Breakthrough pain during cesarean delivery refers to a patient reporting significant pain or intense uncomfortable pressure; it is a patient-reported outcome that occurs in up to one-third of patients, depending on anesthetic risk factors (eg, epidural anesthesia) and obstetric scenarios (eg, intrapartum cesarean delivery, prolonged surgery). In this review, important considerations to better understand what breakthrough pain during cesarean delivery under neuraxial anesthesia is, how to measure and report it, and strategies to prevent, mitigate and manage its occurrence and associated consequ

PURPOSE OF REVIEW: An overview on breakthrough cancer pain (BTCP), including inherent limitations of the terminology, assessment, clinical presentation, and treatment options.
RECENT FINDINGS: The estimated prevalence of BTCP is dependent on the defined cutoffs for controlled background pain and the magnitude of the pain flare. In addition, pain flares outside the definition of BTCP are prevalent. In the 11th Revision of the International Classification of Diseases, the temporal characteristics of cancer pain are described as continuous background pain and intermittent episodic pain. BTCP sho

OBJECTIVE: To characterize cancer-related breakthrough pain (BTcP) among community-dwelling patients with cancer.
METHODS: Data from the National Breakthrough Pain Study, a cross-sectional observational survey describing breakthrough pain (BTP) in the community, were analyzed for a subset of patients with BTcP. Eligible patients were community-dwelling adults with commercial insurance and insurance claims consistent with cancer and chronic pain who consented to a structured telephone interview. Assessments included the Brief Pain Inventory-Short Form (BPI), the Short Form 12 Health Survey, th

Because dental implant post-surgical pain is mainly driven by inflammation, nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen (eg, Advil\'ae, Motrin\'ae IB) or naproxen sodium (Aleve\'ae) are logical choices to address it. These drugs are highly efficacious, non-addicting, and generally well tolerated. Dental implant patients, however, are generally older than those having their impacted third molars removed and are, therefore, likely to present with more comorbidities and drug intake, leading to potentially more adverse effects and serious adverse drug interactions. Fortunately,

Pain is common in patients with cancer (33-64%) and can be divided into background and breakthrough pain (BTP). BTP is a passing, acute pain that occurs despite the use of analgesia to control background pain. BTP may arise spontaneously or be provoked by certain movements or activities. It lasts 30-60 minutes and is generally self-limiting and is often undertreated. We describe 2 patients aged 68 and 57 years with metastatic disease who were admitted for pain management. BTP was inadequately managed during their hospital stay. Both patients had to wait too long before they received their BTP

Breakthrough pain is defined as transitory flares of pain. Breakthrough pain is caused by cancer, cancer complications, treatment or comorbidities. The usual onset to maximum breakthrough pain intensity time is 3 min and duration is 30 min; therefore, the assessment for response needs to be at short intervals. The rapid onset and offset of pain results in inadequate responses when oral opioids are used to manage pain flares. Several strategies have been used to manage breakthrough pain: titration of the chronic opioid, independent titration of rescue opioids and alternative routes. Buccal fent

Breakthrough pain--a transient increase in pain occurring over a baseline of controlled persistent pain--represents an important clinical challenge in the management of cancer and other chronic pain conditions. The fentanyl buccal tablet is a new sugar-free, easily-administered formulation that uses an effervescent drug delivery system to enhance the rate and extent of fentanyl absorption across the buccal mucosa. Clinical trials indicate that the rapid systemic exposure provided by fentanyl buccal tablets translates into clinically significant improvements in pain intensity. Pain relief is ob

BACKGROUND: Breakthrough cancer pain (BTCP) is common among cancer patients and markedly lowers their quality of life. The treatment for BTCP episodes that is recommended in current guidelines involves extended-release formulations in combination with rapid-onset and short-acting opioids. In the past few years, several new preparations of fentanyl, an opioid with a very rapid onset, have been approved for this indication. Treating physicians need to be aware of the clinical differences between the newer fentanyl preparations and immediate-release opioids.
METHODS: We searched the PubMed and E

