Biosurveillance in outbreak investigations.

Following the terrorist attacks of September 11 and the anthrax attacks in 2001, public health entities implemented automated surveillance systems based on disease syndromes for early detection of bioterror events and to increase timeliness of responses. Despite widespread adoption, syndromic surveillance systems' ability to provide early notification of outbreaks is unproven, and there is little documentation on their role in outbreak response. We hypothesized that biosurveillance is used in practice to augment classical outbreak investigations, and we used case studies conducted in 2007-08 t

Biosurveillance: a review and update.

Since the terrorist attacks and anthrax release in 2001, almost $32 billion has been allocated to biodefense and biosurveillance in the USA alone. Surveillance in health care refers to the continual systematic collection, analysis, interpretation, and dissemination of data. When attempting to detect agents of bioterrorism, surveillance can occur in several ways. Syndromic surveillance occurs by monitoring clinical manifestations of certain illnesses. Laboratory surveillance occurs by looking for certain markers or laboratory data, and environmental surveillance is the process by which the ambi

Biosurveillance of Selected Pathogens with Zoonotic Potential in a Zoo.

Monitoring of infectious diseases is one of the most important pillars of preventive medicine in zoos. Screening for parasitic and bacterial infections is important to keep animals and equipment safe from pathogens that may pose a risk to animal and human health. Zoos usually contain many different animal species living in proximity with people and wild animals. As an epidemiological probe, 188 animals (122 mammals, 65 birds, and one reptile) from a zoo in Slovenia were examined for selected pathogens. Antibodies to Toxoplasma gondii and Neospora caninum were detected by ELISA in 38% (46/122)

Bayesian modeling of unknown diseases for biosurveillance.

This paper investigates Bayesian modeling of unknown causes of events in the context of disease-outbreak detection. We introduce a Bayesian approach that models and detects both (1) known diseases (e.g., influenza and anthrax) by using informative prior probabilities and (2) unknown diseases (e.g., a new, highly contagious respiratory virus that has never been seen before) by using relatively non-informative prior probabilities. We report the results of simulation experiments which support that this modeling method can improve the detection of new disease outbreaks in a population. A key contr

Invisible Threats, Relentless Hunters: Biosurveillance of Airborne Plant Pathogens.

Airborne dispersal enables plant pathogens to travel across fields, regions, and continents, fueling rapid epidemics and emerging disease threats. Biosurveillance, the systematic monitoring of airborne inoculum, offers the opportunity to detect pathogens before symptoms appear and informs timely, risk-based management. Recent advances in air sampling, molecular diagnostics, metagenomics, and imaging technologies have expanded the scale and resolution of pathogen monitoring, from single-species qPCR assays to community-level aerobiome surveys. Integration of biosurveillance data with decision-s

Biosurveillance of coronaviruses in Rhinolophus bats from South Africa.

Rhinolophus bats harbour various alpha- and betacoronaviruses and are believed to be the progenitor host of SARS-CoV and SARS-CoV-2. These bats are widely distributed, with 38 species recognized in Africa. Although coronaviruses have been detected in several species in Africa, there is a lack of surveillance among South African rhinolophids. This study conducted longitudinal nucleic acid surveillance for Rhinolophus spp. coronaviruses from September 2021 to January 2024 in a mixed species cave in Limpopo, South Africa, using a hemi-nested RT-PCR assay. Among the 492 gastrointestinal samples co

Digital Biosurveillance for Zoonotic Disease Detection in Kenya.

Infectious disease surveillance is crucial for early detection and situational awareness of disease outbreaks. Digital biosurveillance monitors large volumes of open-source data to flag potential health threats. This study investigates the potential of digital surveillance in the detection of the top five priority zoonotic diseases in Kenya: Rift Valley fever (RVF), anthrax, rabies, brucellosis, and trypanosomiasis. Open-source disease events reported between August 2016 and October 2020 were collected and key event-specific information was extracted using a newly developed disease event taxon

Biosurveillance of emerging biothreats using scalable genotype clustering.

Developments in molecular fingerprinting of pathogens with epidemic potential have offered new opportunities for improving detection and monitoring of biothreats. However, the lack of scalable definitions for infectious disease clustering presents a barrier for effective use and evaluation of new data types for early warning systems. A novel working definition of an outbreak based on temporal and spatial clustering of molecular genotypes is introduced in this paper. It provides an unambiguous way of clustering of causative pathogens and is adjustable to local disease prevalence and availabilit

ORBiT: Oak Ridge biosurveillance toolkit for public health dynamics.

BACKGROUND: The digitization of health-related information through electronic health records (EHR) and electronic healthcare reimbursement claims and the continued growth of self-reported health information through social media provides both tremendous opportunities and challenges in developing effective biosurveillance tools. With novel emerging infectious diseases being reported across different parts of the world, there is a need to build systems that can track, monitor and report such events in a timely manner. Further, it is also important to identify susceptible geographic regions and po

A new prior for bayesian anomaly detection: application to biosurveillance.

