Bone transport with magnetic intramedullary nails in long bone defects.

BACKGROUND: This study describes the outcomes of internal bone transport with magnetic nails in five cases of traumatic segmental femoral bone defects. METHODS: Five patients with open fracture of the femur and diaphyseal bone loss were included between May 2018 and August 2020. The mean femoral defect was 8.7 cm (range 5.6-16.0). INTERVENTION: We used plate-assisted bone segment transport (PABST) with PRECICE magnetic nails. RESULTS: All five patients have fully consolidated. The mean consolidation time and index were 7.5 months and 0.8 mo/cm, respectively. The mean follow-up was 21.3 mont

Bone ingrowth into dynamically loaded porous-coated intramedullary nails.

Research to determine whether porous-coated Vitallium intramedullary rods could be used to bridge segmental defects in long bones was performed using rabbit tibias as a suitable model for human bone growth. One-centimeter segments of the midshaft of tibias of mature rabbits were removed surgically and replaced with equal-sized segments of Silastic tubing to maintain leg length. A coated rod was inserted through the proximal tibial segment, through the tubing, and into the distal bone segment. The legs were taken out of plaster at 3 weeks, and x-rays were taken periodically until sacrifice. At

Intramedullary nails for long bone metastases: why do they fail?

Metastatic disease to long bones is common and often requires stabilization to treat or prevent fracture. Intramedullary fixation is used in many metaphyseal and diaphyseal lesions. The goal of this study was to investigate the causes of and risk factors for reconstructive failure in intramedullary fixation of metastatic disease. We performed a retrospective review of 112 consecutive reconstructions for metastatic disease treated with an isolated intramedullary nail. There were 81 reconstructions in the femur, 25 in the humerus, and 6 in the tibia. All included patients were followed until dea

[The monorail system--bone segment transport over unreamed interlocking nails].

A treatment protocol is demonstrated, consisting of an osteotomy, either proximal or distal, of the bone defect with subsequent segmental transport via an anteromedially (tibia) or laterally (femur) mounted AO external fixation over an unreamed interlocking nail (monorail system). Twenty patients were treated by this method with indications as follows: 13 had a segmental bone defect of the tibia, 3 of the femur. Three patients showed post-traumatic and postinfectious leg-length discrepancies and one was treated for hypertrophic non-union of the femur. Defect distance varied between 5 and 18.5

[Areas of safe use of bone nails on the forearm].

On the basis of the data of topography-anatomical study, carried out at 30 upper extremities, are proposed zones and directions of safe application of wires of the compression-distraction apparatus at 4 forearm levels, developed with allowance made for individual differences in structure and topography of vessels and nerves as well as functional position of the segment. An experimental examination (30 extremities) and clinical approval (30 patients) of the proposed zones and directions have confirmed the practical use of the obtained data that allows to recommend them to be widely introduced i

Bone dependent nail formation.

Two sporadic cases with congenital anonychia and hypoplastic nails combined with ectrophalangia or hypoplastic phalanges are reported. It is suggested that congenital anonychia and hyponychia may be 'bone territory' dependent disorders.

Palliative percutaneous stabilization of lower extremity for bone metastasis using flexible nails and bone cement.

OBJECTIVE: Percutaneous stabilization (PS; percutaneous flexible nailing and intramedullary bone cement injection) was performed at lower extremity long bones in patients with multiple bone metastases with short life expectancy to get mechanical stability and local tumor control. We evaluated the usefulness of PS by clinical status, F-18-FDG PET-CT and bone scintigraphy (BS). METHODS: Patients comprised 15 patients (total 20 sites) who had undergone PS for the metastatic bone tumors of lower extremity long bones (femur and tibia). After percutaneous flexible nailing, bone cement was injected

Complications and risk factors of intramedullary bone lengthening nails: a retrospective multicenter cohort study of 314 FITBONE and PRECICE nails.

BACKGROUND AND PURPOSE: Intramedullary bone-lengthening nails have become increasingly popular. The 2 most used and successful nails are the FITBONE and the PRECICE nails. Uniform reporting is lacking on complications of intramedullary bone-lengthening nails. The purpose was therefore to assess and categorize the complications of lower limb bone-lengthening nails and investigate risk factors. PATIENTS AND METHODS: We performed a retrospective review of patients operated on with intramedullary lengthening nails at 2 hospitals. We included only lower limb lengthening with FITBONE and PRECICE na

The effect of retained intramedullary nails on tibial bone mineral density.

