgical weight loss, as over 50

Feasibility and acceptability of an integrated behavioral medicine service within a post-bariatric surgery clinic.

BACKGROUND: Long-term medical and psychological follow-up after weight loss surgery is associated with improved patient outcomes. Weight regain after weight loss surgery is a common concern that has behavioral and psychological components; however, most patients do not attend behavioral medicine (BMED) follow-up appointments post-surgery. Innovative treatment models are needed to improve access to BMED to optimize long-term outcomes. OBJECTIVES: This study aimed to examine the feasibility and acceptability of an integrated BMED service within a bariatric surgery clinic. SETTING: University m

Thrombotic complications following bariatric surgery: how medical tourism poses challenges to comprehensive care in obesity medicine.

The global prevalence of obesity has more than tripled since 1975. Unfortunately, bariatric surgery waiting lists can last many years therefore many patients seek alternative options such as "medical tourism" by venturing abroad for surgery. We describe two cases of porto-mesenteric venous thrombosis in patients who travelled abroad for bariatric surgery. Upon returning both cases required interventional radiological management, and in the first case, the patient underwent a small bowel resection for bowel ischaemia. Porto-mesenteric complications are significant and have profound lifelong con

Millon Behavioral Medicine Diagnostic Scales Do Not Predict Long-term Weight Loss Outcomes in Bariatric Surgery Patients.

BACKGROUND: Psychological factors identified during presurgical screening have the potential to identify which patients may respond better to bariatric surgery, and which patients may need additional assistance. Previous research has validated a number of potential instruments for use in presurgical screening. METHOD: This study evaluated archival data for 224 patients for a 60-month period following bariatric surgery to examine how presurgical scores on the Millon Behavioral Medicine Diagnostic predict body mass index (BMI) over time. Follow-up data was available on 96% of these patients at

Outcomes and Predictors of Success for Very Low Calorie Diets in an Australian Publicly Funded Bariatric Medicine Service.

OBJECTIVE: Enablers of successful completion and outcomes from Very Low Calorie Diets (VLCD) conducted in real-life clinical settings are poorly understood. This study explored weight and metabolic changes following VLCD and identified baseline factors associated with VLCD completion, weight-loss, and weight-loss maintenance. METHODS: A retrospective chart review was conducted for patients beginning VLCD between February 1, 2018 and October 31, 2020 at an Australian bariatric medicine service. Univariate analyses assessed weight, glycemic control, blood pressure, and cholesterol changes follo

[Bariatric surgery, stomas and other digestive tract reductions: Insufficient data and recommendations to adapt medicines regimens in therapeutic practice].

Surgery modifying digestive tract may alter drugs pharmacokinetics. To maintain concentrations of active substance in their therapeutic ranges, a dosage adjustment or change of drug may be necessary. This is particularly important when no pharmacological or pharmacodynamic parameter reflecting the medication effectiveness is easily measurable. Our objective was to gather the information and documentary tools that can guide prescription in these patients with rearranged digestive tract. We searched information on the documentary portals of French agencies, on gray literature, on MEDLINE and in

Tongue color tones in Traditional Chinese Medicine correlate with liver histology of metabolic dysfunction-associated steatohepatitis in morbidly obese patients undergoing metabolic and bariatric surgery.

OBJECTIVE: To determine the association of tongue color tones in morbidly obese patients with various liver histological characteristics of metabolic dysfunction-associated steatohepatitis (MASH), which is the advanced subtype of metabolic dysfunction-associated steatotic liver disease (MASLD). METHODS: A total of 60 morbidly obese adults who underwent metabolic and bariatric surgery were recruited. Liver biopsies were available for all participants and were subjected to histological assessment. The association of tongue color tones with histological parameters in the liver was investigated.

Although some published suggestions for conducting evaluations exist, no formal guidelines have been published specifying the qualifications, content knowledge, or clinical experience for the behavioral health professionals who conduct them.
