Bone Cements in Depressed Frontal Bone Fractures.

Skull fractures can be classified into four major types; linear, depressed, diastatic, and basilar. Of these, a depressed skull fracture presents a high risk of increased intracranial pressure or hemorrhage to the brain. A compound depressed skull fracture results when a laceration over the fracture exposes the internal cranial cavity to the outside environment. Such depressed skull fractures are indicated for elevation if the defect is more than 10 mm and in the presence of brain injury. Frontal bone contour defects result in marked facial deformity which becomes obvious to the observer. Esth

About radioactivity in some PMMA bone cements.

Various bone cements containing zirconium oxide (ZrO2) as X-ray contrast medium were tested for radioactivity by means of a gamma spectrometer. All measured bone cements (PALACOS, IMPLAST, SULFIX-6) showed a certain degree of radioactivity. The radiation source in the bone cement is the added zirconium oxide, which is polluted by radioactive elements. As these X-ray contrast media remain in the body for decades as components of the bone cement, the radioactive zirconium oxides should be substituted by high purity radiation-free zirconium oxide or barium sulfate.

Injection biomechanics of bone cements used in vertebroplasty.

The incidence of osteoporotic bone fractures is growing exponentially as the western population ages and as life expectancy increases. Vertebroplasty, where acrylic or calcium phosphate cement is injected into the weakened vertebrae to augment them, is an emerging procedure for treating spinal fragility fractures. However, cement injection is currently limited because there are no clear standards for a safe, reproducible and predictable procedure. The purpose of this paper is to examine the role that bone cements play in the underlying bio-mechanisms that affect the outcomes of cement injectio

Resorption patterns of calcium-phosphate cements in bone.

Two calcium phosphate cements, one monophasic and the other biphasic, have been used as bone void filler in a sheep model. The cements were injected into a slot defect in the proximal tibia and into a cylindrical defect in the distal femur. In this study, we focused on the resorption pattern of the two cement formulations and the subsequent biologic reaction. Bone remodeling occurred synchronously with the resorption of the implant material in a creeping substitution process. Cracks and pores in the monophasic cement were filled with osseous tissues. The biphasic cement showed faster resorptio

[Antibiotic-loaded bone cements: from laboratory studies clinical evaluation].

Antibiotic-loaded bone cements (ALBC) were initially used empirically, both for the treatment of infected prostheses and for antibioprophylaxis. We conducted in vitro and animal studies as a prerequisite for proper clinical evaluation. We measured gentamicin diffusion from methacrylate bone cement, and found that the concentration in surrounding fluid was significantly above the minimal inhibitory concentration (MIC). We then implanted ALBC in the proximal femora of 100 ewes, in conditions close to those of total hip replacement (THR). Antibiotic concentrations in bone were above 4 x MIC for m

Antimicrobial activity of bone cements embedded with organic nanoparticles.

Infections after orthopedic surgery are a very unwelcome outcome; despite the widespread use of antibiotics, their incidence can be as high as 10%. This risk is likely to increase as antibiotics are gradually losing efficacy as a result of bacterial resistance; therefore, novel antimicrobial approaches are required. Parabens are a class of compounds whose antimicrobial activity is employed in many cosmetic and pharmaceutical products. We developed propylparaben nanoparticles that are hydrophilic, thus expanding the applicability of parabens to aqueous systems. In this paper we assess the possi

New injectable and radiopaque antibiotic loaded acrylic bone cements.

The use of antibiotic loaded bone cements (ALBCs) has become a common clinical practice in the prevention and treatment of prosthesis-related infections. However, due to antibiotic resistance, there is a general interest in broadening the antibacterial spectrum of currently used drugs. The aim of this work is to formulate ALBCs for specific use in vertebroplasty and kyphoplasty, and to study the effect of the addition of ciprofloxacin alone and in combination with vancomycin on some properties of the cement. The cements were formulated using bismuth salicylate as the radiopacifier. The setting

Injectable and In Situ Hydration-Reinforced Hybrid Bone Cements for Accelerated Bone Regeneration.

Injectable bone cements are recognized as ideal solution for bone augmentation due to their minimally invasive introduction strategy. However, current clinical formulations and composites, while demonstrating improvements in certain areas, often fail to comprehensively address challenges, including rheological injectability, mechanical stability, interconnected porosity, degradability, and bioactivity. To overcome limitations, a novel bone cement comprising linear polyhydroxy PEGylated poly(glycerol sebacate) (L-PEGS) and calcium phosphate cement (CPC) is developed. The incorporation of L-PEGS

Calcium phosphate bone cements as local drug delivery systems for bone cancer treatment.

Bone cancer is usually a metastatic disease, affecting people of all ages. Its effective therapy requires a targeted drug administration locally at the cancer site so that the surrounding healthy organs and tissues stay unharmed. Upon a thorough literature search, a tremendous number of published articles are reporting on development of calcium phosphate cements (CPCs) for the treatment of a variety of diseases, such as osteoporosis, osteoarthritis, osteomyelitis, and other musculoskeletal disorders. However, just a limited number of research employs CPCs specifically for bone cancer treatment

Porosity reduction in bone cement at the cement-stem interface.