BACKGROUND: Intramedullary nailing has become a standard treatment for adult tibial shaft fractures. Retained intramedullary nails have been associated with stress shielding, although their long-term effect on decreasing tibial bone mineral density is currently unclear. The purpose of this study was to determine if retained tibial intramedullary nails decrease tibial mineral density in patients with successfully treated fractures. METHODS: Patients treated with statically locked intramedullary nails for isolated, unilateral tibia shaft fractures were studied. Inclusion required that fracture

Congenital brachydactyly and nail hypoplasia: clue to bone-dependent nail formation.

Congenital hyponychia and anonychia are rare malformations which may form part of syndromes such as nail-patella syndrome, ectodermal dysplasias and brachydactylies, or may occur as an isolated finding. Congenital hyponychia and anonychia are frequently accompanied by underlying skeletal abnormalities. A 20-year-old woman showed congenital bilateral hypoplasia or aplasia of the second, third and fourth toenails with corresponding phalanx dysplasia or aplasia of the affected toes. Malformations of the hands or other congenital defects were absent. The findings in this patient do not exactly fit

3D printed bone nails loaded with ceftriaxone sodium for localized drug delivery.

The convergence of advanced materials science and additive manufacturing technologies has revolutionized medical applications ranging from orthodontic devices to patient-specific bone fixation systems. Despite these advancements, the clinical translation of 3D-printed orthopedic implants remains constrained by suboptimal biocompatibility of conventional manufacturing materials. This study systematically evaluates dental resin-based composites as alternative biomaterials for bone fixation devices, hypothesizing that their rapid photopolymerization kinetics, mechanical robustness, and inherent b

Outcome of Both Bone Forearm Fracture Fixation in Children by Rush Nails.

INTRODUCTION: Forearm fractures are common upper limb injuries among children and usually treated non-operatively. Failure of non-operative treatment, open injuries and multiple fractures are the indications for surgery in paediatric both bone forearm fractures. Intramedullary nailing is considered as minimally invasive procedure with excellent to fair outcomes but it is not free of complications. We reviewed the results and evaluated the outcomes of IM fixation of forearm fractures in children by Rush nails to understand the risks and complications associated with these procedures. METHODS:

Transverse nail ridging (Beau's lines) and rough nails after bone marrow transplantation.

BACKGROUND: Tibial hemimelia is a rare malformation with a wide clinical spectrum of presentation. The severity of this condition can be typed using different classification systems. It can exist as an independent entity or can be associated with upper limb or visceral malformations. The aims of our study are therefore, a. to report the incidence of upper limb deformities in relation to the severity of tibial hemimelia classified by the Jones classification, b. incidence of tibial hemimelia as a part of a syndrome c. to report the overall incidence of the associated upper limb and visceral def

[Evaluation of impaction on bone nail and bone plate of the pertrochanteric fracture].

PURPOSE OF THE STUDY: Impaction in pertrochanteric fracture sites is a well known phenomenon; the screw-plate system is designed to stabilise the fracture. Although easier to use, the risk with the nail-plate system is postoperative penetration of the nail into the joint. The present study was conducted to determine the exact conditions of the impaction, and to identify possible ways to improve the nail-plate system. MATERIAL-METHOD: The study included 129 cases of pertrochanteric fracture, excluding sub-trochanteric fractures. All fractures were fixed with a 130 degrees angulated nail-plate.

[Retrograde bone transport nail in a posttraumatic femoral bone defect].

We report the case of a 28-year-old man who developed nonunion with complex deformity after treatment of a distal femoral fracture with an antegrade femoral nail. The resulting deformity was as follows: 10° varus, 21° external torsion, 1.8 cm of foreshortening and translation malalignment. After resection of the pseudarthrosis, a retrograde segmental transport nail was implanted. During the same surgical procedure, acute internal torsion, valgization and lengthening correction was performed. The segment transport was performed using a magnetically driven internal transport nail. Seven months a
