Barrett esophagus is a metaplastic change in the lining of the distal esophageal epithelium, characterized by replacement of the normal squamous epithelium by specialized intestinal metaplasia. The presence of Barrett esophagus increases the risk of esophageal adenocarcinoma several-fold. Esophageal adenocarcinoma is a malignancy with rapidly rising incidence and persistently poor outcomes when diagnosed after the onset of symptoms. Risk factors for Barrett esophagus include chronic gastroesophageal reflux, central obesity, white race, male gender, older age, smoking, and a family history of B

PURPOSE OF REVIEW: Strides have been made in the past year to identify means of decreasing mortality from esophageal adenocarcinoma. This requires a thorough understanding of the risk factor for its development, Barrett esophagus. This article summarizes the most up-to-date and important advancements involving the epidemiology, screening, pathogenesis, surveillance, and treatment of Barrett esophagus over the past year.
RECENT FINDINGS: Much of the literature over the past year involving Barrett esophagus centered on selecting subsets of patients who would benefit most from screening and surv

There continues to be great interest in Barrett esophagus, the premalignant lesion for adenocarcinoma of the esophagus and the esophagogastric junction. There were several publications this past year related to the epidemiology, pathogenesis, new methods for detection, biomarkers for stratifying patients by risk, and the treatment of patients with Barrett esophagus. Patients with intestinal metaplasia in short lengths of columnar mucosa in the distal esophagus (ie, short-segment Barrett esophagus) appear to be distinct from those with intestinal metaplasia in the gastric cardia. The reported r

Barrett esophagus continues to intrigue investigators and clinicians alike as the new millennium begins. A large number of publications in the past year have discussed issues of epidemiology, prevalence, detection, and treatment of Barrett esophagus. Chronic symptoms of gastroesophageal reflux were identified as a strong risk for esophageal adenocarcinoma. The relative frequency of short and long Barrett and cardia intestinal metaplasia in patients who undergo upper endoscopy have been better defined. Biomarkers in patients with Barrett may eventually be helpful in identifying those at high ri

There are a lot of contradiction and uncertainty related to Barrett's esophagus. Authors review the literature about the incidence, the aetiology, the pathology of the ailment, the possibility of diagnosis and treatment. 25 Barrett's esophagus were observed in the course of controlling of patients after antireflux operations and investigations of patients suffering from gastroesophageal reflux disease. X-ray examination, endoscopy, biopsy, 24-hour pH monitoring of the distal esophagus and esophageal manometry were performed. Because of the possibility of malignant transformation Barrett's esop

The incidence of Barrett carcinoma has increased almost six-fold in the last 20 years. Barrett oesophagus is defined as the presence of metaplastic columnar epithelium in the oesophagus in continuity with the gastric mucosa. It is regarded as an acquired abnormality, developed as a result of chronic (duodenal) gastro-oesophageal reflux. It is especially the intestinal type of columnar epithelium that has a greatly enhanced risk of malignant degeneration. It is advisable that patients with a Barrett oesophagus should regularly be examined endoscopically, with extensive tissue biopsy. Presence o

Barrett's oesophagus is a complication of chronic gastrooesophageal reflux disease and occurs in eight percent of patients with reflux symptoms. The diagnosis is made by endoscopy with biopsy. Dysplasia and early adenocarcinoma may involve only very small irregular areas of the oesophageal mucosal surface. Biopsies should therefore be taken from each quadrant at 1-2 cm intervals. The prevalence of dysplasia is about five to 16 percent, while the prevalence rate of oesophageal adenocarcinoma ranges from nought to ten percent and the incidence is one case per 87 patients years. No consensus conc

The definition of Barrett esophagus is currently under discussion. It is now suggested that a distal esophagus coated with cylinder epithelium with cardia-fundus mucosa should also be classified as Barrett esophagus because the risk of cancer is significantly increased even without histological evidence of intestinal metaplasia with goblet cells. The results of recent epidemiological investigations imply that the cancer risk of cylinder cell metaplasia and low grade intraepithelial neoplasia in Barrett esophagus has previously been overestimated. The histological detection of dysplasia still r

IMPORTANCE: Barrett esophagus is characterized by the replacement of normal esophageal squamous cell epithelium with columnar metaplasia and affects approximately 5% of people in the US and approximately 1% worldwide. Approximately 3% to 5% of patients with Barrett esophagus will be diagnosed with esophageal adenocarcinoma in their lifetime.
OBSERVATIONS: Barrett esophagus affects approximately 2.3% to 8.3% of people with gastroesophageal reflux disease (GERD) and approximately 1.2% to 5.6% of people without GERD. Characteristics associated with Barrett esophagus include older age (prevalence

