[Brain contusion: morphology, pathogenesis and treatment].

Focal cerebral contusions can be dynamic and expansive, leading to a delayed neurological deterioration. In head--injured patients, the rise in intracranial pressure (ICP), subsequent to uncontrollable swelling, is the only and the most frequent cause of death. Studies show that brain swelling, after traumatic brain injury (TBI), is caused by brain edema rather than cerebral blood volume (CBV). CBV is reduced in proportion to cerebral blood flow (CBF) reduction, following a severe TBI. Cerebrovascular damages, leading to subsequent reductions in regional CBF, may play an important role in seco

Blood-brain barrier damage in traumatic brain contusions.

Plasma proteins were used as an endogenous marker of blood-brain barrier damage in 19 patients dying with traumatic cortical contusions. Patients survived for a few hours to 31 days after head injury. Eight proteins (M WT 61-2,500 x 10(3) were demonstrated with standard immunohistochemical techniques. Proteins were not found in "control" brains or in macroscopically normal parasagittal cortex in the head injury patients. Proteins were found in all of the macroscopic contusion in all brains. Protein leakage appeared to be from the contusion itself. Protein staining around histologically normal

Brain Abscess Complicating a Traumatic Intracerebral Hemorrhagic Contusion.

A 63-year-old right-handed man was admitted to our hospital after sustaining a traumatic right-sided subdural hematoma, subarachnoid hemorrhage, and temporal lobe hemorrhagic contusion. He was managed non-operatively and discharged without any neurologic deficits. Two weeks later he presented with worsening headaches and altered sensorium. Imaging studies showed that the patient had developed a cerebral abscess at the site of his intracerebral hemorrhage. The abscess was surgically resected, and the patient was treated with antibiotics with complete resolution of symptoms. This case illustrate

Brain contusion with aphasia following an ice hockey injury.

Head injuries are relatively common in ice hockey, with the majority represented by concussions, a form of mild traumatic brain injury. More severe head injuries are rare since the implementation of mandatory helmet use in the 1960s. We present a case of a 27 year-old male who sustained a traumatic intraparenchymal hemorrhage with an associated subdural hematoma resulting after being struck by a puck shot at high velocity. The patient presented with expressive aphasia, with no other apparent neurologic deficits. Acutely, he was successfully treated with observation and serial neuroimaging stud

Ultrastructural study of brain microvessels in patients with traumatic cerebral contusions.

Brain tissue from 11 patients with traumatic cerebral contusions submitted to surgery was studied. Control biopsy specimens were obtained from 5 patients undergoing ventriculo-peritoneal shunts for "communicating" hydrocephalus. After collection, the small fragments were fixed by immersion in glutaraldehyde-osmium and embedded in Epon. Semi-thin sections stained with toluidine blue were observed with the light microscope. Thin sections stained with lead citrate and uranyl acetate were observed using a Jeol electron microscope. In tissues from patients with head trauma a clear space most probab

Prognostics of Hospitalization Length and Mortality in Patients with Traumatic Frontal Brain Contusions.

Traumatic brain injury has ripple effect on the physical, cognitive, behavioral, and emotional domains of quality of life and portends a long-term neurological disability in survivors. In this study we evaluated the prognostic role of demographic and clinico-radiological variables on the hospitalization length and mortality in 71 of patients with frontal brain contusions. The receiver operating characteristic (ROC) plots were performed, with area under the curve (AUC) values, for graphical comparison of variables that would predict mortality and hospitalization length. We found that the best p

[Headache as the consequence of brain concussion and contusion in closed head injuries in children].

The most frequent type of head injury in children is closed head trauma with brain concussion or contusion, and headache is the dominant complaint of early and late postinjury period. Because of scant number of studies on the problem of occurrence, characteristics and persistence of posttraumatic headache this study was undertaken in a group of 100 children (29 girls and 71 boys), aged 3-14 years old, 90 after brain concussion and 10 after contusion. Children with a history of injuries, central nervous system infections, with headaches before injury and chronic diseases were excluded. In 9 cas

[The orthostatic resistance of the cerebral and peripheral circulations in patients with contusive brain lesions].

Orthostatic tests with printing recording of EEG, REG, RVG were used to examine the functional status of the truncal level of circulatory regulation in patients with brain confusions. Within the first 24 hours after brain injury there was a significant reduction in the pulse blood filling of cerebral vessels in the vascularization areas of carotid and vertebral arteries along with higher vascular tone; pulse blood filling of peripheral vessels in the lower extremities remained normal with a marked decrease in their vascular tone. Orthostatic tests showed the asymmetric enhancement of a dilatin

The effect of cyclosporin A on brain edema formation following experimental cortical contusion.

Cyclosporin A (CsA) has been shown by our laboratory and others to be neuroprotective in the experimental animal model of traumatic brain injury. However, we found that the intrathecal administration resulted in a concomitant increase of brain edema. The aim of this study was to assess whether intravascular administration may also influence brain edema formation. This project includes two independent series in which different doses of CsA were intravenously given to Sprague-Dawley rats of each group. In the first series, the animals were exposed to focal brain injury by a controlled cortical i

Frontal sinus volume predicts incidence of brain contusion in patients with head trauma.

BACKGROUND: The function of the paranasal sinuses is a topic of debate. One hypothesis suggested has been that sinuses act as a "crumple zone," serving to protect the contents of the head from impact. In this study, we examine the interactions of the frontal sinus and the brain in the setting of head trauma. Our formal hypothesis is that frontal sinus volume is less in head trauma patients with contusion than in head trauma patients without contusion. METHODS: Computed tomographic (CT) scans of patients who sustained blunt head trauma performed at the University of Pittsburgh Medical Center f

Synaptic remodeling and free radical formation after brain contusion injury in the rat.

The purpose of this study was to explore whether bilateral frontal cortex contusion in rats would demonstrate changes relevant for understanding the pathology of frontal lobe injury in humans. Rats were allowed to survive for 3, 7, or 18 days postinjury (dpi). In the contused rats, albumin was trapped in frontal cortices, as well as in other brain areas, showing that neurons were exposed to plasma components. In the sham-operated rats, which had only craniotomy but no penetration of dura, the level of trapped albumin was also increased compared to intact controls, suggesting a partial lesion-l

[Perimetric diagnosis of chiasmal injury after contusion of skull and brain (author's transl)].

After a discussion of the pathomechanism of skull trauma by a blow leading to chiasmal lesion, frontobasal injuries of the skull are considered; it is shown that typical defects of the visual field resulting from chiasmal damage can be revealed not only in cases of serious industrial or traffic accidents with brain concussion and skull base fracture, but also following minor injuries without unconsciousness and radiologically visible bone fractures.

Water transport becomes uncoupled from K+ siphoning in brain contusion, bacterial meningitis, and brain tumours: immunohistochemical case review.

Specimens of normal human brain, contused brain, brain with bacterial meningitis, and brain tumours were immunolabelled for aquaporin 4 (AQP4) and Kir4.1. In normal brain tissue, AQP4 and Kir4.1 were detected around the microvessels. In pathological brain tissue, AQP4 was upregulated in astrocytes in oedematous regions and Kir4.1 was upregulated in astrocytes in damaged brain. Changes in alpha syntrophin expression paralleled those of AQP4 and Kir4.1. The following hypothesis is proposed: in astrocytes, under normal conditions, AQP4 couples water transport with Kir4.1 mediated K+ siphoning, bu
