Early Treatment With a Single Dose of Mesenchymal Stem Cell Derived Extracellular Vesicles Modulates the Brain Transcriptome to Create Neuroprotective Changes in a Porcine Model of Traumatic Brain Injury and Hemorrhagic Shock.

BACKGROUND: Cell-based therapies using mesenchymal stem cell derived extracellular vesicles (EVs) improve neurologic outcomes in animal models of traumatic brain injury (TBI), stroke, and hemorrhage. Using a porcine 7-day survival model of TBI and hemorrhagic shock (HS), we previously demonstrated that EV-treatment was associated with reduced brain lesion size, neurologic severity score, and cerebral inflammation. However, the underlying cellular and genomic mechanisms remain poorly defined. We hypothesize that EV treatment modulates the brain transcriptome to enhance neuroprotection and neuro

Acute traumatic midbrain hemorrhage: experimental and clinical observations with CT.

Traumatic brain stem hemorrhage (TBH) after blunt head impact is an uncommon injury and has historically been associated with high mortality. Retrospective clinical review identified 64 patients with TBH admitted during a 5-year period. Complete imaging and clinical records for 45 of these patients demonstrated that TBH could be categorized into three groups. The most frequent site of hemorrhage, in 31 (69%) of 45 patients (group 1), was the midline rostral anterior brain stem, posterior to the interpeduncular cistern, and this injury was associated with a 71% survival rate. This pattern was a

Acute post-traumatic intraventricular hemorrhage analysis of 25 patients with emphasis on final outcome.

A series of 25 patients suffering acute post-traumatic intraventricular hemorrhage (IVH) after closed head injury is reviewed. Post-traumatic IVH was associated with other brain lesion in most cases; intracerebral lesions with contusion, subdural hematoma, and epidural hematoma coexisted with post-traumatic IVH in ten, three, and one instance, respectively. In six cases, no CT abnormality other than IVH was noted. The final outcome in this series is poor, only 10 (40%) patients made a functional survival. Four different groups of IVH were proposed according to the CT scan findings: deep nuclei

Small hemorrhages in the brain stem. A sign of injury?

Microscopic hemorrhages in the brain stem were studied in cases of known head injury and in cases where head injury was known not to have occurred. They were found to be either strictly limited to the perivascular space or to extend into the neuropil. Those limited to the perivascular space appear to be artifacts and should not be considered when determining the cause of death. Extravasations of red cells into the neuropil may be of traumatic genesis. Such hemorrhages may be absent, however, in cases of a fatal primary brain-stem injury.

Primary brain stem lesions caused by closed head injuries.

Traumatic lesions of the brain stem are of two types: primary, which are considered to be caused at the moment of impact, and secondary, associated with supratentorial mass lesion. Of the 239 patients with a serious head injury who showed a severe disturbance of consciousness upon admission and who had CT scan carried out immediately, 21 cases were considered to have a primary brain stem lesion with initial CT scan. A primary brain stem lesion was found in 21 of 239 (8.8%) of patients with serious head injury. Their injuries were caused primarily by traffic accidents. Sixteen of the 21 cases s

Intracranial subarachnoid hemorrhage following lumbar myelography in two dogs.

Intracranial subarachnoid hemorrhage is a rare but serious complication of lumbar puncture in humans. Possible sequelae include increased intracranial pressure, cerebral vasospasm, or mass effect, which can result in dysfunction or brain herniation. We describe two dogs that developed intracranial subarachnoid hemorrhage following lumbar myelography. In both dogs, myelography was performed by lumbar injection of iohexol (Omnipaque). Both the dogs underwent uneventful ventral decompressive surgery for disk herniation; however, the dogs failed to recover consciousness or spontaneous respiration

Placenta-Derived Cells for Acute Brain Injury.

Acute brain injury resulting from ischemic/hemorrhagic or traumatic damage is one of the leading causes of mortality and disability worldwide and is a significant burden to society. Neuroprotective options to counteract brain damage are very limited in stroke and traumatic brain injury (TBI). Given the multifaceted nature of acute brain injury and damage progression, several therapeutic targets may need to be addressed simultaneously to interfere with the evolution of the injury and improve the patient's outcome. Stem cells are ideal candidates since they act on various mechanisms of protectio

[A treatment procedure for nontraumatic cerebellar hemorrhages].

