The plated bayonet: a new instrument to facilitate surgery for intra-axial neoplasms of the spinal cord and brain stem. Technical note.

A new instrument for use in surgically treating intra-axial neoplasms of the spinal cord and brain stem is described. The plated bayonet allows neoplastic tissue in the spinal cord to be separated from functioning neural elements without perforating the adjacent spinal cord. In addition, the plated bayonet facilitates exposure through the very small incision necessary to remove tumors of the brain stem without damaging cranial nerves or other vital structures.

Membranous expression of glucose transporter-1 protein (GLUT-1) in embryonal neoplasms of the central nervous system.

The human erythrocyte GLUT-1 is a transmembrane protein which facilitates transport of glucose in the cell in an energy-independent fashion. Neuroectodermal stem cells show strong membrane immunoreactivitry with this marker at early developmental stages in rodents. Membranous expression by undifferentiated neuroectodermal cells gradually decreases while GLUT-1 becomes confined to the endothelial cells, when these acquire blood-brain barrier function. We thus sought to determine whether GLUT-1 expression was limited to embryonal neoplasms of the central nervous system (CNS) which are presumably

Trigeminal nerve root entry zone pilocytic astrocytoma in an adult: a rare case of an extraparenchymal tumor.

Extra-axial cerebellopontine angle (CPA) tumors account for approximately 10% of all brain neoplasms in adults. Vestibular schwannomas are the most common, followed by meningiomas. Gliomas in the CPA are rare and quite often are the exophytic extension of primary brain stem or cerebellar tumors. We describe a pilocytic astrocytoma of the CPA that was found to arise from the proximal portion of trigeminal nerve without any anatomic continuity with the brain stem and the cerebellum. Pre-operative MRI suggested a schwannoma. The proposed origin of this extremely rare tumor is the root entry zone

Magnetic resonance imaging of occult vascular malformations of the brain stem.

A characteristic appearance of occult cerebrovascular malformations (OCVMs) on high-field spin-echo magnetic resonance imaging (MRI) has been described previously. The authors report a series of 21 of these lesions located in the brain stem and examine the clinical and MRI characteristics. Most of these lesions display a prolonged, fluctuating clinical course, which suggests multiple sclerosis or brain-stem glioma. Although the possibility of hemorrhagic neoplasms mimicking the MRI appearance of OCVMs has been emphasized recently, distinguishing between these two groups of lesions should be po

Paraneoplastic brainstem encephalomyelitis and atypical form of chronic inflammatory demyelinating polyneuropathy in patient with testicular germinal tumor-is this an overlap syndrome? a case report.

Paraneoplastic neurologic syndromes are diagnosed when neurologic symptoms are associated with neoplasm and other causative factors are excluded. They may precede or be simultaneous to various types of neoplasms, mainly malignant. In men up to 45-50 years old the most common cancer causing the paraneoplastic syndrome is testicle tumor, manifesting usually as limbic/brain stem encephalitis and myelitis. Usually effective treatment of underlying neoplasm brings resolution of neurologic symptoms. But corticosteroids and intravenuous immunoglobulins are also used. In the presented case a 37-year-o

[Alteration of the early R1-component of the blink reflex in coma: topographical and prognostic value (author's transl)].

Alteration of the early component of the blink reflex (delayed latency or abolished reflex) is considered to be relatively specific for pontine lesions (multiple sclerosis, vacular lesions of the brain-stem, neoplasms, syringes); provided, the trigeminal and facial nerve are unaffected. In comatose patients the electrically, by stimulation of the supraorbital nerve elicited blink reflex was super-imposed (50 or 100 responses) with the LAB 8/e-computer (off-line). It was found that the R1 component was abolished even in cases with supratentorial lesions and secondary brain-stem alterations, whi

[Isolated cervical spinal cord hemangioblastoma--case report].

Hemangioblastomas are rare tumors which account for 0.9-2.1% of central nervous system neoplasms. The most common site of hemangioblastomas is the cerebellum, while they are rarely located in spinal cord, cerebrum and brain stem. Hemangioblastomas occur as a sporadic entity, and as a manifestation of von Hippel-Lindau syndrome. A 33-year old patient with isolated cervical spinal cord hemangioblastoma is presented.

The dorsolateral, suboccipital, transcondylar approach to the lower clivus and anterior portion of the craniocervical junction.

The authors review their experience with a dorsolateral approach to the anterior rim of the foramen magnum and adjacent region. The operative technique includes exposure of the vertebral artery at C1, partial resection of the occipital condyle and lateral atlantal mass, and extradural drilling of the jugular tubercle. This approach has been applied in six patients who harbored intradural space-occupying lesions located ventral to the lower brain stem. Excision of the neoplasm was virtually total in all but one patient, in whom biopsy was the primary goal of the intervention. No morbidity and n

Donor-derived brain tumor following neural stem cell transplantation in an ataxia telangiectasia patient.

BACKGROUND: Neural stem cells are currently being investigated as potential therapies for neurodegenerative diseases, stroke, and trauma. However, concerns have been raised over the safety of this experimental therapeutic approach, including, for example, whether there is the potential for tumors to develop from transplanted stem cells. METHODS AND FINDINGS: A boy with ataxia telangiectasia (AT) was treated with intracerebellar and intrathecal injection of human fetal neural stem cells. Four years after the first treatment he was diagnosed with a multifocal brain tumor. The biopsied tumor was

Stereotactic radiosurgery and hypofractionated stereotactic radiotherapy for management of vestibular schwannomas: initial experience with 17 cases.