SUMMARY Fentanyl buccal tablets (FBT) have been designed to treat breakthrough pain (BTP) in patients who are already receiving, and who are tolerant to, opioid therapy for their underlying persistent pain. FBT are a formulation that uses an effervescent drug delivery system to enhance penetration across the buccal mucosa. OraVescent technology provides an effervescent reaction that liberates carbon dioxide in the buccal cavity. This reaction causes an initial decrease in pH, which facilitates solubilization, thus driving fentanyl into solution. Subsequently, carbon dioxide increases the local

BACKGROUND: Although breakthrough pain (BTP; pain flares interrupting well-controlled baseline pain) is common among patients with cancer, its prevalence, characteristics, and impact on health-related quality of life (HRQOL) are poorly understood in ethnic minorities.
METHODS: This comparative study examines ethnic and gender differences in BTP characteristics and impact on HRQOL. Patients with stage III or IV cancer of the breast, prostate, colorectal, or lung, or stage II-IV multiple myeloma with BTP completed surveys (upon initial assessment, 3 months, and 6 months) assessing consistent pa

PURPOSE OF REVIEW: Neuraxial techniques are the current gold standard for labour analgesia, but are associated with up to 25% incidence of breakthrough pain. In this review, we aim to update clinicians on the latest research pertaining to the optimization of neuraxial labour analgesia.
RECENT FINDINGS: Attempts to improve the efficacy and reliability of epidural analgesia while minimizing adverse effects have resulted in the rising popularity of combined spinal epidural and the dural puncture epidural (DPE). Recent studies have focused on determining the safety, efficacy, and optimization of

BACKGROUND: Breakthrough pain is a transient increase in pain intensity over background pain. It is a common and distinct component of cancer pain that can have a negative impact for both the patient and carers' quality of life. Breakthrough pain is usually related to background pain and is typically of rapid onset, severe in intensity, and generally self-limiting with an average duration of 30 minutes. At present the current approach to managing breakthrough pain is using supplemental analgesia (also known as rescue medication) at a dose proportional to the total around-the-clock (ATC) opioid

BACKGROUND: The aim of this study was to evaluate patients' global impression (PGI), patient pain goal (PPG), achievement of a patient pain goal response (PPGR) among individuals with advanced cancer admitted to an acute palliative care unit (APCU), after a BTP medication.
METHODS: Longitudinal design was employed to assess 230 patients. Changes in BTP, PGI, PPG and PPGR were measured.
RESULTS: BTP intensity (mean, SD) before administration of a BTP medication (T0), 15 min after (T15) and 30 min after (T30) was 6.5 (1.63), 4.3 (1.5) and 2.3 (1.5), respectively. The mean PPG was 1.61 (SD 1.6)

BACKGROUND: Most breakthrough pain (BTP) studies assess patients with advanced cancer or those receiving inpatient care. Studies in noncancer populations are limited to surveys of pain clinics and patients with other advanced diseases. To better understand BTP, data are needed from less selected populations.
AIM: The aim of this study was to evaluate BTP in opioid-treated ambulatory patients with chronic cancer or noncancer pain treated in community practices.
METHODS: Primary care physicians or community-based oncologists recruited a convenience sample for a cross-sectional study of BTP at

INTRODUCTION: An ongoing national multicenter survey [Italian Oncologic Pain multiSetting Multicentric Survey (IOPS-MS)] is evaluating the characteristics of breakthrough cancer pain (BTP) in different clinical settings. Preliminary data from the first 1500 cancer patients with BTP enrolled in this study are presented here.
METHODS: Thirty-two clinical centers are involved in the survey. A diagnosis of BTP was performed by a standard algorithm. Epidemiological data, Karnofsky index, stage of disease, presence and sites of metastases, ongoing oncologic treatment, and characteristics of backgro

OBJECTIVE: We sought to assess the prevalence and characteristics of breakthrough pain (BTP) in patients with chronic back pain.
DESIGN: Researchers utilized a telephone survey using a pain assessment algorithm. This report represents a subset of patients from a larger survey of 228 patients with chronic pain unrelated to cancer.
PARTICIPANTS: This study employed 117 subjects taking opioids for a primary diagnosis of back pain and receiving care at geographically dispersed pain treatment centers. Subjects had pain lasting at least six months and had "controlled" baseline pain.
RESULTS: Eigh