The results of surgical treatment of 12 patients with non-traumatic intracerebellar hemorrhages are analyzed in this paper. The following parameters were assessed: clinical signs and symptoms, the level of consciousness, location and size of the lesion, the brain stem deformation, the rupture of hematoma into the adjacent ventricle, and hydrocephalus. In all patients with fulminant course as well as in the majority of patients with acute or subacute course when the brain stem compression progresses and seems to be unresponsive to medical treatment, surgery is strongly indicated. Fatal transorbital penetrating intracranial injury caused by a bicycle hand brake.

A transorbital penetrating intracranial injury is a rare and severe traumatic brain injury. Patients with this type of injury may present dramatically, but often the injury is subtle and therefore easily overlooked and not recognized in the first place. We present the case of a 45-year-old female admitted to the emergency department after she fell with her bike and the bicycle brake handle penetrated her left eye. A computerized tomography of the cerebrum showed a fracture of the superior orbital roof with multiple bone fragments extending into the brain near the circle of Willis. A pneumoceph

Non-surgical treatment of massive traumatic corpus callosum hematoma after blunt head injury: A case report.

Massive hematoma of the corpus callosum caused by blunt head trauma is an extremely rare lesion. Most frequent traumatic lesions involve the corpus callosum are diffuse axonal injuries. They might be associated with small hemorrhagic foci in the hemispheric and brain stem white matter, intraventricular hemorrhages, subarachnoid hemorrhages, traumatic lesions of the septum pellucidum and fornix. Many cases of corpus callosum injury present with permanent disconnection syndrome. We present a case of a 32-year-old female suffered blunt head trauma resulted in massive corpus callosum hematoma whic

Transorbital Penetrating Intracranial Injury Caused by Sheppard's Hook.

Intracranial injury resulting from transorbital penetrating objects is rare in a noncombat setting. As such there is a significant lack of data pertaining to the management of non-projectile traumatic brain injuries due to foreign bodies entering the brain. Intracranial complications can include intracerebral hematoma, cerebral contusion, intraventricular hemorrhage, pneumocephalus, brain stem injury, and carotid cavernous sinus fistula. This is the first report of a transorbital penetrating intracranial injury caused by a Sheppard's hook, which was managed utilizing a multi-disciplinary, non-

[Traumatic intraventricular hemorrhage in severe head injury].

Among 329 cases with comatose state caused by severe head injury, 32 had primary intraventricular hemorrhage as revealed on initial CT scan (13.4%). We divided traumatic intraventricular hemorrhage into 4 types. Type 1: massive hemorrhage (9 cases). Type 2: subependymal hemorrhage (8 cases). Type 3: damage of fornix or septum pellucidum (8 cases). Type 4: Nieveau (7 cases). Most of these cases were caused by traffic accident. Shearing injury may be the most accurate mechanism to produce the intraventricular hemorrhage. Shear strain in severe head injury should cause tears of the subependymal v

[Electron microscopic and immunohistochemical studies of acute stages of severe brain injury in rats].

Few morphological studies on fluid-percussion experimental models using mechanically produced severe brain injury have been reported. This study was initiated to evaluate light and electron microscopic and immunohistochemical findings in severe brain injury models using rats. The experimental rats and the methods used were as described for a fluid-percussion model. Fluid-percussion models of traumatic brain injury produce injury rapidly by injecting fluid volumes into the epidural space of the temporal lobe. We used 5 rats which sustained various degrees of injury by high (7.0 atm), medium (5.

Infratentorial traumatic brain hemorrhage: may outcome be predicted by initial GCS?

BACKGROUND: Overall prognosis of nonpenetrating traumatic brain hemorrhage may be predicted by neurologic function days after insult. The relationship between immediate function and outcome has not been examined for infratentorial traumatic brain hemmorhage (iTBH) identified on computed tomographic (CT) scan. Given the severity of brain stem injury, it is conceivable that immediate function may be predictive. METHODS: A retrospective review of 1,500 brain injuries occurring at our institution identified 18 patients (1.2%) with iTBH on CT scan (eight brain stem, five cerebellum, five both). De

Beta-amyloid precursor protein immunohistochemistry in the evaluation of pediatric traumatic optic nerve injury.

OBJECTIVE: The pathologic markers of nonaccidental injury (NAI) of the central nervous system (CNS) of infants and young children include subdural, subarachnoid, and retinal hemorrhages. Immunohistochemical staining for beta-amyloid precursor protein (betaAPP) has been used to investigate traumatic axonal injury of the brain, brain stem, and spinal cord injuries, and has potential as an additional indicator of traumatic head injury. A single study has reported that betaAPP immunostaining of the optic nerve in CNS NAI reveals axonal damage. The purpose of our study was to explore further the ut
