Effect of Posterior Glenoid Bone Loss and Retroversion on Arthroscopic Posterior Glenohumeral Stabilization.

BACKGROUND: Posterior glenohumeral instability is an increasingly recognized cause of shoulder instability, but little is known about the incidence or effect of posterior glenoid bone loss. PURPOSE: To determine the incidence, characteristics, and failure rate of posterior glenoid deficiency in shoulders undergoing isolated arthroscopic posterior shoulder stabilization. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: All patients undergoing isolated posterior labral repair and glenoid-based capsulorrhaphy with suture anchors between 2008 and 2016 at a single institution were iden

Posterior Open-wedge Osteotomy and Glenoid Concavity Reconstruction Using an Implant-free, J-shaped Iliac Crest Bone Graft in Atraumatic Posterior Instability with Pathologic Glenoid Retroversion and Dysplasia: A Preliminary Report.

BACKGROUND: Atraumatic posterior shoulder instability in patients with pathologic glenoid retroversion and dysplasia is an unsolved problem in shoulder surgery. QUESTIONS/PURPOSES: In a preliminary study of a small group of patients with atraumatic posterior shoulder instability associated with glenoid retroversion ≥ 15° and glenoid dysplasia who underwent posterior open-wedge osteotomy and glenoid concavity reconstruction using an implant-free, J-shaped iliac crest bone graft, we asked: (1) What proportion of the patients had persistent apprehension? (2) What were the improvements in patient

Assessment of rotation in proximal humeral osteotomy.

The correlation between the shift measured on bone surface in rotatory osteotomy and the radiologic change of the retroversion angle was analyzed. The humeral head retroversion was measured radiographically in 7 isolated humeri by means of a semiaxial projection. All bones were then osteotomized and the humeral head rotated in order to increase and decrease the retroversion. The retroversion was measured radiographically for each new rotational position of the head. The relationship between the measured shift in rotatory osteotomy and the change of retroversion was found to be linear. Twelve p

Combined Glenoid Osteotomy and Bone Block Augmentation for Severe Posterior Shoulder Dysplasia and Instability: A Case Report.

CASE: A 26-year-old man with recurrent posterior shoulder instability in the setting of glenoid dysplasia (41° of glenoid retroversion and 18% posterior glenoid bone loss) underwent combined opening-wedge osteotomy and posterior bone-block augmentation surgery. Postoperative imaging demonstrated retroversion correction to 9° of residual glenoid retroversion and increase in the glenoid arc by 22%. At 18 months, he reports no pain or instability and has regained full motion and strength. CONCLUSION: Combined glenoid osteotomy and posterior bone block technique is a novel treatment option for po

Augmented glenoid implants in anatomic total shoulder arthroplasty: review of available implants and current literature.

Glenoid bone loss and retroversion increase the complexity of primary shoulder arthroplasty and affect the outcome. Although eccentric reaming, augmented glenoid implants, bone grafting, and reverse arthroplasty have been used to manage bone loss and retroversion, there is no consensus on treatment. Posteriorly augmented glenoid components can correct retroversion and avoid joint line medialization, which occurs with corrective reaming techniques. Full-wedged, half-wedged, and stepped polyethylene posteriorly augmented designs are currently available for use in the United States. The results o

Shoulder arthroplasty in the presence of posterior glenoid bone loss.

➤ Chronic osteoarthritis of the glenohumeral joint, traumatic injury, post-reconstruction arthropathy, and developmental conditions such as glenoid dysplasia can result in posterior glenoid bone loss and excessive retroversion of the glenoid. Shoulder replacement in this setting is technically challenging and characterized by higher rates of complications and revisions.➤ Current options that should be considered for managing glenoid bone loss that results in >15° of retroversion include bone-grafting, augmented glenoid components, and reverse total shoulder replacement.➤ Asymmetric reaming is

Considerations For Surgical Planning Of Humeral Osteotomy In Brachial Plexus Birth Palsy Based On The Elbow Crease And Humeral Retroversion Measurement.

In obstetric brachial plexus, injuries are characterized by contractures, weakness and an association with different bone deformities, such as elongation of the coracoid process and/or acromion, retroversion of the glenoid cavity, flattening of the humeral head and Scapular Hypoplasia with Elevation and Rotation (SHEAR). Another humeral deformity is the retroversion of the humeral head, cases of which are increased in the presence of plexus lesions. The purpose of the current manuscript is to highlight two aspects of this latter deformity. First, the importance of taking into consideration the

Total shoulder arthroplasty using trabecular metal augments to address glenoid retroversion: the preliminary result of 10 patients with minimum 2-year follow-up.

BACKGROUND: Options to address glenoid retroversion include eccentric reaming, bone grafting, modifications to component shape, and reverse shoulder arthroplasty. Trabecular metal (TM) augments have been used extensively in the hip and knee to address bone deficiency in arthroplasty as part of a hybrid combination of high-density polyethylene, polymethyl methacrylate, and TM. This study presents the initial results of the use of specifically designed augments in the shoulder to address glenoid retroversion as part of total shoulder arthroplasty (TSA). MATERIALS: Ten patients (4 women and 6 me

Pelvic morphology differs in rotation and obliquity between developmental dysplasia of the hip and retroversion.

