Predicting the onset of Alzheimer's disease using Bayes' theorem.

Bayes' theorem describes the effect of new information (e.g., a test result) on the probability of an outcome (e.g., a disease). Likelihood ratios for separate tests can be combined to assess the joint effect of their results on disease probability. This approach has been used to develop a test package for Alzheimer's disease that consists of some simple cognitive tests (Paired Associate Learning Test, Trailmaking Test, and Raven's Progressive Matrices) combined with age and family history of dementia. A total of 1,454 subjects who had been recruited into the Medical Research Council Elderly H

Bayes' theorem-based assessment of VDRL syphilis screening miss rates.

BACKGROUND AND OBJECTIVES: Interlaboratory differences (and conflicting practices) in syphilis screening strategies (serial versus parallel test combinations) prompted us to determine an estimate of the diagnostic miss rate in the detection of (1) infected persons and (2) infected but untreated persons potentially affected by late active syphilis. GOAL: We set out to establish the most efficient syphilis screening strategy for two routine tests (VDRL and Treponema pallidum hemagglutination assay [TPHA]) with regard to our tested public health population (average VDRL+ TPHA+ and VDRL- TPHA+ re

Issues in the application of Bayes' Theorem to child abuse decision making.

Proposals for the application of Bayes' Theorem as an aid to child abuse decision making are discussed critically. Subsequently, two examples of the application of research findings concerning signs of child abuse to decision making are demonstrated, using data from research studies of signs of physical abuse and sexual abuse. The calculation of the probability of the presence of abuse using Bayes' Theorem is described, given prevalence information and specific indicators of abuse. In addition, a method for describing the degree of imprecision in estimates of the probability of abuse is discus

A diagnostic algorithm for carpal tunnel syndrome based on Bayes's theorem.

OBJECTIVE: To construct a diagnostic algorithm based on Bayes's theorem and using simple clinical tests to allow accurate diagnosis without resort to nerve conduction studies. METHOD: A cohort of patients being referred with possible carpal tunnel syndrome had clinical and electrophysiological testing, from which the simple calculations for sensitivity, specificity and prevalence were made and subsequently used in the formula of Bayes's theorem. The algorithm was then tested prospectively in a further cohort of similarly referred patients. RESULTS: The algorithm proved to be reliable when te

Evidence-based diagnosis of type 1 von Willebrand disease: a Bayes theorem approach.

The diagnosis of type 1 von Willebrand disease (VWD) is based on the presence of bleeding symptoms, reduced von Willebrand factor (VWF) levels, and autosomal inheritance of the phenotype. To better appreciate the contribution of clinical and laboratory data to the final diagnosis of VWD, we computed the likelihoods of having VWD as a function of the bleeding score (LR(score)), of VWF level (LR(VWF)), and of number of first-degree family members with reduced VWF levels (LR(family)). The 3 likelihoods were therefore combined using the Bayes theorem, giving the final probability (odds) of having

Child Abuse, Misdiagnosed by an Expertise Center-Part II-Misuse of Bayes' Theorem.

A newborn girl had, from two weeks on, small bruises on varying body locations, but not on her chest. Her Armenian grandmother easily bruised, too. Her mother was diagnosed with hypermobility-type Ehlers-Danlos-Syndrome (hEDS), an autosomal dominant connective tissue disorder, with a 50% inheritance probability. Referral to a University Medical Center located "Dutch Expertise Center for Child Abuse" resulted (prior to consultation) in physical abuse suspicion. Protocol-based skeletal X-rays showed three healed, asymptomatic rib fractures. A protocol-based Bayesian likelihood ratio guesstimatio

Modeling the clinical assessment of men with suspected obstructed voiding using Bayes' Theorem.

AIMS: Pressure-flow studies (PFS) are the only reliable way to diagnose bladder outlet obstruction (BOO) in men with lower urinary tract symptoms (LUTS). However, in routine clinical practice, BOO is usually inferred by any of a number of tests (symptoms, flow rate, prostate size...). Bayes' Theorem provides a mathematical method, which may be similar to the process used by clinicians, for combining the results of multiple tests to reach a diagnosis. We have applied Bayes' Theorem to the results of several tests known weakly to predict BOO in men with LUTS to assess if they improve the diagnos

[The use of Bayes' theorem in controlling the coming epidemic peak of epidemic meningitis in the 1980s].

To control the coming epidemic peak of epidemic cerebrospinal meningitis (ECM) in the 1980s in the regions with 100 million population in China, the mathematical models based upon Bayes' theorem (BT) were established and used respectively in provincial, regional and county's level. Reports of ECM from each ten-day's period or each month during the meningitis season were analysed to create forecast models. Records of ECM vaccinating rate in previous years were fully taken into account to modify the theoretical values. Calibration, split-sample, random-sample selection, as well as actual forecas

An objective way to predict remission and relapse in Cushing disease using Bayes' theorem of probability.

