Bipolar disorder not otherwise specified in relation to the bipolar spectrum.

BACKGROUND: The purpose of this study was to determine the clinical characteristics of patients who are diagnosed with bipolar disorder not otherwise specified (BPD NOS) and who are considered to represent part of the bipolar spectrum. The lifetime prevalence of BPD in the general population may be as high as 6% when the full spectrum of bipolar disorders is accounted for. Correct identification of true bipolar patients in clinical settings may result in more appropriate treatment. Our hypothesis was that patients with BPD NOS would be more similar to other bipolar patients than major depressi

Are impulse-control disorders related to bipolar disorder?

We reviewed available evidence regarding a possible relationship between impulse-control disorders (ICDs) and bipolar disorder. Studies examining the phenomenology, course, comorbidity, family history, biology, and treatment response of ICDs were compared with similar studies of bipolar disorder. Although no studies directly compare a cohort of ICD patients with a cohort of mood disorder patients, available data suggest that ICDs and bipolar disorder share a number of features: (1) phenomenologic similarities, including harmful, dangerous, or pleasurable behaviors, impulsivity, and similar aff

Comorbidity of bipolar and eating disorders: distinct or related disorders with shared dysregulations?

BACKGROUND: The co-occurrence of bipolar and eating disorders, though of major clinical and public health importance, remains relatively unexamined. METHODS: In reviewing the literature on this comorbidity, we compared bulimia, anorexia nervosa, bulimia nervosa, binge eating disorders and bipolar disorders on phenomenology, course, family history, biology, and treatment response. RESULTS: Epidemiological studies show an association between subthreshold bipolar disorder and eating disorders in adolescents, and between hypomania and eating disorders, especially binge eating behavior, in adults

Is cerebellar volume related to bipolar disorder?

BACKGROUND: Recent data suggest that cerebellum influences emotion modulation in humans. The findings of cerebellar abnormalities in bipolar disorder (BD) are especially intriguing given the link between the cerebellum emotional and behavioral regulation. The purpose of this study was to evaluate cerebellar volume in patients with euthymic BD type I compared to controls. Moreover, we investigated the possible relationship between cerebellar volume and suicidal behavior. METHODS: Forty-patients with euthymic BD type I, 20 with and 20 without history of suicide attempt, and 22 healthy controls

Health-related quality of life in bipolar disorder.

OBJECTIVES: Health-related quality of life (HRQOL) refers to an individual's overall sense of wellbeing, and subjective physical, psychological, and social functioning. HRQOL is significantly affected in patients with bipolar disorder (BD), a chronic affective disorder characterized by mood fluctuations, sleep and cognitive impairment, and impact on inter-personal relationships, all of which contribute to HRQOL impairment. This article reviews the relevant literature on the potential causes and magnitude of HRQOL impairment in BD and role of treatment interventions in restoring HRQOL in this p

Relation of hyperparathyroidism and hypercalcemia to bipolar and psychotic disorders.

Primary hyperparathyroidism (PHPT) is characterized by hypercalcemia and an elevated level of serum parathyroid hormone. PHPT leads to hypercalcemia and presents with renal, skeletal, and neuropsychiatric symptoms. Neuropsychiatric manifestations of PHPT such as psychotic, depressive, and anxiety disorders are rare. Delirium may also be present in patients with PHPT. Parathyroidectomy is the treatment of choice for patients with physically symptomatic PHPT, but it remains unclear if parathyroidectomy is indicated for its psychiatric manifestations. It is unclear if parathyroidectomy should be

Explicating the structure and relations of the Mood Disorder Questionnaire: Implications for screening for bipolar and related disorders.

BACKGROUND: The Mood Disorder Questionnaire (MDQ; Hirschfeld et al., 2000) is a widely used screening measure in bipolar disorder research. Although this measure assesses a heterogeneous range of content, only limited prior research utilizing relatively small sample sizes has examined its factor structure. METHODS: The MDQ's structure was examined in 700 participants reporting current psychiatric treatment. We extended prior structural work on the MDQ by explicating relations between factors and a wide range of psychopathology and personality measures. RESULTS: The MDQ items were best captur

Attrition factors in clinical trials of comorbid bipolar and substance-related disorders.

BACKGROUND: This study analyzed and defined specific factors that account for attrition in clinical research for patients with bipolar and substance-related disorders. METHODS: Data were analyzed from two completed studies: an open-label trial of lamotrigine in patients with bipolar disorder (BPD) and cocaine-related disorder, and a placebo-controlled trial of quetiapine in patients with BPD and alcohol-related disorders. Correlations and Independent sample t-tests were performed to assess the impact of baseline characteristics including on length of study participation. Significance was set

Comparing anxiety disorders and anxiety-related traits in bipolar disorder and unipolar depression.

The frequent comorbidity of anxiety disorders and mood disorders has been documented in previous studies. However, it remains unclear whether specific anxiety traits or disorders are more closely associated with unipolar major depression (MDD) or bipolar disorder (BPD). We sought to examine whether MDD and BPD can be distinguished by their association with specific types of anxiety comorbidity. Individuals with a primary lifetime diagnosis of either bipolar disorder (N=122) or major depressive disorder (N=114) received diagnostic assessments of anxiety disorder comorbidity, and completed quest

Health-related quality of life in Thai bipolar disorder.