BACKGROUND: Nowadays radiation treatment of patients with vestibular schwannomas (VSs) applied either as stereotactic radiosurgery (SRS) or stereotactic radiotherapy (SRT) represents a real alternative to surgical tumor resection. METHODS: During 2010-2011, 17 consecutive patients (19-75 years old) with a VS underwent treatment with SRS or SRT in our center. Microsurgery was initially offered in all cases but was declined for various reasons. Five lesions recurred after the initial partial resection. Two other patients with neurofibromatosis type 2 underwent initial surgery for a large tumor

Symmetrical Curvilinear Cytotoxic Edema Along the Surface of the Brain Stem: A Probable New Magnetic Resonance Imaging Finding of Leptomeningeal Carcinomatosis.

Lung cancer is one of the most common neoplasms to appear leptomeningeal metastasis (LM). Contrast-enhanced magnetic resonance imaging (MRI) is better diagnostic choice for LM and usually shows focal nodular or diffuse linear enhancement on the leptomeninges along the sulci and tentorium in the brain. We experienced atypical 2 cases of lung cancer in patients who showed unusual brain MRI finding of symmetrical curvilinear or band-like, nonenhancing cytotoxic edema along the surface of the brain stem. This finding is unique and different from the general findings of leptomeningeal metastasis. T

Intraventricular neurocytoma with massive brain stem involvement in a 5-year-old child.

INTRODUCTION: Central neurocytoma is a tumor of neuronal origin that should be taken into consideration in the differential diagnosis of intraventricular neoplasms. Reports of neurocytomas with an extraventricular localization are rare: to our knowledge, the case described here is the first in which a neurocytoma developed within the ventricles but also invaded the brain stem. CASE REPORT: The authors describe the unusual case of a 5-year-old boy with an intraventricular neurocytoma presenting with massive involvement of the basal nuclei and the brain stem. The patient underwent first biopsy

Cancer stem cells in the mammalian central nervous system.

Malignant tumours intrinsic to the central nervous system (CNS) are among the most difficult of neoplasms to treat effectively. The major biological features of these tumours that preclude successful therapy include their cellular heterogeneity, which renders them highly resistant to both chemotherapy and radiotherapy, and the propensity of the component tumour cells to invade, diffusely, the contiguous nervous tissues. The tumours are classified according to perceived cell of origin, gliomas being the most common generic group. In the 1970s transplacental administration of the potent neurocar

[Computed tomography in the diagnosis of complications in the early postoperative period in patients with tumors of the posterior cranial fossa].

The authors analyse the results of clinico-CT studies in complicated course of the early postoperative period in 104 patients (87 adults and 17 children) with tumors of the posterior cranial fossa (PCF). According to the histological structure and predominant topography of the neoplasm, the cases were divided into 46 extracerebral parastem tumors (meningiomas, neurinomas, chordomas, cholesteatomas) among which 24 extended supratentorially and 22 to Blumenbach's clivus; 23 gliomas of the cerebellum and fourth ventricle; 16 primary gliomas of the stem; 19 other new growths (hemangioblastomas, ha

Concurrence of subependymoma and heterotopic leptomeningeal neuroglial tissue.

Heterotopic neuroglial tissues in the leptomeninges are generally regarded as developmental malformations. They are uncommon in the general population (1%) but are more often found in patients with other CNS maldevelopments (25%). Of the 22 cases of subependymoma available for our study, four (18.2%) had concurrent heterotopic neuroglial tissue. The great majority of these neuroglial heterotopias were located over the brain stem and were topographically close to the subependymomas. Since the histogenetic nature of subependymoma, whether it represents a local maldevelopment (hamartoma) or a tru

Primary Ki-1 (anaplastic large cell) lymphoma of the brain and spinal cord.

The authors report a case of primary Ki-1 lymphoma of the brain. The patient was a 4 1/2-year-old black girl who presented with a 4- and 5-day history of headaches, nausea, vomiting, neck stiffness, and difficulty in walking. Computed tomography (CT) scan of the brain showed two discrete densities in the left occipital lobe and in the brain stem. Magnetic resonance imaging (MRI) showed multiple densities scattered over the brain surface and brain stem. Microscopically, the tumor was an anaplastic neoplasm that diffusely infiltrated brain parenchyma. The neoplastic cells were large with amphoph

Usefulness of sagittal computed tomography in neurosurgery.

Sagittal section tomographic images can be obtained either directly or by an electronic reconstructing procedure. These two techniques are extremely useful for neurosurgeons because it is possible to obtain a better clinical and pathological evaluation of the lesion. This procedure is almost recommended in case of pituitary adenoma, cerebello-pontine angle neoplasms, orbital lesions and brain-stem lesions. The authors show the results of the experience of the University of Pavia and discuss the true indications to this neuroradiological technique.

Neuromyoblastoma in the rat.

We report three tumours arising in the brain stem or adjacent cranial nerves of the Alderly Park rat. Light microscopy with special stains, immunocytochemistry and electron microscopy supported the conclusion that both neuronal and myoid differentiation occurred in all neoplasms. This was shown by the presence in neural cells of axon-like cell processes and in myoblastic cells of striated myofibrils. Although medulloblastomas occur in man and rats the tumours reported here were not related to the cerebellum. We have termed this tumour neuromyoblastoma.