OBJECTIVE: In this study on patients with Cushing disease, post-transsphenoidal surgery (TSS), we attempt to predict the probability of remaining in remission, at least for a year and relapse after that, using Bayes' theorem and the equation of conditional probability. The number of parameters, as well as the weightage of each, is incorporated in this equation. DESIGN AND METHODS: The study design was a single-centre ambispective study. Ten clinical, biochemical, radiological and histopathological parameters capable of predicting Cushing disease remission were identified. The presence or abse

The value of Bayes' theorem for interpreting abnormal test scores in cognitively healthy and clinical samples.

The base rates of abnormal test scores in cognitively normal samples have been a focus of recent research. The goal of the current study is to illustrate how Bayes' theorem uses these base rates--along with the same base rates in cognitively impaired samples and prevalence rates of cognitive impairment--to yield probability values that are more useful for making judgments about the absence or presence of cognitive impairment. Correlation matrices, means, and standard deviations were obtained from the Wechsler Memory Scale--4th Edition (WMS-IV) Technical and Interpretive Manual and used in Mont

Heterozygote tests and genetic counseling in maple syrup urine disease: an application of Baye's theorem.

The discriminatory power of a quantitative heterozygote test for Maple Syrup Urine Disease (MSUD) which we have been using is analyzed on the presumption that a single rare mutant allele is involved in MSUD. Bayes theorem then predicts that only a small portion of persons from the general population with activities in the heterozygous range really are heterozygotes. In addition, overlap of heterozygous and normal homozygous activity distributions requires rather high activities in first-cousin matings of a patient's sib in order to obviate the necessity for prenatal diagnosis. Thus, the princi

Prevalence of Birt-Hogg-Dubé Syndrome Determined Through Epidemiological Data on Spontaneous Pneumothorax and Bayes Theorem.

Background: Birt-Hogg-Dubé syndrome (BHD) is a rare inherited disorder characterized by cutaneous fibrofolliculomas, multiple pulmonary cysts, recurrent spontaneous pneumothorax (SP), and renal tumors. More than 40 years after its description, the prevalence of BHD in the general population remains unknown. This study aimed at determining the prevalence of BHD by applying the Bayes theorem of conditional probability to epidemiological data on SP. Methods: We performed a meta-analysis of published data on: (1) the probability of having BHD among patients with apparent primary SP (4 studies), (2

Diagnosing ectopic pregnancy using the bayes theorem and neural network: a validation of a retrospective cohort study.

OBJECTIVE: To evaluate the accuracy of neural networks and Naïve Bayes models in diagnosing ectopic pregnancy, using clinical data, hCG levels, and transvaginal ultrasound findings from a real dataset. METHODS: This was a retrospective cohort study based on a public dataset of 2,495 first-trimester pregnant women with confirmed pregnancy under 13 weeks, documented transvaginal ultrasound reports, and follow-up on pregnancy outcome. The cohort presented a natural imbalance (8.5% ectopic, 91.5% intrauterine pregnancies), reflecting real-world clinical prevalence. Data on risk factors, clinical

Blast counts in bone marrow aspirate smears: analysis using the poisson probability function, bayes theorem, and information theory.

Counts of cells or other phenomena observed through a microscope are numeric observations and, as such, are subject to mathematical and statistical analyses. For example, the Poisson probability function provides the probability of observing a particular number of blasts in a bone marrow aspirate, given an underlying true fraction of blasts present and a particular number of cells evaluated. Furthermore, using the Poisson function, Bayes theorem can provide the probabilities of specific categories of refractory anemia, given a number of observed blasts in a specific total of cells evaluated. The use of Baye's theorem and other indices of agreement in evaluating the use of reagent strips in screening rural schoolchildren for Schistosoma haematobium in Zambia.

The diagnosis of Schistosoma haematobium disease on the basis of history and physical examination alone is often difficult. Tests have thus been developed to allow an early and more accurate diagnosis. However these tests have substantial imperfections and many different results obtained from these tests must be integrated into a diagnostic conclusion about the probability of disease in a given patient. Also the accuracy of these tests in detecting S. haematobium disease is critically dependent not only on its sensitivity and specificity but also on the prevalence or pretest likelihood of dise

[Why can it be difficult for clinicians to diagnose acute lyme disease? From physiopathology to diagnosis, via abductive reasoning and Bayes theorem].

The diagnosis of acute neuroborreliosis may be difficult if it's regarded as a "classical" infectious disease. Through a clinical case, we illustrate the difficulties met and we suggest two ways of reflexion to assist in the diagnosis:--firstly, we explain how the comprehension of the behavior of the pathogen agent, which is similar to a parasitic behavior, can help to choose and interpret the results of additional tests;--secondly, we develop practically the clinical form of Bayes's theorem to demonstrate the interest of a rational Bayesian and abductive approach which should be preferred to

Use of Bayesian statistical approach in diagnosing secondary hypertension.

Bayes's theorem is predominantly used in diagnosing based on the results of various diagnostic tests. This statistical approach is intuitive in differential diagnosis as it explicitly takes into consideration data from medical history, physical examination, laboratory findings and imaging. Bayes's theorem states that the probability of disease occurrence (or occurrence of other outcome) after new information is obtained, called a posteriori probability, depends directly on an a priori probability and the value of likelihood ratio associated with a given test result. This paper describes basic