The incidence of esophageal adenocarcinoma and associated mortality has risen dramatically over the past several decades, and, thus, it is increasingly important to understand its pathogenesis and risk factors. Barrett esophagus is the established precursor to esophageal adenocarcinoma that progresses through a metaplasia-dysplasia-carcinoma sequence. Its risk of transforming to carcinoma is not as high as previously reported and there appears to be a biological heterogeneity among patients with this disease. The overall prevalence of Barrett esophagus in the United States ranges from 1% to 25

The purpose of the treatment of Barrett epithelium in the distal oesophagus is to reduce or even eliminate the increased risk of malignant degeneration in it. This can be achieved by removing the Barrett epithelium, whether or not dysplastic, and to have it replaced by normal squamous epithelium. Drug treatment or surgical antireflux treatment of Barrett epithelium has hardly any effect on the length of the Barrett epithelium or on the occurrence of malignancy. Various forms of endoscopic ablative therapy (laser coagulation, multipolar electrocoagulation, photodynamic therapy and argon plasma

Barrett esophagus is a premalignant change of the esophagus; however, malignant transformation to esophageal adenocarcinoma is rare in patients without dysplasia. Barrett esophagus is estimated to affect up to 5.6% of the U.S. population. Risk factors for Barrett esophagus include gastroesophageal reflux disease, obesity, age older than 50 years, male sex, tobacco use, and a family history of Barrett esophagus or esophageal adenocarcinoma. Patients who experience chronic gastroesophageal reflux symptoms plus additional risk factors should be considered for screening. Mucosal change consistent

Systemic sclerosis is a generalized systemic autoimmune disorder frequently involving the oesophagus causing reflux oesophagitis in the majority of patients. Barrett's oesophagus is one of the most serious complication of the reflux oesophagitis, in which the normal oesophageal squamous epithelium is partially replaced by metaplastic columnar epithelium. The clinical significance of Barrett's oesophagus is the significantly increased risk of developing the adenocarcinoma. In our paper, the authors present 5 cases with Barrett's oesophagus among their 52 examined patients with scleroderma.

There has been a considerable increase in the incidence of adenocarsinoma in the proximal stomach (cancer of the cardia) and distal oesophagus (Barrett's cancer) for the past 20 years. There is probably also a parallel increase in the pathogenetically related conditions reflux oesophagitis and Barrett's oesophagus. In patients with classical Barrett's oesophagus, i.e., metaplastic changes in the mucosa more than 3 cm up from the gastro-oesophageal junction, a follow-up programme with endoscopy and adequate biopsies is recommended in cases where a finding of premalignant changes or malignancy w

Barrett's esophagus (BE) is a precancerous condition marked by the replacement of the normal stratified squamous epithelium in the distal esophagus with metaplastic columnar epithelium, typically resulting from chronic gastroesophageal reflux disease (GERD). This pathological adaptation significantly increases the risk of developing esophageal adenocarcinoma (EAC), a malignancy with rising incidence, particularly in Western populations. This review provides a comprehensive overview of the current understanding and management of BE, drawing on major international guidelines and recent advanceme

Barrett's oesophagus is a premalignant metaplastic change of the oesophageal mucosa. Due to its relationship with oesophageal reflux disease and the development of adenoma-carcinoma of the oesophagus the problem arouses increasing interest. In the wide pathogenesis of the disease most probably the composite effect of the refluxed HCl content and duodenal juices play a part. In the diagnosis in addition to fundamental methods--endoscopy and histology--increasingly chromoendoscopy and fluorescent endoscopy are involved. Dispensarization of patients is essential and depends on the degree of patho

Barrett's oesophagus is a complication of oesophagogastric reflux. Diagnosis is made on the basis of endoscopy and histology showing glandular and intestinal metaplasia above the oeso-gastric junction. The most severe complication is adenocarcinoma. Endoscopic follow up is effective in individual patients. Other solutions would be either to protect the Barrett oesophagus from carcinogenic and cocarcinogenic agents contained in particular in the duodenal reflux fluid, or to ablate Barrett's mucosa by thermal or physicochemical processes, allowing subsequent regrowth of normal malpighian mucosa

Barrett's esophagus develops as a result of chronic acid and bile reflux and carries an increased risk of esophageal adenocarcinoma. Because it has no specific symptoms, many patients do not receive a diagnosis or they present with symptoms of gastroesophageal reflux disease and other related risk factors or complications. Diagnosis relies on endoscopic and histopathological findings, including a visible columnar-cell-lined segment measuring at least 1 cm long that contains intestinal metaplasia with goblet cells. Ongoing surveillance focuses on early detection of malignant progression, partic

Barrett's esophagus is an acquired condition resulting from severe esophageal mucosal injury. It still remains unclear why some patients with gastroesophageal reflux disease develop Barrett's esophagus whereas others do not. The diagnosis of Barrett's esophagus is established if the squamocolumnar junction is displaced proximal to the gastroesophageal junction and if intestinal metaplasia is detected by biopsy. Despite this seemingly simple definition, diagnostic inconsistencies remain a problem, especially in distinguishing short segment Barrett's esophagus from intestinal metaplasia of the g