BACKGROUND: Bipolar disorder (BPD) affects both patients 'functioning and well-being. Quality of life (QoL) has gained increasing attention as an important functional outcome in BPD. The present study was conducted to assess QoL of Thai BPD patients. MATERIAL AND METHOD: The authors obtained cross-sectional demographic, clinical, and functional ratings from 285 BPD outpatients. SF-36 and Thai Mania Rating Scale (TMRS) were used to assess QoL and severity of symptoms respectively. RESULTS: The mean TMRS was 4.42 +/- 5.87. Compared with the Thai general population, SF-36 scores of study popula

Clinical features related to age at onset in bipolar disorder.

BACKGROUND: Early age at illness onset has been associated with poor functional and syndromal outcome in bipolar disorder, although debate remains about the likely robustness of this variable, especially while controlling for other illness parameters. METHOD: Fifty-six consecutive bipolar outpatients underwent semistructured interviews to assess psychopathology and outcome. We hypothesized that early age at onset would be linked with more prevalent rapid cycling, psychosis, comorbid substance abuse, and suicide attempts. RESULTS: Illness onset before age 19 arose in 46% of subjects. Separate

Acute Psychosis Related to Primary Hyperparathyroidism in a Patient With Bipolar Disorder.

Primary hyperparathyroidism (PHPT) can cause hypercalcemia secondary to high parathyroid hormone secretion. Hyperparathyroidism- and hypercalcemia-related acute psychotic symptoms can be challenging to diagnose in patients with mental health-related disorders, and it should be considered a possible differential in these patients besides medications. It can sometimes be the first manifestation of the disease, and diagnosis can be challenging, especially in patients with a previous psychiatric history without checking their biochemistry profile. The hypercalcemia severity can vary from mild to s

Polygenic risk for schizophrenia and bipolar disorder in relation to cardiovascular biomarkers.

Individuals with schizophrenia and bipolar disorder are at an increased risk of cardiovascular disease (CVD), and a range of biomarkers related to CVD risk have been found to be abnormal in these patients. Common genetic factors are a putative underlying mechanism, alongside lifestyle factors and antipsychotic medication. However, the extent to which the altered CVD biomarkers are related to genetic factors involved in schizophrenia and bipolar disorder is unknown. In a sample including 699 patients with schizophrenia, 391 with bipolar disorder, and 822 healthy controls, we evaluated 8 CVD ris

Health-related quality of life in adolescents with bipolar I disorder.

OBJECTIVE: To examine abnormalities in health-related quality of life (HRQOL) and to determine whether pharmacological intervention with divalproex or quetiapine alters HRQOL abnormalities in bipolar adolescents. METHOD: Parents of 23 adolescents diagnosed with a manic or mixed episode associated with bipolar I disorder were asked to rate their child's health using the Child Health Questionnaire-Parental Form 50 (CHQ-PF 50) at baseline, prior to receiving medication and then at 28 days later. RESULTS: Manic adolescents exhibited significantly worse HRQOL than national norms at baseline on al

Clinical variables related to antidepressant-induced mania in bipolar disorder.

BACKGROUND: The development of mania or hypomania during antidepressant treatment is a serious complication of the clinical management of bipolar disorder (BP). The primary aim of this study was to evaluate the clinical variables related to antidepressant-induced mania or hypomania (AIM) in patients with BP. METHODS: DSM-IV BP-I or BP-II patients who had had at least one depressive episode treated with antidepressants were considered. Patients were subdivided into two groups according to the presence (n = 30) or absence (n = 106) of manic or hypomanic episodes occurring during antidepressant

Revolving Door Phenomenon and Related Factors in Schizophrenia, Bipolar Affective Disorder and Other Psychotic Disorders.

BACKGROUND: Patients with chronic mental illness are frequently hospitalized and discharged from psychiatry wards. This situation is referred to as the "revolving door phenomenon" (RDP). In addition to factors related to the patient and the disease, limited number of beds leading to shortened hospital stay are among the reasons associated with frequent hospitalization. This study aims to compare patients with RDP and patients with single hospitalization in terms of clinical, sociodemographic, and treatment-oriented characteristics in order to evaluate the risk factors causing frequent hospital

Circadian-Related Hypothalamic Structure Differs by Chronotype in Bipolar Disorder.

BACKGROUND: Circadian rhythm disturbances represent a core feature of bipolar disorder (BD), with evening chronotype as a marker for poorer outcomes. We hypothesized that BD psychopathology combined with evening chronotype would be associated with structural alterations in circadian-related hypothalamic regions, particularly the suprachiasmatic nucleus (SCN), specific to BD relative to other psychiatric diagnoses. METHODS: We investigated structural neuroimaging data from the UK Biobank (113 BD, 205 major depressive disorder, 91 psychotic disorders, 199 healthy control). The SCN-containing an