The fatigue failure of bone cement, leading to loosening of the stem, is likely to be one mode of failure of cemented total hip replacements. There is strong evidence that cracks in the cement are initiated at voids which act as stress risers, particularly at the cement-stem interface. The preferential formation of voids at this site results from shrinkage during polymerisation and the initiation of this process at the warmer cement-bone interface, which causes bone cement to shrink away from the stem. A reversal of the direction of polymerisation would shrink the cement on to the stem and red

Calcium phosphate bone cements with α-ketoglutarate polyester microspheres promote osteoporotic bone defect repair.

Excessive oxidative stress and inflammation are prevalent in osteoporotic bone defects, significantly impairing the efficacy of bone regeneration in such defects. Calcium phosphate bone cements (CPC) are a commonly employed material for repairing bone defects. However, the slow rate of osteogenic mineralization and biodegradation presents a significant challenge in meeting the requirements of osteoporotic bone treatment. In this study, α-ketoglutarate (α-KG) polyester microspheres were synthesized from α-KG. By introducing different mass ratios of α-KG polyester microspheres, we constructed CP

Bone cement implantation syndrome in patients with cemented endoprostheses for metastatic bone disease in the femur.

BACKGROUND: Patients with bone metastases in the femur (BMf) may experience pathological fractures requiring surgery with cemented endoprostheses (EPR). At cementation and prosthesis insertion, patients are at risk of experiencing hypoxia, hypotension, cardiac failure and potentially death, known as bone cement implantation syndrome (BCIS). We aimed to 1) investigate the incidence and grade of BCIS in patients with BMf treated with cemented EPR, and 2) examine if the incidence or extent of BCIS has decreased after a change of a combined anesthesiologic and surgical protocol. METHODS: We retro

[Allergy to bone cement components].

Intolerance reactions to endoprostheses may lead to allergological diagnostics, which focus mainly on metal allergy. However, bone cement may also contain potential allergens, e.g. acrylates and additives such as benzoyl peroxide (BPO), N,N-dimethyl-p-toluidine, hydroquinone, and antibiotics (particularly gentamicin). In the Munich implant allergy clinic, we found that 28 of 113 patients (24.8%) with cemented prostheses had contact allergies to bone cement components, mostly to gentamicin (16.8%) and BPO (8.0%). The clinical significance of test results cannot always be shown, but we still rec

[Allergy towards bone cement].

Bone cements based on polymethylmethacrylate are typically used for fixation of artificial joints. Intolerance reactions to endoprostheses not explained by infection or mechanical failure may lead to allergological diagnostics, which mostly focuses on metal allergy. However, also bone cement components may provoke hypersensitivity reactions leading to eczema, implant loosening, or fistula formation. Elicitors of such reactions encompass acrylates and additives such as benzoyl peroxide, N,N-dimethyl-p-toluidine, hydroquinone, or antibiotics (particularly gentamicin). Upon repeated contact with

Functional Properties of Low-Modulus PMMA Bone Cements Containing Linoleic Acid.

Acrylic bone cements modified with linoleic acid are a promising low-modulus alternative to traditional high-modulus bone cements. However, several key properties remain unexplored, including the effect of autoclave sterilization and the potential use of low-modulus cements in other applications than vertebral augmentation. In this work, we evaluate the effect of sterilization on the structure and stability of linoleic acid, as well as in the handling properties, glass transition temperature, mechanical properties, and screw augmentation potential of low-modulus cement containing the fatty aci

Insufficient Penetration of Bone Cement Into the Trabecular Bone: A Potential Risk for Delayed Bone Cement Displacement After Kyphoplasty?

OBJECTIVE: Balloon kyphoplasty is a minimally invasive procedure used in the treatment of vertebral compression fractures. Although cement leakage is a well-known complication of the procedure, delayed displacement of the injected bone cement material, occurring several weeks after kyphoplasty, is a rare occurrence. In this report, we describe a case of delayed dislodgement of the bone cement occurring 4 weeks after successful kyphoplasty for an osteoporotic compression fracture of L4. CASE REPORT: A balloon kyphoplasty was successfully performed for the clinical management of an osteoporotic

Strontium release from Sr2+-loaded bone cements and dispersion in healthy and osteoporotic rat bone.

Drug functionalization of biomaterials is a modern and popular approach in biomaterials research. Amongst others this concept is used for the functionalization of bone implants to locally stimulate the bone healing process. For example strontium ions (Sr2+) are administered in osteoporosis therapy to stimulate bone growth and have recently been integrated into bone cements. Based on results of different analytical experiments we developed a two-phase model for the transport of therapeutically active Sr2+-ions in bone in combination with Korsmeyer-Peppas kinetics for the Sr2+ release from bone

Composite bone cements based on halloysite nanotube with enhanced drug elution.

Antibiotic-loaded PMMA (polymethylmethacrylate) bone cement (ALBC) is widely used to prevent and treat periprosthetic joint infections (PJIs), yet its clinical efficacy is limited by issues like burst release and short release duration. To address these challenges, this study developed a composite bone cement (HV-PMMA) loaded with vancomycin-functionalized halloysite nanotubes (HNTs-Van). The results showed that HV-PMMA optimized antibiotic elution: it avoided initial burst release, and the drug elution amount of HV-PMMA was superior to that of traditional ALBC with vancomycin formulation.