BACKGROUND: Developmental dysplasia of the hip (DDH) and acetabular retroversion represent distinct acetabular pathomorphologies. Both are associated with alterations in pelvic morphology. In cases where direct radiographic assessment of the acetabulum is difficult or impossible or in mixed cases of DDH and retroversion, additional indirect pelvimetric parameters would help identify the major underlying structural abnormality. QUESTIONS/PURPOSES: We asked: How does DDH and retroversion differ with respect to rotation and coronal obliquity as measured by the pelvic width index, anterior inferi

Multilevel measurement of acetabular version using 3-D CT-generated models: implications for hip preservation surgery.

BACKGROUND: Numerous structural deformities of the acetabulum are associated with hip osteoarthritis (OA). Acetabular retroversion has been implicated in the development of femoroacetabular impingement. However, it is unknown if retroversion occurs from isolated excessive bone in the superior acetabulum or a torsional phenomenon of the entire pelvic segment. QUESTIONS/PURPOSES: A method was developed to measure acetabular version (AV) using 3-D models thus eliminating positional factors. This method was used to determine if acetabular retroversion is an isolated entity or an effect involving

Acetabular retroversion is prevalent and proportional to the severity of slipped upper femoral epiphysis.

AIMS: Slipped upper femoral epiphysis (SUFE) has well documented biochemical and mechanical risk factors. Femoral and acetabular morphologies seem to be equally important. Acetabular retroversion has a low prevalence in asymptomatic adults. Hips with dysplasia, osteoarthritis, and Perthes' disease, however, have higher rates, ranging from 18% to 48%. The aim of our study was to assess the prevalence of acetabular retroversion in patients presenting with SUFE using both validated radiological signs and tomographical measurements. METHODS: A retrospective review of all SUFE surgical cases prese

A two- stage surgery for severe femoral neck deformity due to fibrous dysplasia: a case report.

Various kinds of surgical treatments have been reported for varus deformity of the proximal femur due to fibrous dysplasia. We report a case of two-stage corrective osteotomy for severe varus-retroversion deformity of the femoral neck due to monostotic fibrous dysplasia. The patient was an 18 year-old man. On initial examination, the spina malleollar distance was 88 cm on the right side and 83 cm on the left. Plain radiography showed prominent varus deformity of his left proximal femur. The morphology was 130 degrees on the right side and 85 degrees on the left. Computed tomography revealed 60

Long-term results after posterior open glenoid wedge osteotomy for posterior shoulder instability associated with excessive glenoid retroversion.

BACKGROUND: Treatment of posterior shoulder instability (PSI) associated with excessive glenoid retroversion is a rare, challenging problem in shoulder surgery. One proposed technique is posterior open wedge glenoid osteotomy to correct excessive glenoid retroversion as described by Scott. However, this operation is rarely performed, and limited long-term outcomes using this approach are available. The goal of this study was to analyze the long-term outcomes of posterior open wedge glenoid osteotomy for PSI associated with excessive glenoid retroversion. METHODS: Six consecutive patients (7 s

A Newly Discovered Tendon Between the Genioglossus Muscle and Epiglottic Cartilage Identified by Histological Observation of the Pre-Epiglottic Space.

Epiglottic retroversion is difficult to explain anatomically. One reason is inadequate structural identification of the ligaments in the submucosal tissue anterior to the epiglottis (pre-epiglottic space, PES). Although studies have shown that tongue root movement plays a role in epiglottic retroversion, few morphological reports have investigated the attachment of the lingual muscles to the epiglottis. This study reconstructed the fiber structure of the PES by comprehensively analyzing fiber alignment in the PES focusing on the hyoepiglottic ligament, which runs between the lingual muscles an

Baseplate version in reverse shoulder arthroplasty: does excessive retroversion or anteversion affect functional activities of daily living?

PURPOSE: While bone grafting and augmented components can help restore reverse shoulder arthroplasty (RSA) baseplate version close to neutral, the indication for version correction in RSA is unclear. The purpose of this study was to compare the clinical outcomes of RSA baseplates in high degrees of retroversion and anteversion to components in more neutral version. METHODS: Patients who underwent RSA with minimum two year follow-up were identified from an institutional registry. An RSA system with a glenosphere that is two-thirds of a sphere was utilized. The effects of broiler chicken growth rate on valgus, varus, and tibial dyschondroplasia.

An experiment was conducted to test the hypothesis that the growth rate of broilers influences their susceptibilities to bone abnormalities, causing major leg problems. Leg angulations, described in the twisted legs syndrome as valgus and bilateral or unilateral varus, were investigated in 2 subpopulations of mixed-sex Arkansas randombred broilers. Valgus angulation was classified as mild (tibia-metatarsus angle between 10 and 25°), intermediate (25-45°), or severe (> 45°). Body weight was measured at hatch and weekly until 6 wk of age. There were 8 different settings of approximately 450 eggs

Posterior Acetabular Reconstruction Can Sometimes Lead To Retroverted Cup Placement In Complex Total Hip Arthroplasty.

INTRODUCTION: Acetabular fractures with uncontained posterior wall and posterior column bone loss make orientation of the acetabular component difficult during total hip arthroplasty (THA). When the posterior buttress is deficient, the socket tends to settle in retroversion, which alters hip biomechanics, promotes edge loading and accelerated polyethylene wear, and predisposes to anterior impingement and instability. Structural femoral head autograft restores bone stock in such defects, but its influence on final cup version is seldom documented. To our knowledge, this is the first series to s