OBJECTIVES: Bayesian anomaly detection computes posterior probabilities of anomalous events by combining prior beliefs and evidence from data. However, the specification of prior probabilities can be challenging. This paper describes a Bayesian prior in the context of disease outbreak detection. The goal is to provide a meaningful, easy-to-use prior that yields a posterior probability of an outbreak that performs at least as well as a standard frequentist approach. If this goal is achieved, the resulting posterior could be usefully incorporated into a decision analysis about how to act in ligh

Artificial intelligence-enhanced biosurveillance for antimicrobial resistance in sub-Saharan Africa.

Antimicrobial resistance (AMR) remains a critical global health threat, with significant impacts on individuals and healthcare systems, particularly in low-income countries. By 2019, AMR was responsible for >4.9 million fatalities globally, and projections suggest this could rise to 10 million annually by 2050 without effective interventions. Sub-Saharan Africa (SSA) faces considerable challenges in managing AMR due to insufficient surveillance systems, resulting in fragmented data. Technological advancements, notably artificial intelligence (AI), offer promising avenues to enhance AMR biosurv

Exploring TopicGPT for Identification of Themes of Emerging Pathogens in Biosurveillance Data.

INTRODUCTION: Characterization of emerging diseases often has to occur when analysts who are in charge of tracking emerging threats have limited time. Topic modeling approaches that require limited analyst input and can be applied on electronic health records (EHR) may provide earlier insights. We applied large language model (LLM) based topic modeling to text from emerging pathogens to discover themes. MATERIALS AND METHODS: EHR notes were extracted from Veterans Affairs (VA) Corporate Data Warehouse. We included up to 200 notes from 5 cohorts including Nationally Notifiable Disease (2005-20

Constructing a global human epidemic database using open-source digital biosurveillance.

We developed a dataset consisting of outbreak data collected from official, open-source surveillance reports representing more than 170 pathogens, 237 countries and territories, and more than 3300 events that occurred primarily between 1963 and 2023. Here we present and analyze a subset of these data, comprising a dataset of human epidemic events with onset between 2015 and 2020. Structuring of epidemiological data in the dataset follows a specific methodology to ensure consistency across all events. This methodology has been designed to produce the most reliable spatiotemporal view of an outb

Identifying respiratory findings in emergency department reports for biosurveillance using MetaMap.

Clinical conditions described in patients' dictated reports are necessary for automated detection of patients with respiratory illnesses such as inhalational anthrax and pneumonia. We applied MetaMap to emergency department reports to extract a set of 71 clinical conditions relevant to detection of a lower respiratory outbreak. We indexed UMLS terms in emergency department reports with MetaMap, filtered the indexed output with a specialized lexicon of UMLS terms for the domain, and mapped the clinical conditions of interest to concepts in the lexicon. We compared MetaMap's ability to accuratel

Interfacing a biosurveillance portal and an international network of institutional analysts to detect biological threats.

The Early Alerting and Reporting (EAR) project, launched in 2008, is aimed at improving global early alerting and risk assessment and evaluating the feasibility and opportunity of integrating the analysis of biological, chemical, radionuclear (CBRN), and pandemic influenza threats. At a time when no international collaborations existed in the field of event-based surveillance, EAR's innovative approach involved both epidemic intelligence experts and internet-based biosurveillance system providers in the framework of an international collaboration called the Global Health Security Initiative, w

A Metagenomic Biosurveillance Network for Emerging Infectious Diseases: A Simulation-Based Model.

In this article, we propose a metagenomic next-generation sequencing (mNGS) system for symptomatic clinical respiratory disease samples in Israel to enable detection early enough to contain novel pathogen outbreaks, limit international spread and expedite countermeasure development. We built an open-source, interactive SEIR (susceptible, exposed, infectious, recovered)-based model extending the work of Sharma et al (2023) for 7 representative known respiratory pathogens with pandemic potential, aiming to estimate costs and detection time for the identification of a novel respiratory pathogen i

Biosurveillance and the Opioid Crisis in Emergency Medicine: Qualitative Study of Physician Perspectives.

BACKGROUND: In 2017, the US Department of Health and Human Services declared a national opioid crisis. In 2022, an estimated 81,806 overdose deaths involved an opioid. Emergency departments are critical in the pathway of care for providing resources and linkages to services. Studies investigating emergency medicine (EM) physicians' perspectives on the opioid crisis have largely focused on prescribing. OBJECTIVES: To investigate EM physicians' perspectives on response strategies to the opioid crisis of biosurveillance and linkages to care. A secondary objective was to map reported challenges a
